Spatial-temporal distribution of neglected tropical diseases burdens in China from 2005 to 2020

Background Out of the 21 neglected tropical diseases (NTDs) listed by the World Health Organization, 15 affect the People’s Republic of China. Despite significant achievements in controlling NTDs, comprehensive assessments of the disease burden based on actual case data and detailed information on spatial and temporal dynamics are still lacking. This study aims to assess the disease burden and spatial-temporal distribution of NTDs in China from 2005 to 2020, to provide a reference for the formulation of national health agendas in line with the global health agenda, and guide resource *** The number of cases and deaths of major NTDs in China from 2005 to 2020 were downloaded from the China Public Health Science Data Center (https://***/Share/***) of the Chinese Center for Disease Control and Prevention and relevant literatures. Simplified formulas for disability-adjusted life years (DALYs) helped estimate the years of life lost (YLLs), years lived with disability (YLDs), and total DALYs. Spatial autocor-relation analysis of the average NTDs burden data for the years 2005 to 2020 was evaluated using Moran’sI *** China’s overall NTDs burden decreased significantly, from 245,444.53 DALYs in 2005 to 18,984.34 DALYs in 2020, marking a reduction of 92.27%. In 2005, the DALYs caused by schistosomiasis and rabies represent a substantial proportion of the total disease burden, accounting for 65.37% and 34.43% respectively. In 2015, Hunan and Sichuan provinces had the highest diversity of NTDs, with 9 and 8 number of different NTDs reported respectively. And the highest disease burden was observed in Sichuan (242,683.46 DALYs), Xizang Zizhiqu (178,318.99 DALYs) and Guangdong (154,228.31 DALYs). The application of Subcritical water technology in improving the bioavailability of hydrophobic water insoluble drugs, active pharmaceutical ingredients was reported in this these. In this research work, the solubility and the nanoparticles generation of the class Ⅱ anti-cancer anti androgen bicalutamide oral drug were studied at temperature ranging from 110℃ to 180℃ and constant pressure of 5.5 MPa. The experiments were performed using a modify solvent anti solvent method combined with a subcritical water technology. The chemical and physical characteristics including the stability and the crystallinity of the processed drug samples was analysed under X-Ray Diffraction （XRD） Technique followed by Fourier Transformed Infra-Red （FTIR） Techniques. Statistical analyses were performed on the experimental data. The solubility data obtained were correlated using a modify Apelblat model. The Van’t Hoff equation was used to determine the thermodynamic parameters including the molar enthalpy （ΔHsol）, molar entropy （ASsol）, and the molar Gibbs free energy from the experimental solubility *** nanoparticles were generated under various collection media temperatures and concentration conditions. Different pharmaceutical excipients/surfactants such as lactose monohydrate, polyethylene glycol, and sodium lauryl sulfate were used for the particles morphology, size and distribution control. The scanning electron microscopy （SEM） analysis was performed on the processed drug nanoparticles. The syntheses of nanoparticles up to 40 run with uniformity in morphology were achieved. The dissolution study of the synthesized nanoparticles drug was carried out. The size distribution analysis was also carried out using Image J software and the normal distribution was correlated using nonlinear （Gaussian function） *** results of the predicted solubility show good agreement with the experimental values proving accurate dependency of solubility to the temperature. Influence of Climatic Changes and Anthropogenic Activities on the Distribution and Habitats of Senegalia senegal in Niger: A Forecast and Ecological Analysis

The primary objective of this study is to assess the impact of climate change on the current and future spatial distribution of areas suitable for the growth of Senegalia senegal, a forest species of significant agro-ecological and socio-economic importance. To achieve this objective, the MaxEnt (Maximum Entropy) model was utilized, incorporating species presence points alongside bioclimatic variables. To project future distributions, three climatic models-CCCMA, HadCM3, and CSIRO were employed under the A2 scenario to predict the species’ distribution by the year 2050. Currently, habitats highly favorable for the conservation of the species are situated in the Sahelo-Sudanian and Sudanian zones, accounting for 6.81% of the national territory. All models forecast a substantial increase in habitats highly favorable for the future conservation of S. senegal, with a total expected growth of 448.9%. Conversely, moderately favorable and unfavorable habitats are predicted to decrease by 106.26% and 78.59%, respectively. These findings provide crucial data for the long-term conservation strategy of this species, which holds significant ecological and agronomic potential for the Sahelian region.

Alternative Phenotypic Profile for B-Cell Abnormality in a Case at Zinder National Hospital

Introduction: Since it is impossible to establish a diagnosis in the presence of hyperlymphocytosis not secondary to lymphocytic hyperactivation, we considered a B-lymphoid hematopathy with a non-specific phenotypic profile. We report one case of this. Observation: This is a forty-eight (48) year old patient with hyperlymphocytosis at 139,000 elements per cubic millimeter, heterogeneous splenomegaly at 25.6 cm in diameter on abdominal ultrasound without detectable deep or peripheral lymphadenopathy. Peripheral blood cytology shows lymphocyte cell proliferation suggestive of the circulating phase of chronic lymphoproliferative B syndrome. The expression profile of cell membrane markers did not allow for the definition of a specific phenotypic profile. Faced with this immunophenotyping result, we considered a B-lymphoid hemopathy with a non-specific phenotypic profile. After three courses, the MINICHOP treatment was used to achieve partial remission with wasting of more than 80% of the evaluable masses. Conclusion: Despite the contribution of immunophenotyping in the diagnosis of lymphoproliferative syndromes, it is possible to consider the diagnosis of a B-lymphoid hemopathy with a phenotypic non-specific profile of CD45+, monotypic kappa, CD19+, FMC7+, CD22+, CD5−, CD79b−, CD23−, CD43−, CD38−, CD200−.

Background: Niger is one of the countries that have the highest fertility rate in the world, with 7, 6 children per woman. Zinder is the most populated region of Niger with 5 million inhabitants that is 1/5 of Niger population. The demographic impact that has affected Niger the last 10 years: unemployment poverty and drug use, spawned violence of all kinds in Zinder region (physical and sexual violence). Sexual abuse is a major risk for poor psychiatric and physical outcomes. As sexual violence has serious consequences on the life and future of women and the community and it concerns our field of intervention, we initiated this study whose objective was to determine the socio-epidemiological and lesional aspects, the consequences and appropriate solutions of sexual violence in the Zinder. Methodology: This was a retrospective collection study of all cases of alleged sexual violence received over a 12-year period from 1 January 2009 to 30 June 2021 in Zinder’s referral maternity wards. Results: During the study period, 441 cases of sexual violence were collected;an annual average of 36 cases (441/12). The frequency is 1.29% on the 34,186 consultations. The mean age was 12.31 years with age extremes of 3 to 43 years. Students accounted for 25.39%. Street vendors were found in 224 cases or 50.79%. The predominant reason for admission is rape in 64.63%. The defloration was 52.15%. Pregnancy was found in 118 cases or 26.75%. HIV serology was positive in 1 victim. School dropout was 26.75%. Conclusion: The victims of sexual violence were minors, mostly street vendors and school children. The lack of means of repression at the time allowed the expansion of this violence. The consequences of sexual violence were severe. Advocacy has improved the phenomen on of sexual violence in Zinder.

Land Use and Gully Dynamics in the Kourfa Watershed, Matankari (Southwest Niger)

The main consequences of climate change in the Sahel have been the metamorphosis of surface conditions. These metamorphoses have resulted in surface degradation, of which silting up of watersheds is the main phenomenon. The objective of this study is to assess the environmental trends of the Kourfa pond watershed. The study is based on diachronic mapping with Landsat satellite images and Google Earth images, over the period 1986 to 2021. The study reveals that vegetation (whose rate of regression doubled between 1986 and 2021) has decreased to the benefit of crop areas (whose rate of increase multiplied by 3.61 between 1986 and 2021). Bare soil and encrusted areas have also decreased, with regression rates almost double than those of 1986. In addition, the Kourfa waterholes have experienced two types of changes over 35 years: one progressive between 2011 and 2016 and the other regressive between 2001 and 2021 compared to 1986. The ravine network has been multiplied by a factor of 2.4, with density more than doubled and the connectivity of the hydrographic networks has risen from 2 to 4, with significant bank recession. This dynamic of the Kourfa pond is linked to the high drainage, the increasing complexity of the gully network and the erosion due to the retreat of the watershed banks, all of which contribute to the silting-up of the Kourfa watershed.

Background: In developing countries maternal mortality and morbidity are most often due to the late to consult, poverty and ignorance. Added to this is, the under-medicalization of the health system. The objective of study is to report a case of late consultation and its consequences and make recommendations to improve: case of abdominal pregnancy with hemorrhagic placenta at the Health and Mother Center (CSME) in Zinder/Niger. Clinical Observation: We describe a case of abdominal pregnancy with hemorrhagic placenta in a 24-year-old woman, without profession, resident in the city of Zinder with a low economic level, consulted for abdomino-pelvic pain. She had a notion of amenorrhea for 5 months without any consultation made. She was pale, feverish and had abdominal pain. Ultrasound revealed a 23-week-old fetus and an empty uterus. The diagnosis was abdominal pregnancy. A laparotomy was performed and a The morbidity was hysterectomy and anemia. The postoperative period course was complicated with anemia. The patient was cured on the 9th day with a good vital prognosis. Conclusion: Abdominal pregnancy occurred in a patient who presented a risk factor. Ultrasound was the key diagnostic test. Surgery was the treatment of choice and the prognosis depends on the earliness of the consultation. Community awareness and early consultation with health services can improve the prognosis of abdominal pregnancy.

Pregnancy in a sickle cell patient is a source of maternal and perinatal morbidities. Thromboembolic diseases of which Deep Vein Thrombosis (DVT) is one of the complications. The frequency of the latter is poorly assessed. Very few studies have been done on this topic. We report a case of DVT in a 35-year-old patient with sickle cell SS pregnant with 27SA diagnosed and cared for at the Maternity Issaka Gazobi (MIG) in Niamey. The management was based on low molecular weight heparin combined with preventive measures against sickle cell anemia and a planned cesarean section which led to a favorable maternal and perinatal prognosis.

Analysis of Maternal Mortality at the Maradi Maternal and Child Health Center/Niger: About 379 Cases

Introduction: Pregnancy, childbirth, and their consequences continue to be the leading cause of death, illness, and disability among women of reproductive age in developing countries. In Niger, maternal deaths account for about 39% of all deaths of women aged 15-49. Our aim was to identify the factors linked to maternal mortality in order to contribute to its reduction. Method: this is a descriptive retrospective study of 379 cases of maternal deaths collected at the Maradi CSME from January 1, 2018, to December 31, ***: Our study’s maternal mortality ratio is 2645.72 per 100,000 live births. The mean age of our patients was 27.46 years, with extremes ranging from 15 to 47 years. The 20-24 age group was the most represented, with 88 cases (23.2%). Housewives were 361 (95.3%), and 334 patients (88.1%) were uneducated. There were 261 of them from rural areas, i.e. 69%. The mean parity in our study was 3.46, with extremes ranging from 0 to 15. Multiparous patients 117 cases (30.9%) represented the most significant proportion. Only 42 patients (11.08%) had performed four or more antenatal consultations. Direct obstetric causes accounted for 56.73% of the causes of maternal death. Anemia was the leading cause of maternal death (31.66%). Nearly half of the patients had died before 24 hours after admission, i.e. 49.86%. Conclusion: Maternal mortality remains a significant challenge for developing countries, given its magnitude and timid decline. The fight to reduce maternal mortality must be the priority of our countries’ health policies.
