A randomized controlled trial on triamcinolone versus saline impregnated merocel post endoscopic sinus surgery:Our experience in a tertiary care centre

Purpose:To assess the potential benefit of impregnating Merocel(a non-absorbable nasal dressing)with a topical steroid solution,for use as a direct and slow local delivery system of steroids after sinus surgery to improve postoperative wound ***:In this randomized controlled trial,40 patients with bilateral chronic rhinosinusitis with nasal polyposis were subjected to functional endoscopic sinus *** the completion of the surgery,Merocel packs were inserted in the bilateral nasal cavities and infiltrated with 4 mL triamcinolone(40 mg/mL)in one nasal cavity(treatment group)and 4 mL normal saline in the other(control group).Nasal packs were removed on the third postoperative day and postoperative healing assessment was done on postoperative Weeks 1,2,4,and *** findings were noted as per Lund Kennedy(LKES)and perioperative sinus endoscopy(POSE)scores and compared on both ***:Significant(P<0.05)improvement was noted in Lund Kennedy score for crusting and polyp at Week 12,for edema at Week 1,and nasal discharge at Weeks 1 and 12,but there was no significant improvement in scarring at any ***,the difference between the treatment and control arms was statistically significant at all postoperative visits except at Week ***,there was a significant improvement in POSE scores at Weeks 1,2,and 12 but not at Week ***:This study positively concludes that the nasal cavity packed with drug-soaked packs had less scarring and edema in the postoperative period and the overall wound healing was much better as compared to saline-soaked packs.

BACKGROUND With the introduction of transcatheter aortic valve replacement and an evolving understanding of the natural progression and history of aortic stenosis,the potential for earlier intervention in appropriate patients is promising;however,the benefit of aortic valve replacement in moderate aortic stenosis remains *** Pubmed,Embase,and the Cochrane Library databases were searched up until 30th of December 2021 using keywords including moderate aortic stenosis and aortic valve *** reporting all-cause mortality and outcomes in early aortic valve replacement(AVR)compared to conservative management in patients with moderate aortic stenosis were *** ratios were generated using random-effects meta-analysis to determine effect *** 3470 publications were screened with title and abstract review,which left 169 articles for full-text *** these studies,7 met inclusion criteria and were included,totalling 4,827 *** studies treated AVR as a time-dependent co-variable in cox-regression multivariate analysis of all-cause *** with surgical or transcatheter AVR was associated with a 45% decreased risk of all-cause mortality(HR=0.55[0.42-0.68],I2=51.5%,P<0.001).All studies were representative of the overall cohort with appropriate sample sizes,with no evidence of publication,detection,or information biases in any of the *** In this systematic review and meta-analysis,we report a 45% reduction in all-cause mortality in patients with moderate aortic stenosis who were treated with early aortic valve replacement compared to a strategy of conservative *** control trials are awaited to determine the utility of AVR in moderate aortic stenosis.

BACKGROUND Many Ayurvedic preparations are claimed to have immune-boosting properties,as suggested in various published randomized clinical trials(RCTs)AIM To compile evidence on the nature and mechanism of immune system enhancement by Ayurvedic preparations in healthy and sick *** After prospectively registering study protocol with PROSPERO,we searched PubMed,DOAJ,Google Scholar,three dedicated Ayurveda research portals,two specialty Ayurveda journals,and reference lists for relevant records published until February 6,2021 using appropriate search *** features and data pertaining to the nature and mechanism of immune system function were extracted from all eligible *** quality was assessed using the Cochrane RoB-2 *** Of 12554 articles screened,19 studies reporting 20 RCTs(17 parallel group design,three crossover design)with 1661 unique patients were included;11/19 studies had Indian first *** population was included in nine studies,of which one study included pregnant women and two included pediatric population;remaining studies included patients with different health conditions,including one study with coronavirus disease 2019 patients.A total of 21 Ayurvedic interventions were studied,out of which five were composite *** predominant route of administration was oral;dose and frequency of administration of the intervention varied across the *** results reported with five RCTs exploring five Ayurvedic interventions were incomplete,ambiguous,or *** the remaining 16 interventions,indirect evidence of immune enhancement was reported with four interventions,while lack of the same was reported with two *** of T helper cells and natural killer cells was reported with three and four interventions,respectively,while the pooled results did not clearly point toward enhancement of other components of the immune system,including cytotoxic T cells,B lymphocyte

Dear editor,Unilateral laterothoracic exanthem(ULE),also called superimposed lateralized exanthema or asymmetric periflexural exanthem of childhood,is mostly seen in children aged between 1-5 years[1].It is often preceded by a respiratory or gastrointestinal *** eruption is usually unilateral,spreading centrifugally,which may present as maculopapular,eczematous or scarlatiniform lesions[2,3].The proposed etiologies include viral infections such as adenovirus,parvovirus,Epstein-Barr virus and most recently,the SARS-CoV-19 virus[4,5].This entity is quite rare in *** we present a case of ULE in a COVID-19 positive adult *** condition is mostly self-limiting and requires symptomatic management.

Patterns of Herbal Combination for the Treatment of Insomnia Commonly Employed by Highly Experienced Chinese Medicine Physicians

Objective：To explore the most effective herbal combinations commonly used by highly experienced Chinese medicine（CM） physicians for the treatment of ***：We collected and analyzed data related to insomnia treatment from the clinics of 7 highly experienced CM physicians in *** sample included 162 patients and 460 consultations in *** outcomes,such as sleep quality and sleep time per day,were manually collected from the medical records by trained CM *** data mining methods,support vector machine（SVM）,logistic regression and decision tree,and multifactor dimensionality reduction（MDR）,were used to determine and confirm the herbal combinations that resulted in positive outcomes in patients suffering from ***：Results show that MDR is the most efficient method to predict the effective herbal *** the MDR model,we identified several combinations of herbs with 100%positive outcomes,such as stir-fried spine date seed,Szechwan lovage rhizome,and prepared thinleaf milkwort root;white peony root,golden thread,and stir-fried spine date seed;and Asiatic cornelian cherry fruit with fresh ***：Results indicate that herbal combinations are effective treatments for patients with insomnia compared with individual *** is also shown that MDR is a potent data mining method to identify the herbal combination with high rates of positive outcome.
