CD24 expression predicts distant metastasis in extrahepatic bile duct cancer

AIM: To evaluate the prognostic significance of CD24 expression in patients undergoing adjuvant chemoradiotherapy for extrahepatic bile duct (EHBD) cancer. METHODS: Eighty-four patients with EHBD cancer who underwent curative resection followed by adjuvant chemoradiotherapy were enrolled in this study. Postoperative radiotherapy was delivered to the tumor bed and regional lymph nodes up to a median of 40 Gy (range: 40-56 Gy). All patients also received fluoropyrimidine chemotherapy for radiosensitization during radiotherapy. CD24 expression was assessed with immunohistochemical staining on tissue microarray. Clinicopathologic factors as well as CD24 expression were evaluated in multivariate analysis for clinical outcomes including loco-regional recurrence, distant metastasisfree and overall survival. RESULTS: CD24 was expressed in 36 patients (42.9%). CD24 expression was associated with distant metastasis, but not with loco-regional recurrence nor with overall survival. The 5-year distant metastasis-free survival rates were 55.1% and 29.0% in patients with negative and positive expression, respectively (P=0.0100). On multivariate analysis incorporating N stage, histologic differentiation and CD24 expression, N stage was the only significant factor predicting distant metastasis-free survival (P=0.0089), while CD24 expression had borderline significance (P=0.0733). In subgroup analysis, CD24 expression was significantly associated with 5-year distant metastasis-free survival in node-positive patients (38.4% with negative expression vs 0% with positive expression, P=0.0110), but not in nodenegative patients (62.0% with negative expression vs 64.0% with positive expression,P=0.8599). CONCLUSION: CD24 expression was a significant predictor of distant metastasis for patients undergoing curative resection followed by adjuvant chemoradiotherapy especially for node-positive EHBD cancer.

Diel vertical migration of the copepod Calanus sinicus before and during formation of the Yellow Sea Cold Bottom Water in the Yellow Sea

To understand the effects of the Yellow Sea Cold Bottom Water （YSCBW） on the diel vertical migration （D- VM） of the copepod Calanus sinicus, we surveyed vertical distribution of C. sinicus at a fixed station in the Yellow Sea before （spring） and during （summer） formation of the YSCBW. Cold water （〈10℃） was observed in the bottom layer when the water column was thermally stratified in summer, but the water column was thermally well-mixed in spring 2010. Samples were collected from five different layers at 3-h intervals using an opening-closing net. Adult females （1-155 ind./m3） showed a clear normal DVM pattern throughout the entire water column in spring, whereas adult males did not migrate. DVM of copepodite V （CV） individuals was not clear, but the maximum abundance of CI-CIV occurred consistently in the upper 10-20 m layer, where there was a high concentration of chlorophyll-a （Chl-a）（0.49-1.19μg/L）. In summer, weak DVM was limited to cold waters beneath the thermocline for adult females （〈30 ind./m3）, but not for adult males. The maximum abundance of CI-CIV also occurred consistently in the subsurface layer （20-40 m） together with high concentrations of Chl-a （0.81-2.36 μg/L）. CV individuals （1-272 ind./m3） moved slightly upward noc- turnally to the near-surface layer （10-20 m）, where the average temperature was 25.74℃, but they were not found in the surface layer （0-10 m; 28.31℃）. These results indicate that the existence of the YSBCW affected food availability at depth and the vertical temperature distribution, leading to variation in the amplitude and shape of stage-specific vertical distributions （CI to adults） in C. sinicus before and during the formation of cold waters in the Yellow Sea during the study period.

Validation of Sea Level Data in the East Asian Marginal Seas: Comparison between TOPEX/POSEIdoN Altimeter and In-Situ Tide Gauges

In an effort to assess the reliability of satellite altimeter systems, the authors conduct a comparative analysis of sea level data that were collected from the TOPEX/POSEIdoN (T/P) altimeter and 10 tide gauges (TG) near the satellite passing ground tracks. The analysis is made using datasets collected from marginal sea regions surrounding the Korean Peninsula at T/P cycles of 2 to 230, which correspond to October 1992 to December 1998. Proper treatment of tidal errors is a very critical step in data processing because the study area has very strong tide. When the T/P data are processed, the procedures of Park and Gamberoni (1995) are adapted to reduce errors associated with the tide. When the T/P data are processed in this way, the alias periods of M2, S2, and K1 constituents are found to be 62.1, 58.7, and 173 days repectively. The compatibility of the T/P and TG datasets are examined at various filtering periods. The results indicate that the low-frequency signals of the T/P data can be interpreted more safely with longer filtering periods (such as up to the maximum selected value of 200 days). When RMS errors for the 200-day low-pass filter period are compared with all 10 tidal stations, the values span the range of 2.8 to 6.7 cm. The results of a correlation analysis for this filtering period also show a strong agreement between the T/P and TG datasets across all stations investigated (e.g.,p- values consistently less than 0.001). Hence according to the analysis, the conclusion is made that the analysis of surface sea level using satellite altimeter data can be made safely with reasonably extended filtering periods such as 200 days.

Clinical outcomes and safety of high-resolution manometry guided superficial partial circular muscle myotomy in per-oral endoscopic myotomy for Jackhammer esophagus: Two cases report

BACKGROUND Jack hammer esophagus is a relatively rare disease and to date,there is no dramatic treatment ***,conventional per-oral endoscopic myotomy (POEM) have expanded their area into Jackhammer ***,several complications such as post procedure motility disorders (e.g.,passage disturbance) are issues after *** overcome these issues,we here introduced high-resolution manometry (HRM)-guided superficial partial circular muscle myotomy,which involves cutting only the superficial layer of the esophageal circular *** SUMMARY We report two cases of patients with Jackhammer esophagus who were treated with HRM-guided extremely superficial partial circular muscle myotomy during *** 1 was a 53-year-old female with medication-refractory odynophagia and case 2 was a 47-year-old man who presented with chest *** were diagnosed with Jackhammer esophagus using HRM,and the hypercontractile segments of the esophagus were ***-guided extremely superficial partial circular muscle myotomy was performed while preserving the lower esophageal ***,the circular and longitudinal muscle layers are preserved but hypercontractile movements are reduced,even after ***’ clinical symptoms dramatically improved right after POEM,and 6-mo follow-up HRM revealed completely resolved *** a 1-year follow-up period,patients were still in good health and remained symptom *** HRM-guided superficial partial circular muscle myotomy may be a promising alternative to conventional POEM for treating Jackhammer esophagus with improved efficacy.

Tailored eradication vs empirical bismuth-containing quadruple therapy for first-line Helicobacter pylori eradication: A comparative,open trial
