Epidemiological, Clinical Aspects and Factors Associated with Typical Gastroesophageal Reflux Disease in the General Population of Parakou, Republic of Benin

Introduction: Gastroesophageal reflux disease (GERD) is a benign and chronic disease that can impair the quality of life. Objective: To study the epidemiological, clinical aspects and factors associated with typical GERD in the general population of Parakou in the Republic of Benin. Methods: This was a descriptive and analytical cross-sectional study with prospective data collection which took place from May 1, 2019 to June 30, 2019. The study population consisted of any subject aged at least 15 years and living in Parakou. Subjects aged at least 15 years, present on the day of the survey and having given their free and informed verbal consent were included. The Short form of QRS®(Reflux-Qual Short form) questionnaire and the “Hospital Anxiety and Depression” score were used to assess the quality of life and anxiety/depression, respectively. The sampling was probabilistic through a 2-stage cluster sampling. The variable of interest was the presence of GERD. Results: Out of 390 subjects surveyed, 28 had symptoms of typical GERD, representing a prevalence of 7.18%. The average age of these subjects was 27.11 ± 7.88 years. Fifteen subjects (53.57%) were male, giving a sex ratio of 1.15. Fourteen (50%) had a secondary level of education, 12 (42.86%) were pupils/students, 18 (64.29%) were single. The monthly income of 22 subjects (78.57%) was lower than the guaranteed interprofessional minimum wage (40,000 FCFA). The average duration of progression of symptoms was 15.89 ± 4.97 months. Symptoms occurred less than twice a week in 22 subjects (78.57%). The clinical manifestations were both diurnal and nocturnal in 12 subjects (42.86%). None of the 28 subjects with GERD had clinical warning signs. Asthma, diabetes, consumption of high fat or spice foods, carbonated drinks, non-steroidal anti-inflammatory drugs, alcoholic beverages and tobacco were statistically associated with the occurrence of GERD. Frequency and Factors Associated with Gender-Based Violence in the Northern Region of Benin from 2016 to 2022

Introduction: Gender-based violence constitutes a public health problem. The objective of this study is to determine the frequency and factors associated with gender-based violence. Study Framework and Method: This was a retrospective, descriptive, cross-sectional study with an analytical aim over the period from January 2016 to December 2022. The study took place in the Integrated Support Center for Victims and Survivors of Gender-Based Violence located in the Departmental University Hospital Center of Borgou and Alibori. The study population consisted of people who were victims of gender-based violence in the northern region of Benin, regardless of their age and gender. Results: The study population consisted of 1198 subjects. The average age of the victims was 21.76 ± 10.02;female victims represented 96.4%, with a sex ratio equal to 0.037 (43/1155). Sexual violence was the most represented (50.25%), followed by psychological (29.30%), physical (24.37%), economic (13.77%) and property (2%). The factors associated with violence were married people and monthly income (physical violence), minors, female gender and single people (sexual violence), polygamous households and monthly income (economic violence), the absence of children burden, married people and monthly income (psychological violence), married people (property violence). Conclusion: Gender-based violence is frequent in the North of Benin and is dominated by sexual violence. The victims are mainly female.

Virological Therapeutic Failure and Associated Factors in People Living with HIV Followed up at the Regional Hospital and University of Borgou (RHU-B) in 2019

Introduction: Antiretroviral treatment has made a considerable reduction in morbidity and mortality in HIV patients, but its effectiveness is compromised by the emergence of cardiovascular and metabolic diseases and also treatment failure. The goal is to study Virological therapeutic failure and the factors associated with HIV patients followed at RUH-B. Methods: This was a cross-sectional, descriptive and analytical study covering the period from 21st July to 21st September 2019. The study enrolled patients 18 years old and above, on antiretroviral treatment for at least 6 months. For the survey, a questionnaire and review of the medical records were used. The variable studied was virological treatment failure. Data analysis was performed using EPI-INFO version 7 and STATA 11 software. The significance level was 5%. Results: A total of 498 patients were included in the study. The mean age was 40 ± 9.90 years. The sex ratio was 0.3. The frequency of treatment failure was 15.3%. The factors associated with the occurrence of treatment failure were: phytotherapy, occurrence of opportunistic infections during follow-up, poor compliance with antiretrovirals, lack of weight gain and anemia during follow-up. Conclusion: This study had shown that treatment failure was frequent in patients. The systematic investigation for failure should be done in all people living with HIV patients on antiretrovirals.

Frequency and Factors Associated with Erectile Dysfunction in People Living with HIV Managed in the Department of Internal Medicine at the Departmental Hospital University of Borgou (DHU-B) in 2017

Introduction: Highly active antiretroviral treatment has considerably reduced the morbidity and mortality associated with HIV infection. However, we are witnessing the emergence of cardiovascular and metabolic diseases. The aim of this work is to study erectile dysfunction (ED) and the associated factors with HIV positive people managed DHU-B. Methods: This was a cross-sectional, descriptive and analytical study covering the period from March 1, 2017 to June 31, 2017. The study involved HIV-positive patients 18 years old and above, male, under antiretroviral treatment and have given their consent. ED was assessed by the IIEF5 score (International Index of Erectile Function). The data collected had been recorded and processed with the EPI INFO software (Version 7.2). The significance level was 5%. Results: A total of 280 patients were included in the study. The average age was 42 ± 10.04 years with extremes of 23 and 67 years. The prevalence of ED was 52.14%. Among patients with ED, 41% had severe ED. Factors associated with ED in multivariate analysis were: age over 40 years, anxiety, depression, arterial hypertension, lipodystrophy, multiple sexual partners and protease inhibitor therapy. Conclusion: ED concerns half of HIV-infected patients followed in the Internal Medicine department. It is therefore essential to provide more comprehensive care, both clinically and psychologically, in order to improve the quality of life of people living with HIV.

Factors Associated with Hypovitaminosis D in People Living with HIV/AIDS (PLWHA) Followed up in Porto-Novo in 2019

Background: Vitamin D deficiency is very common among PLWHA compared to the general population and is promoted by several factors. Aim: To identify the factors associated with hypovitaminosis D in people living with HIV/AIDS (PLWHA) followed up in Porto-Novo in 2019. Methods: This was a cross-sectional, descriptive and analytical study. PLWHA who were monitored on an outpatient basis and who gave their consent were included in this work. Subjects unable to answer questions or under ongoing vitamin D supplementation or suffering from chronic liver diseases were not included. Hypovitaminosis D was defined by a blood level of 25 hydroxy-vitamin D less than 30 ng/mL. Results: A total of 270 PLWHA were included in the study. The mean age of the patients was 39.51 ± 6.85 years with a female predominance (73.3%). The frequency of hypovitaminosis D was 56%. Hypertension, low protein intake, anemia and low glomerular filtration rate were associated with vitamin D deficiency. Conclusion: Vitamin D deficiency is common among PLWHA, which justifies routine screening, especially among those at risk.

Neurological Disorders among the Outpatients Living with Human Immuno Deficiency Virus (HIV) Following at the Parakou Hospital in 2014

Objective: To study the frequency of neurological disorders associated to HIV infection. Methods: This cross-sectional study was carried out from 16th June to 16th September 2014 and included all 289 HIV-outpatients admitted to the Parakou University hospital. All data were recorded, physical examination was performed and the medical sheet of each patient was assessed. The diagnosis of neurological disorder (central and peripheral) was based on clinical features, past medical history, biological test and CT-scan data after multidisciplinary staff. Epi-data and Epi-Info 6.04C were used to perform analysis. Results: They were 73 (25.3%) male with a mean age of 38.0 years +/−?10.1 years. The mean duration of disease was 48.5 months +/−37.1 months. 150 (51.9%) of them were at Stage 3 of the disease. All patients (99.7%) were infected by HIV1, 188 (76.1%) had CD4 cells at the onset of the follow-up less than 350 cells/mm3 and 260 (90.0%) were on ART. Among the 289 patients, 110 had at least one neurological disorder, the frequency of neurological disorders was 38.1%. The main neurological disorders were peripheral neuropathy (34.3%), isolated headache (5.2%), stroke (0.7%), cerebral toxoplasmosis (0.7%) and one case of dementia (0.7%). The main associated factors were the age of patient and exposure on neurotoxic drug, especially ART. Conclusion: The neurological disorders were more frequent among HIV patients with a high frequency of peripheral neuropathy probably due to ART.

Systemic scleroderma is a rare disease in which visceral manifestations occur, particularly peripheral vascular, digestive, cardiopulmonary and renal. It is pathology with a predilection for women. The present clinical case is that of a man with the renal complications of scleroderma and the difficulties of the treatment even in the developed countries like France. In the present case, the management of this disease required a high dose of corticosteroid therapy and extra-renal purification. Early detection of complications through a minimal clinical examination supplemented with paraclinic tests has proved necessary.

Objective: To study the epidemiological, clinical and paraclinical aspects of cirrhosis at Borgou Departmental University hospital Center. Methods: This is a retrospective study for descriptive purpose conducted in the Internal Medicine Department. The study population consists of patients hospitalized in the Internal Medicine Department during the period from 1st January 2009 to 31st December 2016. Results: The frequency of cirrhosis was 1.35%. The sex ratio was 3.76. The average age of patients was 45.22 ± 15.23 years old, with a range from 15 to 82 years. There is a post hepatitis Bcirrhosis predominance in 87.5% of cases, followed by alcoholic cirrhosis in 21.59% of cases. The complications of cirrhosis are dominated by ascites (78.4%) and jaundice (52%). Conclusion: Cirrhosis is a condition that is wide spread. The hepatitis B virusis the main cause of liver cirrhosis followed by alcoholism. Ascites is the most encountered complication. It’s very important now to educate the populations for a behaviour change and to promote vaccination against viral hepatitis.

Frequency of Modifiable Cardiovascular Risk Factors Such as Obesity, Diabetes Mellitus and Hypertension in a Benin Rural Area

The fight against cardiovascular diseases requires the knowledge of their modifiable risk factors. This study aimed to assess the level of modifiable cardiovascular risk factors in a rural area in order to develop a prevention program. Methods: This is an analytical and cross-sectional study on data collected during a fairground medical consultation of adults from 20 to 25 May 2013, in both border districts of the municipality of Djidja (Agouna and Houto). The studied parameters were the capillary blood glucose profile, blood pressure profile, waist circumference (WC) and Body Mass Index (BMI). Diabetes is defined by a fasting blood glucose ≥ 1.26 g/L two times. Hypertension is defined according to the criteria of JNC VII and obesity by BMI ≥ 30 kg/m2;abdominal obesity is defined by a WC above 102 cm in men and 88 cm in women. Data were analyzed using SPSS software version 18.0. Results: A total of 926 people were included among whom 57.8% were women. The average age was 38.43 ± 15.84 years. The prevalence of diabetes mellitus was 2.9%. The prevalence of obesity (BMI ≥ 30 kg/m2) was 8.4%;10.8% of studied population had abdominal obesity and 19.2% had hypertension. Age above 35 years and obesity were significantly associated with hypertension. Risk factors of obesity (IMC ≥ 30 kg/m2) were the place of residence (Agouna), the female gender, age between 35 and 64 and abdominal obesity. Conclusion: The frequency of cardiovascular risk factors, although small compared to the national level, remains a concern in the investigated rural communities. This must lead to undertake a survey on the lifestyle and dietary habits of the inhabitants of these areas.

Lower Limb Arteriopathy in Diabetics at the Oueme-Plateau Provincial Hospital in Porto-Novo: Frequency and Associated Factors

Introduction: The distribution of arteriopathy of the lower limbs (ALL) is not familiar in the city of Porto-Novo. The aim of this study was to determine the prevalence and associated factors of ALL in diabetic patients in treatment at the Oueme-Plateau Provincial Teaching Hospital. Methods: This was a cross-sectional, descriptive, and analytical study from July 12 to October 10, 2021, at the Oueme-Plateau Provincial Teaching Hospital in Porto-Novo. ALL was screened by performing systolic pressure index (SPI) in all diabetic patients aged at least 18 years, seen in consultation or hospitalization during the study period and who had given their consent. Results: The study registered 165 diabetics and 98.2% of them were type 2. Subjects aged between 55 and 65 years were the most represented (33.9%), with an average age of 59.4 years. Women were in the majority (61.2%), with a sex ratio of 0.6. High blood pressure was associated with diabetes in 61.2% of patients. The prevalence of ALL was 48.5% (80 patients). Obliterative arteriopathy of the lower limbs (OALL) was observed in 70 patients (42.4%), mediacalcosis in 8 patients (4.8%), and mixed arteriopathy in 2 patients (1.2%). In 88.5% of patients, the pathology was symptomless. Factors significantly associated with ALL included advanced age (p = 0.01), the age of the glycemic imbalance (p = 0.03), lack of regular physical activity (p = 0.001) and dyslipidemia (p = 0.008). Conclusion: Pelvic limb arteriopathy is usual among diabetics at Oueme-Plateau Provincial Teaching Hospital. The use of SPI, which enables diagnosis, requires simple, low-cost means, and should be encouraged.
