Acute Renal Failure in COVID-19 Patients in Intensive Care at the CHU du Point G in Mali

Introduction: SARS-CoV-2 (Severe Acute Respiratory Syndrome Corona-virus 2) causes an acute respiratory disease with interstitial and alveolar pneumonia, which can affect several organs including the kidneys [1] [2] [3]. As Mali is no stranger to this pandemic, we report our experience of the management of cases of kidney failure observed in the COVID-19 intensive care unit at the Point G University Hospital Centre (CHU). The aim of this work was to characterise acute renal failure in COVID-19 patients in intensive care, describing the management methods used and determining the vital prognosis. Materials and Methods: This was a retrospective descriptive study, covering an 18-month period from April 2020 to September 2021. We included all patients admitted to the COVID-19 intensive care unit on the basis of a positive RT-PCR and/or the presence of ground-glass images on thoracic computed tomography. Results: We selected 232 patients admitted for COVID-19. Acute Renal Failure (ARF) developed in 71 patients (30.6%). The stages of AKI according to KDIGO were Stage 1 in 28.2%, Stage 2 in 18.3% and Stage 3 in 53.5%. The mean age was 63.96 years, with a standard deviation of 16.6, and males accounted for the majority (71.8%). Organic ARF was found in 80.3% of cases. Risk factors and comorbidities for ARF included advanced age (60.6%), male sex (71.8%), hypertension (52.1%), diabetes (21.1%), invasive mechanical ventilation (71.8%) and septic shock (56.3%). Extra renal purification (haemodialysis) was used in 29.6% of patients. Admission to intensive care ranged from 7 days to 14 days in 43.7% of cases. More than half the patients (52.1%) were in critical condition on admission. Death occurred in 76.1% of patients. Conclusion: ARF appears to occur more frequently in patients with severe COVID-19. It is associated with a poor prognosis.

Particle size fractionation by sieving is used to optimize antioxydant potential of natural substances. The aim of the present study was to evaluate particle size effects on antioxidant and hepatoprotective potential of Eucalyptus camaldulensis essential oils (EO) on CCl4-induced hepatic damage in Wistar rats. Animals were daily orally treated with the EOs extracted by hydrodistillation from powder sieved at four particle sizes (≥355, 200 - 355, 100 - 200, ≤100 μm) and those of the unsieved powder at dose of 50 mg/Kg for 7 days. Compounds that are evaluated for these activities are hydrocarbons and oxygenated terpenes that were identified and quantified by GC/MS. Activities of enzymes markers of hepatocellular damage in serum and antioxidant enzymes in the liver homogenates were measured. In this research, EOs significantly prevented the increase in serum ALT and AST (p 4 and significant increase level of plasma HDL-cholesterol. Also, significantly (p < 0.05) decreased the extent of malondialdehyde (MDA) formation and elevated the activities of superoxide dismutase (SOD) and catalase (CAT) liver in comparison to negative control group. The best antioxidant and hepatoprotective activities were those of EOs from two fine powder fractions (≤100 μm and 100 - 200 μm) was correlated to their high concentration in oxygenated terpenes (70.9% and 46.4%, respectively), when compared to the large particles (200 - 355 μm and ≥355 μm, with 33.3% and 41.8%, respectively) and unsieved powder (37.4%).

Tomato is a market gardening product that plays an important nutritional and economic role in Burkina Faso. However, the bad use of pesticides for its production could have negative impacts consumers’ health. The objective of the present study was to assess the concentration of trace metal elements (Pb, Cd and As), 15N isotopes (NO2-, NO3-, NH4+) and phosphorus (PO42-) in tomatoes according to cultivation practices. Thus, three tomato varieties (Mongal F1, F1 Cobra 26 and Roma F1) were grown using organic and conventional production methods on 2 sites at Nongr Massom, (commune of Kadiogo province). Epidemiological and Clinical Study of Cardiac Diseases in the Pediatric Department of the University Hospital Gabriel Touré(UH GT), Bamako (Mali)

Introduction: Children’s heart disease is a major public health problem in developing countries and especially in Mali. The purpose of our work was to determine frequency, different types of heart disease and their short term evolution in the pediatric department. Methods: We performed a retrospective study among children aged 0 to 15 years, hospitalized in the pediatric department from January to December 2015 and whose diagnosis was confirmed using trans-thoracic echocardiography. Results: We included 103 cases of heart disease out of a total of 8613 admissions in the pediatric department, giving an hospital prevalence of 1.2%. Mean age was 4.1 years (from 1 day to 15 years) and children under 5 years were the most affected with 73.80% of cases. Male predominance was noted (sex ratio = 1.2). Respiratory distress was the most common circumstance of discovery (93.20%). Cardiac murmur and tachycardia were the most common cardiac signs with respectively 88.35% and 83.50%. Congenital heart disease accounted for 70.87% and was dominated by ventricular septal defect (VSD) with 30.13%. Acquired heart disease (29.13% of the sample) was dominated by mitral regurgitation (MR) with 56.67%. Mortality rate was 31.9% for congenital heart disease and 11.1% for acquired heart disease. Conclusion: children’s heart disease is responsible for high mortality. Early detection improves the management of this pathology, which remains frequent.

Dried leaves of Eucalyptus camaldulensis were finely grinded and fractionated by sieving into four granulometric classes (355 μm). The obtained powder fractions were used for essential oil (EO) extraction by hydrodistillation and their phytochemical profile and in vitro antioxidant activities were evaluated. The mother powder (unsieved powder) was used for comparison. Particle size exerted a significant influence (p in vitro antioxidant properties of the EOs. Comparatively, the mother powder had the highest contents of α-pinene (55.6%), camphene (3.4%) and limonene (3.7%), while 1,8-cineole (26.6% and 22.4%), exo-fenchol (5.6% and 3.5%), α-campholenol (4.2% and 3.4%), L-trans-pinocarveol (5.5% and 2.7%), L-borneol (12.6% and 6.8%) and α-terpineol (16.4% and 7.6%) are the main compounds of EOs from the <100 μm and 100 - 200 μm fractions, respectively. Kola nuts were regularly chewed by West Africans and Beninese in particularly. The aim of this study was to investigate nutritional and anti-nutrient content of three Benin’s kola nuts (Cola nitida, Cola acuminata and Garcinia kola). Proximate composition of the three species of kola nuts was assessed using standard analytical AOAC methods. Phenolics and flavonoids contents were determined by Folin-Ciocalteu and aluminum trichloride methods, respectively. Mineral composition was determined by Atomic Absorption Spectrometry method. Free and total amino acids were separated and quantified by HPLC. Protein content of the three kola nuts ranges from 4.95% (G. kola) to 10.64% (C. acuminata) whereas fat content ranges from 0.2 ± 0.00 (C. nitida) to 2.5 ± 0.42 (G. kola). Total phenolics abounded (2444.96 ± 81.56 μg Eq AG/100g) in C. acuminata, while flavonoids predominated (561.69 ± 22.10 μgEqQ/100g) in G. kola. The three species are a good source of magnesium and a copper provider was lowest in C. nitida (0.59 ± 0.08 mg/g) and in C. acuminata (0.65 ± 0.02 mg/g). The dominant total essential amino acids were threonine (C. acuminata) and methionine (C. acuminata and G. kola), while the predominant non-essential total amino acids according to species were arginine (C. nitida and G. kola), proline (C. acuminata) and cysteine (G. kola). For the anti-nutrients factors, saponins were in great proportion (8.33% ± 0.25%), while the oxalates were in small proportion (0.44% ± 0.04%). The three species have an interesting nutritional composition, but these seeds have the relatively lowest amino acids content.

Background: It is yet a controversy subject whether low birth weight and infant death are associated to human immunodeficiency virus-1 infection. Objective: To appreciate association between low birth weights, mother to child HIV transmission and infant mortality in HIV-1 infected pregnant women delivering between 2011 and 2016. Materials: We conducted 6 years cohort study in urban Mali. Outcome included preterm delivery, small for gestational age, infant survival status and HIV transmission. Comparison concerned women clinical WHO stage, mother viro-immunological status, and newborn anthropometric parameters. Results: HIV-1 infected women who delivered low birth weight newborn were 20.9% (111/531) versus 16.5% (1910/11.546) in HIV negative patients (p = 0.016). CD4 T cell counts low than 350 T cells count were strongly associated to LBW (p = 0.000;RR = 3.03;95% CI [1.89 - 3.16]). There is no significant association between ART that was initiated during pregnancy (p = 0.061, RR = 0.02;CI 95% (1.02 - 1.99)) or during delivery (p = 0.571;RR = 1.01;CI 95% (0.10 - 3.02)) and LBW delivery. In multivariate analysis ART regimens containing protease inhibitor (PI) were lone regimens associated with LBW ((p = 0.030;RR = 1.001;95% confidence interval [1.28 - 3.80]). Very low birth weight was statistically associated to women HIV infection (adjusted relative risk, 2.02;p = 0.000;95% confidence interval (2.17 - 4.10)). There is no significant difference between mother to child HIV transmission rate in the two HIV-infected pregnant women (10 infected children in group 2: MTCT rate 4.5%) and 3 infected children in group 1 (MTCT rate: 2.7%) (p = 0.56;RR, 0.59;CI 95% (0.18 - 4.39)). In multivariate analysis, LBW was associated with infant death (p = 0.001;RR = 2.04;CI 95% [1.04 - 5.05]). The median weight of infant at the moment of death in group 1 was 851 g (IQR: 520 - 1833 g). Endoscopic Management of Migrated Intra Uterine Device in Sub-Saharan Health Setting Area. Our Experience about Seventeen Cases

Intra uterine device migration is a relatively rare event. The migration of the IUD in the surrounded viscera can be managed by endoscopy approach. Objectives: We reported our experience to determine the epidemiologic characteristics of patients that presented migrated IUD, to report clinical aspects and describe the laparoscopic management. Method: We conducted a descriptive and prospective survey from January 1st, 2014 to October 31, 2016. That survey took place in the department of gynecology and obstetrics of Point G Teaching hospital, Bamako, Mali. Population study concerned patients with intra uterine device complications. Statistic tests used have been X2 or Fisher test according their application conditions. P Results: Seventeen patients were included. Mean age of patients is 29 years with extremes of 13 years and 44 years. The main reference incitement of patients were perforation of the uterus and pelvis pain (27.8% for each), misplaced IUD (22.3%). Complications observed were intra uterine device migrated in to the bladder (1 case), in the abdomen (10 cases), in the ovary (2 cases) and in the uterine tuba (1 case). Three perforations were done without migration in to surrounded viscera. We used endoscopic surgery among all of them. But in one case we switched endoscopy method in to laparotomy because of important intra peritoneal bleeding to end the management of the patient. No death occurred and mean duration of the stay of hospitalization was 2 days. Conclusion: IUD migration is a scarce event. All the surrounded viscera can be the site of its migration. The management of that complication is usually done by endoscopy method.

Caustic Esophageal Stenosis: Epidemiological, Clinical, Endoscopic and Therapeutic Aspects at the Gabriel TouréUniversity Hospital in Bamako

Voluntary or accidental ingestion of a caustic product is increasingly encountered in our country. Caustic esophageal stenosis is the most common sequel. Its treatment involves instrumental dilation and/or surgery which is particularly heavy. The aim of this study was to study the epidemiological, clinical and therapeutic aspects of caustic esophageal strictures in our context. The retrospective study took place from December 2013 to December 2017 in the Hepato-Gastroenterology department of the Gabriel Touré university hospital in Bamako. We included patients with caustic esophageal stenosis referred to the gastrointestinal endoscopy unit for dilation. 67 patients admitted for caustic stenosis were included. The mean age of our patients was 20.76 ± 19.9 years with extremes of 1 and 70 years and a sex ratio of 1.9. In 50.7% of cases, the product ingested was basic in nature. The clinical symptomatology was dominated by dysphagia (100%), vomiting (100%), the emaciated (60%) and cough (40%). In 59 (88%) patients, the stenosis was unique and multiple in 8 (12%). The KILLIAN mouth as the sole site of the stenosis was found in 40.3% of patients. The number of sessions was ≤ 3 in 16.4% and >3 in 83.6% with a mean of 4.59 ± 1.57 sessions. Secondary Stress Induced Central Serous Chorioretinopathy in Bamako: Report of Two Cases

Introduction: Idiopathic central serous chorioretinopathy (ICSC) is characterized by the buildup of fluid leading to circumscribed elevation of the retina within the posterior pole. This condition affects young people between the ages of 20 - 50 years. The disease is seen predominantly in men as compared to women. We report the cases of two (02) patients received in consultation at IOTA-Teaching Hospital between July 2019 and February 2020. Case Presentation: We report two cases of Idiopathic stress central serous chorioretinopathy. The first case is a 37-year-old man that we received in consultation for visual acuity decrease from a sudden installation for 1 hour. The clinical and paraclinical arguments permitted us to retain the diagnosis of Idiopathic stress central serous chorioretinopathy. The patient benefited from laser treatment and the clinical evolution was good. The second case is a 42-year-old man, who consulted for a sudden reduction of visual acuity that had been evolving for 3 weeks. The clinical and paraclinical contexts were in favour of the diagnosis of Idiopathic stress central serous chorioretinopathy. The patient was treated with spironolactone. The clinical evolution was favourable. Conclusion: Our study confirms the existence of central serous chorioretinopathy (CSC) in Black African patients. Studies show the connection between CSC and stress.
