Comparison between the PlusoptiX and IScreen Photoscreeners in Detecting Amblyopic Risk Factors in Children

Purpose: To compare the accuracy of plusoptiX A08 photoscreener (PPS) and iScreen 3000 photoscreener (IPS) in objectively screening for amblyopic risk factors in children age 5 months to 13 years old. Methods: Cross-sectional study of 148 children who received photoscreenings via PPS and IPS and a comprehensive pediatric ophthalmic examination in our office. Patients were considered to have amblyogenic risk factors based on the AAPOS referral criteria guidelines. Results: 45 percent of patients undergoing a pediatric ophthalmology examination were found to have amblyopia or amblyogenic risk factors. In this study, PPS demonstrated an overall sensitivity of 75.4%, specificity of 68.0%, positive predictive value (PPV) of 67.1%, and negative predictive value (NPV) of 76.1%. However, IPS photoscreener had an overall sensitivity of 66.2%, specificity of 87.6%, PPV of 81.8%, and NPV of 75.5%. Discussion: The accuracy of PPS and IPS was compared in different age groups. The sensitivity and specificity were analyzed according to varied amblyogenic risk factors. The statistic results of this study were compared to those of previous studies, including Vision in Preschoolers (VIP) Study and the Iowa PhotoScreening Program. Conclusion: PPS and IPS proved to be useful tools in the objective vision screening in children. PPS was found to have a higher sensitivity, and IPS showed a higher specificity and PPV in detecting amblyopic risk factors. In conclusion, one device may be more beneficial over the other, depending on the patient population and office settings.

Purpose: To report a case of a pediatric corneal limbal dermoid with eyelashes and to describe post-operative changes after excision with reconstruction using amniotic membrane grafting, sutures and fibrinogen-thrombin glue. Case Report: One pediatric patient was identified with a grade II infratemporal corneal-limbal dermoid with conjunctival eyelashes. The dermoid was surgically excised and the cornea reconstructed with amniotic membrane using sutures and fibrinogen/thrombin glue. Preoperative and postoperative measurement of astigmatism, anisometropia and presence of exposure keratopathy were performed.

Trisomy 13, also known as Patau Syndrome, is a congenital malformation that leads to several ocular anomalies, of which cataracts are the most common, as well as iris and retinal colobomas, persistent hyperplastic primary vitreous, persistent tunica vasculosa lentis, and microphthalmos. While most do not survive beyond the neonatal period, those that do survive past one year of age have an 84% chance of making it to age 5. Therefore, treatments for ocular problems should be used to improve the quality of life if the child can withstand surgery. The literature on this topic has described histopathologic findings in postmortem eyes. This case series supports those observations and presents a new discovery of nasolacrimal duct obstructions or stenosis. Deciding on surgical intervention in cases with Trisomy 13 can be challenging, and a variety of factors must be taken into account before surgery is considered. Treatment of these cases can be difficult, necessitating deliberate consideration and determination on a case-by-case basis;however, this case series gives additional evidence to help guide these decisions. Traditionally, trisomy 13 was regarded as being lethal, multiple organ malformations and severe intellectual disability.

Clinical Presentations of Pediatric Horner Syndrome

Purpose: To understand the multiple signs of Horner syndrome and to recommend protocols for pediatricians to obtain an accurate diagnosis of Horner syndrome. Methods: The medical records of 17 pediatric patients with Horner syndrome, neonates to eighteen years of age, were collected and analyzed. Data recorded included age, presenting symptoms, other medical history, allergies, medications, pupil size, presence of anhidrosis, and presence of ptosis. From the available pupil sizes, average degree of anisocoria was calculated. Results: All 17 patients had other clinical findings of Horner syndrome in addition to anisocoria. On initial evaluation, 100% had ptosis and 25% had anhidrosis. Of the available pupil size data, the average level of anisocoria was 2.06 mm, with a standard deviation of 1.17 mm. Conclusion: Physicians are reminded to measure pupil size to determine the degree of anisocoria when present, as it may help distinguish benign conditions from underlying pathology. Educating pediatricians on measurement of anisocoria and additional signs of Horner syndrome will help with proper referral patterns.

Comparison Study of Funduscopic Exam of Pediatric Patients Using the D-EYE Method and Conventional Indirect Ophthalmoscopic Methods

Purpose: The D-EYE device, a new fundoscopic smartphone lens, has demonstrated its utility in a clinical setting to detect and document ocular pathology, but has not been tested in the pediatric population. A prospective study was performed to explore the application of D-EYE in pediatric fundus examinations. Methods: Patients ages 3 - 18 years old underwent dilated fundus examinations by masked examiners using the video function of the D-EYE, while indirect ophthalmoscopy was performed by apediatric ophthalmologist. The examiners independently analyzed the D-EYE videos for the presence or absence of abnormalities, cup-to-disc (c/d) ratios and optic nerve size and color. The D-EYE video findings were compared to indirect ophthalmoscopy findings. Results: The study included 172 eyes from 87 patients. In comparing D-EYE to indirect ophthalmoscopy for detecting fundus abnormalities, the sensitivity was 0.72, specificity was 0.97, positive predictive value (PPV) was 0.77, negative predictive value (NPV) was 0.97, positive likelihood ratio (LR) was 27.8, and negative LR was 0.29. The agreement rate between the D-EYE video graders for the c/d ratio within a value of 0.1 was 97.0%. Multiple, distinct abnormalities were discovered using the D-EYE device, including nystagmus, optic nerve hypoplasia, optic disc edema, peripapillary atrophy, disc pallor, and optic disc drusen. Conclusion: Fundoscopic imaging using the D-EYE smartphone lens reliably detects the presence of fundus abnormalities and has good reliability in assessing c/d ratios. The video capability is useful in patients with nystagmus or those who are poorly compliant with the examination and allowed for effective teaching by the pediatric ophthalmologist.

In this study,a numerical analysis was conducted on the ductile fracture of a 2-mm diameter Mg-1Zn-0.5Mn-0.5Sr-0.1Ca alloy wire during *** hexagonally close-packed crystal structure of Mg alloys causes asymmetric fracture behavior,especially in the compression *** aim of this study is to develop a comprehensive damage model for Mg alloy wire that accurately predicts ductile fracture,with a focus on the compression region.A novel experimental method was introduced to measure the ductile fracture of Mg alloy wires under different stress *** wire drawing process was simulated using the Generalized Incremental Stress-State dependent damage(GISSMO)Model and the Semi-Analytical Model for Polymers(SAMP)*** damage model's prediction and the experimental results were found to be in excellent agreement,especially in determining crack *** analysis established a safe zone diagram for die angle and reduction ratio,and experimental validation confirmed the feasibility of this *** proposed damage model can provide a practical and reliable analysis for optimizing the drawing process of Mg alloy wire.

Bulk geochemistry,Sr,Nd,and O-H isotope systematics are reported for the first time on banded iron formation(BIF)-hosted high-grade iron ore at the northwestern segment of Congo Craton(CC).Located in Mbalam iron ore district,Southern Cameroon,Metzimevin iron ore deposit is a hematite-magnetite BIF system,dominated by SiO_(2)+Fe_(2)O_(3)(97.1 to 99.84 wt%),with low concentrations of clastic elements e.g.,Al_(2)O_(3),TiO_(2),and HFSE,depicting a nearly pure chemical *** REE+Y signature of the iron deposit displays strong positive Eu anomaly,strong negative Ce anomaly,and chondritic to superchondritic Y/Ho ratios,suggestive of formation by mixed seawater-high temperature hydrothermal fluids in oxidising ***^(87)Sr/^(86)Sr ratios of the BIF are higher than the maximum^(87)Sr/^(86)Sr evolution curves for all Archean reservoirs(bulk silicate earth,Archean crust and Archean seawater),indicating involvement of continentally-derived components during BIF formation and ***Ɛ_(Nd)(t)(+2.26 to+3.77)and Nd model age indicate that chemical constituents for the BIF were derived from undifferentiated crustal source,between 3.002 and 2.88 *** variable and diverse O and H isotope data(−1.9‰to 17.3‰and−57‰to 136‰respectively)indicate that the Metzimevin iron ore formed initially from magmatic plumes and later enriched by magmatic-metamorphic-modified meteoric *** balance calculations indicate mineralisation by combined leaching and precipitation,with an average iron enrichment factor of>2.67 and SiO_(2)depletion factor of>*** is associated with an overall volume reduction of 28.27%,reflecting net leaching and volume collapse of the BIF protholith.
