Esophagogastric anastomosis in rats: Improved healing by BPC 157 and L-arginine, aggravated by L-NAME

AIM To cure typically life-threatening esophagogastric anastomosis in rats, lacking anastomosis healing and sphincter function rescue, in particular. METHODS Because we assume esophagogastric fistulas represent a particular NO-system disability, we attempt to identify the benefits of anti-ulcer stable gastric pentadecapeptide BPC 157, which was in trials for ulcerative colitis and currently for multiple sclerosis, in rats with esophagocutaneous fistulas. Previously, BPC 157 therapies have promoted the healing of intestinal anastomosis and fistulas, and esophagitis and gastric lesions, along with rescued sphincter function. Additionally, BPC 157 particularly interacts with the NOsystem. In the 4 d after esophagogastric anastomosis creation, rats received medication(/kg intraperitoneallyonce daily: BPC 157(10 μg, 10 ng), L-NAME(5 mg), or L-arginine(100 mg) alone and/or combined or BPC 157(10 μg, 10 ng) in drinking water). For rats underwent esophagogastric anastomosis, daily assessment included progressive stomach damage(sum of the longest diameters, mm), esophagitis(scored 0-5), weak anastomosis(m L H2 O before leak), low pressure in esophagus at anastomosis and in the pyloric sphincter(cm H2O), progressive weight loss(g) and mortality. Immediate effect assessed blood vessels disappearance(scored 0-5) at the stomach surface immediately after anastomosis creation. RESULTS BPC 157(all regimens) fully counteracted the perilous disease course from the very beginning(i.e., with the BPC 157 bath, blood vessels remained present at the gastric surface after anastomosis creation) and eliminated mortality. Additionally, BPC 157 treatment in combination with L-NAME nullified any effect of L-NAME that otherwise intensified the regular course. We consider GROUP CONTROL BY ADDING INDIVIDUALS(GCAI)in the setting of group identification for two procedural rules-the consensus-start-respecting rule and the liberal-start-respecting *** is known that GCAI for both rules are NP-hard,but whether they are fixed-parameter tractable with respect to the number of distinguished individuals remained *** resolve both open problems in the *** addition,we strengthen the NP-hardness of GCAI by showing that,with respect to the natural parameter the number of added individuals,GCAI for both rules are W[2]-***,the W[2]-hardness for the liberal-startrespecting rule holds even when restricted to a very special case where the qualifications of individuals satisfy the so-called consecutive ones ***,for the consensus-startrespecting rule,the problem becomes polynomial-time solvable in this special *** also study a dual restriction where the disqualifications of individuals fulfill the consecutive ones property,and show that under this restriction GCAI for both rules turn out to be polynomial-time *** reductions for showing W[2]-hardness also imply several algorithmic lowerbounds.

Light curve solutions of six eclipsing binaries at the lower limit of periods for W UMa stars

Photometric observations are presented in V and I bands of six eclipsing binaries at the lower limit of the orbital periods for W UMa stars. Three of them are newly discovered eclipsing systems. The light curve solutions reveal that all shortperiod targets are contact or overcontact binaries and six new binaries are added to the family of short-period systems with estimated parameters. Four binaries have com- ponents that are equal in size and a mass ratio near 1. The phase variability shown by the V-I colors of all targets may be explained by lower temperatures on their back surfaces than those on their side surfaces. Five systems exhibit the O＇Connell effect that can be modeled by cool spots on the side surfaces of their primary components. The light curves of V1067 Her in 2011 and 2012 are fitted by diametrically opposite spots. Applying the criteria for subdivision of W UMa stars to our targets leads to ambiguous results.

Since the initial coronavirus disease 2019(COVID-19)outbreak in China in December 2019,the infection has now become the biggest medical issue of modern *** major contributors that amplified the impact of the disease and subsequently increased the burden on health care systems were high mortality among patients with multiple co-morbidities and overcapacity of intensive care *** the gastroenterology-related community,particular concern was raised with respect to patients with inflammatory bowel disease(IBD),as those patients are prone to opportunistic infections mainly owing to their immunosuppressive-based ***,we sought to summarize current knowledge regarding COVID-19 infection in patients with ***,it seems that IBD is not a comorbidity that poses an increased risk for COVID-19 acquisition,except in patients treated with ***,outcomes of the infected patients are largely dependent on therapeutic modality by which they are treated,as some worsen the clinical course of COVID-19 infection,whereas others seem to dampen the detrimental effects of ***,we discussed the present and the future impact of COVID-19 pandemic and concomitantly increased health care burden on IBD-management.

Elevated serum values of procollagen Ⅲ peptide (PIIIP)in patients with ulcerative colitis who will develop pseudopolyps

AIM: To assess the impact of procollagen Ⅲ peptide as a marker of collagenesis in the development of pseudopolyps in patients with ulcerative ***: Development of pseudopolyps was monitored in 25 patients with ulcerative colitis classified according to Powell-Tuck index as mild (n=12) or moderate (n=13) form of disease. Patients with a mild form of disease were treated with oral mesalazine medication (2-4 g/day) and local mesalazine preparation (suppository). Patients with a moderate form of disease received oral mesalazine medication (2-4 g/day), local mesalazine preparation(suppository) and local methylprednisolone at an initial dose of 60 rag/day, followed by dose tapering. How many significant variables (previously determined by analysis of variance) were elevated in the groups with and without pseudopolyp developement was observed. ROC analysis for calculation of new index was ***: Serum values of procollagen Ⅲ peptide (PⅢP), C-reactive protein (CRP) and C4 complement component (C4)were statistically significantly lower in the group of patients flee from pseudopolyp development bhan bhose who developed one or more pseudopolyps (0.45±0.12 vs 1.42±0.70,P0.05).Discrimination function analysis yielded highest standardized cannon coefficients for PⅢP (0.876), CRP (0.104), C3 (-0.534) and C4 (0.184) (P<0.036). The elevation in two of three laboratory variables (PⅢP, CRP and C4) reached sensitivity of 93 % and specificity of 90 % in the development of ***: It is proposed that an increase in two of the three laboratory parameters (PⅢP, CRP and C4) could improve the accuracy of prediction of the development of pseudopolyps. The digestive system is one of the most common sites of malignancies in *** gastrointestinal tumors represent a massive global health burden both in terms of morbidity and health care expenditures,scientists continuously develop novel diagnostic and therapeutic methods to ameliorate the detrimental effects of this group of *** from the well-established role of the endoscopic ultrasound(EUS)in the diagnostic course of gastrointestinal and hepatobiliary malignancies,we have recently become acquainted with a vast array of its therapeutic possibilities.A multitude of previously established,evidence-based methods that might now be guided by the EUS emerged:Radiofrequency ablation,brachytherapy,fine needle injection,celiac plexus neurolysis,and endoscopic submucosal *** this review we endeavored to provide a comprehensive overview of the role of these methods in different malignancies of the digestive system,primarily in the treatment and symptom control in pancreatic cancer,and additionally in the management of hepatic,gastrointestinal tumors,and pancreatic cysts.

Immune evasion by Plasmodium falciparum parasites: converting a host protection mechanism for the parasite′s benefit

Immune evasion is a strategy used by pathogenic microbes to evade the host immune system in order to ensure successful propagation. Immune evasion is particularly important for the blood stages of Plasmodium falciparum, the causative agent of the deadly disease malaria tropica. Because Plasmodium blood stage parasites require human erythrocytes for replication, their ability to evade attack by the human immune system is essential for parasite survival. In order to escape immunity-induced killing, the intraerythrocytic parasites have evolved a variety of evasion mechanisms, including expansion of plasmodial surface proteins, organ-specific sequestration of the infected red blood cells and acquisition of immune-regulatory proteins by the parasite. This review aims to highlight recent advances in the molecular understanding of the immune evasion strategies by P. falciparum, including antigenic variation, surface protein polymorphisms and invasion ligand diversification. The review will further discuss new findings on the regulatory mechanisms applied by P. falciparum to avoid lysis by the human complement as well as killing by immune factors of the mosquito vector.

Potential of Forest Wood Biomass in Bulgaria, Market and Possibilities for Its Utilization

The total annual increment in forests in Bulgaria is about 14.5 Mio m^3 of the growing stock, of which the harvesting amounts at 5-6 Mio m^3 of fallen wood. An increase of the actually harvested quantities of wood is possible and it may rise up to 8-10 Mio m^3. The volume of forestry waste is 1,066,900 m^3, incl. economically accessible forestry waste are 373,400 m^3. Every year, there are about 3.0 Mio m^3 of wood which are obtained by the wood category, being 2.5 Mio m^3 of that same one used by the population as fire wood for stoves about 40% efficiency. In that connection, a map has been elaborated of the economically accessible forest-wood waste from the wood harvesting. Taking into account the data on the share of that wood, is considered capable for energy production, as well as the fact that a part of fire wood shall not be used in the conventional way but shall be processed, instead, that is to say, transformed into chips, pellets and briquettes, a map of category of woods has been made for the purposes and needs of this elaboration. There are some reserves as regards to thinning operations where small- and medium-sized wood is harvested and from temporarily inaccessible forest exploitation basins or the so called ＂closed basins＂ should arrive to. This paper proposes the most promising technologies for waste processing and transformation into chips, which could be applicable in Bulgaria and in some other countries as well.

The Effects of Dexmedetomidine Continuous Rate Infusion (CRI) on Isoflurane Anaesthesia in Healthy Horses

The concept of the modern anaesthesiological technique partial intravenous anaesthesia (PIVA) is by means of infusion of different pharmacological agents as a supplement to inhalation anaesthetics to reduce the concentration of volatile agents maintaining surgical anaesthesia and to decrease their noxious side effects mainly on cardiovascular and respiratory systems. Alpha-2 agonists are agents with frequent use in equine practice either as sedatives or in general anaesthesia PIVA protocols. The most selective amongst them, dexmedetomidine, is characterized by fewer side effects, lower doses, and fast elimination which make it appropriate for application as a continuous rate infusion (CRI). The purpose of this study was to trace out the effects of dexmedetomidine continuous rate infusion (CRI) as part of PIVA using isoflurane on volatile agent requirements, cardiovascular function, respiration and coagulation parameters, and recovery in healthy horses. Six healthy horses with average age 9.0 ± 5.1 year and mean body weight 247.7 ± 71.4 kg were subjected to either 3-hour lasting isoflurane or isoflurane-dexmedetomidine anaesthesia two weeks apart. The main clinical and anaesthesiological parameters were monitored in 10 minutes intervals. Electrolytes, acid-base, blood gases, and coagulation parameters were measured at the beginning and at the end of each anaesthesia. Recovery times and qualities were also recorded. The results showed that, the addition of dexmedetomidine by CRI at 1.75 μg·kg-1·hour-1 to isoflurane anaesthesia slightly reduced isoflurane requirement for maintenance of surgical anaesthesia but had negligible effects on the recovery time and quality. PIVA using dexmedetomidine and isoflurane produced respiratory acidosis similar to isoflurane anaesthesia alone but with significantly more pronounced hypoxaemia and hyperlactaemia. Both investigated anaesthesia protocols did not influence significantly haemocoagulation parameters.

Adherence to Mediterranean diet and advanced glycation endproducts in patients with diabetes

BACKGROUND In recent years,American Diabetes Association started to strongly advocate the Mediterranean diet(MD)over other diets in patients with diabetes mellitus(DM)because of its beneficial effects on glycemic control and cardiovascular(CV)risk *** levels of advanced glycation endproducts(AGEs)emerged as an indicator of CV risk in *** biopsy being invasive,the use of AGE Reader has been shown to reflect tissue AGEs *** To examine the association between adherence to MD and AGEs in patients with DM type *** This cross-sectional study was conducted on 273 patients with DM type II.A survey questionnaire was composed of 3 separate *** first part of the questionnaire included general data and the habits of the *** second part aimed to assess the basic parameters of participants’diseases and associated *** third part of the questionnaire was the Croatian version of the 14-item MD service score(MDSS).AGEs levels and associated CV risk were measured using AGE Reader(DiagnOptics Technologies BV,Groningen,The Netherlands).RESULTS A total of 27(9.9%)patients fulfilled criteria for adherence to MD,with a median score of 8.0(6.0-10.0).Patients with none/limited CV risk had significantly higher percentage of MD adherence in comparison to patients with increased/definite CV risk(15.2%vs 6.9%,P=0.028),as well as better adherence to guidelines for nuts(23.2%vs 12.6%,P=0.023)and legumes(40.4%vs 25.9%,P=0.013)*** number of patients with glycated hemoglobin(HbA1c)7%(14.9%vs 7.3%,P=0.045).Moreover,those patients followed the MDSS guidelines for eggs(33.0%vs 46.8%,P=0.025)and wine(15.6%vs 29.8%,P=0.006)consumption more *** score had significant positive correlation with disease duration(r=0.179,P=0.003)and negative correlation with body mass index(BMI)values(r=-0.159,P=0.008).In the multiple linear regression model,BMI(β±SE,-0.09±0.04,P=0.037
