Introduction: The choice of adopting unilateral pedicle screw fixation or using bilateral pedicle screw fixation in lumbar spinal stenosis remains controversial. In our context, very few studies have been performed comparing the clinical effectiveness of unilateral versus bilateral fixation in the surgical management of lumbar spinal canal stenosis. Objective: Evaluate the impact on quality of life and clinical efficacy of unilateral spondylodesis compared to bilateral spondylodesis in the surgical management of lumbar spinal canal stenosis at the Yaounde Central Hospital. Methods: This was a retrospective descriptive cross-sectional study for a period of 4 years, from June 2015 to June 2019. It involved all patients operated for lumbar canal stenosis and who underwent spondylodesis or spinal fusion at the neurosurgery department of the Yaounde Central Hospital. Results: A total of 68 participants were recruited during our study period. 32 (47%) of the study population were in the 50 - 60 age group, with a mean age of 56.98 years ranging from 41 to 75 years. Females, housewives and farmers were the most affected. In our study, 72% of patients had unilateral spondylodesis and 28% had bilateral fusion. Preoperatively, 71% of patients had insurmountable pain, refractory to medical treatment. At 3 months postoperatively, 73.7% of patients with bilateral setup had moderate pain compared to 69% of those with unilateral setup. At 6 months postoperatively, 79% of patients with bilateral fusion had mild pain compared to 82% of patients with unilateral setup. At 1 year postoperatively, all patients had mild pain. Preoperatively, 66.2% of patients were unable to walk and 19.1% of patients were bedridden according to the Oswestry score. At 3 months postoperatively, 10.2% of patients with unilateral setup were unable to walk compared to 10.5% of patients with bilateral fixation, while 67.3% of patients with unilateral fixation had moderate disability compared to 52.6% of patient

The lancehead (Bothrops lanceolatus) is a poisonous snake endemic to Martinique, an island in the Lesser Antilles arc. Today, this snake is on the verge of extinction. The recorded number of snakes killed yearly between 1970 and 2002 decreased by 97%. Despite the production of an antivenom in 1993, the local authorities set up a financial reward to encourage the eradication of lancehead snakes. Today the local population still perceives the snake as a threat, due to its fatal venom and its fierce behaviour. The case study of the lancehead in Martinique highlights the need to develop integrated strategies to conserve species of poisonous snakes on densely populated islands. This requires innovative, cross-sectoral strategies that involve decision-makers working along with multi-disciplinary scientists. Approaching the complexity of ecosystems through the socio-ecological prism implies, conversely, linking up the domains of science and technology, life and environmental sciences, and human and social sciences, through interactions based on sharing common

Introduction: Priapism is a rare pathology, known since antiquity. Sickle cell disease is the main aetiology in Africa. The aim of our work was to report our experience in its treatment. Material and Methods: This is a prospective, descriptive study carried out at the urology department of the Libreville University Hospital from January 2018 to December 2020. All sickle cell patients admitted to urology for priapism were included. The parameters studied were socio-demographic, clinical and therapeutic parameters as well as the evolution after treatment. Result: We collected 19 priapisms in sickle cell patients. The average age was 20.9 years with extremes of 4 and 53 years. Fifteen patients were homozygous SS. All patients had stasis priapism. The average consultation time was 22.4 hours. All patients had perioperative medical management combining hyperhydration, analgesia and antibiotic prophylaxis. A vasoactive drug was administered to 13 patients. Sixteen patients had a puncture of the corpus cavernosum. A distal cavernosal-spongiosum shunt under penile block was performed in 6 patients. The outcome was favorable from the outset in 12 cases, marked by complete detumescence of the corpus cavernosum. Partial detumescence was noted in 7 patients with the need for a new puncture of the cavernous body. A complication such as edema of the penis was in only one of our patients. A recurrence was noted in 2 patients. After an average follow-up of 6 months, no sequelae erectile dysfunction was observed. Conclusion: Priapism is a frequent complication among sickle cell patients in Libreville. Medical management associated with a cavernous puncture with administration of vasoactive drugs allows a favourable evolution without after-effects.

Introduction: The management of post-traumatic stenosis of the posterior urethra is divided between early endoscopic realignment and delayed surgery. In the latter case, several techniques are used taking into account several factors. Our objective was therefore to investigate the possible prognostic factors of failure of this surgical management. Material and Methods: We conducted a retrospective study from January 2006 to December 2017 on patients admitted to the Urology Department of the Mohammed 6 University Hospital of Marrakech for management of post-traumatic posterior urethral stenosis. The parameters studied were age, medical and surgical history, causes, characteristics of the stenosis (location, number, length), associated lesions, surgical technique used, and evolution. The descriptive analysis consisted of calculation of absolute and relative frequencies for the qualitative variables, and of position and dispersion parameters for the quantitative variables (mean, standard deviation). In bivariate analysis, the comparison of categorical variables used Pearson’s Chi-square statistical test and Fisher’s test if necessary. The significance threshold was set at p Results: A total of 30 cases were selected. The age of our patients varied between 18 and 68 years, with an average of 38.33 years (16 - 80 years);the most affected age group was between 20 and 39 years. The mode of revelation of urethral damage was dominated by acute retention of urine present in 76% of patients, followed by dysuria (33%). The stenosis extended over 21.91 mm on average (14 - 40 mm). It was accompanied by bone involvement in 15 patients (53.3%). Erectile dysfunction was present in 23 patients (23%). Terminal urethrogram was the main surgical technique used, followed by internal urethrotomy (60% and 30% respectively). 15 patients had a recurrence, with an average of 1-second operation. Located in the north-east of Côte d’Ivoire, the Gouméré-Iguéla sectors were the subject of a geological mapping. These geological formations are located southwest of the Bui trench. In order to improve the petrogenetic knowledge of the study area, a multidisciplinary methodology integrating microscopic observations and geochemical analyses of major and trace elements was carried out on 15 samples considered as representative of the studied outcrops. The macroscopic and microscopic petrographic study allowed to highlight three major lithological units: 1) a volcano-plutonic unit, formed by gabbros and amphibole-pyroxenite, basalt and rhyodacite;2) a volcano-sedimentary unit containing pyroclastites (of basaltic and andesitic composition);3) a sedimentary unit (meta-sedimentary). These rocks are affected by a general metamorphism of green schist facies and a hydrothermal alteration with pervasive and vein alteration marked by the presence of quartz, calcite, epidote, chlorite and sericite. From a geochemical point of view, the results obtained indicate that the plutonites are gabbros while the volcanites and volcanosedimentary rocks have compositions of basalts, basaltic andesites, rhyolites and dacites. They have mainly a tholeitic character related to subduction zones with crustal contamination. The metasediments are arenites emplaced in an active continental margin environment.

Introduction: Penile emergencies are multiple and varied, and they can jeopardize the sexual functional prognosis of the patient. The objective of our study was to evaluate the sexual functional prognosis of patients admitted for penile emergencies. Patients and Methods: We conducted a cross-sectional study from January 1 to 31, 2021, in the urology department of the University Hospital. Variables included sociodemographic, diagnostic, and evolutionary data of sexually active patients admitted for a penile emergency. Results: During this period, 68 patients were admitted for penile emergencies. We included 45 sexually active patients, with a mean age ranging from 34.5 ± 14.1 years with extremes of 16 and 90 years. The most frequent penile emergencies were priapism (62.2%) and penile fracture (22.2%), with a mean time for treatment of 21.4 ± 52.5 hours. The sexual functional prognosis was very satisfactory (normal erection) in 71.1% (n = 32) of patients. Loss of erectile function was observed in 4.4% (n = 2) of cases. Conclusion: Penile emergencies are relatively frequent, dominated by priapism and penile fracture in our context. The sexual functional prognosis is good.

Priapism is a painful and prolonged erection occurring without any sexual stimulation and not resulting in ejaculation. It most often occurs in patients with sickle cell disease. We report here the case of acute recurrent priapism revealing inaugural mode of CML in a 22-year-old patient with no particular pathological history. The study of the onco-hematological karyotype revealed a karyotype with 46 chromosomes with a clonal chromosomal anomaly: The t (9;22) “Philadelphia chromosome”. The evolution was favorable under imatinib.

This study aims to contribute to improve knowledge on geological formations of Comoé basin. The petrographic study of the geological formations of Koun Fao has highlighted two major lithological families: magmatic rocks consisting of monzonites, monzogranites, diorites, biotite granodiorites, syenites, porphyritic micromonzonites and porphyritic dacite and metamorphic rocks from sedimentary origin (quartz schists, meta-greywackes, schists, andalusite chloritoschist and paragneiss). These formations are affected by amphibolite to greenschist facies metamorphism and hydrothermal alteration (pervasive and vein) marked by the presence of quartz, epidotes, chlorites and sericite. Minerals such as andalusite, muscovite and chlorite characterize a local low pressure contact metamorphism in the andalusite chloritoschist. Remote sensing data (Landsat 8 image) coupled with field data allowed the production of a geological map of the area. The study of the structures and microstructures highlighted two deformation mechanisms. These are flattening and simple shear (ductile and brittle). The study area is affected by four deformation phases: D1 marked by a N-S to NNE-SSW elongation, D2 marked by a NW-SE to NNW-SSE compression phase, D3 which is a NE-SW to NNE-SSW transpression phase and D4, responsible for late structures, marked by a NW-SE to NNW-SSE transpression phase.
