Systematic review of cystic duct closure techniques in relation to prevention of bile duct leakage after laparoscopic cholecystectomy

AIM To study the effect of different techniques of cystic duct closure on bile leakage after laparoscopic cholecystectomy(LC) for biliary disease.METHODS A systematic search of MEDLINE, Cochrane and EMBASE was performed. Rate of cystic duct leakage(CDL) was the primary outcome. Risk of bias was evaluated. Odds ratios were analyzed for comparison of techniques and pooled event rates for non-comparative analyses. Pooled event rates were compared for each of included techniques.RESULTS Out of 1491 articles, 38 studies were included. A total of 47491 patients were included, of which 38683(81.5%)underwent cystic duct closure with non-locking(metal)clips. All studies were of low-moderate methodological quality. Only two studies reported separate data on uncomplicated and complicated gallbladder disease. For overall CDL, an odds ratio of 0.4(95%CI: 0.06-2.48)was found for harmonic energy vs clip closure and an odds ratio of 0.17(95%CI: 0.03-0.93) for locking vs non-locking clips. Pooled CDL rate was around 1% for harmonic energy and metal clips, and 0% for locking clips and ligatures. CONCLUSION Based on available evidence it is not possible to either recommend or discourage any of the techniques for cystic duct closure during LC with respects to CDL,although data point out a slight preference for locking clips and ligatures vs other techniques. No separate recommendation can be made for complicated gallbladder disease.

Association between serosal intestinal microcirculation and blood pressure during major abdominal surgery

Background:In clinical practice,blood pressure is used as a resuscitation goal on a daily basis,with the aim of maintaining adequate perfusion and oxygen delivery to target organs.Compromised perfusion is often indicated as a key factor in the pathophysiology of anastomotic leakage.This study was aimed at assessing the extent to which the microcirculation of the bowel coheres with blood pressure during abdominal surgery.Methods:We performed a prospective and observational cohort study.In patients undergoing abdominal surgery,the serosal microcirculation of either the small intestine or the colon was visualized using handheld vital mi-croscopy(HVM).From the acquired HVM image sequences,red blood cell velocity(RBCv)and total vessel density(TVD)were calculated using MicroTools and AVA software,respectively.The association between microcircula-tory parameters and blood pressure was assessed using Pearson’s correlation analysis.We considered a two-sided P-value of<0.050 to be significant.Results:In 28 patients undergoing abdominal surgery,a total of 76 HVM images were analyzed.The RBCv was 335±96μm/s and the TVD was 13.7±3.4 mm/mm ^(2).Mean arterial pressure(MAP)was 71±12 mm Hg during microcirculatory imaging.MAP was not correlated with RBCv(Pearson’s r=−0.049,P=0.800)or TVD(Pearson’s r=0.310,P=0.110).Conclusion:In 28 patients undergoing abdominal surgery,we found no association between serosal intestinal microcirculatory parameters and blood pressure.

Bile leakage after loop closure vs clip closure of the cystic duct during laparoscopic cholecystectomy:A retrospective analysis of a prospective cohort

BACKGROUND Laparoscopic cholecystectomy(LC)is one of the most frequently performed surgical procedures.Cystic stump leakage is an underestimated,potentially life threatening complication that occurs in 1%-6%of the patients.With a secure cystic duct occlusion technique during LC,bile leakage becomes a preventable complication.AIM To investigate the effect of polydioxanone(PDS)loop closure of the cystic duct on bile leakage rate in LC patients.METHODS In this retrospective analysis of a prospective cohort,the effect of PDS loop closure of the cystic duct on bile leakage complication was compared to patients with conventional clip closure.Logistic regression analysis was used to develop a risk score to identify bile leakage risk.Leakage rate was assessed for categories of patients with increasing levels of bile leakage risk.RESULTS Of the 4359 patients who underwent LC,136(3%)underwent cystic duct closure by a PDS loop.Preoperatively,loop closure patients had significantly more complicated biliary disease compared to the clipped closure patients.In the loop closure cohort,zero(0%)bile leakage occurred compared to 59 of 4223(1.4%)clip closure patients.For patients at increased bile leakage risk(risk score≥1)rates were 1.6%and up to 13%(4/30)for clip closure patients with a risk score≥4.This risk increase paralleled a stepwise increase of actual bile leakage complication for clip closure patients,which was not observed for loop closure patients.CONCLUSION Cystic duct closure with a PDS loop during LC may reduce bile leakage in patients at increased risk for bile leakage.

Improving the outcomes in oncological colorectal surgery

During the last several decades,colorectal cancer surgery has experienced some major perioperative improvements.Preoperative risk-assessment of nutrition,frailty,and sarcopenia followed by interventions for patient optimization or an adapted surgical strategy,contributed to improved postoperative outcomes.Enhanced recovery programs or fast-track surgery also resulted in reduced length of hospital stay and overall complications without affecting patient safety.After an initially indecisive start due to uncertainty about oncological safety,the most significant improvement in intraoperative care was the introduction of laparoscopy.Laparoscopic surgery for colon and rectal cancer is associated with better short-term outcomes,whereas long-term outcomes regarding survival and recurrence rates are comparable.Nevertheless,long-term results in rectal surgery remain to be seen.Early recognition of anastomotic leakage remains a challenge,though multiple improvements have allowed better management of this complication.

Effect of interlayer anions on chromium removal using Mg–Al layered double hydroxides:Kinetic,equilibrium and thermodynamic studies

The influence of interlayer anions such as NO3-, SO42-and Cl-on Mg–Al hydrotalcites for Cr(VI) removal from aqueous solution was studied. The structure of the prepared LDHs was characterized by XRD, SEM, FTIR, TGA, BET surface area and p Hzpc. The sorbent ability and sorption mechanisms were also investigated. The LDHs exhibit high removal for Cr(VI), and the sorbed amount depends on the nature of interlayer anion, which decreased in the following order: NO3-N Cl-N SO42-. Nitrate-containing LDH reached a Cr(VI) sorption equilibrium within only 30 min. The effects of operating conditions, including initial concentration, solution p H, agitation time and sorbent amount have been studied in batch mode. The optimum conditions were observed at an initial concentration of 100 mg·L-1, p H = 6, agitation time of 60 min and a sorbent dose of 2 g·L-1. The equilibrium data were fitted to the Langmuir, Freundlich and Dubinin–Radushkevich isotherm models. The Langmuir model was found to sufficiently describe the sorption process, offering a maximum sorption capacity of 71.91 mg·g-1. The sorption kinetic follows pseudo-second-order reaction with high accuracy. Thermodynamic parameters suggested that the sorption process is spontaneous and endothermic in nature.

OBJECTIVE:To characterize the chemical profile of methanolic crude extract and its fractions(Ethyl acetate,n-butanol and aqueous)using liquid chromatographymass spectrometry(LC-MS)analysis,to evaluate their biological and pharmacological properties:antioxidant(1,1-diphenyl-2-pycrylhydrazyl(DPPH),2,2′-azino-bis(3-ethylbenzothiazoline-6-sulphonic)(ABTS·+),galvinoxyle free radical scavenging,reducing power,phenanthroline andβcarotene-linoleic acid bleaching assays),enzymes inhibitory ability against several enzymes[acetylcholinesterase(AChE),buthyrylcholinesterase(BChE),urease and tyrosinase].METHODS:Secondary metabolites were extracted from Tamarix africana air-dried powdered leaves by maceration,the crude extract was fractionated using different solvents with different polarities(Ethyl acetate,n-butanol and aqueous).The amount of polyphenols,flavonoids and tannins(hydrolysable and condensed)were determined using colorimetric assays.A variety of in vitro biochemical tests were carried out to assess antioxidant and oxygen radical scavenging properties using DPPH,ABTS,galvinoxyle free radical scavenging,reducing power,phenanthroline andβcarotene-linoleic acid bleaching methods.Neuroprotective effect was examined against acetylcholinesterase and buthyrylcholinesterase enzymes.The anti-urease and antityrosinase activities were performed against urease and tyrosinase enzymes respectively.The extract’s components were identified using LC-MS and compared to reference substances.RESULTS:The results indicated that Tamarix africana extracts presented a powerful antioxidant activity in all assays and exhibited a potent inhibitory effect against AChE and BChE as well as urease and tyrosinase enzymes.LC-MS analysis identified amount of eight phenolic compounds were revealed in this analysis;Apigenin,Diosmin,Quercetin,Quercetine-3-glycoside,Apigenin 7-O glycoside,Rutin,Neohesperidin and Wogonin in methanolic extract and its different fractions of Tamarix africana from leaves.CONCLUSIONS:Based on these findings,it is reasonable t

SEIR-SW, Simulation Model of Influenza Spread Based on the Small World Network

This study modeled the spread of an influenza epidemic in the population of Oran, Algeria. We investigated the mathematical epidemic model, SEIR（Susceptible-Exposed-Infected-Removed）, through extensive simulations of the effects of social network on epidemic spread in a Small World（SW） network, to understand how an influenza epidemic spreads through a human population. A combined SEIR-SW model was built, to help understand the dynamics of infectious disease in a community, and to identify the main characteristics of epidemic transmission and its evolution over time. The model was also used to examine social network effects to better understand the topological structure of social contact and the impact of its properties. Experiments were conducted to evaluate the combined SEIR-SW model. Simulation results were analyzed to explore how network evolution influences the spread of desease, and statistical tests were applied to validate the model. The model accurately replicated the dynamic behavior of the real influenza epidemic data, confirming that the susceptible size and topological structure of social networks in a human population significantly influence the spread of infectious diseases. Our model can provide health policy decision makers with a better understanding of epidemic spread,allowing them to implement control measures. It also provides an early warning of the emergence of influenza epidemics.

Use of traditional medicine in Algeria as an alternative treatment against COVID-19 in the context of modern medicine: a survey-based study

Objective: The current survey-based study is interested in using traditional medicine as an alternative treatment against COVID-19 by the local population in Algeria. Methods: An online survey using Google form and a face-to-face interview were conducted. The addressed questions were concerned basically with the names of the most commonly-used plants, the reasons for using TM, preparation methods, and effectiveness of treatment based on TM. Results: Obtained answers showed a tendency of respondents to use traditional medicine against this pandemic by using particularly different medicinal plants. The main reasons for choosing the use of traditional medicine are its effectiveness (57.5%), the lack of side effects (36.7%) and being used to practicing it (31.7%), and to a lesser extent abundance and low cost of medicinal plants (10.8%). Leaves were mostly used for the treatment of COVID-19 in the form of infusions. Conclusion: The current study allowed us to identify the medicinal plants and other natural products like natural honey and olive oil used by the local population and to appreciate and know the traditional practices used in the prevention of COVID-19.

A Combinative Assembly Strategy Inspired Reversibly Borate-Bridged Polymeric Micelles for Lesion-Specific Rapid Release of Anti-Coccidial Drugs

Stimuli-triggered drug delivery systems hold vast promise in local infection treatment for the site-specific targeting and shuttling of drugs.Herein,chitosan conjugates(SPCS)installed with sialic acid(SA)and phenylboronic acid(PBA)were synthesized,of which SA served as targeting ligand for coccidium and reversible-binding bridge for PBA.The enhanced drug-loading capacity of SPCS micelles was attributed to a combination assembly from hydrophobicity-driving and reversible borate bridges.The drug-loaded SPCS micelles shared superior biostability in upper gastrointestinal tract.After reaching the lesions,the borate bridges were snipped by carbohydrates under a higher pH followed by accelerated drug release,while SA exposure on micellar surface facilitated drug cellular internalization to eliminate parasites inside.The drugmicelles revealed an enhanced anti-coccidial capacity with a higher index of 185.72 compared with commercial preparation.The dual-responsive combination of physicochemical assembly could provide an efficient strategy for the exploitation of stable,safe and flexible anti-infectious drug delivery systems.

Stimuli-responsive gel-micelles with flexible modulation of drug release for maximized antitumor efficacy

Engineered stimuli-responsive drug delivery devices hold vast promise in biological applications for disease treatment due to their maximized therapeutic efficacy In this study a novel, stably cross-linked, and pH-sensitive biodegradable gel-micelle was constructed with amphiphilic conjugates of trimethylene dipiperidine- methacrylic anhydride-hyaluronic acid-stearylamine （TMDP-MA-HA-SA, TMHS） to improve tumor-targeting with flexible intracellular delivery of paditaxel （PTX）. The cross-linked methacrylate bonds significantly improved the biostability of TMHS gel-micelle （~ 200 nm） over the non-cross-linked under physiological conditions, while hyaluronic acid plays an important role in active tumor targetability. The gradual degradation of cross-linked hyaluronic acid shell was triggered by the concentrated hyaluronidase. Meanwhile, under acidic conditions （pH 〈 6.5）, the tertiary amines of pH-sensitive TMDP moieties were protonated and thereby solubilized the gel-micellar core-portions. The resultant pH-triggered inner-core spaces rapidly prompted PTX release in the presence of multiple cytosolic enzymes that mainly degraded the remaining hydrophobic stearylamine core. During the in vitro cytotoxicity assay, PTX-loaded TMHS gel-micelles （ct~） revealed anticancer efficacy against human hepatocellular carcinoma HepG2 cells with ICs0 of 1.42 gg/mL （PTX concentration）, significantly lower than other groups. In parallel, the in vivo anti-tumor efficacy of CLTMHSptx gel-micelles against BALB/c xenograft tumor animal model demonstrated the greater tumor growth inhibition capacity of 72.06%, compared to other treatment groups at a safe concentration. Consequently, the cross-linked and stimuli-responsive CLTMHSFrx gel-micelles hold a great potential for flexible modulation of intracellular delivery of hydrophobic anticancer drugs with maximized antitumor efficacy.
