Liver preservation prior to transplantation: Past, present, and future

Since Dr. Thomas Starzl performed the first series of successful liver transplants(LTs), important advances have been made in immunosuppression, operative techniques, and postoperative care. In 1988, Belzer's group reported the first successful LT using the University of Wisconsin preservation solution(UW).Since then, UW has replaced EuroCollins solution and allowed prolonged and safer preservation of liver, kidney, and pancreas allografts, thus contributing to the improvement of transplant outcomes. Although UW is still considered the standard of care in the United States and in several countries worldwide, a recent meta-analysis revealed similar LT outcomes among UW, Celsior solution, and the Institut Georges Lopez-1 preservation solution, which were slightly superior to those obtained with histidine-tryptophan-ketoglutarate preservation *** preservation has been recently developed, and liver allografts are preserved mainly through the following methods: hypothermic machine perfusion, normothermic machine perfusion, and subnormothermic machine perfusion. Their use has the potential advantage of improving clinical results in LT involving extended criteria donor allografts. Although associated with increased costs, techniques employing machine perfusion of liver allografts have been considered clinically feasible. This editorial focuses on recent advances and future perspectives in liver allograft preservation.

Estrogen is implicated as playing an important role in aging and tumorigenesis of estrogen responsive tissues;however the mechanisms underlying the mitogenic actions of estrogen are not fully *** we report that estrogen deficiency in mice caused by targeted disruption of the aromatase gene results in a signi-ficant inhibition of telomerase maintenance of telomeres in mouse ovaries in a tissue-specific *** inhibition entails a significant shortening of telomeres and compromised proliferation in the follicular granulosa cell compartment of *** expression analysis showed decreased levels of proto-oncogene c-Myc and the telomerase catalytic subunit,telomerase reverse transcriptase(TERT),in response to estrogen *** replacement therapy led to increases in TERT gene expression,telomerase activity,telomere length and ovarian tissue growth,thereby reinstating ovary development to normal in four *** data demonstrate for the first time that telomere maintenance is the primary mechanism mediating the mitogenic effect of estrogen on ovarian granulosa cell proliferation by upregulating the genes of c-Myc and TERT in *** deficiency or over-activity may cause ovarian tissue aging or tumorigenesis,respectively,through estrogen regulation of telomere remodeling.

Giant appendiceal mucocele: Report of a case and brief review

Mucocele of the appendix is a rare lesion, characterized by distension of the lumen due to accumulation of mucoid substance. This disease is often asymptomatic and pre-operative diagnosis is rare. If untreated, one type of mucocele may rupture producing a potentially fatal entity known as pseudomyxoma peritonei. The type of surgical treatment is related to the dimensions and to histology of the mucocele. Appendectomy is used for simple mucocele or for cystadenoma. Right hemi-colectomy is recommended for cystadenocarcinoma. In this paper, we report a case of a 51-year-old woman with a mobile, painless mass in the right lower quadrant of abdomen caused by a giant appendiceal mucocele. Imaging showed a large, tubular,cystic structure extending below from the inferior wall of the cecum. Surgery revealed a giant retro-cecal appendix measuring 17 cm in length and 4 cm in diameter. The final pathologic diagnosis was mucocele caused by mucinous cystadenoma.

Non-alcoholic fatty liver disease: What the clinician needs to know

Non-alcoholic fatty liver disease(NAFLD) is the most frequent cause of liver disease in the Western world. Furthermore, it is increasing worldwide, paralleling the obesity pandemic. Though highly frequent, only about one fifth of affected subjects are at risk of developing the progressive form of the disease, non-alcoholic steatohepatitis with fibrosis. Even in the latter, liver disease is slowly progressive, though, since it is so prevalent, it is already the third cause of liver transplantation in the United States, and it is predicted to get to the top of the ranking in few years. Of relevance, fatty liver is also associated with increased overall mortality and particularly increased cardiovascular mortality. The literature and amount of published papers on NAFLD is increasing as fast as its prevalence, which makes it difficult to keep updated in this topic. This review aims to summarize the latest knowledge on NAFLD, in order to help clinicians understanding its pathogenesis and advances on diagnosis and treatment.

Recent Advances in Drug Delivery Systems

Drug targeting to specific organs and tissues has become one of the critical endeavors of the century since the use of free drugs in conventional dosage forms generally involves difficulties in achieving the target site at the appropriate dose after or during a proper time period. Consequently, the search for new drug delivery approaches and new modes of action represent one of the frontier research areas. New drug delivery systems include lipidic, proteic and polymeric technologies to provide new sustained drug delivery with better body distribution, drug protection from harsh external environment and avoidance of drug clearance. Many of these technologies have reached the market therefore proving the benefits of these new carriers. This review covers the generalities of those new carriers and their new advances in drug delivery.

Lightning Enhancement in the Amazon Region Due to Urban Activity

Urbanization has an increasing contribution to anthropogenic climate forcing. The impact arises mainly from the Urban Heat Island (UHI) effect and aerosol anthropogenic emissions. An important but not completely understood consequence of this forcing is its effect on local lightning activity. Changes in lightning activity may result in a feedback on the climate system. In this article, it investigates changes in the lightning activity in the city of Manaus, located in the Amazon region of Brazil. It is found that, over the city, the lightning activity is larger than in the regions around it and it has been increasing in the last four decades simultaneously with the increasing of its urban area. Our results suggest that such changes are caused by the UHI effect. The observations reported here are unique and relevant because Manaus is located in the central part of the Amazon rainforest and inside one of the three global lightning chimneys in the world.

Soils result from the interaction of five independent formation *** one factor varies,while the others remain constant,different soils can be ***,we demonstrated an opposing trend,wherein two soils were similar,despite considerable differences in all factors of soil *** sampled two Inceptisols(Oxic Dystrudepts) formed on different parent materials(gneiss *** schist),climate(tropical altimontane ***,drier plateau),topography(1 650 m,45% slope vs.1 000 m,8% slope),time(rejuvenated ***,stable surface),and vegetation(rainforest *** savanna).The two soils had similar chemical properties,whereas the soil on mica schist had finer particle size distribution,lower porosity,and lower saturated hydraulic *** properties were related to a coarser blocky microstructure compared to the soil on *** soils presented active mineral weathering and pronounced pedoplasmation,demonstrated by clay contents>300 g kg^(-1),although only the Dystrudept on gneiss possessed coarse rock *** C horizons of both soils presented fragmented clay coatings suggestive of argilluviation,likely relict,because they were not observed in the B *** similarities in many properties of the two Dystrudepts,despite contrasting factors of soil formation,suggest converging evolution and that soil classification at the subgroup level was efficient in grouping similar formative processes in tropical ***,this work revealed that similar pedogenic processes acting on different factors of soil formation can result in similar soil properties,at least for Inceptisols where further soil development is hindered by topographic limitations.

Oxidative stress （OS） is a key process in the development of many neurodegenerative diseases, memory disorders, and other pathological processes related to aging. Tibolone （TIB）, a synthetic hormone used as a treatment for menopausal ymptoms, decreases lipoperoxidation levels, prevents memory impairment and learning disability caused by ozone （O3） exposure. However, it is not clear if TIB could prevent the increase in phosphorylation induced by oxidative stress of the microtubule-associated protein Tau. In this study, the effects of TIB at different times of administration on the phosphorylation of Tau, the activation of glycogen synthase kinase-3β （GSK3β）, and the inactivation of Akt and phosphatases PP2A and PTEN induced by O3 exposure were assessed in adult male Wistar rats. Rats were divided into 10 groups： control group （ozone-free air plus vehicle [C]）, control ＋ TIB group （ozone-free air plus TIB 1 mg/kg [C ＋ TIB]）; 7,15, 30, and 60 days of ozone exposure groups [O3] and 7, 15, 30, and 60 days of TIB 1 mg/kg before ozone exposure groups [O3 ＋ TIB]. The effects of O3 exposure and TIB administration were assessed by western blot analysis of total and phosphorylated Tau, GSK3β, Akt, PP2A, and PTEN proteins and oxidative stress marker nitrotyrosine, and superoxide dismutase activity and lipid peroxidation of malondialdehyde by two different spectrophotometric methods （Marklund and TBARS, respectively）. We observed that O3 exposure increases Tau phosphorylation, which is correlated with decreased PP2A and PTEN protein levels, diminished Akt protein levels, and increased GSK3β protein levels in the hippocampus of adult male rats. The effects of O3 exposure were prevented by the long-term treatment （over 15 days） with TIB. Malondialdehyde and nitrotyrosine levels increased from 15 to 60 days of exposure to O3 in comparison to C group, and superoxide dismutase activity decreased. Furthermore, TIB administration limited the changes induced by O3 exposure. Our results
