Cumulative positive fluid balance is a risk factor for acute kidney injury and requirement for renal replacement therapy after liver transplantation

AIM To analyze whether fluid overload is an independent risk factor of adverse outcomes after liver transplantation(LT).METHODS One hundred and twenty-one patients submitted to LT were retrospectively *** regarding perioperative and postoperative variables previously associated with adverse outcomes after LT were *** fluid balance(FB) in the first 12 h and 4 d after surgery were compared with major adverse outcomes after *** Most of the patients were submitted to a liberal approach of fluid administration with a mean cumulative FBover 5 L and 10 L,respectively,in the first 12 h and 4 d after *** FB in 4 d was independently associated with occurrence of both AKI and requirement for renal replacement therapy(RRT)(OR = 2.3;95%CI:1.37-3.86,P = 0.02 and OR = 2.89;95%CI:1.52-5.49,P = 0.001 respectively).Other variables on multivariate analysis associated with AKI and RRT were,respectively,male sex and Acute Physiology and Chronic Health Disease Classification System(APACHE II) levels and sepsis or septic *** was shown to be independently related to AST and APACHE II levels(OR = 2.35;95%CI:1.1-5.05,P = 0.02 and 2.63;95%CI:1.0-6.87,P = 0.04 respectively),probably reflecting the degree of graft dysfunction and severity of early postoperative course of *** effect of FB on mortality after LT was *** Cumulative positive FB over 4 d after LT is independently associated with the development of AKI and the requirement of *** was not independently related to FB,but to surrogate markers of graft dysfunction and severity of postoperative course of LT.

Immobilization in Spheres of a Cocktail Rich in Xylanase Produced by the Fungus Fusarium sp. EA 1.3.1 for Hydrolysis of Sugarcane Bagasse

Second generation ethanol is produced from the degradation of lignocellulosic biomass using enzymes as catalysts, with emphasis on xylanases. These biocatalysts are often costly, but stable at high temperatures, and their reuse is of great value, so the immobilization of the enzymes can increase their applicability on an industrial scale. We sought to immobilize a cocktail rich in xylanase produced by the fungus Fusarium sp. EA 1.3.1 in alginate spheres, optimize the immobilization method, characterize the immobilized derivatives, improve their physical-chemical characteristics, and perform the hydrolysis of sugarcane bagasse to release sugars. The Fusarium sp. EA 1.3.1 has been identified and used for cocktail rich in xylanase production that was immobilized in alginate spheres. During this process, the drip equipment, and the concentration of the solutions of sodium alginate and calcium chloride were evaluated. The best results were obtained with the glass rod and with concentrations of 3.14% and 2.10% for the solutions, respectively. The apparent optimum conditions of pH and temperature reaction were studied, and the values of pH 6.5 and 60°C were obtained. The immobilized conjugate also presented greater stability at this temperature than that of the soluble cocktail. The conjugate could be recycled up to six times, and its activity was maintained after 75 days of storage. Finally, the hydrolysis in natural sugarcane bagasse was achieved, and greater amounts of reducing sugars were obtained in the reaction with the conjugate. Thus, the cocktail rich in xylanase produced by the fungus Fusarium sp. EA1.3.1 was successfully immobilized on alginate spheres and possesses the potential to be used as a catalyst in industrial processes such as the lignocellulosic ethanol industry.

The volatile constituents of the essential oils from leaves from the weed plants Parthenium hysterophorus and Ambrosia polystachya were identified and quantified by GLC-MS and GLC. Allelopathic activities were determined by methods that evaluated the volatile effects and the direct application of these essential oils on the seed germination and seedling vigor of lettuce. We identified 27 compounds in the essential oil from P. hysterophorus, and the main constituents were germacrene-D (35.9%), trans-β-ocimene (8.5%) and β-myrcene (7.6%). In the essential oil from A. polystachya, 40 constituents were identified and the principal compounds were germacrene-D (29.3%), trans-β-ocimene (13.6%) and β-caryophyllene (9.8%). In both methods, the essential oil from A. polystachya presented a greater potential for reducing seed germination and seedling vigor in lettuce than the essential oil from P. hysterophorus, and this activity might be attributed to its higher content of monoterpenes.

Temporal Variations of the Chemical Composition of Three Seaweeds in Two Tropical Coastal Environments

The seaweeds Chaetomorpha antennina, Gymnogongrus griffithsiae and Ulva fasciata were studied regarding tissue concentrations of total nitrogen, total phosphorus, total protein, hydrosoluble protein, total carbohydrate, chlorophyll a and total carotenoid throughout a 39-month survey in two coastal environments of Rio de Janeiro State, Brazil. One of the sites (Itapuca Stone) has high concentrations of dissolved nutrients and an intense long-term process of cultural eutrophication;the second site (Bananal Inlet) is thought to have lower concentrations of dissolved nutrients and no relevant anthropic impact. Seaweeds experienced changes in the concentrations of the substances in the thalli;however they did not show any cyclic seasonal pattern, except for pigments, with lower values in summer in both sites. The differences found for each species in each sampling at the sites were small (e.g. U. fasciata, more total nitrogen at Itapuca Stone) or absent (e.g. C. antennina, no significant differences for hydrosoluble protein in the sites). Differences in the concentrations of dissolved nutrients in the sites did not generate contrasting chemical profiles in the seaweeds. There is no evidence of nitrogen- or phosphorus-limitation in any season. It is presumable that the concentrations of dissolved nutrients at the nutrient-poorer site are sufficient to generate high concentrations of the substances in the thalli of the species tested, similar to the concentrations measured in the eutrophic site. Experimental data are needed to elucidate the factors that promote the success of the species tested under contrasting nutrient availability and environmental disturbance.

Self-Care Practice of Patients with Mechanical Heart Valve Prosthesis Accompanied in Nursing Consultation

The objective was to evaluate the practice of self-care of patients with mechanical heart valve prosthesis. A descriptive, cross-sectional study developed in two outpatient valvopathies of teaching hospitals of Fortaleza/CE, from October 2013 to January 2014. The sample consisted of 127 patients with mechanical heart valve prosthesis. It was held an interview using instrument based on the Theory of Orem’s Self-Care and Brazilian Guidelines for Valvular Heart Diseases. The data were presented in tables and charts. Results: Universal self-care practices of larger adhesion: body hygiene (97.6% washed-haired, daily-bath 92.1%);oral hygiene (brushing teeth before sleeping, 87.4%);fluid intake (drinking-water 95.2%);food intake (salt intake ≤ 2 g/day, 92.1%, fruit and vegetable consumption 79.5%);intestinal eliminations (without blood or mucus—96%, non-parasitic 94.4%);urinary elimination (no blood nor pus-96.8%, urination 4 to 6 times a day, 96%). As self-care developmental requirements predominated: never used illegal drugs, tobacco, alcohol, or stopped at the discovery of the disease (70%). As self-care health deviation requirements we have: making use of certain medication at the right dose (95.2%);attending medical appointments (cardiologist—92.1%;nursing—84.2%);INR control (identifies signs of bleeding— 85.8%). We conclude that patients did not perform all recommended self-care practices, being necessary to establish strategies to reduce the self-care deficit.

Contact Precaution: Acceptance of Health Professionals to Teaching Hospital

The adoption by health professionals to the practice of patient isolation is a decision that reduces patients’ risks of occupational exposure to potentially contaminated biological material and prevents nosocomial infections. Verify the compliance of health professionals to the practices of contact precautions in patients colonized by microorganisms in a teaching hospital. This was a prospective observational cohort study in a special-sized hospital located in southeastern Brazil. The observation of health professionals in delivery of health care to patients with contact precaution was performed regarding hand hygiene, use of overcoat, glove, and mask. We carried out 1502 observations involving the following professional categories: nursing technicians and/or nursing assistants (n = 1028;68.4%), nurses (n = 200;13.3%), physical therapists (n = 185;12.3%), and physicians (n = 89;5.9%), totaling 971.8 hours. Regarding the level of compliance of professionals to contact precaution practices with hand hygiene after the procedures, the use of overcoats, gloves, and masks, all were statistically significant (p Klebsiella pneumoniae was the most positive pathogen identified in laboratory tests. This study provided the mapping of the compliance of health professionals to the practices of contact precautions in order to support a safer management of patient care reducing the risks of Healthcare-Associated Infections (HAI). This study also showed that health professionals are using practice management to deal with their behavior to protect their health.

Root Growth, Nutrition and Yield of Maize with Applied Finely Limestone in Surface of Cerrado Soil

The surface applies lime to correcting soil acidity in no-tillage and promotes root development and yield of maize. However, limestone with a particle size smaller than 0.30 mm cannot promote this effect. The aim of this study was to determine the root attributes, nutrition and productivity of maize with limestone ground in no-tillage. The experimental design was randomized blocks with five treatments and four replications. The treatments consisted of different particle sizes of limestone applying superficially to the soil under NT 0) control, without lime, 1) 0.30 mm to 0.20 mm 2) 0.20 mm to 0.10 mm, 3) 0, 10 mm to 0.05 mm 4) 0.05 mm to 0.01 mm. Attributes root, leaf nutrition and grain yield of maize were evaluated in two years 2009 and 2010. The elongation of roots in a 20-40 cm depth was greater when using limestone with particle size between 0.30 mm and 0.20 mm. The root length of maize plants correlated with the levels of Mn in maize leaves. The root development promoted by liming effect was not reflected in the productivity of maize.

Mesenchymal Stem Cells Express Low Levels of Cardiomyogenic Genes and Show Limited Plasticity towards Cardiomyogenic Phenotype

Mesenchymal stem cell differentiation towards osteogenic, chondrogenic and adipogenic lineages have been extensively described and reproduced in the literature. In contrast, cardiomyogenic differentiation still remains largely controversial. In this study the authors aim to shed new light into this unclear phenomenon and test whether BMMSC （bone marrow mesenchymal stem cells） and ATMSC （adipose tissue derived mesenchymal stem cells） are able to differentiate into functional cardiomyocytes, investigating two differentiation protocols. AT and BMMSC behaved differently when cultured in differentiation media and presented lower levels of proliferation and alkaline phosphatase production, expression of cardiomyocyte-specific transcription factors such as GATA-4, Nkx2-5 and proteins such as ct and 13 Myosin Heavy Chains. Furthermore, MSC started to express higher levels of Connexin-43 and c~ sarcomeric actinin protein. Unfortunately, though, MSC did not present cardiomyocyte-like electrophysiological properties. In order to analyze a possible explanation for such limited plasticity, the authors decided to address the issue using a quantitative approach. Gene expression was quantified by Real time PCR, and, for the first time, the authors show that a possible explanation for limited plasticity of MSC is that even though differentiated cells presented differential gene expression, the levels of key cardiomyogenic genes did not reach expression levels presented by adult cardiomyocytes, nor were maintained along differentiation, reaching peaks at 4 days of stimulation, and decaying thereafter.

Root Growth, Nutrition and Yield of Maize with Applied Different Limestone Particle Size in the Cerrado Soil

The limestone with smaller particle size provides rapid correction of soil acidity. However, the limestone with large particles can promote bigger root growth, and may still have a residual effect on the soil. The aim of this study was to evaluate the residual effect of the application and incorporation of different limestone with a particle size bigger than 0.30 mm in root growth, nutrition and yield of maize in Cerrado soil. A randomized completed block utilized in this experiment, with five treatments and four replications. The grain sizes of dolomitic limestone incorporated into the Oxisol were: 0 (control, no-lime), 2.00-0.82 mm, 0.82-0.56 mm, 0.56-0.30 mm, 0.30-0.20 mm, in doses of 6.5 t·ha-1, 3.9 t·ha-1, 2.6 t·ha-1 and 1.3 t·ha-1 respectively. The limestone applied in introduction no-tillage. The maize were evaluated of root attributes, nutrition and grain yield in two years 2009 and 2010. The use of limestone particle size of 2.00-0.82 mm, and the quantity of 6.5 t·ha-1 applied to the soil and incorporated showed elongation and root growth of maize thin depth of 20-40 cm in the soil. In that, same proposed management of the limestone was the highest maize grain yield in the first year. The rooting characteristics evaluated correlated with leaf Ca and Cu and maizegrain yield.
