A 67-year-old man with celiac disease developed recurrent diarrhea,profound weakness and weight loss, with evidence of marked protein *** clinical course was refractory to a strict gluten-free diet and steroid *** studies led to definition of unrecognized collagenous sprue that caused ulceration and small intestinal *** PCR showed identical monoclonal T-cell populations in antemortem duodenal biopsies and postmortem jejunum,careful pathological evaluation demonstrated no frank ***,overt or even cryptic T-cell lymphoma may complicate collagenous sprue,however, small intestinal ulcers and perforation may also develop *** dramatic findings here may reflect an underlying or early molecular event in the eventual clinical appearance of overt T-cell lymphoma.

Introduction:Climate change is expected to impose significant tension on the geographic distribution of tree ***,tree species range shifts may be delayed by their long life spans,capacity to withstand long periods of physiological stress,and dispersal *** could theoretically break this biological inertia by killing forest canopies and facilitating species redistribution under changing *** investigated the capacity of wildfire to modulate climate-induced tree redistribution across a montane landscape in the central Rocky Mountains under three climate scenarios(contemporary and two warmer future climates)and three wildfire scenarios(representing historical,suppressed,and future fire regimes).Methods:Distributions of four common tree species were projected over 90 years by pairing a climate niche model with a forest landscape simulation model that simulates species dispersal,establishment,and mortality under alternative disturbance regimes and climate ***:Three species(douglas-fir,lodgepole pine,subalpine fir)declined in abundance over time,due to climate-driven contraction in area suitable for establishment,while one species(ponderosa pine)was unable to exploit climate-driven expansion of area suitable for *** fire frequency accelerated declines in area occupied by douglas-fir,lodgepole pine,and subalpine fir,and it maintained local abundance but not range expansion of ponderosa ***:Wildfire may play a larger role in eliminating these conifer species along trailing edges of their distributions than facilitating establishment along leading edges,in part due to dispersal limitations and interspecific competition,and future populations may increasingly depend on persistence in locations unfavorable for their establishment.

A landslide susceptibility mapping study was performed using dynamic hillslope hydrology. The modified infinite slope stability model that directly includes vadose zone soil moisture(SM) was applied at Cleveland Corral, California, US and Krishnabhir, Dhading, Nepal. The variable infiltration capacity(VIC-3l) model simulated vadose zone soil moisture and the wetness index hydrologic model simulated groundwater(GW). The GW model predictions had a 75% NASH-Sutcliffe efficiency when compared to California's in-situ GW measurements. The model performed best during the wet season. Using predicted GW and VIC-3l vadose zone SM, the developed landslide susceptibility maps showed very good agreement with mapped landslides at each study region. Previous quasi-dynamic model predictions of Nepal's hazardous areas during extreme rainfall events were enhanced to improve the spatial characterization and provide the timing of hazardous conditions.

AIM: To systematically examine the impact of the hepatitis C virus (HCV) diagnosis on patients' level of social support in a large-scale study. METHODS: Patients evaluated and treated for HCV in a tertiary referral center were enrolled in a cross-sectional study. Demographic data, functional and emotional status as measured by the Hospital Anxiety and Depression Scale (HAD) and the Sickness Impact Profile (SIP), severity of liver disease, mode of acquisition, and physical and psychiatric comorbidities were collected from patients or abstracted from the medical record. All participants completed a semi-structured interview, addressing questions of social support. RESUlTS: A total of 342 patients (mean age 45.2 years; 37% women) were enrolled. Ninety-two (27%) patients described lower levels of support by family and friends. Nearly half of the participants (45%) noted the loss of at least one relationship due to the disease. Fears related to transmitting the disease (25%) were common and often associated with ignorance or even discrimination by others (19%). Nearly one fifth of the patients did not share information about their disease with others to avoid being stigmatized. lower levels of social support were significantly associated with living alone, being unemployed, being excluded from antiviral therapy, having psychiatric comorbidities, contracting HCV through intravenous drug use, having high levels of anxiety and depression as measured by the HAD and negative mood state as measured by the SIP. Patients reporting lower levels of social support also noted more physical symptoms as measured by the SIP. CONClUSION: Patients with hepatitis C often face significant social problems, ranging from social isolation to familial stress. The most common concerns reflect a limited insight of patients and their relatives and friends about the disease, the risk factors for its spread, and about potential consequences. Osteogenesis imperfecta（OI） comprises a group of heritable connective tissue disorders generally defined by recurrent fractures, low bone mass, short stature and skeletal fragility. Beyond the skeletal complications of OI,many patients also report intolerance to physical activity, fatigue and muscle weakness. Indeed, recent studies have demonstrated that skeletal muscle is also negatively affected by OI, both directly and indirectly. Given the well-established interdependence of bone and skeletal muscle in both physiology and pathophysiology and the observations of skeletal muscle pathology in patients with OI, we investigated the therapeutic potential of simultaneous anabolic targeting of both bone and skeletal muscle using a soluble activin receptor 2B（ACVR2B） in a mouse model of type Ⅲ OI（oim）. Treatment of 12-week-old oim mice with ACVR2 B for 4 weeks resulted in significant increases in both bone and muscle that were similar to those observed in healthy,wild-type littermates. This proof of concept study provides encouraging evidence for a holistic approach to treating the deleterious consequences of OI in the musculoskeletal system.

Distance caregivers(DCGs) are a growing phenomenon in the United States Family members are struggling to provide care to loved ones with chronic illnesses such as cancer,from a *** local caregiving research,distance caregiving research is limited and inconsistent definitions of distance make it difficult to compare *** date,DCGs have not been afforded the opportunities for educational and emotional support that local caregivers have received from the health care *** they are not usually present at medical appointments,DCGs do not receive first-hand information from the health care team about the patient's condition,disease progression,and/or treatment *** caregivers report feeling left out of important family *** experience anxiety related to the uncertainty of the family members' well-being and guilt related to not being available to help local caregivers *** challenges of distance caregiving are especially evident when the distance caregiver has a parent with advanced ***-centered care,attending to the needs of the whole family regardless of their geographic location is critical for quality cancer *** this manuscript,the sparse literature on distance caregiving is *** for future research and for the development of creative technologically advanced interventions for this underserved caregiving population are suggested.

The microbiomes associated with bee nests influence colony health through various mechanisms,although it is not yet clear how honeybee congeners differ in microbiome assembly processes,in particular the degrees to which floral visitations and the environment contribute to different aspects of *** used DNA metabarcoding to sequence bacterial 16S rRNA from honey and stored pollen from nests of 4 honeybee species(Apis cerana,***,***,and ***)sampled throughout Yunnan,China,a global biodiversity *** developed a computational pipeline integrating multiple databases for quantifying key facets of diversity,including compositional,taxonomic,phylogenetic,and functional ***,we assessed candidate drivers of observed microbiome dissimilarity,particularly differences in floral visitations,habitat disturbance,and other key environmental *** revealed that microbiome alpha diversity was broadly equivalent across the study sites and between bee species,apart from functional diversity which was very low in nests of the reclusive *** in microbiome composition across Yunnan was driven predominantly by pollen *** disturbance negatively impacted both compositional and phylogenetic alpha diversity of nest microbiomes,but did not correlate with microbial *** herein make progress in understanding microbiome diversity associated with key pollinators in a biodiversity hotspot,and provide a model for the use of a comprehensive informatics framework in assessing pattern and drivers of diversity,which enables the inclusion of explanatory variables both subtly and fundamentally different and enables elucidation of emergent or unexpected drivers.
