Stevens-Johnson syndrome and concurrent hand foot syndrome during treatment with capecitabine:A case report

BACKGROUND Capecitabine is used in combination with lapatinib as palliative treatment for human epidermal growth factor receptor 2-positive metastatic breast cancer.The most frequently reported adverse events attributed to capecitabine include diarrhea,hyperbilirubinemia,and hand-foot syndrome(HFS).A number of cutaneous adverse events have been attributed to capecitabine,including Stevens-Johnson syndrome(SJS)as a rare and potentially life-threatening mucocutaneous condition.We report the first case involving concurrent SJS and HFS after capecitabine and lapatinib treatment.CASE SUMMARY A 70-year-old woman with a history of breast cancer treatment visited our hospital for evaluation of painful skin lesions.Six weeks earlier,she had been prescribed capecitabine plus lapatinib as treatment for metastatic breast cancer.She subsequently developed worsening erythema and bullae on her palms and soles,as well as reddish macules on her back and chest wall.Histopathological evaluation of the chest wall lesions revealed extensive eosinophilic epidermal necrosis and separation of the epidermis from the dermis.The capecitabine plus lapatinib treatment was discontinued immediately and treatment was started using systemic steroids.This treatment resolved most lesions,although the lesions on her palms and soles required Vaseline gauze dressings,which resulted in reepithelialization.Therefore,we determined that the patient had concurrent SJS and HFS.Although the dermatological problems resolved,the patient ultimately died because of multiple organ failure.CONCLUSION Oral capecitabine treatment carries a risk of both HFS and also life-threatening adverse cutaneous drug reactions,such as SJS.

Development of a variable rate applicator for uniform fertilizer spreading

Environmental impact and economic concerns have driven a variable rate technology(VRT).Spinner spreaders were mainly used for granular fertilizer application since they are simple in design,inexpensive,and can cover large areas.However,the spreader was not adequate for VRT because uniformity changes drastically while varying application rates.Thus,the purpose of this study was to develop a variable rate applicator with uniform spreading patterns.A commercial spreader was modified with a controller and electric actuators for controlling fertilizer discharge directions and amounts.Database was established to determine the optimum discharge direction according to the fertilizer application amount.The uniformity of spreading patterns in accordance with the spread amount per set unit area was evaluated by the statistical coefficient of variation(CV)lower than 15%is assumed to prevent damage to the crop.Test results showed that CVs were 8%,9%,and 8%,respectively,for a tractor in race track mode(adjacent passes in same direction of travel)at 200 kg/hm^(2),300 kg/hm^(2),and 400 kg/hm^(2).This indicates that the fertilizer was spread uniformly,while the coefficient of variation was 12%at 200 kg/hm^(2) in back and forth mode(adjacent passes in the opposite direction of travel).Overall,the results suggest that the race track mode is suitable for operation of a tractor to ensure uniform spreading of fertilizer when applying at variable rates.The future goal is to establish a system for automatic variable rate application according to location in connection with soil analysis and geographic information systems.

Gastric dysplasia may be an independent risk factor of an advanced colorectal neoplasm

AIM: To evaluate the relationship between gastric dysplasia and Helicobacter pylori (H pylori) and the occurrence of colorectal adenoma, and to defi ne the necessity for colonoscopy in patients with gastric dysplasia or H pylori infection.METHODS: From May 2005 to February 2008, 133 patients with established gastric dysplasia by gastroduo-denoscopy (EGD) were additionally investigated by colonoscopy. The authors compared results with those of 213 subjects who underwent both EGD and colonoscopy during the same period at the author’s Health Promotion Center as a control group. H pylori infection was evaluated in both the gastric dysplasia and control groups.RESULTS: The mean age of all 346 study subjects was 54.1 ± 10.5 years, and there were 258 (73%) men and 87 (27%) women. No signif icant difference was found between the H pylori positive and negative subjects in terms of the prevalence of colorectal adenoma and advanced colorectal adenoma (P = 0.261). Patients with gastric dysplasia showed no elevated risk of colorectal adenoma (OR = 0.910, 95% CI: 0.587-1.411, P = 0.738), but had a signif icantly higher risk of having advanced colorectal adenoma (OR = 3.382, 95% CI: 1.700-6.342, P = 0.000).CONCLUSION: The study emphasizes the need for colon surveillance in patients with gastric dysplasia, regardless of H pylori infection.

Osteochondroma formation after avulsion fracture of anterior inferior iliac spine:A case report

BACKGROUNDHip avulsion fractures occur mostly during adolescence when actions such askicking or running cause forceful contraction of attached muscle.Osteochondroma is benign tumor that mostly occurs at the metaphysis of a longbone, being usually asymptomatic.CASE SUMMARYA 15-year-old patient experienced feeling and sound of a break while kicking aball in soccer game three years prior to his visit to our hospital. A simple X-rayrevealed an avulsion fracture of the apophysis of the anterior inferior iliac spine(AIIS). Later in the follow-up X-ray, a palpable mass was found and demonstratedby magnetic resonance imaging to be a pedunculated osteochondroma in thesuperolateral aspect of the AIIS. For surgical treatment, we performed osteotomyfor surgical excision and excisional biopsy. A mass with smooth surface and anunclear superolateral AIIS border was found intraoperatively. Pathologic examshowed definite diagnosis of osteochondroma. Postoperatively, discomfort duringhip flexion was improved, and the hip joint range of motion during walking wasrecovered at the last follow-up, which was three weeks after the surgery.CONCLUSIONThis is a rare case to demonstrate relevant previous trauma history prior to theformation of osteochondroma.

Therapeutic benefit of repetitive transcranial magnetic stimulation for severe mirror movements A case report

Congenital mirror movements retard typical hand functions, but no definite therapeutic modality is available to treat such movements. We report an 8-year-old boy with severe mirror movements of both hands. His mirror movements were assessed using the Woods and Teuber grading scale and his fine motor skills were also evaluated by the Purdue Pegboard Test. A 2-week regimen of repetitive transcranial magnetic stimulation produced markedly diminished mirror movement symptoms and increased the fine motor skills of both hands. Two weeks after the completion of the regimen, mirror movement grades had improved from grade 4 to 1 in both hands and the Purdue Pegboard Test results of the right and left hands also improved from 12 to 14 or 13. These improvements were maintained for 1 month after the 2-week repetitive transcranial magnetic stimulation regimen. After 18 months, the mirror movement grade was maintained and the Purdue Pegboard test score had improved to 15 for the right hand while the left hand score was maintained at 13. This occurred without any additional repetitive transcranial magnetic stimulation or other treatment. These findings suggest that repetitive transcranial magnetic stimulation for this patient had a therapeutic and long-term effect on mirror movements.

Effects of Acetylsalicylic Acid and Calcium Chloride on Photosynthetic Apparatus and Reactive Oxygen-Scavenging Enzymes in Chrysanthemum Under Low Temperature Stress with Low Light

The effects of acetylsalicylic acid （ASA）, CaCl2, and ASA ＋ CaCl2 on the photosynthetic apparatus and antioxidant enzyme activities were investigated in chrysanthemum Jinba （a cut flower cultivar） under low temperature stress with low light （TL stress）（16/12℃, day/night, PFD 100 μmol m^-2 s-1）. The results showed that under TL stress, the net photosynthesis rate （Pn）, carboxylation efficiency （CE）, apparent quantum yield （AQY）, maximal photochemical efficiency （Fv/Fm） of PSII, quantum yield of PSII electron transport （ФPSII）, and photochemical quenching （qP） of the chrysanthemum leaves in all treatments were significantly decreased, but the decreases were alleviated by ASA, CaCl2, and ASA ＋ CaCl2 treatments compared with the controls. The alleviating effect of ASA ＋ CaCI2 was better than either ASA or CaCl2 single treatment. Moreover, the ASA ＋ CaCl2 treatment highly improved the chlorophyll content, relatively improved the number and size of chloroplast and starch grain in the leaves of chrysanthemum plants compared with ASA and CaCl2 treatments. It was indicated that ASA and/or CaCI2 could regulate the photosynthetic functions in the leaves of chrysanthemum plants to enhance the resistance against TL stress. On the other hand, reduction in relative conductance rate implied that ASA and/ or CaCl2 could protect from membrane injury in leaves of chrysanthemum plants. The activities of SOD, POD, and CAT in the treated leaves of chrysanthemum were increased as compared with the controls. It was suggested that ASA and/or CaCl2 had positive regulation effects on the defence enzyme activities in chrysanthemum leaves which could protect the photosynthetic apparatus to a certain degree under the TL stress. In brief, the treatment of ASA together with CaCl2 was better for chrysanthemum plants to adapt TL stress than single ASA or CaCl2 treatments.

The Effect of Reflector with Sound-Absorbing Material on Supersonic Jet Noise

This paper describes an experimental work to investigate the effect of a reflector on supersonic jet noise radiated from a convergent-divergent nozzle with a design Mach number 2.0. In the present study, a metal reflector and reflectors made of three different sound-absorbing materials (grass wool and polyurethane foam) were employed,and the reflector size was varied. Acoustic measurement is carried out to obtain the acoustic characteristics such as frequency, amplitude of screech tone and overall sound pressure level (OASPL). A high-quality schlieren optical system is used to visualize the detailed structure of supersonic jet. The results obtained show that the acoustic characteristics of supersonic jet noise are strongly dependent upon the jet pressure ratio and the reflector size. It is also found that the reflector with sound-absorbing material reduces the screech tone amplitude by about 5-13dB and the overall sound pressure levels by about 2-5dB, compared with those of the metal reflector.

A simple, flexible and high-throughput cloning system for plant genome editing via CRISPR-Cas system

CRISPR-Cas9 system is now widely used to edit a target genome in animals and plants. Cas9 protein derived from Streptococcus pyogenes（Sp Cas9） cleaves double-stranded DNA targeted by a chimeric single-guide RNA（sg RNA）. For plant genome editing, Agrobacterium-mediated T-DNA transformation has been broadly used to express Cas9 proteins and sg RNAs under the control of Ca MV 35 S and U6/U3 promoter, respectively. We here developed a simple and high-throughput binary vector system to clone a 19 20 bp of sg RNA, which binds to the reverse complement of a target locus, in a large T-DNA binary vector containing an Sp Cas9 expressing cassette. Twostep cloning procedures：（1） annealing two target-specific oligonucleotides with overhangs specific to the Aar I restriction enzyme site of the binary vector; and（2） ligating the annealed oligonucleotides into the two Aar I sites of the vector, facilitate the high-throughput production of the positive clones. In addition, Cas9-coding sequence and U6/U3 promoter can be easily exchanged via the GatewayTMsystem and unique Eco RI/Xho I sites on the vector, respectively. We examined the mutation ratio and patterns when we transformed these constructs into Arabidopsis thaliana and a wild tobacco, Nicotiana attenuata. Our vector system will be useful to generate targeted large-scale knock-out lines of model as well as non-model plant.

Endoscopic assessment and management of sporadic duodenal adenomas: The results of single centre multidisciplinary management

AIM To review the role of multidisciplinary management in treating sporadic duodenal adenomas(SDA).METHODS SDA managed at North Shore Hospital between 2009-2014 were entered into a prospective database. Pathology, endoscopic and surgical management as well as follow up were reviewed.RESULTS Twenty-eight patients(14 male: Median age 68 years) presented with SDA [18 were classified as non ampullary location(NA), 10 as ampullary location(A)]. All SDA were diagnosed on upper gastrointestinal endoscopy and were imaged with a contrast enhanced CT scan of the chest, abdomen and pelvis. Of the NA adenomas 14 were located in the second part, 2 in the first part and 2 in the third part of the duodenum. Two patients declined treatment, 3 patients underwent surgical resection(2 transduodenal resections and 1 pancreaticoduodenectomy), and 23 patients were treated with endoscopic mucosal resection(EMR). The only complication with endoscopic resection was mild pancreatitis post procedure. Patients were followed with gastroduodenoscopy for a median of 22 mo(range: 2-69 mo). There were 8 recurrences treated with EMR with one patient proceeding to pancreaticodeuodenectomy because of high grade dysplasia in the resected specimen and 2 NA recurrences were managed with surgical resection(distal gastrectomy for a lesion in the first part of the duodenum and a transduodenal resection of a lesion in the third part of the duodenum).CONCLUSION SDA can be treated endoscopically with minimal morbidity and piecemeal resection results in eradication in nearly three quarters of patients. Recurrent SDA can be treated with endoscopic reresection with surgical resection indicated when the lesions are large(> 4 cm in diameter) or demonstrate severe dysplasia or invasive cancer.
