The Erasm.s Plus program.e’Innovative Education and Training in high power laser plasm.s’,otherwise known as PowerLaPs,is *** PowerLaPs program.e em.loys an innovative paradigm.in that it is a m.lti-centre program.e where teaching takes place in five separate institutes with a range of different aim. and styles of *** ’in class’ tim. is lim.ted to four weeks a year,and the program.e spans two *** aim. to train students from.across Europe in theoretical,applied and laboratory skills relevant to the pursuit of research in laserplasm. interaction physics and inertial confinem.nt fusion(ICF).Lectures are interm.ngled with laboratory sessions and continuous assessm.nt *** program.e,which is led by workers from.the Technological Educational Institute(TEI)of Crete,and supported by co-workers from.the Queen’s University Belfast,the University of Bordeaux,the Czech Technical-University in Prague,Ecole Polytechnique,the University of Ioannina,the University of Salam.nca and the University of York,has just com.leted its first *** far three Learning Teaching Training(LTT)activities have been held,at the Queen’s University Belfast,the University of Bordeaux and the Centre for Plasm. 为了研究肥胖对妇女孕育神经管缺陷(Neural tube defect NTD)胎儿的危险性,我们对加利福亚地区肥胖妇女孕育NTD胎儿的情况进行了一次大样本调查.从调查结果发现,肥胖妇女孕育NTD胎儿的危险性要高于正常体重的妇女.其中,尤以孕育脊柱裂(Spina bifida)、颅脊柱裂(Craniorachishisis)和露脑(Iniencephaly)畸形胎儿的危险性更大.即使考虑其他各种可能的影响因素,如叶酸摄取、糖尿病、减肥药使用、年龄、教育、饮酒等也未能改变上述分析结果.

背景单克隆抗体的广泛的应用导致了超敏反应(HSRs)发生率的增加,这阻碍了单克隆抗体作为一线治疗方法在临床上的应用。HRSs的症状、诊断方法及定向治疗方法均尚未标准化。目的我们基于临床表型、潜在的内型和生物标志物进行脱敏的治疗方法提出一种新的基于循证医学的m.Ab HSR分类方法。方法描述了104例患者对16种m.Abs HSR的表型、内型和生物标志物,并与526次皮下及静脉脱敏治疗结果进行比较。结果初始反应呈现4种模式:Ⅰ型样反应占63%,细胞因子释放反应占13%,混合反应占21%,Ⅳ型迟发反应占3%。与之形成鲜明对比的是,在脱敏治疗期间突发的HSRs为23%,其中52%为细胞因子释放反应,32%为Ⅰ型反应,12%为混合型反应,4%为Ⅰ型伴Ⅳ型迟发超敏反应。58例患者对10种m.Abs进行皮肤试验,41%的患者出现阳性。在脱敏治疗期间,1例患者血清类胰蛋白酶水平升高,8例患者IL-6水平升高,均为细胞因子释放表型。结论 m.Abs引起的HSRs可界定为Ⅰ型、细胞因子释放型、混合(Ⅰ型/细胞因子释放型)型和Ⅳ型反应,可通过生物标志物如皮肤试验、类胰蛋白酶和IL-6进行鉴别分型。当确诊为m.Abs HSR时,这些表型有助于患者接受个性化和精准医学治疗,并为脱敏治疗的实施提供依据。脱敏治疗是一种安全有效的治疗手段,可使出现过变态反应的m.Abs仍能作为一线治疗方法。

背景与目的本科学声明提供了血管性因素对认知损害（co,airiveim.airm.nt）和痴呆促成作用证据的纵览。血管性因素在老年认知损害和痴呆中的作用十分普遍。本文对血管性认知损害（vascularcognitiveim.airm.nt，VCI）的定义、神经病理学、基础科学和病理生理学研究、神经影像学的作用、血管性和其他相关危险因素的作用以及潜在的防治方法进行了回顾，旨在为临床医生提供一份总体指导，以进一步加深对VCI和痴呆及其防治的理解。方法写作组成员由写作组联合主席基于先前在相关领域的研究成果提名，并经美国心脏协会（Am.ricanHeartAssociation，AHA）卒中委员会科学声明监督委员会、流行病学和预防委员会以及文稿监督委员会批准。写作组采用系统文献回顾（主要涵盖1990年至2010年5月期间的文献）、以往公布的指南、个人资料以及专家观点总结现有证据，提出当前知识的不足，并对适当的问题根据AHA标准制定推荐意见。所有写作组成员均有机会对推荐意见进行评论并认可了最终版本。经AHA的同行评议以及卒中委员会领导阶层、流行病学和预防委员会以及科学声明监督委员会审阅后，最终获得AHA科学咨询和协调委员会批准。结果VCI概念的提出涵盖了与所有类型脑血管性脑损害（而不仅仅是卒中）相关的整个认知障碍（cognitivedisorder）谱，其范围从轻度认知损害到完全痴呆。神经血管单元和脑血流调节机制障碍可能是VCI的潜在病理生理学过程的重要组成部分。脑淀粉样血管病已成为阿尔茨海默病（Alzheim.rdisease，AD）、微梗死、微出血、脑出血以及VCI风险的一个重要标志。老年认知损害的神经病理学改变常常是AD与微血管性脑损害的混合状态，二者可叠加和协同增高认知损害风险。就此而言，m.I和其他神经影像学技术在VCI的确定和识别中起着重要作用，并提供证据说明以白质高信号和小的深部梗死为特征的皮质下型VCI很常见。在许多情况下，VCI的风险标志同样也是卒中的传统危险因素。这些危险因素包括但不仅限于心房颤动、高血压、糖尿病和高胆固醇血症。而且，这些血管危险因素也可能是AD的风险标志。颈动脉内膜一中膜厚度和动脉僵硬度是动脉老化的标记物，也可能成为VCI的风险标志。目前，美国食品药物管理局尚未批准任何VCI的特异性治疗方法。不过，对卒中和心血管病传统危险因素的评价和控制，可能对包括老年人群在内的VCI的预防具有作用。结论血管性因素在认知损害和痴呆中的促成作用很重要。近年来，在临床前、神经病理学、神经影像学、生理学和流行病学研究基础上，对VCI的认识得到了不断提高。推荐使用跨学科的、转化的以及交互的方法进一步加深对此领域的了解，并更好地掌握其神经心理学特征。需要开展前瞻性定量临床一病理学一神经影像学研究，以提高对神经影像学改变的病理学基础，以及血管病变和AD病变在VCI和AD临床演变中的复杂的丰互作用的认识。可能需要在中年期尽早肝始对血管危险标志进行长期干预，以预防或延迟VCI或AD发病的研究。住高危人群I}1强化控制血管危险因素则是另一个重要的研究方向。

背景卒中是一种主要的残疾和死亡原因。脑卒巾联盟提议，建立初级卒中中心（prim.ry stroke center，PSC）和综合性卒中中心（com.rehensive stroke center，csc）以分别为需要基本或高级下预的卒中患者提供适当的医疗服务。从2003年开始，美国医疗卫生机构认证联合委员会和各州已指定了一些PSC，目前正在考虑CSC的指定工作。为了推进该过程，我们提出了一套CSC应追踪监测医疗质量并促进质量改进的标准和相关数据。方法和结果我们分析了现有指南、综述和其他文献，以识别区分CSC与PSC的主要特征；起草了一套标准和相关数据元素，以评价这些卒中医疗方面的关键部分；然后通过反复修改达成共识。我们提出的这套标准和相关数据元素，涵盖了CSC内缺血性脯血管病、非外伤性蛛网膜下腔出血和腩出血患者专、№医疗的主要方面。结论我们提出这套标准，旨在为CSC的标准化数据采集提供一个框架，以促进当地医疗质量改进，并允许对来自不同CSC的数据进行合并分析。这将有助于将来CSC国家执行标准的制定。
