Background: Postpartum hemorrhage (PPH) is the major contributor to maternal mortality and morbidity worldwide as well as in Tanzania. Studies have shown Pre-eclampsia as a risk indicator for Postpartum hemorrhage and d-dimer tends to rise in women with pre-eclampsia. Few studies that have shown the association between d-dimer and PPH have been controversial and differ according to ethnicity and lifestyle. Hence there is no suitable reference interval for d-dimer in predicting Postpartum hemorrhage among women with pre-eclampsia. Broad Objective: This study aimed to assess the association, sensitivity, and specificity of d-dimer as a laboratory predictor of postpartum hemorrhage among women with pre-eclampsia at KCMC hospital. Methodology: This was a hospital-based analytical cross-sectional study conducted at KCMC Hospital in Northern Tanzania from September 2022 to March 2023. A total of 195 women with pre-eclampsia were included in this study. Plasma d-dimer levels were taken from women with pre-eclampsia pre-delivery. Haematocrit was compared before and after delivery, and a fall of 10% was considered as Postpartum hemorrhage together with clinical assessment of the patient. Participants were divided among those who had severe features and those who did not have severe features and were further categorized into those who had PPH and those who did not have PPH. Logistic regression was used to determine the association between d-dimer and PPH adjusting for other factors. The Receiver Operating Curve (ROC) was used to evaluate the predictive value. Results: Higher median d-dimer levels were seen among women who had PPH compared to those who had no PPH. d-dimer was seen to be associated with PPH, thus for every unit increase of µg/ml of d-dimer among women who had pre-eclampsia without severe features there was a 14% significant increase in the odds of having postpartum hemorrhage and a 45% significant increase of having postpartum hemorrhage among those who had pre

Quality of life and palliative care needs of elderly patients with advanced heart failure

Objective To examine the quality of life and palliative care needs of elderly patients with advanced heart failure （HF）. Methods This was a correlation descriptive study conducted at a 650-bed sub-acute hospital. Patients who were aged 65 or over, diagnosed with HF of New York Heart Association Class III or IV symptoms, and mentally sound were eligible to the study. The Edmonton Symptom Assessment Scale, the overall quality of life single item scale, and the McQill Quality of Life Questionnaire （MQoL）, were used for measurement. Multi- ple regression analysis was performed to determine factors for predicting quality of life. Results A convenience sample of 112 patients was recruited. Their age was 81.5 ± 8.5 years. The three most distressing symptoms reported by the patients were tiredness （5.96 ± 2.78）, drowsiness （5.47± 2.93）, and shortness of breath （5.34 ± 2.96）. Their mean overall quality of life single item scale score was 4.72 ± 2.06 out of 10. The mean MQoL physical subscale score was the lowest （4.20 ± 1.767）, whereas their mean psychological subscale was the highest （7.14 ±2.39）. However, in a multivariate analysis model, quality of life was significantly associated with existential wellbeing, physical wellbeing, psychological wellbeing and educational level. Conclusions The findings highlight that spiritual concerns are significant palliative care needs among elderly patients with advanced HF, in addition to symptom management. This is in line with the argument that palliative care that places great emphasis on holistic care should be integrated to the care of this group of patients.

Results:during January 2009 to July 2009,528 phlebotomine specimens were collected including 329 males(62.3%) and 199 females(37.7%),approximately 238(45%) of them were captured from indoor and 290(55%) from outdoor resting *** species belonging to the genera Phlebotomus(8%) and Sergentomyia(92%) were ***:Public health measures such as case detection and treatment,the control of sandflies,the conjunction elimination of infected stray dogs and health education can be effective in controlling the disease.

The objective of this study was to develop an adaptive thermal comfort equation for naturally ventilated buildings in hot-humid climates. The study employed statistical meta-analysis of the American Society of Heating, Refrigerating and Air-Conditioning Engineers （ASHRAE） RP-884 database, which covered several climatic zones. The data were carefully sorted into three climate groups including hot-humid, hot-dry, and moderate and were analyzed separately. The results revealed that the adaptive equations for hot-humid and hot-dry climates were analogous with approximate regression coefficients of 0.6, which were nearly twice those of ASHRAE and European standards 55 and EN15251, respectively. The equation using the daily mean outdoor air temperature had the highest coefficient of determination for hot-humid climate, compared with other mean temperatures that considered acclimatization of previous days. Acceptable comfort ranges showed asymmetry and leaned toward operative temperatures below thermal neutrality for all climates. In the hot-humid climate, a lower comfort limit was not observed for naturally ventilated buitdings, and the adaptive equation was influenced by indoor air speed rather than indoor relative humidity. The new equation developed in this study can be applied to tropical climates and hot humid summer seasons of temperate climates.

AIM： TO investigate colonic endocrine cells in lympho- cytic colitis （LC） patients. METHOdS： Fifty-seven patients with LC were in- cluded. These patients were 41 females and 16 males, with an average age of 49 years （range 19-84 years）. Twenty-seven subjects that underwent colonoscopy with biopsies were used as controls. These subjects underwent colonoscopy because of gastrointestinal bleeding or health worries, where the source of bleed- ing was identified as haemorrhoids or angiodysplasia. They were 19 females and 8 males with an average age of 49 years （range 18-67 years）. Biopsies from the right and left colon were obtained from both patients and controls during colonoscopy. Biopsies were fixed in 4% buffered paraformaldehyde, embedded in paraffin and cut into 5 μm-thick sections. The sections immunostained by the avidin-biotin-complex method for se- rotonin, peptide YY （PYY）, pancreatic polypeptide （PP） enteroglucagon and somatostatin cells. The cell densi- ties were quantified by computerised image analysis using Olympus software. RESULTS： The colon of both the patient and the control subjects were macroscopically normal. Histo- pathological examination of colon biopsies from con- trols revealed normal histology. All patients fulfilled the diagnosis criteria required for of LC： an increase in intraepithelial lymphocytes （〉 20 lymphocytes/100 epithelial cells） and surface epithelial damage with increased lamina propria plasma cells and absent or minimal crypt architectural distribution. In the colon of both patients and control subjects, serotonin-, PYY-, PP-, enteroglucagon- and somatostatin-immunoreac- tive cells were primarily located in the upper part of the crypts of Lieberk0hn. These cells were basket- or flask-shaped. There was no statistically significant dif- ference between the right and left colon in controls with regards to the densities of serotonin- and PYY- immunoreactive cells （P = 0.9 and 0.1, respectively）. Prospective case series on trabecular-iris angle status after an acute episode of phacomorphic angle closure

AIM:To investigate the trabecular-iris angle with ultrasound biomicroscopy (UBM) post cataract extraction after an acute attack of phacomorphic angle closure. ·METHOdS:This prospective study involved 10 cases of phacomorphic angle closure that underwent cataract extraction and intraocular lens insertion after intraocular pressure (IOP) lowering. Apart from visual acuity and IOP, the trabecular-iris angle was measured by gonioscopy and UBM at 3 months post attack. ·RESULTS:In 10 consecutive cases of acute phaco-morphic angle closure from december 2009 to december 2010, gonioscopic findings showed peripheral anterior synechiae (PAS) ≤90° in 30% of phacomorphic patients and a mean Shaffer grading of (3.1±1.0). UBM showed a mean angle of (37.1°±4.5°) in the phacomorphic eye with the temporal quadrant being the most opened and (37.1°±8.0°) in the contralateral uninvolved eye. The mean time from consultation to cataract extraction was (1.4 ± 0.7) days and the mean total duration of phacomorphic angle closure was (3.6 ±2.8) days but there was no correlation to the degree of angle closure on UBM (Spearman correlation P =0.7). The presenting mean IOP was (50.5±7.4) mmHg and the mean IOP at 3 months was (10.5±3.4) mmHg but there were no correlations with the degree of angle closure (Spearman correlations P =0.9). CONCLUSION:Anopentrabecular-irisangleandnormal IOP can be achieved after an acute attack of pha-comorphic angle closure if cataract extraction is performed within 1 day-2 days after IOP control. Gonioscopic findings were in agreement with UBM,which provided a more specific and object angle measurement. The superior angle is relatively more narrowed compared to the other quadrants. All contralateral eyes in this series had open angles.
