背景与目的利妥昔单抗是一种靶向CD20的单克隆抗体,在治疗边缘区淋巴瘤(marginal zone lymphoma,MZL)中可以单药或联合化疗使用,可提高缓解率和生存率,但治疗后仍可能会复发。因此,仍需探索如何延长MZL患者的缓解期的治疗方案。这项多中心、单臂、开放标签的Ⅱ期研究评估了对一线R-CVP(rituximab,cyclophosphamide,vincristine,and prednisolone;利妥昔单抗、环磷酰胺、长春新碱和泼尼松龙)治疗有反应的Ⅲ–Ⅳ期CD20阳性MZL患者进行利妥昔单抗维持治疗2年的生存率。本研究旨在明确R-CVP后利妥昔单抗维持治疗的策略是否有进一步研究的价值。方法在利妥昔单抗维持治疗之前,Ⅲ–Ⅳ期MZL患者应接受6–8周期的R-CVP一线治疗。每个周期为3周,第1 d静脉输注利妥昔单抗(375 mg/m^2)、环磷酰胺(750 mg/m^2)和长春新碱(1.4 mg/m^2;最大2 mg),第1–5 d口服泼尼松龙(100 mg)。对R-CVP治疗达到完全缓解(complete response,CR)、部分缓解(part response,PR)或疾病稳定(stable response,SD)的患者进行利妥昔单抗维持治疗,每8周静脉注射利妥昔单抗375mg/m^2,直到12个周期。主要终点为无进展生存期(progression-free surviva,PFS),次要终点为总生存期(overall survival,OS)和治疗安全性。结果共纳入47例患者,其中45(96%)例接受了利妥昔单抗维持治疗。15(33%)例患者有淋巴结MZL。在进行R-CVP一线治疗后,分别有20(44%)例、22(49%)例和3(7%)例的患者达到了CR、PR和SD。中位随访38.2个月后,患者的3年PFS率为81%。在利妥昔单抗维持治疗期间,R-CVP治疗后有6例PR和1例SD患者获得CR。LDH升高和B症状是PFS的重要预后因素(P=0.003),3年OS率为90%。利妥昔单抗维持治疗的耐受性良好,常见的治疗性不良事件为感觉性神经病(18%)、肌痛(13%)、疲劳(9%)和中性粒细胞减少(9%)。结论一线R-CVP治疗后利妥昔单抗维持治疗在Ⅲ–Ⅳ期MZL患者中不仅表现出较好的PFS,而且药物毒性反应评价良好。

薄膜晶体管液晶电视(TFT-LCDTV)因为具有薄、轻、紧凑和可随意放置的特点,已经占据了大部分电视机市场。除了这些物理特性以外,最重要的特性是已具有了良好像质的对比度。为了将对比度提高到1:600以上,对偏振片膜、背光源板、滤色片树脂、电极锥角和摩擦条件等都进行了研究。优化的背光板组合,光滑的电极锥角和摩擦方法的控制是提高对比度的主要控制因素。应用新开发的滤色片树脂,对获得高对比度最为有效。

One-year clinical outcomes in invasive treatment strategies for acute ST-elevation myocardial infarction complicated by cardiogenic shock in eld-erly patients

Objective To investigate the clinical outcomes of an invasive strategy for elderly (aged≥75 years) patients with acute ST-segment elevation myocardial infarction (STEMI) complicated by cardiogenic shock (CS). Methods Data on 366 of 409 elderly CS patients from a total of 6,132 acute STEMI cases enrolled in the Korea Acute Myocardial Infarction Registry between January 2008 and June 2011, were collected and analyzed. In-hospital deaths and the 1-month and 1-year survival rates free from major adverse cardiac events (MACE;defined as all cause death, myocardial infarction, and target vessel revascularization) were reported for the patients who had undergone invasive (n=310) and conservative (n=56) treatment strategies. Results The baseline clinical characteristics were not significantly different between the two groups. There were fewer in-hospital deaths in the invasive treatment strategy group (23.5%vs. 46.4%, P<0.001). In addition, the 1-year MACE-free survival rate after invasive treatment was significantly lower compared with the conservative treatment (51%vs. 66%, P=0.001). Conclusions In elderly patients with acute STEMI complicated by CS, the outcomes of invasive strategy are similar to those in younger patients at the 1-year follow-up.

本研究审视了工作疲劳、工作要求(繁文缛节、角色冲突、工作过载)和公共服务动机对基层公务员离职意向的综合影响。在对4 974名韩国基层公务员调查的基础上,本研究检查了工作疲劳的潜在中介作用和公共服务动机的复杂调节作用,以确定基层公务员的离职意向。与此前研究一致的是,我们发现工作要求通过工作疲劳对基层公务员离职意向产生直接和间接影响。但是,我们发现,公共服务动机以两种不同的方式减弱基层公务员的离职意向。首先,我们发现,公共服务动机对离职意向有直接负面影响。此外,它还会减弱工作要求和工作疲劳之间,以及工作要求与离职意向之间的正面联系。

Warfarin is a widely used *** differences in drug response,a narrow therapeutic range and the risk of bleeding render warfarin difficult to use *** 18-year-old woman with antiphospholipid syndrome received long-term warfarin therapy for a recurrent deep vein *** years later,she developed right flank *** diagnosed intrahepatic and subgaleal hemorrhages secondary to anticoagulation *** stopping oral anticoagulation,a follow-up computed tomography showed improvement in the *** restarting warfarin because of a recurrent thrombosis,the intrahepatic hemorrhage *** decided to start clopidogrel and hydroxychloroquine instead of *** patient has not developed further recurrent thrombotic or bleeding *** hemorrhage is a very rare complication of warfarin,and our patient experienced intrahepatic and subgaleal hemorrhage although she did not have any risk factors for bleeding or instability of the international normalized ratio control.

The development of boceprevir and telaprevir was a major step forward in the treatment of chronic hepatitis *** addition,the treatment of these infections has been recently revolutionized by the approval of sofosbuvir and ***,there are several challenges associated with the application of noveldrugs,such as new and more frequent adverse events,new drug interactions,and excessively high treatment *** additional concern is viral *** considerations highlight the fact that direct-acting antiviral agents are not a panacea and may not be the best option for all patients who are in need of *** retrospective study revealed that the sustained virologic response was not significantly reduced following peginterferon and ribavirin retreatment compared with the new *** suggest that patients who experience relapse shortly after completing treatment with peginterferon and ribavirin have a reasonable chance of achieving a sustained virologic response when retreated with these drugs alone.

＜正＞ We examined POMC mRNA and beta - endorphin expression in mouse hypothalamus elicited by intraplantar formalin. POMC mRNA increased at 2 hr after injection,and returned to control level at 10 hr in the arcuate nu. of hypothalamus. In the tail - flick test,formalin attenuated opioids - induced antinociception. Increase of POMC was also observed after systemic morphine and attenuated by naltrexone. Thus,formalin - induced increase of hypothalamic POMC may be mediated by endogenous opioid system. We further examined alterations of signal molecules. pERK was increased within 30 min and remained at a high level up to 10 hr in the arcuate nu..pCaMKII was increased within 2 hr but decreased at 10 hr. However. POMC mRNA expression was reduced by pretreatment with PD98059 or KN93. Furthermore. pIkB was increased at 2 hr and remained at high level up to 10 hr. Using confocal IF. we confirmed that cells contain beta - endorphin after formalin also express pERK. pCaMKII and pIkB. In conclusion. POMC mRNA expression in arcuate nu. of hypothalamus induced by intraplantar formalin may be mediated by pERK as well as pCaMKII. Furthermore. NFkB pathway may play an important role in the regulation of POMC gene expression.

AIM:To characterize single-cell-derived mouse clonal mesenchymal stem cells (mcMSCs) established with bone marrow samples from three different mouse strains. METHODS:We established mcMSC lines using subfractionation culturing method from bone marrow samples obtained from long *** lines were characterized by measuring cell growth, cell surface epitopes, differentiation potential, lineage-specific gene expression and T-cell suppression capability. Nonclonal MSCs isolated by the conventional gradient centrifugation method were used as controls. RESULTS:All mcMSC lines showed typical nonclonal MSC-like spindle shape morphology. Lines differed inoptimal growth density *** surface epitope prof iles of these mcMSC lines were similar to those of nonclonal MSCs. However, some lines exhibited different expression levels in a few epitopes, such as CD44 and CD105. Differentiation assays showed that 90% of the mcMSC lines were capable of differentiating into adipogenic and/or chondrogenic lineages, but only 20% showed osteogenic lineage differentiation. T-cell suppression analysis showed that 75% of the lines exhibited T-cell suppression capability. CONCLUSION:mcMSC lines have similar cell morphology and cell growth rate but exhibit variations in their cell surface epitopes, differentiation potential, lineage-specifi c gene expression and T-cell suppression capability.
