Background: Tuberculosis (TB) is one of the top lethal infectious diseases worldwide. In recent years, interferon-γ (INF-γ) release assays (IGRAs) have been established as routine tests for diagnosing TB infection. However, produced INF-γ assessment cannot permit to distinguish active ATB from latent TB infection (LTBI), especially in TB epidemic areas. In addition to IFN-γ, interleukin-2 (IL-2), secreted by activated T cells, is involved in immune response against Mycobacterium tuberculosis. This could be involved in the follow up of treatment response. The aim of our study was to determine IFN-γ and IL2 cytokines profiles of patients under antituberculosis treatment. Materials and Methods: A six months’ cross-sectional study was conducted at the Jamot Hospital of Yaoundé, from May to August 2021. Sociodemographic and clinical data as well as 5 mL of blood were collected from each participant. INF-γ and IL-2 were determined using indirect Enzyme linked Immuno-Sorbent Assay (ELISA) according to the manufacturer’s recommendations and spectrum exam in combination with radiography and GeneXpert were used as standard. P-values Results: The results showed that men were more infected 14/61 (31.8%) with a high presence in active and resistant TB groups. The mean age was 41.3 ± 13.1 years with a 95% CI = [38.2 - 44.7], the age group with the highest infection rate was ranged between 31 and 40 years. The IL-2 and INF-γ means were respectively 327.6 ± 160.6 pg/mL and 26.6 ± 13.0 pg/mL in ATB patients, 251.1 ± 30.9 pg/mL and 21.4 ± 9.2 pg/mL in patients with resistant tuberculosis, while it was 149.3 ± 93.3 pg/mL and 17.9 ± 9.4 pg/mL in cured patients, 15.1 ± 8.4 pg/mL and 5.3 ± 2.6 pg/mL in participants presumed healthy (p γ and IL-2rates were observed between the different groups. Conclusion: Monitoring the serum levels of INF-γ and IL-2 would be useful for the follow-up of anti-tuberculosis patients, particularly in the both cytokines association case.

Seroprevalence of SARS-CoV-2 among Blood Donors in the Republic of Congo

Corona Virus Disease 2019 (COVID-19) is a highly transmissible and pathogenic viral infection caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), which erupted in Wuhan, China, in 2019 and has spread worldwide [1]. The objective of this study was to assess the seroprevalence of SARS-CoV-2 among blood donors in the Republic of Congo. Method: This is an analytical and cross-sectional study that was carried out during the period from July to December 2021. Biological analyses were performed with the serological tests n2019 rapid IgG/IgM from Beijing Diagreat Biotechnologies and antigen tests from Abbot using the serum for the detection of anti-SARS-Cov-2 antibodies and nasal mucus for the detection of SARS-Cov-2 antigens. Statistical analysis was performed using SPSS version 22. The Results: Out of 2553 donors recruited in our study, we observed a predominance of male sex with 86.1% or a ratio of 6.19, the age group of 18 - 30 years was dominant with 45.9%, family donation represented 59.2%, the dominant profession was workers with 46.9% and the dominant blood group was O Rh positive with 54%. The prevalence of anti-SRAS-CoV-2 antibodies and antigens were respectively 31.4% for anti-SRAS CoV-2 IgG antibodies, 36.7% for anti-SRAS CoV-2 IgM antibodies and 2.93% for SARS-CoV-2 antigen.

Introduction: Sickle cell disease is a public health problem in the Republic of Congo where the prevalence of sickle cell trait is estimated at 1.25%. The objective of this study is to describe the variations of hematological and biochemical parameters of hemolysis in sickle cell patients in critical and inter-critical periods. Methods: This is a descriptive cross-sectional study including sickle cell patients followed regularly at the National Reference Center for Sickle Cell Disease (CNRDr) from November 2019 to March 2020. A sample of 167 patients (sickle cell subjects in crisis and in steady state as well as control subjects) was randomly selected during the study period. The blood count was performed using a Sysmex-XN 350 automated system and the biochemical parameters were determined using the Cobas e 311 automated system. Statistical analysis was performed with SPSS version 22 software. Results: The study showed that the mean cholesterol level in controls was 4.16 ± 0.77 ul compared with 9.64 ± 4.34 ul in sickle cell crisis subjects. Hb and HCT levels were significantly higher in controls compared with sickle cell subjects in crisis. During crisis, total bilirubin, direct bilirubin, triglycerides, LDH, AST, and CRP were significantly elevated. Hematological parameters such as Hb and HCT were elevated in controls, while the mean WBC value and RET were higher in sickle cell patients in steady state. Microscopy-positive and drug-resistant pulmonary tuberculosis (MPT+) is one of the most feared diseases due to the cost of its management and the associated mortality. The GeneXpert, a new molecular test, is in greater demand for the diagnosis of MPT+ resistance cases. The application of GeneXpert to new cases of MPT+ is not effective in the country’s TB screening centres. The objective of this study is to assess the contribution of GeneXpert to the determination of MPT+ resistance cases in Bangui. The study was cross-sectional and covered the period from February to July 2022. The diagnosis of tuberculosis was first performed by microscopy with Ziehl Neelsen hot stain. The GeneXpert was then used to test for resistance in the sputum of all patients with positive microscopy. The collected data was entered into Excel 2013 and analysed with Epi Info 3.3.7. We analysed data from 755 patients, 80 of whom had resistance. The 80 patients ranged in age from 6 to 68 years (mean age = 35 years). The prevalence of resistant TB was 10.60% (80/755). Primary resistance accounted for 73.75% and secondary resistance for 26.25%. The age group 20 - 39 years (57.50%), male (72.50%), 8th district (17.50%), people living in couples (53.75%), farmers (13.75%) were the socio-demographic characteristics most affected by resistance. Treatment failure (13.75%), relapses (13.75%), the notion of contagion (28.75%), a history of smoking (40%) and alcohol (61.25%) were the clinical antecedents reported by the patients. Treatment failure and relapse were the variables associated with the occurrence of resistant PMT+ (p 0.05). A considerable proportion of the overall Mycobacterium tuberculosis resistance to anti-tuberculosis drugs (10.60%) was identified by GeneXpert. Treatment failure and relapse were the factors associated with the risk of resistance.

Prevalence and Risk Factors for Hepatitis Delta Virus Transmission among HBsAg Positive Blood Donors in Brazzaville, Congo

Background: Hepatitis D virus (HDV) is a defective virus that requires the presence of hepatitis B virus (HBV) for replication. It is a major cause of severe acute and chronic hepatitis B. The objective of this study was to determine the prevalence and risk factors of HDV in blood donors. Methods: This was a descriptive, cross-sectional study involving HBsAg-positive blood donors covering the period from July to December 2020. Testing for HDV RNA was performed by conventional two-step RT-PCR. Data were analysed using SPSS version 22 software. Result: Of 113 HBsAg positive samples included in the study 22 or 19.5% were HDV positive. The prevalence of HDV was higher in the age group 31 - 45 years (45.5%), in male donors (71.4%) and in donors with secondary education (50.0%). Furthermore, 55.4% belonged to the family donor category. A significant statistical difference was observed between HDV infection, tattoos, piercings and multiple sexual partners. Conclusion: This study shows a high prevalence of HDV among blood donors in Brazzaville.
