Laparoscopic gastrectomy has evolved differently in Eastern and Western countries.Feasibility,safety and oncological outcomes of laparoscopic gastrectomy were addressed step-by-step by several randomized controlled trials from the East.Few phase III studies were published from the West that largely did not show a difference between the laparoscopic and open approach.Despite this,laparoscopic gastrectomy is seen as the standard for the surgical treatment of early and advanced gastric cancer in many European countries.Here,we review and comment on some important studies on laparoscopic gastrectomy for gastric cancer from Eastern and Western countries and also comment on current and future challenges.

The Liver Cancer Study Group of Japan(LCSGJ)recently published the first version of the clinical practice guidelines for intrahepatic cholangiocarcinoma(iCCA)(1).The study group followed the GRADE methodology for evidence-based medicine.Japan is a world leader in the treatment of cholangiocarcinoma,with Japanese medical centers realizing superior results compared to other developed countries and many landmark articles originating from these centers.While there are differences between the Japanese iCCA population and patients in other countries,these guidelines contain important lessons for healthcare providers across the world.

Living microtissues are used in a multitude of applications as they more closely resemble native tissue physiology,as compared to 2D cultures.Microtissues are typically composed of a combination of cells and materials in varying combinations,which are dictated by the applications’design requirements.Their applications range wide,from fundamental biological research such as differentiation studies to industrial applications such as cruelty-free meat production.However,their translation to industrial and clinical settings has been hindered due to the lack of scalability of microtissue production techniques.Continuous microfluidic processes provide an opportunity to overcome this limitation as they offer higher throughput production rates as compared to traditional batch techniques,while maintaining reproducible control over microtissue composition and size.In this review,we provide a comprehensive overview of the current approaches to engineer microtissues with a focus on the advantages of,and need for,the use of continuous processes to produce microtissues in large quantities.Finally,an outlook is provided that outlines the required developments to enable large-scale microtissue fabrication using continuous processes.

Background There is increasing epidemiological evidence supporting the association between onchocerciasis and seizures,reinforcing the concept of onchocerciasis-associated epilepsy(OAE).The aim of this paper is to provide an update on the new knowledge about OAE and to propose recommendations to the World Health Organization how to address this public health problem.Main text During the 2nd International Workshop on OAE held on 19-21 September,2023,in Antwerp,Belgium,participants recognised OAE as a substantial yet neglected public health problem,particularly in areas of sub-Saharan Africa where onchocerciasis remains hyperendemic.Evidence from prospective population-based studies suggest that strengthening onchocerciasis elimination efforts leads to a significant reduction of OAE incidence.There is a need to validate an OAE case definition to estimate the burden of disease and identify onchocerciasis-endemic areas requiring intensification of onchocerciasis elimination programmes and integration of epilepsy care t is expected that raising awareness about OAE will boost the population uptake of ivermectin.The implementation of a commu-nity-based epilepsy treatment programme offering free anti-seizure medications(ASMs)has shown high effectiveness in reducing the frequency of seizures and improving the overall quality of life of people with epilepsy.Conclusions To reduce OAE burden,enhanced collaboration between onchocerciasis and mental health pro-grammes at community,national,and international levels is required.Urgent efforts are needed to ensure the uninter-rupted provision of free ASMs in onchocerciasis-endemic areas.Furthermore,OAE should be included in the quantif-cation of the onchocerciasis disease burden.

Protective Effects of Lycopene on Furan-treated Diabetic and Non-diabetic Rat Lung

We assessed the effects of furan and lycopene on the histopathological and biochemical changes on lungs, body and lung weights, and food consumption of rats. Furan and diabetes caused histopathological changes, increment in malondialdehyde levels, and decrease in antioxidant enzyme activities. Lycopene showed a protective effect against these damages, except for glutathione-S-transferase and glutathione peroxidase activities. Consequently, furan and diabetes resulted in lung toxicity. Our findings demonstrate that furan treatment resulted in more alterations in histology and biochemical parameters in diabetic rats and lycopene showed protective effects against these alterations.

Esophageal cancer(EC) remains one of the most common and aggressive diseases worldwide.This review discusses some debates in the modern management of the disease.Endoscopic procedures for early cancer(T1a-b) are now embedded in routine care and the challenge will be to more accurately select patients for endoscopic resection with or without adjuvant therapy.Perioperative multimodal therapies are associated with improved survival compared to surgery alone for locally advanced esophageal cancer.However, there is no global consensus on the optimal regimen.Furthermore, histological subtype(adenocarcinoma vs.squamous cell cancer) plays a role in the choice for treatment.New studies are underway to resolve some issues.The extent of the lymphadenectomy during esophagectomy remains controversial especially after neoadjuvant chemoradiation.The ideal operation balances between limiting surgical trauma and optimizing survival.Minimally invasive esophagectomy and enhanced recovery pathways are associated with decreased morbidity and faster recovery albeit there is no consensus yet what approach should be used.Finally, immune checkpoint inhibitors present promising preliminary results in the novel treatment of advanced or metastatic EC but their widespread application in clinical practice is still awaited.

Background:Adrenal insufficiency(AI)is a wellknown cause of potentially life-threatening disorders.Defects at each level of the hypothalamic-pituitary-adrenal axis can impair adrenal function,leading to varying degrees of glucocorticoid(GC)deficiency.Iatrogenic AI induced by exogenous GCs is the most common cause of AI.The criteria for the diagnosis and management of iatrogenic AI,neonatal AI,and critical illness-related corticosteroid insuffi ciency(CIRCI)are not clear.Data sources:We reviewed the recent original publications and classical data from the literature,as well as the clinical,diagnostic and management strategies of pediatric AI.Results:Practical points in the diagnosis and management of AI with an emphasis on iatrogenic AI,neonatal AI,and CIRCI are provided.Given the lack of sensitive and practical biochemical tests for diagnosis of subtle AI,GC treatment has to be tailored to highly suggestive clinical symptoms and signs.Treatment of adrenal crisis is well standardized and patients almost invariably respond well to therapy.It is mainly the delay in treatment that is responsible for mortality in adrenal crisis.Conclusion:Education of patients and health care professionals is mandatory for timely interventions for patients with adrenal crisis.

Adherence to surveillance guidelines for dysplasia and colorectal carcinoma in ulcerative and Crohn's colitis patients in the Netherlands

AIM: To study adherence to the widely accepted surveillance guidelines for patients with long-standing colitis in the Netherlands. METHODS: A questionnaire was sent to all 244 gastroenterologists in the Netherlands. RESULTS: The response rate was 63%. Of all gastroenterologists, 95% performed endoscopic surveillance in ulcerative colitis (UC) patients and 65% in patients with Crohn's colitis. The American Gastroenterological Association (AGA) guidelines were followed by 27%, while 27% and 46% followed their local hospital protocol or no specific protocol, respectively. The surveillance was correctly initiated in cases of pancolitis by 53%, and in cases of left-sided colitis by 44% of the gastroenterologists. Although guidelines recommend 4 biopsies every 10 cm, less than 30 biopsies per colonoscopy were taken by 73% of the responders. Only 31%, 68% and 58% of the gastroenterologists referred patients for colectomy when low-grade dysplasia, high-grade dysplasia (HGD) or Dysplasia Associated Lesion or Mass (DALM) was present, respectively. CONCLUSION: Most Dutch gastroenterologists performendoscopic surveillance without following international recommended guidelines. This practice potentially leads to a decreased sensitivity for dysplasia, rendering screening for colorectal cancer in this population highly ineffective.
