The thera.eutic a.proa.h of a.optive lymphocyte tra.sfer (a.T) using lymphocytes primed a.d expa.ded ex-vivo by exposure to tota. tumor RNa.(ttRNa. conta.ning dendritic cells (DCs) a.d a.ministered a.ter lymphodepletive host conditioning in pa.ients with refra.tory mela.oma.with bra.n meta.ta.es ha. shown excellent objective responses indica.ing tha. the centra. nervous system (CNS) is not a. immune privileged site a.d further pa.ed the wa. for utiliza.ion of a.simila. a.proa.h in other ca.cers. We ha.e shown tha. the use of a.T + ttRNa.DCs following either myeloa.la.ive or non-myeloa.la.ive host conditioning is fea.ible a.d sa.e a.d a.pea.s to prolong surviva. in a.proportion of children with recurrent medullobla.toma.who ha. fa.led sta.da.d cytotoxic thera.y. Further refinements in this promising a.proa.h a.e needed to improve outcomes a.d extend this trea.ment to a.broa. ra.ge of CNS ma.igna.cies.
