Population-based X-ray gastric cancer screening in Hiroshima prefecture, Japan

BACKGROUND X-ray gastric cancer(GC)screening has been shown to decrease mortality.Population-based X-ray GC screening has been performed in Hiroshima Prefe-cture,Japan,since 1983 but time trends and the efficacy of the method over 39 years have not been assessed.METHODS This was a population-based retrospective study.The data were derived from aggregated data of the Hiroshima Regional Health Medical Promotion Organization,including the number and rate of participants and those requiring esophagogastroduodenoscopies(EGDs),the number and rate of participants diagnosed as having GC,and the positive predictive value of the abnormal findings detected by X-ray and confirmed by EGDs.The number and rate of esophageal cancers were also collected.Further,the cost of detecting one GC was evaluated.RESULTS The number of participants has decreased during the last four decades,from 39925 in 1983 to 12923 in 2021.The rate of those requiring EGDs decreased significantly in recent years(P<0.001).The number of participants diagnosed as having GC has also declined,from 76 to 10 cases.However,the rate of cases diagnosed as GC among the participants remained around 0.1%.The positive predictive value increased significantly in recent years except during 1983-1991.The number and rate of accidentally detected esophageal cancers have risen recently,from 0%in 2008 to 0.02%in 2021,one-fifth of the diagnosis rate of GC.One GC diagnosis costs approximately 4200000 Japanese Yen(30000 United States Dollars)for the X-ray screenings and EGDs.CONCLUSION X-ray GC screening in Hiroshima has been efficient,but one challenge is the cost.Esophageal cancers may also need to be considered because they have gradually increased in recent years.

BACKGROUND Sessile serrated lesions(SSLs)are considered precancerous colorectal lesions that should be detected and removed to prevent colorectal cancer.Previous studies in Vietnam mainly investigated the adenoma pathway,with limited data on the serrated pathway.AIM To evaluate the prevalence,risk factors,and BRAF mutations of SSLs in the Vietnamese population.METHODS This is a cross-sectional study conducted on patients with lower gastrointestinal symptoms who underwent colonoscopy at a tertiary hospital in Vietnam.SSLs were diagnosed on histopathology according to the 2019 World Health Organi-zation classification.BRAF mutation analysis was performed using the Sanger DNA sequencing method.The multivariate logistic regression model was used to determine SSL-associated factors.RESULTS There were 2489 patients,with a mean age of 52.1±13.1 and a female-to-male ratio of 1:1.1.The prevalence of SSLs was 4.2%[95%confidence interval(CI):3.5-5.1].In the multivariate analysis,factors significantly associated with SSLs were age≥40[odds ratio(OR):3.303;95%CI:1.607-6.790],male sex(OR:2.032;95%CI:1.204-3.429),diabetes mellitus(OR:2.721;95%CI:1.551-4.772),and hypertension(OR:1.650,95%CI:1.045-2.605).The rate of BRAF mutations in SSLs was 35.5%.CONCLUSION The prevalence of SSLs was 4.2%.BRAF mutations were present in one-third of SSLs.Significant risk factors for SSLs included age≥40,male sex,diabetes mellitus,and hypertension.

Identifying high-risk individuals for gastric cancer surveillance from western and eastern perspectives: Lessons to learn and possibility to develop an integrated approach for daily practice

Current evidence shows that individuals with gastric dysplasia, severe and extensive gastric atrophy, extensive gastric intestinal metaplasia and the incomplete subtype of intestinal metaplasia are at high risk for gastric cancer(GC) development. There are several approaches to identifying these subjects,including noninvasive methods, esophagogastroduodenoscopy and histology.The main approach in Western countries is histology-based while that in Eastern countries with a high prevalence of GC is endoscopy-based. Regarding asymptomatic individuals, the key issues in selecting applicable approaches are the ability to reduce GC mortality and the cost-effectiveness of the approach. At present, population-based screening programs have only been applied in a few Asian countries with a high risk of GC. Pre-endoscopic risk assessment based on demographic and clinical features, such as ethnicity, age, gender, smoking and Helicobacter pylori status, is helpful for identifying subjects with high pre-test probability for a possibly cost-effective approach, especially in intermediate-and low-risk countries. Regarding symptomatic patients with indications for esophagogastroduodenoscopy, the importance of opportunistic screening should be emphasized. The combination of endoscopic and histological approaches should always be considered as endoscopy provides a real-time assessment of the patient’s risk level. In addition, imaging enhanced endoscopy(IEE) has been shown to facilitate targeted biopsies resulting in better correlation between endoscopic and histological findings. Currently, the use of IEE is recommended for endoscopic examinations, and the Operative Link for Gastric Intestinal Metaplasia or Operative Link on Gastritis Assessment grading systems are recommended for histological examinations whenever available. However,resource limitations are an important barrier in many regions worldwide. BACKGROUND Major societies provide differing guidance on management of Barrett’s esophagus(BE),making standardization challenging.AIM To evaluate the preferred diagnosis and management practices of BE among Asian endoscopists.METHODS Endoscopists from across Asia were invited to participate in an online questionnaire comprising eleven questions regarding diagnosis,surveillance and management of BE.RESULTS Five hundred sixty-nine of 1016(56.0%)respondents completed the survey,with most respondents from Japan(n=310,54.5%)and China(n=129,22.7%).Overall,the preferred endoscopic landmark of the esophagogastric junction was squamocolumnar junction(42.0%).Distal palisade vessels was preferred in Japan(59.0%vs 10.0%,Pducation and research are possible reasons.

Tuberculosis is a dangerous disease to human life,and we need a lot of attempts to stop and reverse it.Significantly,in theCOVID-19 pandemic,access to medical services for tuberculosis has become very difficult.The late detection of tuberculosis could lead to danger to patient health,even death.Vietnamis one of the countries heavily affected by the COVID-19 pandemic,andmany residential areas as well as hospitals have to be isolated for a long time.Reality demands a fast and effective tuberculosis diagnosis solution to deal with the difficulty of accessingmedical services,such as an automatic tuberculosis diagnosis system.In our study,aiming to build that system,we were interested in the tuberculosis diagnosis problem from the chest X-ray images of Vietnamese patients.The chest X-ray image is an important data type to diagnose tuberculosis,and it has also received a lot of attention from deep learning researchers.This paper proposed a novel method for tuberculosis diagnosis and visualization using the deeplearning approach with a large Vietnamese X-ray image dataset.In detail,we designed our custom convolutional neural network for the X-ray image classification task and then analyzed the predicted result to provide visualization as a heat-map.To prove the performance of our network model,we conducted several experiments to compare it to another study and also to evaluate it with the dataset of this research.To support the implementation,we built a specific annotation system for tuberculosis under the requirements of radiologists in the Vietnam National Lung Hospital.A large experiment dataset was also from this hospital,and most of this data was for training the convolutional neural network model.The experiment results were evaluated regarding sensitivity,specificity,and accuracy.We achieved high scores with a training accuracy score of 0.99,and the testing specificity and sensitivity scores were over 0.9.Based on the X-ray image classification result,we visualize prediction results as heat-maps and also analyze t

Influence of different land-use types on selected soil properties related to soil fertility in A Luoi District,Thua Thien Hue,Vietnam

Land use type affects the physicochemical properties of soil.The value of OM and TN is high in acacia soils.Strong acidity of soil is considered a serious constraint for agriculture.Suitable cash crops are acacia,cassava,banana,rice and maize.Soil fertility is affected by land-use types and land management,which exacerbates soil erosion and various other forms of soil degradation in the mountainous regions of Viet-nam.This study was conducted in A Luoi District,Thua Thien Hue,Vietnam to identify the effects of land-use types on specific soil physicochemical characteristics related to soil fertility.Soil physicochemical properties,such as organic matter(OM),total nitrogen(TN),total phosphorous(TP),and K+were significantly affected by land-use type.The results showed that the soils were sandy in rice but clay loam for acacia and cassava.The mean bulk density value of acacia soil was significantly greater than that of other soils.TN were higher in the acacia soils than those in the rice,maize,and banana soils.The OM content was significantly higher in the acacia,cassava,and banana soils than those in the rice and maize soils.The mean of exchangeable K+in the rice soil was higher compared to those in other soils and was affected by land-use type.The high exchangeable acidity content in the soils was probably due to intensive precipitation.However,both land use type and management did not affect the CEC value.Overall,the inappropriate land use caused the disturbance of soil physicochemical properties,indicating that the conditions of rice and maize soils are becoming worse than acacia soils.Therefore,lowering the intensity of cultivation,adopting incorporated soil fertility management,and applying organic fertilizer should preserve the existing conditions and enhance soil properties.

Effectiveness on post-stroke hemiplegia in patients:electroacupuncture plus cycling vs electroacupuncture alone

OBJECTIVE:To evaluate the effectiveness of cycling in combination with electroacupuncture in treatment of poststroke hemiplegia patients at National Hospital of Acupuncture,Vietnam.METHODS:The study was designed as a single-centre,outcome-assessor-blinded parallel randomised controlled trial with 120 post-stroke hemiplegia patients randomly assigned into two groups:electroacupuncture plus cycling(CT group) and electroacupuncture(AT group).Patients were assessed before and after the treatment(using muscle grading,modified Rankin,Barthel,Orgorozo scores and electromyography).Statistical Man–Whitney U test,and Fisher’s exact tests were used to compare between CT and AT groups.RESULTS:The results reported statistically significant improvement in motor function in patients suffering from hemiplegia following ischemic stroke in both CT and AT groups.Patients in CT group experienced a greater improvement compared to those in AT group including better muscle contraction(increased frequency and amplitude of electromyography and increased muscle grading scale);increased recovery(Orgogozo scale),increased independency(Barthel scale) and decreased disability(Modified Rankin scale)(P < 0.01).CONCLUSIONS:Combination with cycling training significantly improves the recovery of post-stroke patients treated with electroacupuncture.

Novel hybrid models of ANFIS and metaheuristic optimizations (SCE and ABC) for prediction of compressive strength of concrete using rebound hammer field test

In this study,we developed novel hybrid models namely Adaptive Neuro Fuzzy Inference System(ANFIS)optimized by Shuffled Complex Evolution(SCE)on the one hand and ANFIS with Artificial Bee Colony(ABC)on the other hand.These were used to predict compressive strength(Cs)of concrete relating to thirteen concrete-strength affecting parameters which are easy to determine in the laboratory.Field and laboratory tests data of 108 structural elements of 18 concrete bridges of the Ha Long-Van Don Expressway,Vietnam were considered.The dataset was randomly divided into a 70:30 ratio,for training(70%)and testing(30%)of the hybrid models.Performance of the developed fuzzy metaheuristic models was evaluated using standard statistical metrics:Correlation Coefficient(R),Root Mean Square Error(RMSE)and Mean Absolute Error(MAE).The results showed that both of the novel models depict close agreement between experimental and predicted results.However,the ANFIS-ABC model reflected better convergence of the results and better performance compared to that of ANFIS-SCE in the prediction of the concrete Cs.Thus,the ANFIS-ABC model can be used for the quick and accurate estimation of compressive strength of concrete based on easily determined parameters for the design of civil engineering structures including bridges.

BACKGROUND Mantle cell lymphoma(MCL)of the gastrointestinal tract is a rare malignancy,accounting for about 0.2%of malignant colorectal tumors.MCL synchronous with rectal adenocarcinoma is extremely rare.We know of only a few cases reported in the literature.We describe the case of a patient with synchronous rectal adenocarcinoma and intestinal MCL.CASE SUMMARY A 63-year-old man was admitted to our hospital due to abdominal pain and hematochezia over the past month.The patient was diagnosed with middle rectal cancer cT2N0M0 and underwent surgery.However,we found a large tumor in the small intestine during surgery.The patient underwent total mesorectal excision for rectal cancer and resectioning of the ileal segment containing the large mass.Pathology and immunohistochemistry revealed the presence of both rectal adenocarcinoma and pathognomonic MCL stage IIE presenting as multiple lymphomatous polyposis.The patient subsequently underwent RDHAP/RCHOP chemotherapy and was maintained with rituximab.A Positron Emission Tomography and Computed Tomography(PET/CT)scan showed that the disease responded well to treatment without tumor-increased metabolism in the gastrointestinal tract.CONCLUSION Synchronous rectal adenocarcinoma and intestinal MCL presenting as multiple lymphomatous polyposis are extremely rare.MCL is often discovered fortuitously when rectal cancer is diagnosed.The coexistence of these tumors poses treatment challenges.

Crack identification in functionally graded material framed structures using stationary wavelet transform and neural network

In this paper,an integrated procedure is proposed to identify cracks in a portal framed structure made of functionally graded material(FGM)using stationary wavelet transform(SWT)and neural network(NN).Material properties of the structure vary along the thickness of beam elements by the power law of volumn distribution.Cracks are assumed to be open and are modeled by double massless springs with stiffness calculated from their depth.The dynamic stiffness method(DSM)is developed to calculate the mode shapes of a cracked frame structure based on shape functions obtained as a general solution of vibration in multiple cracked FGM Timoshenko beams.The SWT of mode shapes is examined for localization of potential cracks in the frame structure and utilized as the input data of NN for crack depth identification.The integrated procedure proposed is shown to be very effective for accurately assessing crack locations and depths in FGM structures,even with noisy measured mode shapes and a limited amount of measured data.
