The unbalanced and inadequate use of fertilizers is one of the causes of soil degradation. Combined with the ever-increasing population, it is necessary to find sustainable agricultural production alternatives. The present work aims to determine the effect of different rates and mixtutes of organic amendments on soil fertility and the performance of Sunflower (Helianthus annuus L.). In the field, treatments consisted of solid household waste and faecal sludge in the ratios of 3/5 (V1), and a mixture of faecal sludge and household waste in the ratio of 3/5 with 900 worms (V2). At the end of the composting process, V1, V2 composts and the poultry manure (PM) were applied at rates of 4, 5 and 6 t∙ha−1 in a randomized complete block design with three replications. Soil samples were collected before and after the experiment and analyzed. The main results revealed that at the end of the composting process, there was a progressive improvement in the physico-chemical properties of V1 and V2 composts. In particular, the C/N ratio, phosphorus (P) and total nitrogen (TN) initially at 16.49 ± 0.42 (V1, V2), 21.06 ± 0.07 mg∙kg−1 (V1, V2), 0.76% ± 0.08% (V1, V2) respectively, increased after 60 days to 12.40 ± 0.41 (V1), 9.74 ± 0.28 (V2) for C/N, 21.94 ± 0.63 mg∙kg−1 (V1) and 22.04 ± 0.04 mg∙kg−1 (V2) for P, 0.96% ± 0.0% (V1) and 1.22 ± 0.04 (V2) for TN. The application of 6 t∙ha−1of PM had the greatest influence on the diameter and weight of the flower heads (27.16 ± 4.01 t∙ha−1 and 230.83 ± 2.64 t∙ha−1), while 4 t∙ha−1 of V2 gave the tallest sunflower plants (110.07 ± 73.28 cm) as well as the diameter at the crown (19.30 ± 9.07 cm). However, CEC was most influenced by 4 t∙ha−1 of V1, while 4 t∙ha−1 of PM had the greatest effect on organic carbon and phosphorus. A New Look at Building Facades as Infrastructure

Like the hard surfaces of streets and sidewalks in an urban environment, the vertical and horizontal surface area on the outside of urban buildings contributes to the constant heating of large cities around the world. However, little is done to design this surface to benefit the public sphere. Instead, the facade of a building performs either as a component that focuses only on the quality of comfort for interior occupants, while ignoring effects on the exterior of the building, or as an identifiable aesthetic for the building＇s owners, This essay proposes the rethinking of the building facade as a steward of outdoor pedestrian welfare, and the conception of public health as an added function of the building envelope- a concept that may fall into the jurisdiction of public works. If the huge total surface area of a city＇s buildings is thought of as part of the city＇s infrastructure, then its public contribution may not only make outdoor areas comfortable, clean, and enjoyable, but also help to alleviate the bigger problem of rising temperatures in cities.

Abnormal Umbilical Doppler: Incidence and Neonatal Outcomes among Hypertensive Disorders in Pregnancy at KCMC: A Hospital-Based Prospective Cohort Study

Hypertensive disorders of pregnancy (HDP) are the most common maternal and perinatal health challenges. Globally, the incidence of HDP increased from 16.30 million to 18.08 million, with a total increase of about 10.9% from 1990 to 2019. Umbilical Doppler study in hypertensive disorders of pregnancy helps to predict neonatal outcomes and prevent neonatal and maternal morbidity and mortality. Objective: This study aims to determine the incidence of abnormal umbilical Doppler among hypertensive pregnant women, to identify the adverse neonatal outcomes associated with abnormal umbilical Doppler, and also to detect the diagnostic predictive values of umbilical Doppler to neonatal outcomes at KCMC. Material and methods: A hospital-based prospective cohort study included women with hypertensive disorders of pregnancy from the gestational age of 28 weeks and above, followed up to delivery during the study period from August 2022 to March 2023. Multivariate logistic regression analysis was used to determine the association between AUD and neonatal outcomes. Results: Out of 112 women with HDP, the incidence of abnormal umbilical Doppler was 38 (33.93%). Abnormal umbilical Doppler was associated with neonates with low birth weight aOR (95% of CI) of 4.52 (1.59 - 12.83) p = 0.005 and neonatal ICU admission 9.71 (2.90 - 32.43) p Conclusion: The incidence of abnormal umbilical Doppler is high in hypertensive disorders of pregnancy which is associated with an increase in neonatal low birth weight and neonatal ICU admissions, the sensitivity of abnormal umbilical Doppler in prediction of low birth weight and neonatal ICU admission is significant hence the routine use of umbilical Doppler assessment among hypertensive pregnant women is crucial.

Effect of fish oil enriched enteral diet on inflammatory bowel disease tissues in organ culture: Differential effects on ulcerative colitis and Crohn's disease

AIM： To investigate the influence of fish oil enriched enteral diet on intestinal tissues taken from Crohn＇s disease （CD）, ulcerative colitis （UC） and non-inflamed non-IBD control patients in vitro. METHODS： Colonoscopic biopsies from patients with active CD （n = 4）, active UC （n = 7）, and non-inflamed non-IBD control patients （n = 4） were incubated （three dilutions of 1：20, 1：10, and 1：5） with Waymouth＇s culture medium and enteral elemental diet （EO28, SHS, Liverpool, UK） modified in the fatty acid composition with fish oil （EF） in an organ culture system for 24 h. In each experimental set-up, incubation with Waymouth＇s medium alone as control was included. Tissue viability was assessed by adding bromodeoxyuridine （BrdU） to the culture fluid and immunohistochemically staining for BrdU uptake. Cytokine ratio of IL-1ra/IL-1β （low ratio indicative of inflammation） and production of those cytokines as a percentage of medium control were assayed in the culture supernatant. RESULTS： Incubation of CD-affected tissue with EF （1：20, 1：10, and 1：5） modestly and non-significantly increased IL-1ra/IL-1β ratio as compared with medium control （CD 39.1±16.1; 26.5±7.8, 47.1±16.8 vs control 13.0±2.2）, but incubation of UC-affected tissues increased IL-1ra/IL-1β ratio significantly in all three dilutions （UC 69.1±32.2, P〈0.05; 76.1±36.4, P = 0.05; 84.5±37.3, P〈0.02; vs control 10.2±3.7）. Incubation of non-inflamed non-IBD control tissue did not increase the IL-1ra/IL-1β ratio in any dilution compared to medium control （69.3±47.0, 54.1±30.6, 79.4±34.0 vs control 76.1±37.3）. Average percentage production of IL-1β indexed against medium control was significantly less in UC after EF incubation as compared with CD （UC 24.0±4.8 vs CD 51.8±8.1; P〈0.05）. Average percentage production of IL-tra was markedly higher in UC （135.9±3.4） than that in control patients （36.5±4.3）（p〈0.0001）. Symptom clusters and quality of life among patients with advanced heart failure

Objectives To identify symptom clusters among patients with advanced heart failure （HF） and the independent relationships with their quality of life （QoL）. Methods This is the secondary data analysis of a cross-sectional study which interviewed 119 patients with advanced HF in the geriatric unit of a regional hospital in Hong Kong. The symptom profile and QoL were assessed by using the Edmonton Symptom Assessment Scale （ESAS） and the McGill QoL Questionnaire. Exploratory factor analysis was used to identify the symptom clusters. Hier- archical regression analysis was used to examine the independent relationships with their QoL, after adjusting the effects of age, gender, and comorbidities. Results The patients were at an advanced age （82.9± 6.5 years）. Three distinct symptom clusters were identified： they were the distress cluster （including shortness of breath, anxiety, and depression）, the decondition cluster （fatigue, drowsiness, nausea, and reduced appetite）, and the discomfort cluster （pain, and sense of generalized discomfort）. These three symptom clusters accounted for 63.25% of variance of the patients＇ symptom experience. The small to moderate correlations between these symptom clusters indicated that they were rather independent of one another. After adjusting the age, gender and comorbidities, the distress （β = -0.635, P 〈 0.001）, the decondition （β = -0.148, P = 0.01）, and the discomfort （β = -0.258, P 〈 0.001） symptom clusters independently predicted their QoL. Conclusions This study identified the distinctive symptom clusters among patients with advanced HF. The results shed light on the need to develop palliative care interventions for optimizing the symptom control for this life-limiting disease.
