Objective: To assess the effectiveness of ultrasound-guided supraginguinal block (SIB) in the management of pain after total hip replacement. Material and Methods: This was a prospective, randomized, single-blind controlled study carried out in the anesthesia-intensive care unit of the Donka National Hospital in Conakry, over a period of 06 months (01/01/2020 to 30/06/2020). It concerned 32 patients: 16 patients in the “ultrasound-guided SIB” group and 16 patients in the “standard analgesia” group. Results: The pain scores assessed by the simple verbal scale and collected at the different time intervals (6H, 12H, 24H, 36H, 48H) showed mean scores 3 in the standard analgesia group (P Conclusion: Our study demonstrated that echo-guided SIB provided better analgesia compared to standard analgesia for the management of postoperative pain after total hip replacement.

COVID-19 appears to be associated with a disproportionate risk of thrombosis. The occurrence of thrombosis is a frequent complication of many medical and surgical conditions. Their prevention by LMWH is the option of choice as well as their early diagnosis in this pandemic context. In addition, several recent observations support that severe pulmonary embolism is very common in hospitalized patients with COVID-19. We report the case of late pulmonary embolism in a 26-year-old patient with a medical history of (SARS-CoV2) and newly discovered diabetes. The clinical examination found exertional dyspnea, intense chest pain, blood pressure at 145/85mmHg, heart rate at 129 bpm, respiratory rate at 25 cycles/min, blood sugar at 1.45 g/l, oxygen saturation at 92%. In front of this clinical picture, the thoracic CT angiography to ask shows a pulmonary embolism of the right segmental and sub-segmental branches less marked on the left, involving the lingular and basal branches; the management was limited to anticoagulation associated with rehabilitation sessions (respiratory physiotherapy).

Case Report for Two Hemodialysed Patients Diagnosed for COVID-19 at the Donka National Hemodialysis Center

In December 2019, coronavirus disease (COVID-19) due to infection with the novel SARS-CoV-2 virus began to break out. Hemodialysis patients are at high risk because of the co-morbidities. We report the clinical and biological characteristics of two patients who developed COVID-19 infection in our dialysis center in Donka National Hospital. None of the patients had contact with an infected person. The age was 38 and 54 years. The symptoms common to both patients were: fatigue, diarrhea, fever. Lymphopenia was present in both patients. None of them had a chest X-ray or chest scan because they were positive for the test before admission.

This study used 2D seismic profiles to investigate the Cenozoic evolution of faults in the Huizhou Sub-basin. It aims to define the basin structure style through describing the geometric fea- tures of the faults and quantitatively analyzing their activities. The results indicate that the boundary faults in the Huizhou Sub-basin display en echelon arrangement in plan view, which indicates that, it was caused by a kind of oblique extension. Calculating the fault slips shows long-term activities of faults occurred in the basin, and some boundary faults kept active after 5.5 Ma. The evolution history of the fault system is reconstructed. Initially, during the Eocene-Early Oligocene, mainly NNE-NE and NW trending faults and NE striking grabens and half-grabens formed in the basin and a series of faults system controlled the deposition. During the Late Oligocene--Early Miocene, the structural ac- tivities were relatively weak, the fault activity and the fault growth rate decreased sharply. Finally, in the late stage from Late Miocene to the present, the structure movement was re-activated, and some faults were also reactivated. Our study will help better understand the structural features and evolu- tion of the petroleum-bearing basins in the northern margin of the South China Sea.

An extensive database of reservoir sedimentation surveys throughout continental United States is compiled and analyzed to determine specific degradation SD relationships as function of mean annual rainfall R, drainage area A, and watershed slope S. The database contains 1463 field measurements and specific degradation relationships are defined as function of A, R and S. Weak trends and significant variability in the data are noticeable. Specific degradation measurements are log normally distributed with respect to R, A, and S and 95% confidence intervals are determined accordingly. The accuracy of the predictions does not significantly increase as more independent variables are added to the regression analyses.

The Huizhou sag is situated on the continental shelf of the northern continental margin of the South China Sea. In this paper we present a grid of reflection seismic and well data to characterize the basin structure and prominent unconformities. We employ EBM and 2DMOVE softwares to explore the subsidence history and stratigraphic development history of the basin. We found a rapid subsidence period since 15.5 Ma. Moreover, we calculated the stretching factors of the upper crust and the whole crust in the Huizhou sag. The results show the values are 1.10–1.13 and 1.08–1.31, respectively, indicating faulting in Huizhou sag is relatively small. It is noteworthy that the faults map reveals en echelon distribution at the north and south margins of the basin. We suggest en echelon faults here are caused by the subduction of Proto-South China Sea toward NW Borneo block and cease of the South China Sea. Considering the pronounced unconformities, subsidence rates, fault activities and sediment thickness, the Cenozoic tectonic evolution of the basin can be divided into rifting（49–32 Ma）, post-rifted（32–15.5 Ma） and rapid subsidence（15.5–0 Ma） stages. Our study will shed new light on the tectonics of SE Asia and petroleum exploration in the South China Sea.

Evaluation of the Effectiveness and Nature of Analgesia in the Medical-Surgical Emergency Department of CHU-Donka

Introduction: Pain assessment is an essential step towards pain relief. In our context, however, it is insufficient in emergency departments. The aim was to assess the nature and effectiveness of the analgesia used in the emergency department of the CHU-Donka. Method: This was a single-centre prospective observational study conducted over one month (November 2020) in the medical-surgical emergency department of CHU-Donka. All patients aged 18 or over admitted to the emergency department with moderate to severe acute pain were included. Results: We enrolled 880 patients, 615 of whom (69.88%) were in pain. Males predominated (65.2%), with a sex ratio of 1.87. The average age was 44.78 ± 16.41 years, with extremes ranging from 18 to 85 years. The most represented age group was 18 - 34 years, with 66.18%. Initial assessment of pain by nursing staff in 3 cases. Initial assessment of pain by the investigator was 100%, 74% moderate and 26% severe. No reassessment of pain by nursing staff. Reassessment by the interviewer was 100% and found 5% no pain, 61% mild pain, 24% moderate pain and 10% severe pain. With an average numerical scale of 3.77 ± 2.61 and extremes from 0 to 10. Average length of stay was 2.85 ± 1.48 hours, with extremes ranging from 45 minutes to 8 hours. Analgesia was administered with paracetamol alone (43.58%), combined with tramadol or nefopam. No patient received morphine. Conclusion: This study revealed a lack of pain assessment in our department.

Background:Despite the progress made in the control of Neglected Tropical Diseases(NTD),schistosome and soil-transmitted helminth infections are far from being effectively managed in many parts of the ***,the key element of all control strategies,is faced with some difficulties in terms of access to *** study aims to describe the factors involved in the success or failure of the community-directed intervention(CDI)approach through control programmes,which aims to achieve consistent high coverage at affordable and sustainable costs in endemic ***:The CDI approach was carried out from December 2007 to October 2008 in ten villages of the district of Diéma,*** inclusion,each child part of the study’s sample was interviewed and submitted for a physical *** study focused on:data collection,treatment of the eligible population,evaluation of the treatment coverage,performance of community drug distributors(CDDs),and the involvement and perception of ***:A total of 8,022 eligible people were studied with a mean coverage rate of 76.7%.Using multiple regression,it was determined that receiving praziquantel as treatment was associated with five factors:belonging to the Fulani or Moorish ethnic minority versus the Bambara/Soninke,use of the central versus the house-to-house drug distribution mode,the ratio of the population to the number of CDDs,the lack of supervision and belonging to the age group of 15 years or above(p<0.05).As well as that,it was found that the presence of parallel community-based programmes(HIV,tuberculosis)that provide financial incentives for community members discouraged many CDDs(who in most cases are volunteers)to participate in the CDI approach due to a lack of ***:The findings indicate that the success of the CDI approach depends on,amongst other things,the personal characteristics of the respondents,as well as on community factors.

Cooperative file sharing mechanism with Network Coding in wireless Mesh networks

This article describes a new model of a cooperative file sharing system in a wireless Mesh network. The authors＇ approach is to develop an efficient and cooperative file sharing mechanism based on opportunistic random linear Network Coding. Within this mechanism, every node transmits random linear combination of its packets according to cooperative priority, which is computed in a distributed manner according to the node-possible contribution to its neighbor nodes. With this mechanism, the more a node contributes to others, the more the node has chances to recover the entire file first. The performance metrics of interest here are： the delay until all the packets in a file have been delivered to all nodes, and an ideal packet size, by the use of which the authors can get the minimum transmission delay. Through extensive simulation the authors compare their mechanism with the current transmission process in a wireless Mesh network without random linear Network Coding. The authors found that using their mechanism, the nodes can cooperatively share the entire file with less transmission time and delay than the current transmission process without random linear network.
