Nasopharyngeal carcinoma (NPC) is among the most common malignancies in southern *** of genomic DNA,which occurs during the complex pathogenesis process for NPC,represents a pivotal mechanism in the inactivation of tumor suppressor genes (TSGs).In many circumstances,loss of TSGs can be detected as diagnostic and prognostic markers in *** short arm of chromosome 3 (3p) is a frequently deleted chromosomal region in NPC,with 3p21.1-21.2 and 3p25.2-26.1 being the most frequently deleted minimal *** recent years,our research group and others have focused on the identification and characterization of novel target TSGs at 3p,such as RASSF1A,BLU,RBMS3,and CHL1,in the development and progression of *** this review,we summarize recent findings of TSGs at 3p and discuss some of these genes in detail.A better understanding of TSGs at 3p will significantly improve our understanding of NPC pathogenesis,diagnosis,and treatment.

＜正＞ The adoption of the 1997 stage classification of AJC for nasopharyngeal carcinoma（NPC）has allowed not only betterprediction of prognosis,but also allowed grouping of patients with different patterns of failure after treatment. Chua et al had analyzed the different stage sub—groups and the pattern of *** information,together with theadvances in radiotherapy and combination chemotherapy,allowed the adoption of different strategies to deal with the differentpatterns of failure.

Comparative genomic hybridization analysis of genetic aberrations associated with development of esophageal squamous cell carcinoma in Henan, China

AIM: To characterize cytogenetic alterations in esophageal squamous cell carcinoma (ESCC) and its metastasis. METHODS: A total of 37 cases of primary ESCC and 15 pairs of primary ESCC tumors and their matched metastatic lymph nodes cases were enrolled from Linzhou, the high incidence area for ESCC in Henan, northern China. The comparative genomic hybridization (CGH) was applied to determine the chromosomal aberrations on the DNA extracted from the frozen ESCC and metastatic lymph node samples from these patients. RESULTS: CGH showed chromosomal aberrations in all the cases. In 37 cases of primary ESCC, chromosomal profile of DNA copy number was characterized by frequently detected gains at 8q (29/37, 78%), 3q (24/37, 65%), 5p (19/37, 51%); and frequently detected losses at 3p (21/37, 57%), 8p and 9q (14/37, 38%). In 15 pairs of primary ESCC tumors and their matched metastatic lymph node cases, the majority of the chromosomal aberrations in both primary tumor and metastatic lymph node lesions were consistent with the primary ESCC cases, but new candidate regions of interest were also detected. The most significant finding is the gains of chromosome 6p with a minimum high-level amplification region at 6p12-6q12 in 7 metastatic lymph nodes butonly in 2 corresponding primary tumors (P = 0.05) and 20p with a minimum high-level amplification region at 20p12 in 11 metastatic lymph nodes but only in 5 corresponding primary tumors (P < 0.05). Another interesting finding is the loss of chromosome 10p and 10q in 8 and 7 metastatic lymph nodes but only in 2 corresponding primary tumors (P < 0.05). CONCLUSION: Using the CGH technique to detect chromosomal aberrations in both the primary tumor and its metastatic lymph nodes of ESCC, gains of 8q, 3q and 5p and loss of 3p, 8p, 9q and 13q were specifically implicated in ESCC in Linzhou population. Intraperitoneal Chemotherapy for Gastric Cancer with Peritoneal Carcinomatosis: Is HIPEC the Only Answer?

Gastric cancer with peritoneal carcinomatosis is notorious for its dismal prognosis. While the pathophysiology of peritoneal dissemination is still controversial, the rapid downhill course is universal. Patients usually suffer abdominal distension, intestinal obstruction and various complications before they succumb after a median of 3 - 6 months. Although not adopted in most international treatment guidelines, intraperitoneal chemotherapy has growing evidence compared with conventional systemic chemotherapy for the treatment of peritoneal carcinomatosis. Cytoreductive surgery with hyperthermic intraperitoneal chemotherapy is well-established for clinical benefit but is technically demanding with substantial treatment-related morbidities and mortality. On the other hand, normothermic intraperitoneal chemotherapy in the form of bidirectional neoadjuvant treatment is promising with various newer chemotherapeutic agents. Regardless of the treatment technique applied, the essential element of success is meticulous patient selection and availability of expertise. Future direction is along the line of personalized treatment with the application of translational science.

Delineation of Biological Tumor Volume from Positron Emission Tomography Images in Nasopharyngeal Carcinoma

Objective: To develop an auto-segmentation method for delineating the biological tumor volume of nasopharyngeal carcinoma from positron emission tomography images. Methods: A phantom consisting of a water tank with fixed background fluorodeoxyglucose [18F-FDG] activity and spheres with diameters ranging from 2.1 to 5 cm with varying activities of FDG were used to simulate tumors of different sizes and FDG uptake. The phantom was scanned with a PET/CT scan at different sphere to background intensity ratios. An optimum fixed percentage threshold (FT) approach and a signal-to-background ratio (SBR) approach were developed to estimate the true size of the spheres from the PET images. Both approaches were further evaluated in patient images for validation. Twenty-two patients with NPC from stage T1 to T4 were included. The PET based biological tumor volumes (BTV) were delineated with both FT (BTVFT) and SBR (BTVSBR) approaches and compared with the gross tumor volume localized from MRI (GTVMR). The mean volumes of BTVFT and BTVSBR were compared and the degree of overlap between GTVMR and both BTVs was evaluated. Paired t-tests were used for statistical analysis. Results: The optimal FT value was 36.5% of maximal intensity, and SBR approach was represented by an inverse linear regression model. The estimated volume of spheres segmented by both approaches shows no significant difference from the true volume of spheres (p > 0.05), but the average absolute errors were smaller from SBR approach than FT approach (p = 0.008). GTVMR was larger than both BTVFT (p = 0.003) and BTVSBR (p SBR with GTVMR is significantly larger than with BTVFT (0.52 and 0.42 respectively, p MR for radiotherapy planning purpose.
