Hypertension and Work Stress among City Hall Workers, Bohicon, Benin

Introduction: In the general population as well as in the workplace, hypertension is one of the main causes of morbidity and mortality in the world. The particularities of hypertension in the world of work remain little studied. The objective of this study was to determine the prevalence and factors associated with hypertension among workers in a town hall in Benin in 2019. Methods: This was a descriptive, analytical cross-sectional study conducted from December 2018 to January 2019. A comprehensive recruitment was conducted. Data were collected during a face-to-face interview, supplemented by measurement of anthropometric parameters and blood pressure as well as a work stress assessment. Frequencies were compared using the chi-square test and Fisher’s test. The significance level was 5%. Results: A total of 86 workers were included. Their mean age was 41.55 ± 9.82 years with a sex ratio of 6.1. The prevalence of hypertension was estimated to be 18.6% [95% CI: 11.96% - 29.75%]. Hypertension was higher in workers under work stress (41.18%) as opposed to non-stressed workers (14.49%) (p = 0.032);in those who were overweight (30.77) as opposed to non-overweight (2.94%) (p Conclusion: Work stress and obesity are modifiable factors associated with hypertension in workers at the Bohicon city hall. These factors deserve to be corrected by occupational health promotion and prevention actions.

Diabetes in Borgou Department in Benin: Prevalence and Associated Factors

An epidemic trend is observed in the evolution the epidemiology of diabetes mellitus worldwide. In a nationwide survey conducted in 2008, prevalence of diabetes was estimated to 2.6% in the hole Beninese population and to 4.6% in Borgou department representing the highest in the country. Aim: To determine the prevalence of diabetes mellitus in the adult population and investigate associated factors in Borgou department. Method: A survey was conducted from September 30th to November 20th 2011. In a random sampling using the STEP wise approach of surveillance of chronic diseases recommended by the World Health Organization, 4597 subjects over 18 years old (1935 men and 2662 women) were selected. After informed consent, every subject was submitted to the STEP questionnaire and anthropometric measures. Capillary blood was drawn in fast condition to determine glycaemia. Diabetes was defined by fasting glycaemia above 1.26 g/l. Chi square test was used for statistical analysis and difference was considered significant with p < 0.05. Results: Prevalence of diabetes was 12.4%. There was not a significant difference between men (8.5%) and women (9.4%), p = 0.33. Prevalence of previously known diabetes was 1.2% suggesting a large proportion of undiagnosed diabetes. Prevalence of diabetes increased significantly with age (p < 0.0001). There was no association with instruction level (p = 0.21). Other factors significantly associated with diabetes were place of residency (p < 0.0001), ethnicity (p = 0.002), marital status (p < 0.0001), obesity (p = 0.017) and high blood pressure (p < 0.0004). Conclusion: There is a rapid increase in diabetes prevalence in Borgou department in Benin confirming the epidemic trend of the disease. A large proportion of these diabetic subjects remained undiagnosed, so untreated and then, exposed to precocious chronic complications and precocious mortality. Epidemiological, Clinical and Etiological Aspects of Sciatica in Hospital Setting, CNHU-HKM Cotonou

Sciatica, mostly clinical-based diagnosis, is a potentially debilitating condition as a result of disco-radicular conflict between L4 and L5 or L5 and S1. This study aims at describing the epidemiological, clinical and etiological aspects of sciatica in Hubert Koutoukou Maga Teaching Hospital (CNHU-HKM) of Cotonou. It was a prospective, descriptive and analytical study conducted in CNHU-HKM Cotonou neurology department from 1st January to 30th June 2016. The study population comprised 115 low back pain patients seen in consultation. Data collection was carried out through a standard information sheet with socio-demographic, clinical, para-clinical, etiological and therapeutic data. Epi data 3.1 was used for data entry and Stata11 for processing. Frequency of sciatica was 18.6%, with 95% CI [11.5% - 25.7%]. The study population age ranged from 29 to 73 years, with 52 ± 16 as mean value, while sex ratio was estimated at 0.6. Bilateral sciatica representing 53% was present in L5 and S1 nerve roots in 44.4% of cases. It was exacerbated in prolonged standing (24.4%) and relieved in supine position (44.3%), with paresthesia (59.1%) and intermittent radicular claudication (40.9%). Spinal osteoarthritis was the most prevalent etiology (47%). Sciatica is becoming increasing prevalent and should be regarded as problem of health concern.

Etiologies of Ischemic Stroke in Sub-Saharan Africa, Case of Benin

The purpose of our study was to investigate etiologies of ischemic stroke in the neurology university clinic of CNHU-HKM, Cotonou. Method: It was a cross-sectional, prospective and analytical study carried out from 1st November 2014 to 31st August 2015 in the neurology university clinic of CNHU-HKM. We included all patients with stroke whose ischemic nature was confirmed through brain CT scan or magnetic resonance imaging. Data analysis was conducted with Census and Survey Processing System (CSPRO). We carried out bivariate then multivariate analysis in identifying associated factors. Threshold value is estimated at p Results: 104 ischemic stroke patients participated in the study, representing 59.4% of all types of stroke. Mean age was 61.9 ± 12.3 years [26 - 87 years] with 1.6 as sex ratio. Among vascular risk factors, hypertension accounted for 85.6% of stroke cases. 29.8% suffered from aphasia. Embolic heart diseases and atherosclerosis were predominant, in a proportion of 53% and 26% respectively. Atrial Fibrillation (AF) accounted for 82% of embolic heart diseases. Conclusion: Etiologic investigation of stroke is important and should be systematically carried out. This study enabled to corroborate atherosclerosis predominance in etiologic investigation of ischemic stroke. Patients’ outcome could be improved by promptly managing the etiology through adoption of appropriate treatment.

Short Term (3 Months) Prognosis of Stroke in Parakou

Background: The burden of stroke is very high in sub-Saharan Africa. The identification of the factors influencing poor outcome can help to reduce this burden. Objective: To study factors influencing the prognosis of stroke at Parakou. Methods: It was a prospective study conducted over two periods: inclusion period between 1st January and 30 June 2013;monitoring period of survivors between 1st July and 30 September. All consecutive patients admitted in the hospitals for stroke in the study period were included. The functional outcomes were assessed by the RANKIN scale (RANKIN > 2). Epi-info version 7 and SPSS version 16 were used for the statistical analyses. Results: We recruited 85 patients;mean age was 52 ± 15 years. The housewives and the unschooled represented respectively 33% and 65%. The mortality rates at 1 and 3 month were respectively 27% and 32%. Factors associated with mortality were female gender, stroke severity, disorders of consciousness, hyperthermia and hyperglycemia at admission, overweight, pneumonia, anemia, hyperleucocytosis, higher total and LDL serum cholesterol levels at 1 month;housewife status and pneumonia were associated at 3 month. 47% of survivors were independent at 1 month. Stroke severity and length of hospital stay were associated with disability at 1 month whereas previous stroke and disability history, stroke severity, weight and length of hospital stay were related with 3 month poor functional outcome. Conclusion: This study showed the poor outcome of stroke patients in Parakou which were influenced by many factors. Taking account of these factors in the strategies of care in the acute phase may improve the prognosis of stroke patients.

Prevalence of migraine among university students at Parakou,Benin:A cross-sectional study

Objective: To determine the prevalence of migraine and its associated factors in students at the University of Parakou. Methods: It was a cross-sectional study carried out from February 1st to April 30th 2011. It included all students registered during the academic year 2010-2011 and selected by systematic random sampling. Migraine was defined according to IHS 2004 criteria. To assess the link between migraine and body mass index we collected height and weight of each student. Multivariate analysis was used to study the factors associated with migraine, odds-ratio and their confidence interval were estimated. SPSS Soft-ware was used to perform statistical analysis. Results: During the study period 1200, students returned their questionnaires. They were 842 males and 358 females. 171 fulfilled migraine criteria, and the overall prevalence of migraine was 14.2% (CI 95%: 11.6% - 17.4%). The main associated factors were the sex with an OR: 2.3 (CI 95%: 1.6 - 3.2) and family history of headache with an OR: 1.6 (CI 95%: 1.1 - 2.4). Migraine with aura was the frequent form (59.1%) and visual aura the frequent form of aura. No association was found between migraine and body mass index but students with obesity had high risk of migraine with aura OR: 3.8 (CI 95%: 1.5 - 9.7) compared with those with normal weight. The main triggering factors were sleeplessness (90.1%) and mental fatigue (85.4%). Conclusion: The results suggest a high prevalence of migraine among students at Parakou University and no association between obesity and migraine.

Post-Stroke Epilepsy within a Teaching Hospital in Cotonou, Benin

Seizures occurring after stroke are risk factors for the onset of ensuing epilepsy. However, the incidence of vascular epilepsy is low. The purpose of this study was to study vascular epilepsy features in a teaching university. It was a retrospective, descriptive and analytical study, conducted from 1st January 2015 to 31st December 2016 in Cotonou Teaching Hospital (CNHU-HKM), Benin. It involved 35 patients brought together through comprehensive sampling. Vascular epilepsy hospital attendance rate was 2%. There were simple partial seizures in 74.28% of cases while cerebral ischemia was recorded in 44.44% of patients. Neurological consultation after stroke occurrence was a protective factor against the occurrence of vascular epilepsy with significant p at 0.001.

Factors associated with stroke direct cost in francophone West Africa, Benin example

Background: In sub-Saharan Africa, stroke constitutes a public health issue. Few studies were conducted to assess the cost involved in its treatment. Objective: To determine the factors involved in direct cost of stroke in Cotonou-Benin. Method: It consists in a transversal and prospective research of economic type with analytical and descriptive aim. It was conducted from 20thFebruary 2011 to 30thSeptember 2011. The research dealt with 122 stroke patients. With regard to the economic approach, bottom-up was the data collection technique which was adopted. Cost was estimated not only based on the patient himself/herself but considering societal aspect. Cost estimation period was hospitalization period. Data analysis was conducted via software such as Epi info and SPSS. Results: Overall expenses in terms of direct cost varied from $144.9 to $9393.9;average expenses were $1030.1 ± $101.7. Patients aged 50 and above had higher stroke hospitalization cost ($1277.4) than those aged below 50 ($857.4) p = 0.001;male patients made more expenses than females (FCFA 1157.5 against $831.8) p = 0.01;direct cost of stroke was increased in proportion to neurological deficit (score NIHSS) p = 0.043. This cost was higher in cases of hemorrhagic stroke than ischemic stroke (FCFA $1375 against $1098) p = 0.002. Stroke direct cost was also increased in proportion to severance of disability level of patients. Stroke type (hemorrhagic) and RANKIN score were firmly correlated to stroke hospitalization cost. Conclusion: Stroke is very expensive for patientsin Benin and they constitute a burden for both patients and their family. There is a great need to increase awareness regarding risk factor control in order to reduce the cost involved in treating this malady.

Primary Headache Disorders in HIV-Infected Patients within a Hospital Setting in Sub-Saharan Africa, Benin Case: Prevalence and Associated Factors

Introduction: Headache disorders are common in HIV-infected patients. These disorders are either primary or secondary. Objective: Determine prevalence and factors associated with primary headache disorders in HIV+ patients. Method: It was a cross-sectional, descriptive and analytical study conducted from 2nd May 2017 to 2nd August 2017 which included HIV+ patients aged 18 years and above. Diagnosis of primary headache disorders was carried out by a Neurologist on the basis of ICHD-II diagnostic criteria. The?dependent variable was primary headache disorders. Epi Info Version 7.***.1.0 and STATA11 were used for data analysis. P Results: Primary headache disorders prevalence was 25.2% (124/493).Tension-type headache represented 77.41%. HIV+ patients were aged 42.63 ± 10.14 years. Sex-ratio was estimated at 0.14. Factors associated with primary headache disorders in HIV+ patients were: male (0.36 [0.17 - 0.77];P = 0.008), Dendi ethnicity (14.8 [2.08 - 101.99];P = 0.007), trader (3.09 [01.25 - 7.62];P = 0.02), WHO clinical stage (3.20 [1.43 - 7.17];P = 0.005), screening duration (1.01 [1.00 - 1.01];P = 0.00), hypertension (10.28 [4.44 - 23.83];P ≤ 0.001). Conclusion: This study helped to determine the standard profile of HIV+ patient with primary headache disorders. It will contribute to save costs associated with paraclinical examinations, while in search of secondary etiologies within an African context with limited financial resources.

Introduction: Cognitive disorders frequency arising after a cardio-cerebral vascular disease (stroke) is currently on the rise due to the ageing population and the increase in the number of survivors after stroke occurrence. Objective: Determining post-stroke cognitive decline and identifying associated factors. Method: It consists in a cross-sectional, prospective, descriptive and analytical study which was conducted from April 1 to August 31, 2013 in the Neurology Department of CNHU-HKM in Cotonou. The study involved 100 patients who have known stroke for at least the past 6 months and were all admitted and discharged later on. The disease survivors were re-contacted and examined again at home or at hospital. The cognitive decline (CD) was estimated by using a modified and adapted MMSE to suit our cultural era. Results: All patients were aged 58.9 years ± 13.6 years. Sex-ratio was 1.4. Cognitive decline frequency was 20%. Post-stroke cognitive decline frequency per sex was 11.6% and 8.4% respectively for females and males. Ischemia stroke patients had a higher cognitive decline (22.5%). 58.3% had severe CD. Moreover the CD frequency increased with time, from 16.7% in a year to 50% in 3 years. Total MMSE varied from 6 to 23 with 21 as median. From the unvaried analysis, the presence of sphincter disorders (1.26 [0.35-4.59], p = 0.004), consciousness disorders (15.67 [1.46-168], p = 0.04), and convulsion disorders (3.77 [1.01 -14.00], p = 0.003) was associated with cognitive decline. From multi-varied analysis, the sole presence of convulsive attacks (11.5 [1.79-73.58], p = 0.01) was individually associated with post-stroke cognitive decline. Conclusion: This study reveals the importance of CD after stroke. The occurrence of convulsions at stroke acute stage is a prognostic factor of CD medium or long-term occurrence. The overall coverage improvement depends on the integration of these data in stroke reach-out programs.
