The Effects of Different Type Vasodilator Agents on Radial Artery Blood Flow: A Placebo-Controlled Clinical Study

Background: The radial artery (RA) is being used as a second arterial conduit in coronary artery bypass grafting (CABG) patients. Vasospasm during the harvesting is the most common problem, yet. Our goal of this research is to evaluate the effects of different vasodilators on RA blood flow. Material and Method: In our 250 CABG patients who were included into the study, patients were divided into five groups which have the same number of the patients. Before the harvesting of the RA, Allen test and modified Allen test were performed. In group 1, sodium nitroprusside (SNP) was administered via central line. In group 2, nitrogliceryne was applied as a vasodilator agent. In group 3, diltiazem as a calcium channel blocker was used. In group 4, dobutamine was used with a dose of 3micro./kg/min. In the remaining patients, as a control group (CG), normal saline solution was used with the same protocol. RA length and radial artery flow, mean arterial blood pressure, central venous pressure, and heart rate were measured before and 10 minutes after drug administration. Results: No significant differences were found between the groups in respect to age, body surface area, cardiopulmonary bypass time, aortic cross clamp time. A multivariate general linear model was created and revealed the drug (specifically SNP and nitroglycerin) as a predictor to increase flow in the radial artery (p = 0.001 and p = 0.002), respectively. SNP has significantly decreased mean arterial pressure when we compared to nitroglycerin (p 0.05). Conclusions: Our study showed that SNP and nitroglycerin are very effective agents to provide the increasing of RA blood flow. However, statistical comparisons exhibited that intravenous SNP was a very effective vasodilating agent for RA than nitroglycerin.

Comparison of double balloon enteroscopy in adults and children

AIM: To compare results of double balloon enteroscopy (DBE) procedures in pediatric and adult patients. METHODS: The medical files of patients who underwent DBE at Gazi University School of Medicine, Ankara, Turkey between 2009 and 2011 were examined retrospectively. Adult and pediatric patients were compared according to DBE indications, procedure duration, final diagnosis, and complications. DBE procedures were performed in an operating room under general anesthesia by two endoscopists. An oral or anal approach was preferred according to estimated lesion sites. Overnight fasting of at least 6 h prior to the start of the procedure was adequate for preprocedural preparation of oral DBE procedures. Bowel cleansing was performed by oral administration of sennosides A and B solution, 2 mL/kg, and anal saline laxative en-ema. The patients were followed up for 2 h after the procedure in terms of possible complications. RESULTS: DBE was performed in 35 patients (5 pediatric and 30 adult). DBE procedures were performed for abdominal pain, chronic diarrhea, bleeding, chronic vomiting, anemia, and postoperative evaluation of anastomosis. Final diagnosis was diffuse gastric angiodysplasia (n = 1); diffuse jejunal angiodysplasia (n = 1); ulceration in the bulbus (n = 1); celiac disease (n = 1); low differentiated metastatic carcinoma (n = 1); PeutzJeghers syndrome (n = 1); adenomatous polyp (n = 1) and stricture formation in anastomosis line (n = 1). During postprocedural follow-up, abdominal pain and elevated amylase levels were noted in three patients and one patient developed abdominal perforation. CONCLUSION: With the help of technological improvements, we may use enteroscopy as a safe modality more frequently in younger and smaller children.

Hypersexuality after Modafinil Treatment： A Case Report

Modafinil is employed for the treatment of narcolepsy and other sleep disorders like idiopathic hypersornnia. Modafinil is involved in the modulation of orexin, a hypothalamic neuropeptide that regulates wakefulness. It also indicated that the drug interferes with the activity of additional neurotransmitters like hypocretin, histamine, gamma-aminobutyricacid, glutamate and norepinephrine. Modafinil can also block the dopamine transporter by increasing brain dopamine levels. Common side effects of modafinil include headache, nausea, nervousness, anxiety and trouble sleeping. Other side effects are affective lability, hyposexuality, personality change, aggression, abnormal dreams, irritability, suicidal ideas, mania, hypomania and psychotic exacerbation. In this case report, we present a 45 year old woman with idiopathic hypersomnia which is characterized primarily excessive day time sleepiness. Before modafinil treatment ESS （Epworth Sleepiness Scale） score of patient was 13. Treatment with Modafmil 200 mg daily within two weeks produced a subjective improvement in her daytime sleepiness （ESS score after treatment is 4）. But one week after the start of modafmil treatment, her sexual desire increased. Before this treatment, her sexual intercourse frequency was 1-2 times per week and after this treatment she wanted coitus every day. She has continual thoughts about sexuality. She didn＇t have any additional medication use or medical condition that increase sexual desire. Modafinil dose was decreased 100 mg/daythen to 50 mg/day. Her ESS score was 7 and daytime sleepiness had no effect on her life. After decreasing modafinil treatment her sexual desire returned to normal. Hypersexuality has not been reported as a side effect of modafinil yet. The pathophysiological mechanism of modafinil induced hypersexuality is not clear. In conclusion, clinicians should be aware of this rare significant side effect of modafinil and should maintain treatment with the lowest possible dos

Parotid mass, Metaplastic Whartin’s Tumor case report

The Whartin Tumor is the second most frequent tumor of the parotid gland following pleomorphic adenoma. Among the Whartin tumors, Metaplastic Whartin Tumor (MWT) is rare. In case of a MWT, histo-pathological replacement of oncosytic cells by squamous cells, ruptured epidermoid and lymphoe-pithelial cystic areas, a large necrosis area, fibrosis and granuloma formation may be seen. MWT may display similar clinical characteristics as the malign parotid tumor that causes sialadenitis, abscess formation, skin ulceration, and facial paralysis. Although the histopathogenesis of Metaplastic Whartin Tumors is not exactly known, it is postulated that the histo-pathological processes caused by trauma, infection, or radiotherapy have a role in the formation of these tumors. This case study presents a 48-year-old female patient with a complaint of panicula before her left ear persisting for 5 months. Following a superfacial parotidectomy carried out 3 weeks after the fineneedle aspiration biopsy (FNAB), her pathological assessment was found to be in line with Metaplastic Whartin Tumor. Our case study presents the case of a Metaplastic Whartin Tumor case with a review of literature on the subject, accompanied by radiological and histological analyses.

New Method for the Production of Barium Humate from Turkish Coal

Metal humates have been generally produced from water soluble alkaline humates with the related metal salts. In this study, a new method for the production of barium humate (Ba-HA) from Turkish coal was developed. Insoluble Ba-HA was produced from the result of the extraction of coal directly with barium hydroxide at 120℃. The effect of the amount of raw materials on the solubility and on the content of barium metal was investigated. The pilot plant trial was performed by using 100 kg of coal in one batch based on laboratory results. Ba-HA was characterized by means of FTIR and TGA. The calcination of Ba-HA at the different temperatures and in atmosphere of nitrogen and air was realized to provide the increase in the surface area and the decrease in the chromatographic groups giving yellow color. In terms of the surface area and UV-absorbance values, the calcination condition was selected as the temperature of 350℃ and atmosphere of air. The calcinated Ba-HA was used in the adsorption of some heavy metals. The results show that Ba-HA may have potential to be used as a low cost, natural and eco-friendly adsorbent.

Plasma catalase,glutathione-s-transferase and total antioxidant activity levels of children with attention deficit and hyperactivity disorder

Objective: In this study, we plan to measure plasma Catalase (CAT), Antioxidant Activity (AOA) and Glu- tathione-S-Transferase (GST) levels to understand whether oxidative stress develops or not and whether or not the detoxification mechanism properly functions in children with Attention Deficit and Hyperactivity Disorder (ADHD) with unknown etiology and pathogenesis. Method and Results: Plasma CAT, AOA, and GST activities were spectrophotometrically measured in forty patients (average age 10.27 ± 2.54) and thirty-five (average age, 9.97 ± 2.59) healthy individuals as the control group. While the CAT activity showed no difference in the patient group (P > 0.05) compared to the control group, AOA and GST levels were found significantly meaningful (P = 0.001). Conclusion: In this pilot study ,the study shows that no oxidative stress develops in individuals with ADHD in high AOA and stable CAT activity, and that the de- toxification mechanism functions extremely in high GST activity. These findings need to be supported by other studies.

Takayasu arteritis is a large vessel vasculitis of the young women with giant cells and granuloma formation. The diagnosis and management of the disease are really not so easy because of the insidious onset and the difficulties in assessment of disease activity. Nearly 60% of the patients are corticosteroid resistant or dependent and relapses are very frequent during taper of the dose [1]. The novel biologic agents as Anti-TNF, Rutiximab and Tocilizumab provide acceptable response rates with low toxicity. Herein, we reviewed the efficacy of Tocilizumab in Takayasu arteritis.

Herein, we combined the EULAR and ACR recommendations for clinicians and also pointed some important facts peculiar to our country.

The Bowing Parameters of Ca<sub>χ</sub>Mg<sub>1-χ</sub>O Ternary Alloys

On the basis of first principles calculations using density functional theory, we explore the structural and electronic properties of two binaries: CaO and MgO in rock salt structures. Structural properties of the semiconductor CaχMg1-χO alloys are derived from total-energy minimization within the General Gradient Approximation. The band gap bowing parameters dependence is very powerful Calcium composition. The results offer that an average bowing parameter of CaχMg1-χO alloys is b = ~0.583$ eV. We analyzed the volume deformation, charge transfer and structural relaxation effects of the CaχMg1-χO alloys.
