Metabolomics evaluation of the effects of green tea extract on acetaminophen-induced hepatotoxicity in mice

Green tea has a considerable amount of polyphenolic catechins and has been promoted to have beneficial effects on health including protective effects against oxidative ***(APAP) is a widely used analgesic drug which is usually safe at recommended therapeutic doses but overdose can cause acute liver *** studies explored the effects of green tea extract(GTE) on APAP-induced hepatotoxicity using UPlC /MS-and NMR-based metabolomic *** B6C3F1 mice were orally administered GTE under three scenarios either prior to or after a single dose of *** /MS and NMR were employed for metabolomics analysis of the biochemical changes in mice livers with GTE pretreatment 3 h or 3 d prior to APAP exposure or GTE exposure 6 h after *** upon the data,GTE given prior to APAP ameliorated the APAP-induced hepatotoxicity in a dose dependent manner whereas GTE given after APAP potentiated the *** belonging to multiple classes including but not limited to acylcarnitines,energy metabolism,bile acids,and lysoPCs were identified and *** metabolites were significantly altered by APAP exposure alone compared to *** pretreatment generally resulted in these metabolite levels returning to control levels or being less *** given after APAP,however,caused more changes in the metabolites over what was induced by APAP alone indicating more severe *** with biochemistry and histopathology results,the changes in liver metabolites indicated perturbations of fatty acid metabolism,energy metabolism,bile acids metabolism and phospholipids metabolism induced by APAP with differing effects on these metabolites depending upon the time of GTE *** results indicate that the time at which GTE was given highly influenced the severity of APAPinduced *** studies clearly highlight the need to understand the interactions between herbs and other dietary supplements with commonly used ove

Colonoscopy surveillance for high risk polyps does not always prevent colorectal cancer

AIM To determine the frequency and risk factors for colorectal cancer(CRC) development among individuals with resected advanced adenoma(AA)/traditional serrated adenoma(TSA)/advanced sessile serrated adenoma(ASSA). METHODS Data was collected from medical records of 14663 subjects found to have AA, TSA, or ASSA at screening or surveillance colonoscopy. Patients with inflammatory bowel disease or known genetic predisposition for CRC were excluded from the study. Factors associated with CRC developing after endoscopic management of high risk polyps were calculated in 4610 such patients who had at least one surveillance colonoscopy within 10 years following the original polypectomy of the incident advanced polyp. RESUlTS84/4610(1.8%) patients developed CRC at the polypectomy site within a median of 4.2 years(mean 4.89 years), and 1.2%(54/4610) developed CRC in a region distinct from the AA/TSA/ASSA resection site within a median of 5.1 years(mean 6.67 years). Approximately, 30%(25/84) of patients who developed CRC at the AA/TSA/ASSA site and 27.8%(15/54) of patients who developed CRC at another site had colonoscopy at recommended surveillance intervals. Increasing age; polyp size; male sex; right-sided location; high degree of dysplasia; higher number of polyps resected; and piecemeal removal were associated with an increased risk for CRC developmentat the same site as the index polyp. Increasing age; right-sided location; higher number of polyps resected and sessile endoscopic appearance of the index AA/TSA/ASSA were significantly associated with an increased risk for CRC development at a different site. CONClUSION Recognition that CRC may develop following AA/TSA/ASSA removal is one step toward improving our practice efficiency and preventing a portion of CRC related morbidity and mortality.

Structure of Isolated <i>Acer grandidentatum</i>(Bigtooth Maple) Communities and Potential Population Changes

Isolated, relic populations of Acer grandidentatum (bigtooth maple, Sapindaceae) were found in deep canyons in central Texas. Associated with them on hilltops were communities of Juniperus ashei (ash juniper). We determined the structure of relic A. grandidentatum populations and compared them to adjacent J. ashei populations. The overstory and understory density was counted and basal area was determined for all woody species in both communities using the quadrat procedure. Acer grandidentatum was the highest density (788 ± 964 or 52%) and basal area (29 ± 35 or 52%) overstory species in the canyon communities while J. ashei had the highest density (1589 ± 146 or 92%) and basal area (51 ± 13 or 88%) in the hilltop communities. In the understory of the canyon communities, there were five juvenile tree species including A. grandidentatum which had the fourth highest density (13% or 176 ± 110 plants/ha). In the hilltop communities, J. ashei was the high density understory tree species (52% or 994 ± 400 plants/ha). There were no A. Grandidentatum overstory or understory plants in the hilltop Juniperus communities. Acer grandidentatum had an inverse quadratic size distribution with few juveniles. Although we counted a number of seedlings, there was little recruitment over the last 16 years and very little potential recruitment in the previous 20 years. Juniperus ashei had an inverse exponential size distribution with many understory plants and considerable recruitment. Weibull age/size distributions showed one A. grandidentatum established in the 1670’s and 2 in the 1760’s, with peak establishment in the 1970’s. Causes of the lack of recent recruitment are uncertain, but it could be episodic due to changes in herbivory, management or other environmental conditions.

Registered Dietitian Wellness Insurance Benefit Makes a Difference in Adult Weight Management: A Pre-Post Study

Registered Dietitian (RD) services as part of insurance wellness programs offer a promising potential venue for improving public health. This study was conducted to evaluate the effectiveness of RD nutrition counseling services provided as part of an insurance benefit on body weight and associated health parameters. Eligible members could enroll to receive 6 RDs visits a year for assistance with weight management. The study RDs were randomized into either Usual Care (UC) or lifestyle Case Management (lCM) groups. Body weight, waist circumference, and systolic and diastolic blood pressure measurements of program enrollees were evaluated for between group as well as start and end program comparisons. There was a statistically significant difference in the number of RD follow-up visits between the two groups as lCM patients had more RD contact than UC patients. Weight and waist circumference changes from baseline to end of study show statistically significant changes with a trend for improvement in systolic blood pressure. Additionally, a clinically significant reduction in weight was achieved in a quarter of program enrollees. In conclusion this study shows that through a coordinated health promotion program RDs’ services are of value to an insured population.

Exogenous Protein as an Environmental Stimulus of Biofilm Formation in Select Bacterial Strains

A screening of environmental conditions that would elicit robust biofilm in a collection of Serratia marcescens isolated from soil revealed that exogenous milk protein increased biofilm productivity up to ten-fold. A select screening of fish pathogens, freshwater and human isolates identified several other species that responded similarly to exogenous protein. The optimal protein concentration was species specific;S. marcescens at 5% milk protein, Aeromonas sp. at 2% - 3%, Flavobacterium columnare at 1% and Pseudomonas aeruginosa at 0.1% - 0.4%. Media supplemented with milk protein also increased the cell counts in biofilm as well as the protein incorporated into the biofilm matrix. These data suggest that relatively high concentrations of exogenous protein may serve as an environmental trigger for biofilm formation, particularly for pathogenic bacteria exposed to relatively high concentrations of protein in bodily fluids and mucosal surfaces.

Barriers and facilitators to an outreach rehabilitation program delivered in nursing homes after hip fracture surgical repair

Objective: To identify and understand facilitators and barriers to implementing an Outreach rehabilitation program designed to improve post-operative recovery following hip fracture in long-term care residents. Residents of nursing home facilities are at considerable risk of hip fracture and minimal recovery following a hip fracture. Methods: Data were gathered over June-August, 2012 through semi-structured interviews or focus groups. Fifteen persons (n = 15) who were members of the Outreach rehabilitation team (n = 8) or relevant nursing home staff (n = 7) were interviewed. Data analysis was guided by principles of grounded theory method. Findings: Three major themes that contributed to or hindered the Outreach rehabilitation program emerged, namely, 1) the division, the separate operation and delivery of rehabilitation services;2) building bridges, or negotiating ways to communicate and work together, and 3) strength in the structure, the acceptance of the program and the perceived benefits of the program. One main challenge to program implementation con- cerned coordinating additional rehabilitation with the rehabilitation provided within the nursing homes. Facility staff was largely unaware of the program and were unprepared to work with Outreach team members. As the program progressed, the facility staff and Outreach team were able to collaborate to overcome resident health issues impeding recovery such as cognitive impairment, language barriers and post-surgical pain control needs. Facilitators included the consistency of Outreach team members and accessible facility staff, which contributed to effective communication and trust between the Outreach team and facility staff. Facilitators also included support for the program by the Outreach team and facility staff, as well as the potential benefits of improved mobility and functional status among some program recipients.
