Intranasal ketamine as an analgesic agent for acute pain management in emergency department: A literature review

Ketamine is a well-known dissociative anesthetic agent, and has been used over 50 years. Intranasal pathway is a mucosal way for absorbing agents to directly affect in brain via olfactory sheets, bypassing first pass metabolism and the blood brain barrier. The current uses of intranasal ketamine as an analgesic agent for acute pain management in emergency department are discussed in this review article. Using 'ketamine', 'pain or analgesia', and'intranasal' as keywords, a search of google scholar, Pubmed, web of science, and Medline database from 1970 until 2017 was performed. Finally, from 1204 papers extracted via primary search, 1088 papers were omitted and finally 10 studies were considered for further assessment. There were four observational studies, one case series and report and 5 clinical trials. Ketamine was used for acute pain control due to musculoskeletal trauma, burns, and painful procedures. A total of 390 cases were included in these studies. The studies used ketamine with doses ranging 0.45-1.25 mg/kg via intranasal pathway. Intranasal ketamine provides relatively rapid, well tolerated, and clinically significant analgesia for emergency department patients. Considering the lack of adequate studies and undetermined intranasal dose, it is better to conduct further high quality investigation in both adults and pediatrics.

Objective:Only a few numbers of studies have been published on the use of abobotulinumtoxinA(Dysport®)in idiopathic detrusor overactivity(IDO).This study reported the long-term follow-up of women with IDO who were treated with intravesical Dysport®injections.Methods:Two hundred and thirty-six patients with IDO who had failed first-line conservative and antimuscarinic therapy received 500-900 units of Dysport®between April 2014 and July 2015.All patients were followed up for 5 years after their initial injection and interviewed on the phone.Results:A total of 236 women with IDO aged from 18 years to 84 years(mean±standard deviation:49.6±15.9 years)were included in our study.The median follow-up time for patients was 36.5(range:10-70)months,and the median recovery time after injection was 18.5(range:0-70)months.A total of 83(35.2%)patients stated that they had subjective improvement of their symptoms whereas 84(35.6%)patients did not report any improvement in symptoms.The initial International Consultation on Incontinence Questionnaire Overactive Bladder mean score was 6.9(standard deviation 3.4).There was a positive association between the median recovery time and the components of the International Consultation on Incontinence Questionnaire Overactive Bladder questionnaire.Conclusion:In a sub-population of overactive bladder patients with IDO who have failed first-line therapy,a single intravesical Dysport®injection can resolve patient symptoms completely or reduce the symptoms to an acceptable level that can be controlled with antimuscarinics or re-injection on demands.

Precipitation Behaviour at the Interface of an Additively Manufactured M789-N709 Hybrid Alloy

A novel hybrid alloy, designed for plastic injection dies, was fabricated by depositing M789 steel on wrought N709 steel through laser powder bed fusion (LPBF). The M789-N709 interface (with a thickness of about 300 μm) is characterized by the variation of titanium content from 0 wt% in the N709 section to about 1 wt% near the M789 region. A thermodynamic and kinetic simulation after aging treatment was performed to reveal the precipitation behavior in the M789-N709 interface. Different sizes and amounts of ETA-Ni3(Ti,Al) strengthening precipitates exist, with a maximum volume fraction and radius of about 5% and 5 nm, respectively (near the M789 region). Moreover, the volume fraction of precipitates significantly increases with Ti additions of up to 0.6 wt% (i.e., 100 μm from the N709 region);however, beyond this threshold, the increase in the amount of precipitation becomes gradual. The thermodynamic and kinetic simulations performed in the interface are validated by electron diffraction spectroscopy (EDS), electron probe microanalyzer (EPMA) and atomic probe tomography (APT) results;the measured precipitate size from the APT analysis is consistent with the simulation results. Two morphologies of precipitates were detected: elongated and spherical;both contribute to the robustness of the interface. The robust M789-N709 interface formed during the process shows good compatibility between the alloys, which is critical for extended tool life. These new insights about the precipitation at the interface of M789-N709 alloy are crucial in assessing the feasibility of N709 as a cost-effective base material to minimize the use of M789 when printing plastic injection dies.

A Novel Modeling, Simulation and Optimization Approach of Crude Oil Cold Stripping Process

Cold stripping is the most common process for crude oil sweetening in oilfields particularly at offshore installations because of its low price and relatively easy operating conditions in comparison with other sweetening processes. In this paper the cold stripping process in tray column has been modeled mathematically in static and dynamic modes, and solved with the MATLAB software. This process has been used in the existing treatment facilities of an offshore oil production complex for verifying the model results. With the help of HYSYS software the effective parameters on the process have been discussed and the optimized conditions finalized after some plant modifications for improving the performance of stripper columns have been proposed.

Some Geometric Properties of the m-Möbius Transformations

Möbius transformations, which are one-to-one mappings of onto have remarkable geometric properties susceptible to be visualized by drawing pictures. Not the same thing can be said about m-Möbius transformations fm mapping onto . Even for the simplest entity, the pre-image by fm of a unique point, there is no way of visualization. Pre-images by fm of figures from C are like ghost figures in Cm. This paper is about handling those ghost figures. We succeeded in doing it and proving theorems about them by using their projections onto the coordinate planes. The most important achievement is the proof in that context of a theorem similar to the symmetry principle for Möbius transformations. It is like saying that the images by m-Möbius transformations of symmetric ghost points with respect to ghost circles are symmetric points with respect to the image circles. Vectors in Cm are well known and vector calculus in Cm is familiar, yet the pre-image by fm of a vector from C is a different entity which materializes by projections into vectors in the coordinate planes. In this paper, we study the interface between those entities and the vectors in Cm. Finally, we have shown that the uniqueness theorem for Möbius transformations and the property of preserving the cross-ratio of four points by those transformations translate into similar theorems for m-Möbius transformations.

Predictors of death in hospitalized elderly patients with COVID-19 in Mashhad, Iran, in 2021: A historical cohort study

Objective:The present study aimed to identify predictive factors for mortality among elderly individuals infected with COVID-19.Methods and Materials:This historical cohort study was conducted from July to December 2021 in the specialized departments for COVID-19 patients at one of the hospitals in Mashhad,Iran.Data were collected from the medical records of 404 elderly patients.Sampling was conducted using the convenience sampling method.Data were gathered through a demographic and clinical checklist developed by the researcher.Univariate and multivariate Cox regression were used to analyze the data.Results:The mortality rate among elderly individuals was 25%(n=101).Multiple regression analysis revealed significant associations between mortality and age(hazard ratio[HR]=0.58,95%confidence interval[CI]:0.38,0.88;P=0.011),level of consciousness(HR=0.31,95%CI:0.19,0.50;P<0.001),and SpO2(HR=0.58,95%CI:0.37,0.92;P=0.022).The probability of survival after the 19th day of hospitalization was 50%.Conclusions:Determining predictors of death allows for early identification of elderly individuals at risk and enables the health-care team to provide more effective care,ultimately saving the lives of elderly individuals by allocating appropriate facilities and equipment.

Combining artificial neural network and multi-objective optimization to reduce a heavy-duty diesel engine emissions and fuel consumption

Nondominated sorting genetic algorithm Ⅱ(NSGA-Ⅱ) is well known for engine optimization problem. Artificial neural networks(ANNs) followed by multi-objective optimization including a NSGA-Ⅱ and strength pareto evolutionary algorithm(SPEA2) were used to optimize the operating parameters of a compression ignition(CI) heavy-duty diesel engine. First, a multi-layer perception(MLP) network was used for the ANN modeling and the back propagation algorithm was utilized as training algorithm. Then, two different multi-objective evolutionary algorithms were implemented to determine the optimal engine parameters. The objective of the present study is to decide which algorithm is preferable in terms of performance in engine emission and fuel consumption optimization problem.

Multipliers and Classification of m-Möbius Transformations

It is known that any m-Möbius transformation is an ordinary Möbius transformation in every one of its variables when the other variables do not take the values a and 1/a, where a is a parameter defining the respective m-Möbius transformation. For ordinary Möbius transformations having distinct fixed points, the multiplier associated with one of these points completely characterizes the nature of that transformation, i.e. it tells us if it is elliptic, hyperbolic or loxodromic. The purpose of this paper is to show that fixed points exist also for m-Möbius transformations and multipliers associated with them can be computed as well. As in the classical case, the values of those multipliers describe completely the nature of the transformations. The method we used was that of a thorough study of the coefficients of the variables involved, with which occasion we discovered surprising symmetries. These were the results allowing us to prove the main theorem regarding the fixed points of a m-Möbius transformation, which is the key to further developments. Finally we were able to illustrate the geometric aspects of these transformations, making the whole theory as intuitive as possible. It was as opening a window into a space of several complex variables. This allows us to prove that if a bi-Möbius transformation is elliptic or hyperbolic in z1 at a point z2 it will remain the same on a circle or line passing through z2. This property remains true when we switch z1 and z2. The main theorem, dealing with the fixed points of an arbitrary m-Möbius transformation made possible the extension of this result to these transformations.

Combining flamelet-generated manifold and machine learning models in simulation of a non-premixed diffusion flame

Flamelet Generated Manifold(FGM)is an example of a chemistry tabulation or a flamelet method that is under attention because of its accuracy and speed in predicting combustion characteristics.However,the main problem in applying the model is a large amount of memory required.One way to solve this problem is to apply machine learning(ML)to replace the stored tabulated data.Four different machine learning methods,including two Artificial Neural Networks(ANNs),a Random Forest(RF),and a Gradient Boosted Trees(GBT),are trained,validated,and compared in terms of various performance measures.The progress variable source term and transport properties are replaced with the ML models.Particular attention was paid to the progress variable source term due to its high gradient and wide range of its value in the control variables space.Data preprocessing is shown to play an essential role in improving the performance of the models.Two ensemble models,namely RF and GBT,exhibit high training efficiency and acceptable accuracy.On the other hand,the ANN models have lower training errors and take longer to train.The four models are then combined with a one-dimensional combustion code to simulate a counterflow non-premixed diffusion flame in engine-relevant conditions.The predictions of the ML-FGM models are compared with detailed chemical simulations and the original FGM model for key combustion properties and representative species profiles.

Knowledge, attitude and use of complementary and integrative health strategies: a preliminary survey of Iranian nurses

BACKGROUND： Disagreement exists regarding the need for knowledge about complementary and integrative health（CIH） strategies, as well as for the need to consider such strategies in clinical nursing practice. OBJECTIVE： This study was conducted to assess the knowledge, attitude and use of CIH strategies among nurses in Iran. DESIGN, SETTING, PARTICIPANTS AND INTERVENTIONS： A cross-sectional study of nurses working in two hospitals of Zabol University of Medical Sciences, in southeast Iran, was conducted from October 2014 to April 2015. The questionnaire, developed specifically for this research, was used to assess the knowledge, attitude and use of CIH by nurses. Descriptive and inferential statistics were used to interpret the survey responses.RESULTS： Most nurses（n=95, 60.5%） have average knowledge about CIH strategies with most holding a positive attitude about use（n=81, 51.6%）. The majority（n=90, 57.3%） of nurses, however, never applied CIH methods. Where CIH was used, massage was most often clinically applied（n=129, 82.2%） and a large percentage believed it useful for treating illness（n=136, 87.9%）. Other CIH methods commonly used included prayer and herbal medicine.CONCLUSION： Nurses have positive attitudes about CIH though knowledge was typically weak. Educational programs should carefully consider how knowledge about CIH methods could be strengthened within curricula.
