Right sided diverticulitis in western countries:A review

BACKGROUND Although the treatment guidelines for left sided diverticulitis are clear,the management of right colonic diverticulitis is not well *** disease can no longer be ignored due to significant spread throughout *** To analyse epidemiology,diagnosis and treatment of right-sided diverticulitis in western *** MEDLINE and PubMed searches were performed using the key words "rightsided diverticulitis","right colon diverticulitis","caecal diverticulitis","ascending colon diverticulitis" and "caecum diverticula" in order to find relevant articles published until *** A total of 18 studies with 422 patients were *** diagnosis was made only in 32.2%,mostly intraoperatively or via CT *** main reason for misdiagnosis was a suspected acute appendicitis(56.8%).The treatment was a nonoperative management(NOM)in 184 patients(43.6%)and surgical in 238 patients(56.4%),seven of which after NOM *** rate was low(5.45%),similar to eastern studies and inferior to left-sided *** patients were successfully conservatively retreated in most *** The management of right-sided diverticulitis is not well clarified in the western world and no selective guidelines have been considered even if principles are similar to those with left-sided *** diagnosis is one of the most important problems and CT scan seems to be the best imaging *** offers a safe and effective treatment;surgery should be considered only in cases of complicated diverticulitis or if malignancy cannot be *** studies are needed to clarify the correct treatment.

ZrB_(2)-SiC ceramics toughened with oriented paper-derived graphite for a sustainable approach

A new additive technique for manufacturing of short fibre-reinforced ultra-refractory ceramics is *** technique allows the fabrication of solvent-free,thin(~100µm),flexible,and easy-to-handle sheets suitable for fabricating homogeneous or layered structures.A large range of compositions,in terms of matrix and fibre volumetric contents,from 0%to 100%is *** amount of short carbon fibres incorporated in the sheets ranged from 20 to 50 vol%,whereas the fibre length ranged from 3 to 5 *** matrix composition investigated with this technique consisted of ZrB_(2)/SiC/Y_(2)O_(3).By increasing the fibre amount from 35 to 50 vol%,an improvement of mechanical properties was ***-point flexural strength(σ)ranged from 107 to 140 MPa,depending on the amount of carbon fibres(Cf).The same holds true for the work of fracture,ranging from 108 to 253 J/m^(2).Functionally graded composites were fabricated by overlapping sheets with a fibre gradient(0%-50%).

High-risk HPVs, microbiota and epithelial carcinogenesis: state of the art and research contribution of in vitro 3D models

Persistent high-risk human papillomavirus (HR-HPV) infection is associated with anogenital and head & neck squamous epithelial (HNSCC) tumors, which altogether cause about 550,000 new cases every year. Several evidences suggest that the microbiota could have a role on the inflammatory, epithelial mesenchymal transition and tumorigenesis processes promoted by HR-HPV infection, yet the mechanisms involved remain to be clarified. In this review we report the state of the art on this topic and on the most promising in vitro developed models for studying the host-pathogen interactions. Using MEDLINE, several terms were searched and combined to select the most pertinent papers. The investigation was limited to the international indexed articles published in PubMed in the last 10 years. This review reports the latest knowledge in the field of the microbial-associated anogenital tumors and HNSCC. In addition, we also discuss the in vitro epithelial culture systems that reproduce the pathophysiological features of the tumoral microenvironment and the in vivo response to microbial agents, thus representing a useful tool for analyzing at cellular and molecular levels the role played by infective agents in tumorigenesis.

Propulsion tests on ultra-high-temperature ceramic matrix composites for reusable rocket nozzles

Ultra-high-temperature ceramic matrix composites(UHTCMCs)based on a ZrB_(2)/SiC matrix have been investigated for the fabrication of reusable nozzles for *** de Laval nozzle prototypes,obtained by sintering with either hot pressing(HP)or spark plasma sintering(SPS),were tested 2-3 times in a hybrid rocket motor for proving *** were extracted after oxidation tests to study the microstructural changes and oxidative and thermomechanical stresses induced by the repeated *** to a reference graphite nozzle,no measurable erosion was observed for the UHTCMC-based *** oxidation mechanism consisted in the formation of a ZrO_(2)intermediate layer,with a liquid silicon oxide(SiO_(2))layer on the surface that was displaced by the action of the gas flux towards the divergent part of the nozzle,protecting it from further *** specimens obtained by HP and SPS displayed similar performance,with very slight differences,which were attributed to small changes in *** tests demonstrated the capability of complex-shaped prototypes made of the developed UHTCMCs to survive repeated exposure to environments representative of a realistic space propulsion application,for overall operating time up to 30 s,without any failure nor measurable erosion,making a promising step towards the development of reusable rocket components.

Misdiagnosis of Angina Pectoris Due to Severe Coronary Artery Disease: An Anecdotal Case

Background: Misdiagnosis of angina pectoris is not uncommon in athletes. Physicians can fail to make a correct clinical diagnosis if they don’t have the skills and experience needed to recognize this disease. Case Report: This case report describes an exertional angina misdiagnosis due to a severe coronary artery disease in a 50-year-old male athlete initially examined by both a general practitioner and a cardiologist. The athlete subsequently underwent physical examination in a sports cardiology medicine center where diagnosis of angina pectoris caused by severe mono-vessel coronary artery disease was made, requiring an angioplasty with stenting. Conclusions: Although most of the severe symptoms in a heart attack are hard to miss, sometimes in athletes angina pectoris is not readily recognized. However, this is dangerous and can lead to the sudden death of the athlete. Diagnosis must be timely and accurate, in order for the patient to receive the best prognosis.

Unusual Presentation of Non-Hodgkin’s B-Cell Lymphoma with Unilateral Right Limb Lymphedema

Background: In clinical practice and setting of general practice it is common to see patients with leg edema. To correctly identify the etiology of the edema and then properly manage the cause is not always easy. The unilateral lymphedema of the lower limb has rarely been reported as an initial presentation for lymphoma, especially in females, usually without classic signs or symptoms, but often with inguinal lymphadenopathy or abdominal masses. Case Report: In this article, we report a rare case of unilateral lower limb edema in a healthy obese woman who complained about the appearance of the disease for several months and for whom deep vein thrombosis and other diseases had been excluded. The histological examination of the biopsy of an enlarged lymph node in the right groin, which was compressing the iliac and femoral vein, revealed the presence of B cell non-Hodgkin lymphoma with high-grade malignancy. Conclusions: A common challenge for primary care physicians is to determine the cause and find an effective treatment for leg edema of unclear etiology. Non-Hodgkin’s B-cell Lymphoma should be considered in the differential diagnosis in patients with unilateral leg edema when the swelling is chronic and deep venous thrombosis is promptly excluded.

Amiodarone-induced bronchiolitis obliterans organizing pneumonia in patient following percutaneous transluminal coronary angioplasty

Background: Many patients are affected by idiopathicbronchiolitis obliterans organizing pneumonia (BOOP). There are several known causes of BOOP, and several systemic disorders have BOOP as an associated primary pulmonary lesion. Numerous agents including cytotoxic and noncytotoxic drugs have the potential to cause pulmonary toxicity. Descriptions of amiodarone-related BOOP continue to be reported throughout the world. Case Report: We reported a patient with original clinical presentation who developed recurrent sustained ventricular tachycardia (SVT) despite the presence of implantable cardioverter-defibrillator (ICD), hypoxaemia and interstitial pneumonitis in both lung bases. After percutaneous transluminal coronary angioplasty, he developed bronchiolitis obliterans organizing pneumonia (BOOP). Conclusions: To our knowledge, such complications after percutaneous coronary procedure in patients with amiodarone therapy for arrhythmia prophylaxis, are not very frequent in literature.
