Retrograde intrarenal surgery for lower pole stones utilizing stone displacement technique yields excellent results

Objective:To evaluate the long-term stone-free rate(SFR)of retrograde intrarenal surgery(RIRS)in the treatment of lower pole renal calculi using only basket relocation and identify independent predictors of stone-free ***:All consecutive patients undergoing RIRS lower pole renal calculi at a single highvolume tertiary center were analyzed *** pole stones were relocated to the upper pole,where laser lithotripsy was *** patients were followed up in the clinic following the surgery and yearly *** stone-free status was assessed with a combination of an abdominal ultrasound and abdominal X-ray,or an abdominal non-contrast computed tomography if the stones were known to be ***:A total of 480 consecutive patients who underwent RIRS for treatment of lower pole renal calculi,between January 2012 and December 2018,were analyzed from a prospectively maintained database of 3000 *** a median follow-up time of 18.6 months,the mean SFR was 94.8%.The procedures were unsuccessful in 26(5.4%)patients due to unreachable *** median stone size of the unreachable stones was 12 mm(range 10e30 mm).Multivariable logistic regression analysis revealed two predictors of SFR for lower pole stones:a small cumulative stone burden(odds ratio[OR]:0.903,95%confidence interval[CI]:0.867e0.941,p<0.0001)and preoperative ureteral stent insertion(OR:0.515,95%CI:0.318e0.835,pZ0.007).Conclusion:The long-term SFR of RIRS for the treatment of lower pole stones with basket displacement with appropriate patient selection is high.

Melatonin is an endogenous monoamine hormone secreted by the pineal *** signaling has been shown to play a role in circadian rhythm regulation,lipid and glucose metabolism,and obesity,and it has anti-inflammatory and antioxidant *** melatonin receptor 1B gene(MTNR1B)is expressed in,among other tissues,the brain and pancreatic beta cells,and risk variants for T2D have been reported as impairing early insulin secretion and increasing fasting glucose levels.^(1) Variants in the MTNR1B gene also have been reported in patients with depression(MDD).

Quantification and depth distribution analysis of carbon to nitrogen ratio in forest soils using reflectance spectroscopy

Forest soils have large contents of carbon(C)and total nitrogen(TN),which have significant spatial variability laterally across landscapes and vertically with depth due to decomposition,erosion and ***,the ratio of C to TN contents(C∶N),a crucial indicator of soil quality and health,is also different depending on soil *** attributes can cost-effectively and rapidly be estimated using visible-near infrared-shortwave infrared(VNIR-SWIR)***,the effect of different soil layers,particularly over large scales of highly heterogeneous forest soils,on the perfor-mance of the technique has rarely been *** study evaluated the potential of VNIR-SWIR spectroscopy in quantification and variability analysis of C∶N in soils from different organic and min-eral layers of forested sites of the Czech *** each site,we collected samples from the litter(L),fragmented(F)and humus(H)organic layers,and from the A_(1)(depth of 2-10 cm)and A_(2)(depth of 10-40 cm)mineral layers providing a total of 2505 *** vector machine regression(SVMR)was used to train the prediction models of the selected attributes at each individual soil layer and the merged layer(profile).We further produced the spatial distribution maps of C∶N as the target attribute at each soil *** showed that the prediction accuracy based on the profile spectral data was adequate for all ***,F was the most accurately predicted layer,regardless of the soil attribute.C∶N models and maps in the organic layers performed well although in mineral layers,models were poor and maps were reliable only in areas with low and moderate C∶*** the other hand,the study indicated that reflectance spectra could efficiently predict and map organic layers of the forested ***,in mineral layers,high values of C∶N(≥50)were not detectable in the map created based on the reflectance *** general,the study suggests that VNIR-SWIR spectrosc

基于藏北地区普若岗日冰川1976-2016年多时相Landsat系列卫星影像数据,以及气温、降水量数据,采用ARCGIS空间分析、线性趋势分析、相关分析等方法,研究了普若岗日冰川和流域湖泊面积变化及其对气候变化的响应。结果表明:近41年普若岗日冰川面积呈显著退缩趋势,41年内冰川面积减少了52.32 km^(2),减小率为12.01%;冰川西北部退缩最为严重,冰川面积减少了12.29 km^(2),减小率为15.77%;且冰川西北部、北部、东部和东南部末端消融显著;其他区域也有不同程度的退缩。流域湖泊面积呈显著增长趋势,共增长了27.17 km^(2),增长率为22.22%;流域气温总体上呈显著增加趋势,平均每10年增加0.48℃;降水量呈不显著上升趋势,平均每10年增加15 mm。降水对冰川和流域湖泊面积影响较小,温度的持续升高是冰川和流域湖泊变化的主要因素。

Nurses’ Job Satisfaction and Motivation: Patients’ and Nurses’ Perceptions

Background: High quality care is the main concern of healthcare administrators and it was found to be related to nurses’ job satisfaction and motivation. Purpose: The study aimed to compare patients’ and nurses’ perceptions of nurses’ job satisfaction and motivation. Method: In this cross-sectional study the participants were 315 released patients and 329 nurses from eight hospitals in northern Israel. The participants completed three questionnaires: demographics, nursing job satisfaction, and nursing job motivation. Results: Nurses perceived both job satisfaction and motivation higher than the patients. Duration of hospitalization and being accompanied by family, were positively correlated to patients’ perceptions of these variables. Conclusion/Implications for Practice: To optimize care quality, it is important to assess both patients’ and nurses’ perceptions of nurses’ job satisfaction and motivation. Such an assessment may shed light on the nursing experience and on the way that it is reflected in the patients’ experience. A system-wide process is needed to set a national standard for service quality while emphasizing strategies that promote recognition of nurses’ work, for example, compliments and acknowledgment. This could enhance nurses’ sense of work engagement, which, in turn, may increase the patients’ perception of nurses’ job satisfaction and motivation.

Characteristics of gas geochemistry in Yangbajing geothermal field, Tibet

CO 2 is a main component of gases in the Yangbajing geothermal field, usually higher than 85 in volume; others are N 2, H 2S, H 2, CH 4, etc. Helium R/R a ratios, ranging from 0.14 to 0.46, indicate a crustal component to be dominant in the gases. δ 13C-CO 2 values and δ 34S-H 2S values are in the range -7.72‰--11.33‰ and 0.2‰-8.3‰, respectively. The gases are inferred to mostly derive from the partial melting of the Nyainqentanglha core complex. The gases of shallow and deep reservoirs are distinctive in chemical compositions and δ 34S-H 2S values, which shows their different flowing paths and physical-chemical processes. Sulphur does not approach isotopic equilibrium between H 2S and SO 2- 4 species in both reservoirs. Significance of steam chemical monitoring is also discussed.

Alpine wetlands in the Lhasa River Basin, China

The Lhasa River Basin is one of the typical distribution regions of alpine wetlands on the Tibetan Plateau. It is very important to get a better understanding of the background and characteristics of alpine wetland for monitoring, protection and utilization. Wetland construction and distribution in the basin were analyzed based on multi-source data including field investigation data, CBERS remote sensing data and other thematic data provided by 3S technology. The results are （1） the total area of wetlands is 209,322.26 hm^2, accounting for 6.37% of the total land area of the basin. The wetlands are mainly dominated by natural wetland, with artificial wetland occupying only 1.09% of the wetland area; marsh wetland is the principal part of natural wetland, dominated by Kobresia littledalei swampy meadow which is distributed in the river source area and upstream of Chali, Damshung and Medro Gongkar counties. The ratio and type of wetlands in different counties differ significantly, which are widely distributed in Chali and Damshung counties （accounting for 62% of the total wetland area）. （2） The concentrated vertical distribution of wetlands is at an elevation of 3600-5100 m The wetlands are widely distributed throughout the Yarlung Zangbo River Valley from river source to river mouth into the Yarlung Zangbo River. Marsh wetland is dominant in the source area and upstream of the river, with the mosaic distribution of lakes, Kobresia litUedalei and Carex moorcroftii swampy meadow, shrubby swamp and river; as for the middle-down streams, the primary types are river wetland and flooded wetland. The distribution is in a mosaic pattern of river, Kobresia humilis and Carex moorcroftii swampy meadow, Phragmites australis and subordinate grass marsh, flooded wetland and artificial wetland.
