Cooperative laparoscopic endoscopic and hybrid laparoscopic surgery for upper gastrointestinal tumors: Current status

AIM: To investigate the cooperative laparoscopic and endoscopic techniques used for the resection of upper gastrointestinal ***: A systematic research of the literature was performed in Pub Med for English and French language articles about laparoscopic and endoscopic cooperative,combined,hybrid and rendezvous techniques. Only original studies using these techniques for the resection of early gastric cancer,benign tumors and gastrointestinal stromal tumors of the stomach and the duodenum were included. By excluding case series of less than 10 patients,25 studies were identified. The study design,number of cases,tumor pathology size and location,the operative technique name,the endoscopy team and surgical team role,operative time,type of closure of visceral wall defect,blood loss,complications and length of hospital stay of these studies were evaluated. Additionally all cooperative techniques found were classified and are presented in a systematic ***: The studies identified were case series and retrospective cohort studies. A total of 706 patients were operated on with a cooperative technique. The tumors resected were only gastrointestinal stromal tumors(GIST) in 4 studies,GIST and various benign submucosal tumors in 22 studies,early gastric cancer(p T1 a and p T1b) in 6 studies and early duodenal cancer in 1 study. There was important heterogeneity between the studies. The operative techniques identified were:laparoscopic assisted endoscopic resection,endoscopic assisted wedge resection,endoscopic assisted transgastric and intragastric surgery,laparoscopic endoscopic cooperative surgery(LECS),laparoscopic assisted endoscopic full thickness resection(LAEFR),clean non exposure technique and non-exposed endoscopic wallinversion surgery(NEWS). Helicobacter pylori (H. pylori) is the main cause of chronic gastritis and a major risk factor for gastric cancer. This pathogen has also been considered a potential trigger of gastric autoimmunity, and in particular of autoimmune gastritis. However, a considerable number of reports have attempted to link H. pylori infection with the development of extra-gastrointestinal autoimmune disorders, affecting organs not immediately relevant to the stomach. This review discusses the current evidence in support or against the role of H. pylori as a potential trigger of autoimmune rheumatic and skin diseases, as well as organ specific autoimmune diseases. We discuss epidemiological, serological, immunological and experimental evidence associating this pathogen with autoimmune diseases. Although over one hundred autoimmune diseases have been investigated in relation to H. pylori, we discuss a select number of papers with a larger literature base, and include Sjögrens syndrome, rheumatoid arthritis, systemic lupus erythematosus, vasculitides, autoimmune skin conditions, idiopathic thrombocytopenic purpura, autoimmune thyroid disease, multiple sclerosis, neuromyelitis optica and autoimmune liver diseases. Specific mention is given to those studies reporting an association of anti-H. pylori antibodies with the presence of autoimmune disease-specific clinical parameters, as well as those failing to find such associations. We also provide helpful hints for future research.

BACKGROUND Synovial sarcoma(SS) is a rare type of soft tissue sarcoma that is usually developed from areas where synovial tissue exists, especially at the ***, several cases of retroperitoneal SS(RSS) have been described. We herein report a case of RSS presented in our *** SUMMARY A 69-year-old female patient was admitted with a large, palpable, firm mass in the right abdominal space SS. Computerized tomography scan depicted a concentric, sharply marinated retro-peritoneal lesion which was displacing the right kidney and the lower edge of the liver. Subsequently, the patient underwent surgical excision of the mass with additional right nephrectomy and resection of the right adrenal gland and a part of the diaphragm. The final histological diagnosis of the tumour was grade II monophasic *** RSS is encountered in the biphasic type, the monophasic fibrous, and the monophasic epithelial category as well. Relevant clinical manifestations are not always documented at early stages. Therefore, the final diagnosis is posed after complete histological examination taking into consideration the results of immunochemistry and genetic analysis. Therapeutic approach happens often late when metastases at the lungs and the liver are apparent. Thus, 5-year survival rates remain low.

Exploring the barriers of utilizing theoretical knowledge in clinical settings: A qualitative study

Objectives: Although effective performance in clinical settings requires the integration between theory and practice,there is a gap between theoretical knowledge as taught in the classroom and what the students experience in clinical *** study aimed to elicit and explore the barriers of utilizing theoretical knowledge in clinical ***: A qualitative study was adopted with a conventional content analysis *** nursing and paramedic's students,faculty members and experienced nursing staff participated in the *** were collected by semi-structured individual interviews until data saturation and concurrently analyzed via MAXQDA ***: Five main categories emerged as barriers of utilizing theoretical knowledge in the clinical settings ***-standard practices in clinical settings;lack of trust in clinical competence;lack of perceived professional support;insufficiencies in teaching and learning process;and differences between doing things in simulated and real clinical ***: Transferring theory into practice in a structured manner requires professional support in the workplace,trust and the opportunity for direct experience,using valid and up-to-date knowledge by clinical staff and bridging the simulated situations with real life scenarios.

Multiplex RT-PCR-based detections of CEA, CK20 and EGFR in colorectal cancer patients

AIM: To develop a multiplex reverse transcription polymerase chain reaction (RT-PCR) method detecting cir-culating tumor cells in the peripheral blood of colorectal cancer (CRC) patients. METHODS: Peripheral blood samples were collected from 88 CRC patients and 40 healthy individuals from the blood donors' clinic and subsequently analyzed by multiplex RT-RCR for the expression of carcinoembryonic antigen (CEA), cytokeratin 20 (CK20) and epidermal growth factor receptor (EGFR) mRNA. The analysis involved determining the detection rates of CEA, CK20 and EGFR transcripts vs disease stage and overall survival. Median follow-up period was 19 mo (range 8-28 mo). RESULTS: Rates of CEA, CK20 and EGFR detection in CRC patients were 95.5%, 78.4% and 19.3%, respectively. CEA transcripts were detected in 3 healthy volunteer samples (7.5%), whereas all control samples were tested negative for CK20 and EGFR transcripts. The increasing number of positive detections for CEA, CK20 and EGFR transcripts in each blood sample was positively correlated with Astler-Coller disease stage (P< 0.001) and preoperative serum levels of CEA (P=0.029) in CRC patients. Data analysis using Kaplan-Meier estimator documented signif icant differences in the overall survival of the different CRC patient groups as formed according to the increasing number of positivity for CEA, CK20 and EGFR transcripts. CONCLUSION: These data suggest that multiplex RTPCR assay can provide useful information concerning disease stage and overall survival of CRC patients.

Fault Detection of Fuel Injectors Based on One-Class Classifiers

Fuel injectors are considered as an important component of combustion engines. Operational weakness can possibly lead to the complete machine malfunction, decreasing reliability and leading to loss of production. To overcome these circumstances, various condition monitoring techniques can be applied. The application of acoustic signals is common in the field of fault diagnosis of rotating machinery. Advanced signal processing is utilized for the construction of features that are specialized in detecting fuel injector faults. A performance comparison between novelty detection algorithms in the form of one-class classifiers is presented. The one-class classifiers that were tested included One-Class Support Vector Machine (OCSVM) and One-Class Self Organizing Map (OCSOM). The acoustic signals of fuel injectors in different operational conditions were processed for feature extraction. Features from all the signals were used as input to the one-class classifiers. The one-class classifiers were trained only with healthy fuel injector conditions and compared with new experimental data which belonged to different operational conditions that were not included in the training set so as to contribute to generalization. The results present the effectiveness of one-class classifiers for detecting faults in fuel injectors.
