Clinical and prognostic implications of atrial fibrillation in patients undergoing transcatheter aortic valve implantation

AIM: To study a cohort of consecutive patients under-going transcatheter aortic valve implantation (TAVI) and compare the outcomes of atrial fibrillation (AF) patients vs patients in sinus rhythm (SR). METHODS: All consecutive patients undergoing TAVI in our hospital were included. The AF group comprised patients in AF at the time of TAVI or with history of AF, and were compared with the SR group. Procedural, echocardiographic and follow-up variables were compared. Likewise, the CHA 2 DS 2-VASC stroke risk score and HAS-BLED bleeding risk score and antithrombotic treatment at discharge in AF patients were compared with that in SR patients. RESULTS: From a total of 34 patients undergoing TAVI, 17 (50%) were allocated to the AF group, of whom 15 (88%) were under chronic oral anticoagulation. Patients in the AF group were similar to those in the SR group except for a trend (P = 0.07) for a higher logistic EuroSCORE (28% vs 19%), and a higher prevalence of hypertension (82% vs 53%) and chronic renal failure (17% vs 0%). Risk of both stroke and bleeding was high in the AF group (mean CHA 2 DS 2-VASC 4.3, mean HAS-BLED 2.9). In the AF group, treatment at discharge included chronic oral anticoagulation in all except one case, and in association with an antiplatelet drug in 57% of patients. During a mean follow-up of 11 mo (maximum 32), there were only two strokes, none of them during the peri-procedural period: one in the AF group at 30 mo and one in the SR group at 3 mo. There were no statistical differences in procedural success, and clinical outcome (survival at 1 year 81% vs 74% in AF and SR groups, respectively, P = NS). CONCLUSION: Patients in AF undergoing TAVI show a trend to a higher surgical risk. However, in our cohort, patients in AF did not have a higher stroke rate compared to the SR group, and the prognosis was similar in both groups.

In this study, the tribological behavior of an ammonium-based protic ionic liquid(PIL) as an additive in a base mineral oil(MO) is investigated on a steel–steel contact at room temperature and 100℃. Tri-[bis(2-hydroxyethylammonium)] citrate(DCi) was synthesized in a simple and low-cost way, and the ionic structure of DCi was confirmed by proton nuclear magnetic resonance(^(1)H NMR). The stability measurement of 1 wt% DCi to a MO was investigated, and the lubricating ability and anti-wear properties of DCi as an additive in MO were also examined using a custom-designed reciprocating ball-on-flat tribometer. Optical microscope and profilometry were used to obtain the worn morphology of the steel disks. Scanning electron microscopy(SEM) and energy dispersive X-ray spectroscopy(EDS) were carried out to investigate the wear mechanism and to analyze the surface interactions between the rubbing components. When 1 wt% DCi is added into the base MO, frictional performance is improved at both temperatures studied with a friction reduction of 29.0% and 35.5%, respectively. Moreover, the addition of 1 wt% DCi to MO reduced the wear volume 59.4% compared to the use of MO. An oxygen-richened tribolayer is confirmed by EDS on the disk surface when DCi was used as additive under 100℃.

In this paper, we offer a graded equivalence between the quotient categories defined by any graded Morita-Takeuchi context via certain modifications of the graded cotensor functors. As a consequence, we show a commutative diagram that establish the relation between the closed objects of the categories gr^c and M^C, where C is a graded coalgebra.

The production of silver nanoparticles(AgNPs) has increased tremendously during recent years due to their antibacterial and physicochemical properties. As a consequence, these particles are released inevitably into the environment, with soil being the main sink of disposal. Soil interactions have an effect on AgNP mobility, transport and bioavailability. To understand AgNP adsorption processes, lab-controlled kinetic studies were *** tests performed with five different Mediterranean agricultural soils showed that cation exchange capacity and electrical conductivity are the main parameters controlling the adsorption processes. The adsorption kinetics of different sized(40, 75, 100 and 200 nm)and coated(citrate, polyvinylpyrrolidone and polyethyleneglycol(PEG)) AgNPs indicated that these nanoparticle properties have also an effect on the adsorption *** assess the mobility and bioavailability of AgNPs and to determine if their form is maintained during adsorption/desorption processes, loaded soils were submitted to leaching tests three weeks after batch adsorption studies. The DIN 38414-S4 extraction method indicated that AgNPs were strongly retained on soils, and single-particle inductively coupled plasma mass spectrometry confirmed that silver particles maintained their nanoform, except for 100 nm PEG-AgNPs and 40 nm citrate-coated AgNPs. The DTPA(diethylenetriaminepentaacetic acid) leaching test was more effective in extracting silver,but there was no presence of AgNPs in almost all of these leachates.

In mammals, spinal cord injury (SCI) is a devastating event that can lead to a permanent loss of motor, sensory and autonomic functions below the site of injury. In the last years, the role of different neurotransmitter systems on regeneration and recovery from SCI has been deciphered. For example, studies in lampreys have shown that neurotransmitters play a key role in modulating the survival and regeneration of brainstem descending neurons after SCI (Romaus-Sanjurjo et al., 2018;Sobrido-Camean et al., 2018).
