Purification of selenoprotein P from rat plasma has allowed detailed characterization of the protein from that species. Chromatographic studies have revealed the existence of at least two isoforms of the *** isoform is a truncated protein with termination of protein synthesis occurring at the second selenocysteine codon. Immunohistochernical studies have shown that the rat protein is associated with capillary endothelial cells in the liver, kidney,and brain. In vivo experiments were designed to study the relationship between selenoprotein P and lipid peroxidation. F2 isoprostanes were measured to quantitate the extent of lipid peroxidation caused by diquat. Using selenium-deficient rats supplemented with seleniurn, it was demonstrated that selenoprotein P appearance correlated with disappearance of diquat-induced lipid peroxidation. This finding is consistent with the protein serving to protect the plasma membrane from oxidative damage. Development of a radioimmunoassay to measure selenoprotein P has allowed assessment of the protein in humans. Measurements of the protein in selenium-deficient Chinese subjects indicated that it can be used as an index of selenium nutritional status in humans

BACKGROUND:Fatty liver is a common chronic liver disease *** is associated with an increasing morbidity in China in recent *** aim of this study was to analyze the effect of drinking alcohol on the hemoglobin and biochemical values of patients with fatty ***:We investigated the clinical and laboratory data of 669 patients with fatty *** the 669 patients,166 consumed alcohol more than 60 g per week for at least 2 years,and 503 did not have a history of long-term alcohol *** further analyzed the relationship between alcohol consumption and clinical characteristics of these ***:The values of aspartate transaminase (AST),gammaglutamyl transpeptidase (GGT),and hemoglobin in the long-term consumption group were significantly higher than those in the non long-term consumption group (P<0.05).In the patients without long-term alcohol consumption,the values of GGT and hemoglobin in patients with light alcohol consumption were significantly higher than those in non alcohol consumers (P<0.05).CONCLUSION:Alcohol consumption is associated with significantly increased values of AST,GGT,and hemoglobin in patients with fatty liver,suggesting their potential roles in hepatic steatosis.

BACKGROUND:Proximal migration of pancreatic stent (PMPS)is an infrequent event but its management can be technically challenging and there are no standard retrieval *** study aimed to determine the results of an endoscopic stent retrieval algorithm in terms of feasibility and efficacy of the endoscopic procedure. METHODS:During the period from January 2008 to December 2009,15 patients(8 women and 7 men with a mean age of 51.9 years)with PMPS were included in this *** retrieval was approached initially with balloon extraction followed by rat-tooth forceps and basket.A rescue approach such as using a stent retriever was attempted when other approaches failed. RESULTS:All the PMPSs(six 5Fr,nine 7Fr)were retrieved successfully within one ERCP *** extraction was successful in 9(60%)*** the 6 failed cases of balloon extraction,wire-guided rat-tooth forceps grasp was successful in 4,and stone extraction basket grasp was successful in 1 in whom forceps grasp *** stent was finally rescued with a stent retriever when balloon extraction,forceps and basket grasp all *** patients with successful balloon extraction,44.4% (4/9)developed post-ERCP hyperamylasemia but none of them developed post-procedure pancreatitis. CONCLUSIONS:With this algorithm,5Fr and 7Fr PMPS were successfully retrieved in all of the *** PMPSs can be safely retrieved with the commonly-used approaches in this *** less used approaches can be used as a rescue method.

Serum lipid profiles were determined in 2,703 male and 792 female offcials of the Electricity Generating Authority of Thailand (EGAT), aged 35-54 yrs, and 519 Ramathibodi Hospital Staff (RHS) consisting of 66 mcn and 453 women, aged 19-61 yrs. They are affluent-urban Thais. Mean serum total cholesterol (TC), low density lipoproteincholesterol (LDL-C) and triglyceride (TG) levels in EGAT officials were higher than those in RHS with the same sex, whereas the opposite result was observed for their serum high density lipoprotein-cholesterol (HDL-C) levels. Men also showed higher serum TC, LDL-C,and TG but lower HDL-C levels than women. The major type of dyslipidemia in EGAT officials and RHS was hypercholesterolemia due to elevated serum LDL-C levels, but the elevated LDL-C level in RHS was less severe than that in EGAT officials. Besides, the prevalence of serum HDL-C level of 1.55 mmol/L, a negative risk factor for coronary heart disease, in RHS was higher than that in EGAT officials. The mean percentages of dietary fat-calories consumed by EGAT officials and RHS were 39% and 31%, *** it is plausible that dietary fat intaly influenccs their serum lipid levels. The beneficial effects of linoleate intake on serum lipoprotein profiles are illustrated in RHS

AIM:To investigate the relationship between reflux and body mass index(BMI) in the asymptomatic obese population using the impedance-p H ***:Gastroesophageal reflux is frequent in the obese ***,the relationship between acid reflux and BMI in asymptomatic obese people is ***-six obese(BMI > 25 kg/m2) people were enrolled in this prospective *** evaluated the demographic findings and 24-h impedance p H values of the whole ***,acid(p H 30 kg/m2).Demographic and endoscopic findings,and impedance results were similar in these two ***,there was a positive correlation between BMI and total and supine p H<4 episodes(P=0.002,r=0.414;P=0.000,r=0.542),p H<4 reflux time(P=0.015,r=0.319;P=0.003,r=0.403),and De Meester score(P=0.012,r=0.333).CONCLUSION:Acid reflux is correlated with BMI in asymptomatic obese individuals.

The immune system has evolved a variety of regulatory mechanisms to ensure the peripheral self-tolerance as well as the optimal capacity to elicit effective anti-infection immunity. At present, there is no satisfactory conceptual framework to explain how the peripheral immunity is regulated at a biological system level, which enables the immune system to perform its essential functions to mount effective immunity to virtually any foreign antigens but avoid harmful immune responses to self. In this regard, during the past few years, an “affinity/avidity model of peripheral T cell regulation” has been proposed and tested, which opens up a new paradigm to understand how the peripheral immunity, to both self and foreign antigens, is regulated. The paradigm is based on the discovery of a subset CD8^＋ T cells with TCRs which specifically recognize a unique set of self-peptides presented by the MHC class Ib molecule Qa-I differentially expressed on T cells as a function of the affinity/avidity of T cell activation. These Qa-1 restricted CD8^＋ T cells represent an example of how the immune system utilizes a unified mechanism to regulate adaptive immunity to both self and foreign antigens. Thus, by selectively down-regulating T cells of intermediate affinity/avidity, to any antigens, the immune system controls the adaptive immunity without the necessity to distinguish self from non-self in the periphery at the level of T cell regulation. AIM: The effect of ethacrynic acid on pancreaticexocrine secretion function and potential mechanisms ofinterference with the secreory process in pancreatic acinarcells were investigated. METHODS: After incubationwith ethacrynic acid for 30 min, caerulein-stimulatedamylase release and cholecystokinin (CCK) receptorbinding characteristics were assessed in isolated ratpancreatic acini. The level of thiol groups (glutathioneand protein thiols) and cytosolic free calcium weremeasured in pancreatic acinar cells. RESULTS:Ethacynic acid decreased caerulein (0. 1 nmol/L)-stimulated amylase release and the level of pancreaticacinar glutathione in a concentration-dependent fashionwithout a marked increase in cell damage. Ethacrynicacid also inhibited the caerulein (1 nmol/L)-inducedCa2+mobilization in pancreatic acinar cells. But neitherprotein thiol nor CCK-receptor binding characteristics wasaltered by ethacrynic acid. CONCLUSION: Ethacrynicacid inhibit pancreatic exocrine secretion by depletion ofglutathion
