Clinico-Angiographic Profile and Prevalence of Restenosis in Patients Undergoing Percutaneous Transluminal Coronary Angioplasty to Left Main Coronary Artery: An Observational Cohort Study

Background: Patients who underwent percutaneous coronary intervention in left main coronary artery (LMCA) requires special concern, being high risk and increasing patient population. The aim of this study was to assess the clinical profile, angiographic status, and prevalence of restenosis in patients who underwent percutaneous transluminal coronary angioplasty (PTCA) in LMCA. Materials and Methods: This observational cohort study included 17 patients who underwent PTCA in LMCA during one-year study period at tertiary care centers in Government Medical College, Kozhikode, India. Data including various risk factors, clinical and angiographic details, stent used, procedural complications and outcomes including rate of restenosis were analyzed. Results: A total of 17 patients (mean age 53.88 ± 9.80 years) with 76.47% of males were included in the study. Smoking and hypertension were the most common risk factors presented in 52.94% and 47.06% of patients respectively. Single vessel disease (SVD) of LMCA was the most common pattern observed in 47.10%;the rate of restenosis was observed in 11.76% patients. Revascularization was performed in one patient (5.88%) with coronary artery bypass graft and in one patient (5.88%) with PTCA using drug eluting stent (DES). The overall procedural success was 88.24% in this study. Survival rate was 100% at one-year follow-up period. Conclusion: Our study involved patients who underwent PTCA in LMCA, showed smoking as a most prevalent risk factor for coronary artery disease and SVD as a most common pattern, comparatively low rate of restenosis and 100% of survival rate at one-year follow-up period.

Sexual Behaviour among Students in a Tertiary Educational Institution in Southeast Nigeria

Background: Sexual behaviour is the manner in which humans experience and express their sexuality. When it is risky, it can lead to untoward health consequences. Objective: The study was undertaking to assess the sexuality pattern of 276 students from a tertiary educational institution in Anambra state of south eastern Nigeria to determine the pattern of risk-behavior among them. Subjects/Methods: This is a cross-sectional questionnaire based study of undergraduates in a tertiary educational institution. Results: Majority of the respondents 190/276 (68.8%) are sexually active. Most of the sexually active respondents are unmarried 156 (82.1%) while only 34 (17.9%) were married. A considerable number of the respondents 34 (17.9%) had 2 to 5 sex partners in the past one year while 26 (13.7%) had at least 6 sex partners the past year also. Interestingly, 140 (73.7%) respondents were having sex for pleasure and while only 14 (7.4%) had sex for money. Conclusion: Majority of the undergraduates in this study indulge in premarital sexual intercourse and usually often with multiple partners. This constitutes high risk sexual behavior. Recommendations: Safer-sex sexuality education is recommended for students in tertiary educational institutions to minimize this ugly trend.

Laparoscopic Pyelolithotomy: Initial Experience with 62 Patients

Objective: To evaluate the role of the laparoscopic approach in the management of pelvic calculi in a urology centre in Douala, Cameroon. Materials and Methods: This was a retrospective study carried out from 2014 to 2019 involving 62 patients with calculi at the renal pelvis managed through laparoscopic pyelolithotomy. Results: We recruited 62 patients (38 men and 24 women) with a median age of 36 [25 - 48] years. Nephritic colic was the most frequently encountered presenting complaint (92% of cases). In 56.45% of cases, the stone was on the left side of the body. Twelve patients had kidney acute failure at the time of diagnosis. Five patients received antibiotics for urinary tract infections before the operation. Before surgery, eight double J stents were placed in 8 patients;in five because of urinary tract infection and in three because of nephritic colic. In all the cases, the definitive diagnosis was made using abdominal CT scans. The median size of the calculi was 22 [17 - 28] mm. Dilatation of the pyelocaliceal cavity was noticed in all the patients. The median surgery duration was 100 [90 - 120] minutes. Drainage of the urinary tract using a double J stent was performed in all the patients, whereas percutaneous drainage was performed only in 9.67% of cases. There was no case of conversion to open surgery. Control ultrasound performed one month after the removal of the double J stent did not reveal any residual stones in the pyelocaliceal cavities. Conclusion: Laparoscopic pyelolithotomy is a mini-invasive technique that is adequate for the management of voluminous pelvic calculi.

Evaluation of Extracorporeal Shockwave Lithotripsy in the Management of Renal and Ureteral Calculi

Aim: To evaluate the efficacy of extracorporeal lithotripsy in the management of renal and ureteric calculi in a urology center in Douala, Cameroon. Materials and Methods: This is a retrospective study carried out over six years, between January 2014 and December 2020. all the patients were treated using a Direx Integra lithotripter, with the number of shockwaves ranging from 1200 to 3500, without anaesthesia and were discharged a few hours after the procedure on the same day. In a majority (63.75%) of the cases, the calculi were incidental findings. A Double-J stent was indicated in two patients and preceded extracorporeal lithotripsy because of renal colic and signs of urinary tract infection. Results: We recruited a total of 122 patients with a mean age of 42.19 ± 13.08. We had 65 (53.3%) males and all patients had at least one calculus confirmed by CT scan with a mean size of 13.84 ± 4.17 mm, 85 (69.7%) patients became completely stone-free after a maximum of four sessions of extracorporeal lithotripsy (ESWL). 21 (17.2%) patients had intermediate results, being asymptomatic and/or having less than three residual fragments that measured less than 4 mm. The failure rate was 13.9%, with 17 patients still having more than three fragments measuring more than 4 mm after 4 ESWL sessions. 1 (0.8%) had septic shock as a post ESWL complication while 6 (4.9%) benefitted from a complimentary medical and/or surgical treatment (double J stent placement). Conclusion: The management of renal and ureteral calculi through extracorporeal lithotripsy in adults seems to be particularly effective for renal calculi measuring less than 20 mm and ureteral calculi measuring less than 15 mm. Extracorporeal lithotripsy, which can be performed on an outpatient basis (and without anaesthesia) is associated with minimal complications, and remains the option of choice for most upper urinary tract calculi.

Endoscopic Management of Bladder Stones: Initial Experience at a Single Center in Cameroon

Purpose: This study aimed to evaluate the efficacy and safety of endoscopic lithotripsy with the lithoclast (EMS, Switzerland) and laser Holmium YAG in the management of bladder calculi. Materials and methods: This was a retrospective study carried out from January 2013 to December 2019 on 32 patients with bladder calculi. all the patients underwent either Lithoclast or Laser lithotripsy using a 22F Storz cystoscope at the Centre medico-chirugicale d’urologie in Douala, Cameroon. Data on patients’ ages, clinical symptoms, stone sizes, type of lithotripsy, surgery duration, and results of lithotripsy were collected and analyzed using Epi info 7. Results: We recruited 32 participants (24 men and 8 women) with a median age of 41.28 [22 - 68] years into this study. In 9 (28.12%) participants, macroscopic hematuria was the main presenting complaint, followed by lower urinary tract symptoms in 8 (25%) patients. A cystoscopy was performed in 17 (53.12%) patients to confirm the diagnosis of a bladder stone, and ultrasonography of the upper urinary tract was performed in 29 (90.6%) cases to certify the absence of another stone. Lithoclast EMS was used to manage the stones in 23 (71.87%) patients while laser lithotripsy was used in 9 (28.13%). A dormia basket was used to remove stone fragments in 10 (31.25%) patients. The mean surgery duration was 33.59 ± 14.2 minutes, and the bladder stones were successfully managed in all the participants of this study. Minor complications such as pain during micturition were found in 28 (87.37%) patients, with complete resolution occurring two weeks after surgery. Conclusion: Endoscopy with lithotripsy is a safe and effective method of management of bladder stones. This technique is also associated with short surgical procedures and postoperative hospitalization periods. We believe that it is an excellent treatment modality in the management of bladder stones.

Objective:This study aimed to determine the predictive ability of instructors’online caring behaviors,online learning burnout,and satisfaction with online learning on nursing students’academic ***:This was a cross-sectional study employing the administration of online self-repor t measures among 606 undergraduate students in one large nursing school in the *** analysis involved the use of descriptive statistics,Pearson’s r,and linear ***:The mean scores in the instructors’online caring behaviors scale,online learning burnout scale,online learning satisfaction scale,and self-repor ted academic performance were 3.88±0.70,4.00±0.84,2.51±0.96,and 3.25±0.79 ***(β=0.216,P=0.000),burnout(β=0.-098,P=0.019),and instructors’caring behaviors(β=0.096,P=0.030)explained 11.7%of the variability in the students’academic ***:Instructors’caring behavior and levels of burnout and satisfaction are linked to students’academic success in the vir tual learning *** study calls for nursing instructors to be mindful of signs of burnout,employ proactive measures that promote satisfactory learning,and foster more caring behaviors in this time of global health crisis.

Characterization of Defaulters from Tuberculosis Treatment in a Tertiary Hospital in South Eastern Nigeria

Background: Tuberculosis is second only to HIV/AIDS as the greatest killer worldwide, due to a single infectious agent. Directly Observed Treatment Short-Course (DOTS) is presently the WHO recommended programme to fight tuberculosis worldwide. There is a need to understand the characteristics of patients who default from treatment for tuberculosis. This will help modify the strategies to reduce such default to the barest minimum and achieve higher levels of adherence. Objective: The aim of this study was to describe the characteristics of patients that defaulted from treatment for TB at Nnamdi Azikiwe University Teaching Hospital (NAUTH) Nnewi DOTS clinic for the period 1st January 2011 to 31st December 2012. Materials and Methods: This was a retrospective study conducted at the DOTS clinic at Nnamdi Azikiwe University Teaching Hospital Nnewi, Anambra State, Nigeria. The records of patients who received treatment from the clinic from 1st January 2011 to 31st December 2012 (2 years) were reviewed. The data collected include patients’ demographic characteristics, treatment category, patient type, baseline sputum smear result, and retroviral status. From the data, default rate was calculated and its relationship with other variables noted. Associations between patients’ characteristics were determined using chi square test of independence. The significance level was set at p = 0.05. Results: A total of 765 patients enrolled for TB treatment in the DOTS clinic of the study area within the study period of 1st January 2011 and December 31st 2012. The mean age at commencement of the treatment was 33.14 years (±18.09). The outcome of treatment showed that 260 (34%) had treatment completed, 230 (30.1%) cured, 120 (15.7%) defaulted, 103 (13.5%) died, 40 (5.2%) were transferred-out, and 12 (1.6%) failed in the treatment, giving a treatment success rate of 64.1%. Factors Influencing Sexual Behavior among Female Adolescents in Onitsha, Nigeria

Background: The sexual health indices of the adolescents are very poor. This study ascertained the factors influencing the sexual behaviour among female adolescents in Onitsha, Anambra state, Nigeria. Method: This was a cross sectional design in which a total of 800 female adolescents were selected from secondary schools in the area using multistage sampling method;and also selected from a major market (for those who were not at school) in the same geographical area using cluster sampling technique. Data were collected from the respondents about socio-demographic profile, reproductive and sexual health knowledge, attitude, behavior, sources of sexual health information. Result: The highest proportion of students who had ever had sex was found among the 16 - 17 years age group while the modal age group was 18 - 19 years for non students. also “not living with both parents” and poor family background, were markedly associated with increased likelihood to engage in sexual activity. Poverty level was particularly high among the non-student adolescents. Furthermore, wrong knowledge of fertile period, low risk perception of HIV and premarital, and use of condoms among the respondents were all significantly associated with increased chance to engage in sex. The student adolescents got sexuality information mainly from the teachers;as opposed to their non-student counterparts who had youth organization and friends as main sources. Conclusion: Family values, poverty alleviation, change of attitude through appropriate health and sexuality education especially targeting the non-student adolescents are highly recommended. Sexual negotiation skill, accessible formal education for all should be encouraged to create the right sexual behavior among the adolescent girls.
