Multitarget stool DNA tests increases colorectal cancer screening among previously noncompliant Medicare patients

AIM To determine the uptake of noninvasive multitarget stool DNA (mt-s DNA) in a cohort of colorectal cancer(CRC) screening non-compliant average-risk Medicare *** This cross sectional primary care office-based study examined mt-s DNA uptake in routine clinical practice among 393 colorectal cancer screening non-compliant Medicare patients ages 50-85 ordered by 77 physicians in a multispecialty group practice (USMD Physician Services, Dallas, TX) from October, 2014-September, 2015. Investigators performed a Health Insurance Portability and Accountability Act compliant retrospective review of electronic health records to identify mt-s DNA use in patients who were either > 10 years since last colonoscopy and/or > 1 year since last fecal occult blood test. Test positive patients were advised to get diagnostic colonoscopy and thereafter patients were characterized by the most clinically significant lesion documented on histopathology of biopsies or excisional tissue. Descriptive statistics were employed. Key outcome measures included mt-s DNA compliance and diagnostic colonoscopy compliance on positive *** Over 12 mo, 77 providers ordered 393 mt-sD NA studies with 347 completed (88.3% compliance). Patient mean age was 69.8 (50-85) and patients were 64% female. Mt-sD NA was negative in 85.3%(296/347) and positive in 14.7%(51/347). Follow-up colonoscopy was performed in 49 positive patients (96.1% colonoscopy compliance) with two patients lost to follow up. Index findings included: colon cancer(4/49, 8.2%), advanced adenomas (21/49, 42.9%), non-advanced adenomas(15/49, 30.6%), and negative results (9/49, 18.4%). The positive predictive value for advanced colorectal lesions was 51.0% and for any colorectal neoplasia was 81.6%. This study was carried out at the Mamou Higher Institute of Technology during the period from March 10 to April 15, 2022, with the aim of designing and testing a solar dryer with forced convection by drying potatoes. The dryer was designed using local materials. Its main geometric parameters are: 1) height of the drying chamber (90 cm), 2) length of the drying chamber (50 cm), 3) width of the drying chamber (43 cm), 4) surface of the racks (0.1806 m2), 5) surface of the heat accumulator (0.2537 m2). The experiment focused on the vacuum test of the dryer for two days and that of the drying of the sweet potato for three days from 8:30 a.m. to 5:30 p.m. The average vacuum test temperature values of the three environments are respectively accumulator (43°C), dryer chamber (41°C) and ambient environment (34°C). Four kilograms (4 kg) of boiled sweet potato were dried. The average temperatures in the accumulator and in the drying chamber during the three days of drying are respectively 33°C and 39°C. The final mass of the dried product is 1.2 kg, with a quantity of water extracted of 2 liters or 63% of the initial mass of the product. The average drying rate is 0.074 kg/h. The drying kinetics showed a decreasing rate in the absence of the heating period and the constant rate period.

The study of the characteristic graph of an atom in all its aspects, allows to describe the concepts of atomistic. From this graph, some concepts of the recomposition of the electronic cloud have been described by specific graphs derived from it. The results are very conclusive. This graph illustrates each of the concepts of atomistics such as blocks, orders, periods and electronic layers. These concepts have been represented by lines, segments and even points. This has made it possible to draw up tables with orders, periods and even their correlations. Thus, this work promotes a better understanding of theoretical concepts by transposing the “abstract” aspect to a more “concrete” aspect of these concepts. This eventually facilitates the learning of this essential part of chemistry at its core. These results indicate that the research hypothesis has been verified.

The role of the oesophago-gastric junction (OGJ) in gastro-oesophageal reflux disease is still not completely understood, and there is no clinically used method to assess the OGJ function in patients. Only indirect methods such as pH studies are carried out. The OGJ acts a valve controlling the flow of solids, liquids and gases between the oesophagus and the stomach. Manometry can determine if a sphincter is toned or relaxed; but, it cannot confirm that the sphincter region is actually open. Distension is a new technique for measuring function in the OGJ. By measuring the cross-sectional area through the narrow region in the junction during distension of a catheter mounted bag, much more information on the opening and closing patterns of the junction can be determined. This technique has already been demonstrated to show changes in the OGJ after surgical treatments for reflux disease. New measurement ideas around the concept of distending the OGJ offer new hope that a clinically useable test for compliance at the junction can be developed and could potentially help in determining appropriate therapy.

Agroclimatological Characteristics of Rainy Seasons in Southwestern Burkina Faso during the 1970-2013 Period

The different drought phases observed during the 1970-2010 period have underlined important weaknesses of West African agricultural systems. The droughts resulted in important decreases in crop production, triggering a significant deficit in food availability. Many studies have identified changes in rain events seasonal patterns as the key drivers of agricultural production failure during these drought phases. In this study, seven agriculturally-relevant intra-seasonal rainfall characteristics (i.e., annual rainfall amount, onset and cessation of the rainy season, dry spells, extreme rain events, hot spells, and strong winds) and associated constraints to crop growth are described for the main cereals (maize, millet, and sorghum) in southwestern Burkina Faso. These characteristics are calculated or determined using daily climate data from a local network of 16 weather stations spanning the 1970-2013 period. A computation of the intensity and the occurrence of these phenomena during the rainy seasons helped to draw the rainy seasons’ nomenclature. Findings suggest that the rainy seasons during the drought phases are characterized by low annual rainfall amount, late onset, early cessation and more frequent long dry spells (>7 days). Furthermore, the long dry spells mostly occurred during the most sensitive phases of crop development: germination at the beginning of the rainy season and flowering at the end of the rainy season. Also, the intensity and the probability of occurrence of the other extreme events (hot spells and strong winds) during rainy seasons are very high in the establishment phase. Thus, adaptation strategies to mitigate these unfavorable climate conditions include a selection of short-cycle crop varieties combined with supplementary irrigation systems during long dry spells.

We have prepared three groups of Ge–As–Se glasses in which the Se content is 5.5 mol%, 10 mol%, and 20 mol%rich, respectively. We explored the possibility of suppressing the formation of the Ge–Ge and As–As homopolar bonds in the glasses. Thermal kinetics analysis indicated that the 5.5 mol% Se-rich Ge_（11.5）As_（24）Se_（64.5） glass exhibits the minimum fragility and thus is most stable against structural relaxation. Analysis of the Raman spectra of the glasses indicated that the Ge–Ge and As–As homopolar bonds could be almost completely suppressed in 20 mol% Se-rich Ge_（15）As_（14）Se_（71） glass.

Pollution by polycyclic aromatic hydrocarbons（PAHs） is widespread due to tmsuitable disposal of industrial waste. They are mostly defined as priority pollutants by environmental protection authorities worldwide. Phenanthrene, a typical PAH, was selected as the target in this paper. The PAH-degrading mixed culture, named ZM, was collected from a petroleum contaminated river bed. This culture was injected into phenanthrene solutions at different concentrations to quantify the biodegradation process. Results show near-complete removal of phenanthrene in three days of biodegradation if the initial phenanthrene concentration is low. When the initial concentration is high, the removal rate is increased but 20%-40% of the phenanthrene remains at the end of the experiment. The biomass shows a peak on the third day due to the combined effects of microbial growth and decay. Another peak is evident for cases with a high initial concentration, possibly due to production of an intermediate metabolite. The pH generally decreased during biodegradation because of the production of organic acid. Two phenomenological models were designed to simulate the phenanthrene biodegradation and biomass growth. A relatively simple model that does not consider the intermediate metabolite and its inhibition of phenanthrene biodegradation cannot fit the observed data. A modified Monod model that considered an intermediate metabolite （organic acid） and its inhibiting reversal effect reasonably depicts the experimental results.
