Alcoholic vs. Nonalcoholic Steatohepatitis: Vascular Branching Heterogeneity on Magnetic Resonance Imaging as a Diagnostic Marker

Background and Aims: Distinguishing alcoholic steato-hepatitis (ASH) and nonalcoholic steatohepatitis (NASH) with biopsy alone is often difficult without a reliable clinical con-text. A novel finding on liver imaging, perivascular branching heterogeneity, has shown promise in distinguishing between these chronic liver diseases. Our study investigated the role of this finding on imaging to differentiate between ASH and NASH. The aim of this study was to determine the utility and reproducibility of this novel radiographic marker to help dis-tinguish ASH from NASH. Methods: This was a retrospective cohort study conducted between 2016 and 2020 in patients with both liver biopsy-confirmed steatohepatitis/chronic hep-atitis and abdominal magnetic resonance imaging within 13 months of each other. Two radiologists, blinded to patient clinical history and diagnosis, categorized the appearance of the liver as: 1- homogeneity, 2- mild heterogeneity, 3- mod-erate heterogeneity, 4- possible perivascular branching, 5- definite perivascular branching. Results: Of the 90 patients in the study, 60 were identified as NASH and 30 as ASH. The area under the curve (AUC) for both reader 1 and 2 when us-ing the 5-point scale was 0.69 (CI: 0.56-0.82, p=0.006) and 0.72 (CI: 0.60-0.85, p=0.001), respectively. The positive predictive value (PPV) for identification of ASH when scoring 5 was 64.7% and 66.7% for reader 1 and 2, respectively. In-terclass correlation coefficient was 0.74 in patients with ASH, indicating moderate reliability among both readers. Conclu-sions: Identification of this perivascular branching pattern on imaging is a promising novel diagnostic marker that can be used with other methods to help distinguish between ASH and NASH.

AIM： To study the appearances of acute interstitial edematous pancreatitis （IEP） on non-enhanced MR imaging. METHODS： A total of 53 patients with IEP diagnosed by clinical features and laboratory findings were underwent MR imaging. MR imaging sequences included fast spoiled gradient echo （FSPgR） fat saturation axial T1-weighted imaging, gradient echo T1-weighted （in phase）, single shot fast spin echo （SSFSE） T2-weighted, respiratory triggered （R-T） T2-weighted with fat saturation, and MR cholangiopancreatography. Using the MR severity score index, pancreatitis was graded as mild （0-2 points）, moderate （3-6 points） and severe （7-10 points）. RESULTS： Among the 53 patients, IEP was graded as mild in 37 patients and as moderate in 16 patients. Forty-seven of 53 （89%） patients had at least one abnormality on MR images. Pancreas was hypointense relative to liver on FSPgR T1-weighted images in 18.9% of patients, and hyperintense in 25% and 30% on SSFSE T2-weighted and R-T T2-weighted images, respectively. The prevalences of the findings of IEP on R-T T2-weighted images were, respectively, 85% for pancreatic fascial plane, 77% for left renal fascial plane, 55% for peripancreatic fat stranding, 42% for right renal fascial plane, 45% for perivascular fluid, 40% for thickened pancreatic Iobular septum and 25% for peripancreatic fluid, which were markedly higher than those on in-phase or SSFSE T2-weighted images （P 〈 0.001）. CONCLUSION： IEP primarily manifests on non- enhanced MR images as thickened pancreatic fascial plane, left renal fascial plane, peripancreatic fat stranding, and peripancreatic fluid. Survival outcomes in early-onset oesophageal adenocarcinoma patients:A systematic review and meta-analyses

Monoamine Oxidase-B Inhibitor Rasagiline Effects on Motor and Non-Motor Symptoms in Individuals with Parkinson’s Disease: A Systematic Review and Meta-Analysis

Objective: In the manuscript titled Monoamine Oxidase-B Inhibitor Rasagiline Effects on Motor and Non-Motor Symptoms in Individuals with Parkinsons Disease: A Systematic Review and Meta-Analysis, the objective was to conduct a systematic review with meta-analysis to investigate the effects that Rasagiline has on motor and non-motor symptoms in individuals with PD. Introduction: Rasagiline is a second-generation monoamine oxidase-B (MAO-B) inhibitor used both as monotherapy and adjunctive therapy for Parkinsons Disease (PD). Methods: A systematic literature search and meta-analysis were performed with randomized control trials that investigated the effects of Rasagiline on motor and non-motor symptoms in individuals with PD. The systematic search was conducted in PubMed, Cochrane, and EBSCO databases. Methodological quality was assessed using the Cochrane grading Recommendations Assessment, Development and Evaluation approach. Results: Fourteen studies were included in our review. There were trivial to small and statistically significant improvements in motor symptoms for individuals with PD treated with Rasagiline compared to placebo. Non-motor symptoms showed no significant improvement with Rasagiline compared to placebo in five of six meta-analyses. Results were based on very low to moderate certainty of evidence. Conclusion: 1 mg/day Rasagiline significantly improved Parkinsonian motor symptoms in individuals with PD compared with placebo. For all outcomes, the 1 mg/day Rasagiline group was favored over the placebo group.

Scavenging and release of REE and HFSE by Na-metasomatism in magmatic-hydrothermal systems

Exploitable or potentially exploitable deposits of critical metals,such as rare-earth(REE)and high-field-strength elements(HFSE),are commonly associated with alkaline or peralkaline igneous ***,the origin,transport and concentration of these metals in peralkaline systems remains poorly *** study presents the results of a mineralogical and geochemical investigation of the Na-metasomatism of alkali amphiboles and clinopyroxenes from a barren peralkaline granite pluton in NE China,to assess the remobilization and redistribution of REE and HFSE during magmatic-hydrothermal *** amphiboles and aegirine-augites from the peralkaline granites show evolutionary trends from sodic-calcic to sodic compositions,with increasing REE and HFSE concentrations as a function of increasing Na-index[Na^(#),defined as molar Na/(Na+Ca)ratios].The Na-amphiboles(i.e.,arfvedsonite)and aegirine-augites can be subsequently altered,or breakdown,to form hydrothermal aegirine during late-or post-magmatic *** compositions analyzed by insitu LA-ICPMS show that the primary aegirine-augites have high and variable REE(2194-3627 ppm)and HFSE(4194-16,862 ppm)contents,suggesting that these critical metals can be scavenged by alkali amphiboles and *** to the primary aegirine-augites,the presentative early replacement aegirine(Aeg-I,Na^(#)=0.91-0.94)has notably lower REE(1484-1972)and HFSE(4351-5621)*** contrast,the late hydrothermal aegirine(Aeg-II,Na^(#)=0.92-0.96)has significantly lower REE(317-456 ppm)and HFSE(6.44-72.2 ppm)*** that the increasing Na^(#)from aegirine-augites to hydrothermal aegirines likely resulted from Na-metasomatism,a scavenging-release model can explain the remobilization of REE and HFSE in peralkaline granitic *** scavenging and release of REE and HFSE by Na-metasomatism provides key insights into the genesis of globally significant REE and HFSE *** high Na-index of th
