Solutions for submucosal injection: What to choose and how to do it

During the past decades, endoscopic resection techniques have gradually improved and gained more importance for the management of premalignant lesions and early cancers. These endoscopic resection techniques can be divided in 3 major groups: snare polipectomy, endoscopic mucosal resection(EmR) and endoscopic submucosal dissection(ESD). The use of submucosal injection is essential for the majority of EmR techniques and is an integral part of ESD,whereas during polipectomy it is not crucial in most cases except to prevent bleeding in large polyps and/or those with large stalks as an alternative to mechanical methods. Injection provides a lifting up effect of the lesion separating it from the muscular layer, thereby reducing thermal injury and the risk of perforation and bleeding while also facilitating en-bloc resection by improving technical feasibility. With this work, we aim to review the most common endoscopic resection techniques and the importance of submucosal injection in each one of them. for that, we present some of the most commonly used submucosal injection solutions, taking into account their advantages and disadvantages. We also discuss, based on current recommendations and our own experience, how and when to preform submucosal injection, depending on lesions features and endoscopic resection technique that′s being used, to assure complete resection and to prevent associated adverse events. finally, we also present and discuss some new proposed submucosal injection solutions,endoscopic resection techniques and devices that may have a major impact on the future of therapeutic endoscopy.

This paper proposes a generative approach for the automatic typesetting of books in desktop *** presented system consists in a computer script that operates inside a widely used design software tool and implements a generative process based on several typographic rules,styles and principles which have been identified in the *** performance of the proposed system is tested through an experiment which included the evaluation of its outputs with *** results reveal the ability of the system to consistently create varied book designs from the same input content as well as visually coherent book designs with different contents while complying with fundamental typographic principles.

Secondary forests and human-made forest gaps are conspicuous features of tropical ***,behavioral responses to these aspects of anthropogenically modified forests remain poorly ***,we analyze the effects of small human-made clearings and secondary forests on tropical bats by examining the guild-and species-level activity patterns of phyllostomids sampled in the Central Amazon,***,we contrast the temporal activity patterns and degree of temporal overlap of 6 frugivorous and 4 gleaning animalivorous species in old-growth forest and second-growth forest and of 4 frugivores in old-growth forest and forest *** activity patterns of frugivores and gleaning animalivores did not change between old-growth forest and second-growth,nor did the activity patterns of frugivores between old-growth forest and ***,at the species level,we detected significant differences for Artibeus obscurus(old-growth forest ***-growth)and ***(old-growth forest ***).The degree of temporal overlap was greater than random in all sampled ***,for frugivorous species,the degree of temporal overlap was similar between old-growth forest and second-growth;whereas for gleaning animalivores,it was lower in second-growth than in old-growth *** the contrary,forest clearings were characterized by increased temporal overlap between *** in activity patterns and temporal overlap may result from differential foraging opportunities and dissimilar predation ***,our analyses suggest that activity patterns of bats in second-growth and small forest clearings,2 of the most prominent habitats in humanized tropical landscapes,varies little from the activity patterns in old-growth forest.

Radiotherapy(RT)is the backbone of multimodality treatment of more than half of cancer *** new modern RT techniques,late complications may occur such as radiation proctitis(RP).The natural history of RP is *** symptoms may resolve spontaneously or require conservative *** the other hand,for similar and uncomplicated clinical contexts,symptoms may persist and can even be refractory to the progressive increase in treatment *** the last decades,an enormous therapeutic armamentarium has been considered in RP,including hyperbaric oxygen therapy(HBOT).Currently,the evidence regarding the impact of HBOT on RP and its benefits is *** prospective and randomised studies are necessary to validate HBOT’s effectiveness in the‘real world’clinical *** article reviewed the relevant literature on pathophysiology,clinical presentation,different classifications and discuss RP management including a proposal for a therapeutic algorithm with a focus on HBOT.

Supporting group decision-making in ubiquitous contexts is a complex task that must deal with a large amount of factors to succeed. Here we propose an approach for an intelligent negotiation model to support the group decision-making process specifically designed for ubiquitous contexts. Our approach can be used by researchers that intend to include arguments, complex algorithms, and agents' modeling in a negotiation model. It uses a social networking logic due to the type of communication employed by the agents and it intends to support the ubiquitous group decision-making process in a similar way to the real process, which simultaneously preserves the amount and quality of intelligence generated in face-to-face meetings. We propose a new look into this problem by considering and defining strategies to deal with important points such as the type of attributes in the multicriterion problems, agents' reasoning, and intelligent dialogues.

Chronic hepatitis C is a leading cause of morbidity and mortality, mainly related to fibrosis/cirrhosis and portal hypertension. Direct antiviral agents are highly effective and safe and can now cure > 90% of the patients. Sustained viral response (SVR) after interferon-based regimens has been associated with improvement in liver function, fibrosis and portal hypertension in a significant proportion of patients, although a point of no return seems to exist from which viral elimination is no longer capable of preventing portal hypertension progression and liver decompensation. Indeed, although SVR is associated with improvement of hepatic venous pressure gradients and therefore a decreased risk of de novo esophageal varices, several studies show that viral clearance does not eliminate the risk of variceal progression, liver decompensation and death in patients with pre-established portal hypertension. Although evidence about the effects of direct antiviral agents (DAAs) on clinically significant outcomes is still scarce and with short follow-up, DAAs can decrease the burden of the disease if patients are timely treated before significant fibrosis and portal hypertension develops. Studies with longer follow-up are waited to establish the real magnitude of hepatitis C treatment on portal hypertension. future studies should also focus on predictors of portal hypertension resolution since it can influence management and avoid unnecessary monitoring

In this review,the state of the art in understanding the structural phase relations in perovskite-structured BifeO_(3)-based polycrystalline solid solutions is presented and *** about the close relation between the structural phase and overall physical properties of the reviewed systems are pointed out and *** is shown that,by adjusting the structural symmetric arrangement,the ferroelectric and magnetic properties of BifeO_(3)-based polycrystalline solid solutions can be tuned to find specific multifunctional ***,an intrinsic mechanism linking structural arrangement and physical properties cannot be identified,revealing that this subject still deserves further discussion and investigation.

Green macroalgae,e.g.,Ulva lactuca,are valuable bioactive sources of nutrients;but algae recalcitrant cell walls,composed of a complex cross-linked matrix of polysaccharides,can compromise their utilization as feedstuffs for monogastric *** study aimed to evaluate the ability of pre-selected Carbohydrate-Active enZymes(CAZymes)and sulfatases to degrade *** cell walls and release nutritive compounds.A databank of 199 recombinant CAZymes and sulfatases was tested in vitro for their action towards *** cell wall *** enzymes were incubated with the macroalga,either alone or in combination,to release reducing sugars and decrease fluorescence intensity of Calcofluor White stained cell *** individual action of a polysaccharide lyase family 25(PL25),an ulvan lyase,was shown to be the most efficient in cell wall *** ulvan lyase treatment,in triplicate measures,promoted the release of 4.54 g/L(Pmono-and oligosaccharides release of 11.4 and 11.2 mmol/100 g of dried alga(Pf 41.7%(Pfluorescence,in comparison to *** ability of ulvan lyase treatment to promote the release of nutritional compounds from alga biomass was also evaluated.A release of some monounsaturated fatty acids was observed,particularly the health beneficial 18:1c9(Pficant release of total fatty acids(P>0.05),proteins(P?0.861)or pigments(P>0.05)was *** results highlight the capacity of a single recombinant ulvan lyase(PL25 family)to incompletely disrupt *** cell *** enzyme could enhance the bioaccessibility of *** bioactive products with promising utilization in the feed industry.

Atmospheric temperatures over northern regions of the world are rising at twice the global average,and one of the most conspicuous effects is the rapid and ongoing decline in summer sea ice in the Arctic *** improve understanding about the causes and consequences of this decline,the project‘multidisciplinary drifting Observatory for the Study of Arctic Climate’(https://***/)commenced in September 2019,under the auspices of the International Arctic Science Committee(IASC).This project is an ambitious year-round set of observations of multiple aspects of the Arctic Ocean,sea ice and atmosphere,involving five icebreakers(notably the German vessel Polarstern that was frozen into the Arctic pack ice),and 600 science personnel from more than 20 nations.
