The biochemical efficacy of primary cryoablation combined with prolonged total androgen suppression compared with radiotherapy on high-risk prostate cancer： a 3-year pilot study

To gain beneficial effects in the management of high-risk prostate cancer, an integrated approach that combines local therapy and androgen deprivation therapy （ADT） was used. We compared biochemical responses between primary cryosurgical ablation of the prostate （CSAP） combined with prolonged ADT and radiation combined with ADT, which is the established modality in high-risk disease. A total of 33 high-risk patients received CSAP combined with ADT for 3 months before and up to 24 months after treatment. This patient group was matched with another 33 patients who had undergone three-dimensional conformal radiation therapy （3D-CRT） with the same protocol for ADT. Biochemical recurrence （BCR） was assessed by the American Society for Therapeutic Radiation Oncology （ASTRO） definition, the Phoenix definition and a prostate-specific antigen （PSA） cutoff of 0.5 ng mL^-1. Median follow-up was 61.0 ± 11.9 months for the CSAP ＋ ADT group and 86.0±15.8 months for the 3D-CRT ＋ ADT group. In the CSAP group, major complications including rectourethral fistula and incontinence were not noted. In the CSAP ＋ ADT group, 57.0% had BCR using the ASTRO definition, 21.2% using the Phoenix definition and 54.5% using a PSA cutoff of 0.5 ng mL^-1. In the 3D-CRT ＋ ADT group, 54.5%, 21.2% and 54.5% had BCR using the ASTRO, Phoenix and PSA definition, respectively. In the CSAP ＋ ADT group, the BCR-free survival （BRFS） was 54 ± 10 months using the ASTRO definition, 65 ± 5 months using the Phoenix definition and 51 ± 4 months using a PSA cutoff of 0.5 ng mL-1. In the 3D-CRT ＋ ADT group, the BRFS was 68 ± 12, 93 ± 19 and 70 ± 18 months using the ASTRO, Phoenix and PSA definition, respectively. By the log-rank test, the BRFS values for each group were not statistically different. This intermediate-term result indicated that primary CSAP combined with prolonged ADT offers a parallel biochemical response compared with radiotherapy in high-risk prostate cancer.

Effect of natural extract eye drops in dry eye disease rats

AIM:To investigate the therapeutic effect of natural extract eye drops containing bee venom,musk,and deer antlers in dry eye disease(DED)animal ***:Scopolamine-injected DED rats and lacrimal gland-excised rats were allocated into control,saline,and natural extract groups respectively and a normal group(lacrimal gland excision was not performed)in lacrimal gland-excised *** eye drop instillation 4 times a day for 5 d,corneal fluorescein staining(CFS)scores,tear MUC5AC levels,and tear lactic dehydrogenase(LDH)levels were ***:In scopolamine-injected rats,the natural extract-treated group had significantly lower CFS scores(1.7±0.5,4.7±1.4,3.8±1.9,P=0.006)and tear LDH levels(0.10±0.01,0.19±0.01,0.16±0.08 OD,P=0.014)but higher tear MUC5AC levels(12.9±3.7,7.9±2.0,9.7±3.6 ng/m L,P=0.041)compared with the control and saline-treated *** were no significant differences between the control and saline-treated *** lacrimal gland-excised rats,the natural extract-treated group also had lower CFS scores(4.3±1.2,11.5±2.3,9.0±1.9,P<0.001,P=0.001)and tear LDH levels(0.30±0.08,0.48±0.12,0.39±0.05 OD,P<0.05)but higher tear volume(4.3±0.9,1.9±0.7,2.8±1.1 mm,P=0.005,P=0.124)and tear MUC5 AC levels(8.2±2.0,2.9±1.2,5.4±2.2 ng/m L,P<0.001,P=0.047)compared with the control and saline-treated *** were no significant differences in the CFS scores,tear MUC5AC level,and tear LDH level between the normal and natural extracttreated ***:The natural extract consisting of bee venom,musk,and deer antlers may have effectiveness in DED treatment by restoring the damaged ocular surface,increasing tear volume,and recovering the tear mucin layer in DED rats.

Efficacy and safety of a patient-positioning device(EZ-FIX)for endoscopic retrograde cholangiopancreatography

AIM: To assess the efficacy and safety of a patientpositioning device(EZ-FIX) for endoscopic retrograde cholangiopancreatography(ERCP).METHODS: A total of 105 patients were randomized to the EZ-FIX(n = 53) or non-EZ-FIX(n = 52) group in this prospective study. Midazolam and propofol,titrated to provide an adequate level of sedation during therapeutic ERCP, were administered by trained registered nurses under endoscopist *** outcome measures were the total dose of propofol and sedative-related complications, including hypoxia and hypotension. Secondary outcome measures were recovery time and sedation satisfaction of the endoscopist, nurses, and ***: There was no significant difference in the rate of hypoxia, but there was a statistical trend(EX-FIX group; n = 4, 7.55%, control group; n = 6, 11.53%,P = 0.06). The mean total dose of propofol was lower in the EZ-FIX group than in the non-EZ-FIX group(89.43 ± 49.8 mg vs 112.4 ± 53.8 mg, P = 0.025).In addition, the EZ-FIX group had a shorter mean recovery time(11.23 ± 4.61 mg vs 14.96 ± 5.12 mg, P< 0.001). Sedation satisfaction of the endoscopist and nurses was higher in the *** group than in the nonEZ-FIX group. Technical success rates of the procedure were 96.23% and 96.15%, respectively(P = 0.856).Procedure-related complications did not differ by group(11.32% vs 13.46%, respectively, P = 0.735).CONCLUSION: Using EZ-FIX reduced the total dose of propofol and the recovery time, and increased the satisfaction of the endoscopist and nurses.

Effect of electrical stimulation on neural regeneration via the p38-RhoA and ERK1/2-Bcl-2 pathways in spinal cord-injured rats

Although electrical stimulation is therapeutically applied for neural regeneration in patients, it remains unclear how electrical stimulation exerts its effects at the molecular level on spinal cord injury （SCI）. To identify the signaling pathway involved in electrical stimulation improving the function of injured spinal cord, 21 female Sprague-Dawley rats were randomly assigned to three groups： control （no surgical intervention, n = 6）, SCI （SCI only, n = 5）, and electrical simulation （ES; SCI induction followed by ES treatment, n = 10）. A complete spinal cord transection was performed at the 10^th thoracic level. Electrical stimulation of the injured spinal cord region was applied for 4 hours per day for 7 days. On days 2 and 7 post SCI, the Touch-Test Sensory Evaluators and the Basso-Beattie-Bresnahan locomotor scale were used to evaluate rat sensory and motor function. Somatosensory-evoked potentials of the tibial nerve of a hind paw of the rat were measured to evaluate the electrophysiological function of injured spinal cord. Western blot analysis was performed to measure p38-RhoA and ERK1/2-Bcl-2 pathways related protein levels in the injured spinal cord. Rat sensory and motor functions were similar between SCI and ES groups. Com- pared with the SCI group, in the ES group, the latencies of the somatosensory-evoked potential of the tibial nerve of rats were significantly shortened, the amplitudes were significantly increased, RhoA protein level was significantly decreased, protein gene product 9.5 expression, ERK1/2, p38, and Bcl-2 protein levels in the spinal cord were significantly increased. These data suggest that ES can promote the recovery of electrophysiological function of the injured spinal cord through regulating p38-RhoA and ERK1/2-Bcl-2 pathway-related protein levels in the injured spinal cord.

Effects of precipitation strengthening heat treatment for Al-Mg alloy

The corrosions resulting from defects in painting layers frequently occur in Al alloys, so the application of corrosion preventing systems is also very important. Optimum conditions in terms of electrochemistry in relation to solution treatment, quenching and artificial aging treatment were established in order to optimize precipitation strengthening conditions intended to enhance the strength of Al alloys. Slow strain rate tests （SSRT） at various applied potentials were conducted in potential range from -1.8 to 0.5 V. The results show that the maximum tensile strengths, elongations and time-to-fracture are shown to be high values. After precipitation strengthening heat treatment, a tendency appear that time-to-fracture increases as elongation increases. In the potential range from -1.3 V to -0.7 V, the specimens show excellent mechanical properties, and thus this range is considered to be a corrosion prevention range.

How good is cola for dissolution of gastric phytobezoars?

AIM:To evaluate the efficacy of cola treatment for gastric phytobezoars,including ***:A total of 17 patients(range:48 to 78 years) with symptomatic gastric phytobezoars treated with cola and adjuvant endoscopic therapy were *** liters of cola lavage(10 cases) or drink(7 cases) were initially used,and then endoscopic fragmentation was done for the remnant bezoars by using a lithotripsy basket or a polypectomy *** overall success of dissolving a gastric phytobezoars with using three liters of cola and the clinical and endoscopic findings were compared retrospectively between four cases of complete dissolution by using only cola and 13 cases of partial dissolution with ***:After 3 L of cola lavage or drinking,a complete dissolution of bezoars was achieved in four patients(23.5%),while 13 cases(76.5%) were only partially ***(4 of 6 cases) were observed more frequently than diospyrobezoars(0 of 11) in the group that underwent complete dissolution(P = 0.006).Gender,symptom duration,size of bezoar and method of cola administration were not significantly different between the two *** of 13 patients with residual bezoars were completely treated with a combination of cola and endoscopic ***:The rate of complete dissolution with three liters of cola was 23.5%,but no case of diospyrobezoar was completely dissolved using this ***,pretreatment with cola may be helpful and facilitate endoscopic fragmentation of gastric phytobezoars.

Predictive factors associated with malignancy of intraductal papillary mucinous pancreatic neoplasms

AIM:To identify preoperative predictive factors associated with malignancy of intraductal papillary mucinous neoplasms(IPMNs) of the ***:Between April 1995 and April 2010,129 patients underwent surgical resection for IPMNs at our institute and had confirmed pathologic *** medical records were retrospectively reviewed and immunohistochemical staining for mucin(MUC) in pancreatic tissues was ***:Univariate analysis showed that the following five variables were closely associated with malignant IPMNs preoperatively:absence of extrapancreatic malignancy;symptoms;tumor size > 4 cm;main pancreaticduct(MPD) size > 7 mm;and lymph node enlargement on preoperative computed tomography(CT).Multivariate analysis revealed that the following two factors were significantly associated with malignant IPMNs preoperatively:MPD size > 7 mm [odds ratio(OR) = 2.50];and lymph node enlargement on preoperative CT(OR = 3.57).No significant differences in the expression of MUC1,MUC2 and MUC5AC were observed between benign and malignant ***:MPD size > 7 mm and preoperative lymph node enlargement on CT are useful predictive factors associated with malignancy of IPMNs.

Generation of tumor spheroids using a droplet-based microfluidic device for photothermal therapy

Despite their simplicity,monolayer cell cultures are not able to accurately predict drug behavior in vivo due to their inability to accurately mimic cell-cell and cell-matrix *** contrast,cell spheroids are able to reproduce these interactions and thus would be a viable tool for testing drug ***,the generation of homogenous and reproducible cell spheroids on a large scale is a labor intensive and slow process compared to monolayer cell ***,we present a droplet-based microfluidic device for the automated,large-scale generation of homogenous cell spheroids in a uniform *** the microfluidic system,the size of the spheroids can be tuned to between 100 and 130μm with generation frequencies of 70 *** demonstrated the photothermal therapy(PTT)application of brain tumor spheroids generated by the microfluidic device using a reduced graphene oxide-branched polyethyleneimine-polyethylene glycol(rGO-BPEI-PEG)nanocomposite as the PTT ***,we generated uniformly sized neural stem cell(NSC)-derived neurospheres in the droplet-based microfluidic *** also confirmed that the neurites were regulated by ***,this droplet-based microfluidic device could be a powerful tool for photothermal therapy and drug screening applications.
