Contribution to the Valorization of Plants Used in the Management of Rheumatic Diseases in Burkina Faso

Joint pain is a real public health problem. According to recent studies, it is increasingly common in sub-Saharan Africa and represents a cause of greater loss of working days than accidents, heart disease and infections. The aim of this study was to study the pharmacochemical and therapeutic evidence of ten (10) plants used in the management of joint pain by traditional health practitioners in Ouagadougou. We conducted a meta-analysis of the data from December 2018 to September 2019. The anti-inflammatory activity of the ten (10) plants retained the maximum consensus internationally. Trichilia emetica Vahl (35.25%), Guiera senegalensis ***. (31%), Calotropis procera (Ait) Ait.F. (28.29%) and Crescientia cujete L. (25.12%) had more than 25% internationally reliability for their effects on joint pain. The structural analogy between conventional drugs and the molecules found in these plants has been confirmed. Additional studies should be carried out in order to improve and secure the use of these plants, which represent a prominent option in the therapeutic offer.

Rare Association between Chronic Tophaceous Gout and Rheumatoid Arthritis in a Black African Subject: A Case Report

Rheumatoid arthritis (RA) and gout are common diseases, but their coexistence is rare. We describe the case of a 76-year-old man with hypertension who had been treated for gout for 20 years on allopurinol and colchicine. He was seen in consultation for deforming polyarthritis of the small and large joints, which had been evolving for about 2 years. The acute episode occurred 10 days earlier with the onset of bilateral and symmetrical polyarthritis affecting the large and small joints. The physical examination revealed a peripheral joint syndrome with ulnar gale force deformities of the hands and several buttonhole deformities of the fingers. In addition, there were nodules, some of which were fistulised, giving rise to a chalky slurry. The biology revealed an inflammatory syndrome in addition to rheumatoid factors and ACPA, which were elevated. Biological analysis of the nodular fluid revealed clusters of sodium urate crystals and ultrasound scans of the joints revealed a double-contour appearance in several joints. The diagnosis of RA was made using the 2010 ACR/EULAR criteria. The patient was treated as an outpatient with corticosteroids before being put on methotrexate. It is important to understand that these two conditions can occur at the same time, so it is important to consider them when treating patients with gout or RA.

Study of Axisymmetric Infinite Guide Lined with Locally Reacting Material without Flow Using DtN Operators

The present work proposed a new method for the modeling by the finite element method of the acoustic propagation problems in infinite axisymmetric cylindrical guides lined with locally reacting absorbent materials without flow. The method deals with the development of an efficient transparent boundary condition based on DtN operators. The method developed in this study is successfully applied to a straight axisymmetric lined guide by imposing a mode on one of the artificial boundaries of the truncated guide. The results are in good agreement with analytical solutions. Applying the method for a non-uniform axisymmetric lined guide which is a complex case, proved its effectiveness and the results compared to those of PML layers are in very good agreement.

Ventilation is one of the factors contributing to energy consumption in buildings and food preservation. The solar chimney proves to be an alternative for reducing conventional energy consumption. Thus, in this study, the performance of a solar chimney with two active faces for thermally drawing air from a chamber for preserving agri-food products was evaluated. These performances were experimentally assessed through data measurements: temperatures and velocities within the chimney, and their analysis using Excel and MATLAB. The obtained results were compared with those from literature to verify their validity. From this study, it is found that the maximum temperature at the chimney outlet reaches 49.4˚C with an average value of 43.7˚C. Additionally, the heating evolution of the chimney air presents four (04) identical phases in pairs, reflecting the chimney’s operation throughout day. The temperature difference between the outlet and inlet of the chimney reaches a maximum of 17˚C with an average of 12.6˚C. Regarding airflow, the maximum air velocity at the chimney outlet is 0.8 m/s, and the average velocities have consistently been greater than or equal to 0.46 m/s. Thus, it can be concluded that the solar chimney is capable of providing ventilation for the preservation chamber through thermal draft.

Gomphrena serrata L. (Amaranthaceae) is a medicinal plant widely used traditionally in rural areas to treat many diseases, including gastrointestinal helminthiasis of small ruminants. This work aims to estimate the plant extracts phytochemical potential and study the activities of the aqueous decoction (GS_AD), aqueous macerate (GS_AM), and hydroalcoholic macerate (GS_HEM) of Gomphrena serrata extracts on two developmental stages (eggs and adult worms) of Haemonchus contortus. Phytochemical screening was carried out. The acute toxicity test was performed according to the Organisation for Economic Co-operation and Development (OECD) test guideline 423. The Haemonchus contortus eggs hatching inhibition test was performed left to incubate for 48 hours in the presence of the different plant extracts at increasing concentrations of 0.125;0.25;0.5;0.625;1;2;3 mg/mL. For the motility inhibition test on adult worms, the worms were contacted with the extracts at concentrations of 0.75;1;1.25;1.5;2;3;4 mg/mL and left to incubate for 24 hours. Albendazole and levamisole were used as standard. Phytochemical screening revealed the presence of tannins, saponins, reducing compounds, coumarins and derivatives, anthocyanins, steroids, triterpenes, and flavonoids. An acute toxicity study showed that the oral LD50 value of G. serrata was estimated to be 5000 mg/kg. All extracts showed an effect on both stages of parasite development. The hatching inhibition rate was 100% from 3 mg/mL for all three extracts. The rate of inhibition of motility was 100% from 4 mg/mL for each extract. HEM is more efficient on eggs and Haemonchus contortus adult worms than other extracts with IC50 of 0.457 mg/mL and LC50 of 1.329 mg/mL, respectively. These results suggest that Gomphrena serrata, a plant used by farmers, has anthelmintic properties.

Growth, Population Parameters and Stock Status of Sarotherodon galilaeus in Samandeni Reservoir, Burkina Faso

Mango tilapia, Sarotherodon galilaeus is one of the most caught fish species in the Samandeni multi-species fishing sites of which, few data on its biology and exploitation are available. The study aimed to Assess the stock status of S. galilaeus. Sampling was conducted from March, 2021 to February 2022 based on commercial fish catches to analyze growth parameters, first sexual maturity size and harvest status of the stock. A total of 572 specimens including 297 females and 275 males were examined. The stock assessment was performed by using the Length based Bayesian method of Biomass (LBB) and that of growth by the ELEFAN method. The growth parameters showed a seasonality of growth and females appeared to grow faster than males. On the other hand, males had a greater asymptotic length than females. Results on the estimated length of fish at first maturity showed that females firstly reached the maturity compared to males. The relative biomass (B/B0) estimated for the stock was higher than the relative biomass that produces maximum sustainable yield (BMSY/B0) indicating healthy biomass. In addition, the length at first sexual maturity was less than the length at the first catch, indicating the absence of overfishing of growth. In addition, extending the study to the various stocks of the reservoir would be important for the sustainable management of the Samandeni high economic fishing area.

Ultrasound of the Rotator Cuff in Non-Fractured Shoulder Trauma: 30 Cases at the Yalgado ouedraogo University Hospital of Ouagadougou (Burkina Faso)

Introduction: Ultrasound is the imaging technique of choice for the study of rotator cuff lesions. However, in the case of shoulder trauma, it is rarely requested in our context. This study aimed to show ultrasound lesions of the rotator cuff in cases of non-fracture shoulder trauma at the CHUYO in Ouagadougou. Methodology: This was a prospective descriptive cross-sectional study running from August 1 to November 30, 2017, in the medical imaging department of CHUYO. The study population consisted of patients received in the department for ultrasound in the context of non-fracture shoulder trauma. Results: We collected 20 cases (66.67%) of rotator cuff lesions out of 30 non-fracture shoulder injuries. The mean age of the patients was 27.6 years. Road traffic accidents accounted for 60% of injuries, sports accidents for 30%, and domestic accidents for 10%. Ultrasound lesions were mainly tendon ruptures (36.67%) and tendinitis (23.33%). Ruptures were non-transfixing in 90.90% of cases. The supraspinatus was the most affected (81.81%). Conclusion: Ultrasound can help diagnose rotator cuff injuries, particularly in non-fractured shoulder trauma.

Chest Radiography: General Practitioners’ Compliance with Recommendations

Introduction: Chest radiography is the most frequently prescribed imaging test in general practice in France. We aimed to assess the extent to which general practitioners follow the recommendations of the French National Authority for Health in prescribing chest radiography. Methodology: We conducted a retrospective analysis study, in two radiology centers belonging to the same group in Saint-Omer and Aire-sur-la-Lys, of requests for chest radiography sent by general practitioners over the winter period between December 22, 2013, and March 21, 2014, for patients aged over 18 years. Results: One hundred and seventy-seven requests for chest X-rays were analyzed, 71.75% of which complied with recommendations. The most frequent reason was the search for bronchopulmonary infection, accounting for 70.08% of prescriptions, followed by 11.2% for requests to rule out pulmonary neoplasia, whereas the latter reason did not comply with recommendations. Chest X-rays contributed to a positive diagnosis in 28.81% of cases. The positive diagnosis was given by 36.22% of the recommended chest X-rays, versus 10% for those not recommended. Conclusion: In most cases, general practitioners follow HAS recommendations for prescribing chest X-rays. Non-recommended chest X-rays do not appear to make a major contribution to diagnosis or patient management, confirming the value of following the recommendations of the French National Authority for Health.

On a Compound Poisson Risk Model Perturbed by Brownian Motion with Variable Premium and Tail Dependence between Claims Amounts and Inter-Claim Time

This paper considers the compound Poisson risk model perturbed by Brownian motion with variable premium and dependence between claims amounts and inter-claim times via Spearman copula. It is assumed that the insurance company’s portfolio is governed by two classes of policyholders. On the one hand, the first class where the amount of claims is high, and on the other hand, the second class where the amount of claims is low, this difference in claim amounts has significant implications for the insurance company’s pricing and risk management strategies. When policyholders are in the first class, they pay an insurance premium of a constant amount c1 and when they are in the second class, the premium paid is a constant amount c2 such that c1 > c2. The nature of claims (low or high) is measured via random thresholds . The study in this work will focus on the determination of the integro-differential equations satisfied by Gerber-Shiu functions and their Laplace transforms in the risk model perturbed by Brownian motion with variable premium and dependence between claims amounts and inter-claim times via Spearman copula. .
