Ambulatory Radio Iodine Therapy in the Management of Hyperthyroidism in Africa: African Systematic Review and Perspectives in Burkina Faso

Introduction: In hyperthyroidism, selective irradiation of the thyroid gland with radioactive iodine is a radical treatment and an alternative to surgery. The aim of this review is to assess the medium-term efficacy of outpatient treatment of hyperthyroidism with iodine-131 in Africa. Methods: We identified the studies carried out in Africa on outpatient radiation therapy between 2016 and 2020. For each article included, we noted the country concerned and the year of publication, the numbers studied, the socio-demographic characteristics of the patients, the indications for radio iodine therapy, the dose administered, the results of the hormonal dosage 6 months after radiation. Results: 13 retrospective studies were included to constitute a total population of 925 patients. The average age was 40.77 years, the sex ratio of 1/5.4 with a clear female predominance. The 3 main etiologies of hyperthyroidism justifying outpatient radio iodine therapy were Graves’ disease (55.89%), toxic multinodular goiter (22.70%) and toxic adenoma (21.40%). The average dose of iodine 131 administered per course is 13.7 mCi. No short-and medium-term complications were reported. The radio iodine therapy was effective in 86.08% (n = 796) of the patients with extremes of 72% and 100%. Conclusion: Radio iodine therapy is effective in Africa. It is simple, inexpensive on an outpatient basis and well tolerated. The introduction of outpatient radio iodine therapy could improve the management of patients with hyperthyroidism in Burkina Faso.

Contribution of Satellite Observations in the Optical and Microphysical Characterization of Aerosols in Burkina Faso, West Africa

In this work, we proceed to an optical and microphysical analysis of the observations reversed by the MOdiS, SeaWiFS, MISR and OMI sensors with the aim of proposing the best-adapted airborne sensor for better monitoring of aerosols in Burkina Faso. To this end, a comparison of AOD between satellite observations and in situ measurements at the Ouagadougou site reveals an underestimation of AERONET AOD except for OMI which overestimates them. Also, an inter-comparison done based on the linear regression line representation shows the correlation between the aerosol models incorporated in the airborne sensor inversion algorithms and the aerosol population probed. This can be seen through the correlation coefficients R which are 0.84, 0.64, 0.55 and 0.054 for MOdiS, SeaWiFS, MISR and OMI respectively. Furthermore, an optical analysis of aerosols in Burkina Faso by the MOdiS sensor from 2001 to 2016 indicates a large spatial and temporal variability of particles strongly dominated by desert dust. This is corroborated by the annual and seasonal cycles of the AOD at 550 nm and the Angström coefficient measured in the spectral range between 412 nm and 470 nm. A zoom on a few sites chosen according to the three climatic zones confirms the majority presence of mineral aerosols in Burkina Faso, whose maxima are observed in spring and summer.

Introduction: Microbiology of effusion fluids in children in Burkina Faso is characterized by the scarcity of data. This work aimed to study the bacteriological and antibiotics susceptibility profile of bacteria involved in effusion fluid infections in paediatrics in order to improve the choice of probabilistic antibiotics therapy. Methods: A cross-sectional, descriptive study was used in children aged 0 to 15 years from 2017 to 2020 at the Charles De Gaulle Pediatric University Hospital Center (CHUP-CDG) in Ouagadougou. Classical bacteriology methods such as macroscopy, Gram staining, identification galleries and antibiotics susceptibility testing were used. Results: Of 231 samples, 64 bacteria were isolated. The most common bacterial strains of pleural fluid were Staphylococcus aureus (25%) and 40% for Enterobacteriaceae. Of the peritoneal fluid, 77% were Enterobacteriaceae with 57% Escherichia coli;and from joint fluid, 33% were S. aureus and 22% for P. aeruginosa. The overall susceptibility profile showed 29% extended-spectrum beta-lactamase-producing Enterobacteriaceae (ESBL), 10% methicillin-resistant S. aureus (MRSA), and 8% carbapenemases. Conclusion: Bacteriological profile is characterized by ESBL-producing Enterobacteriaceae and MRSA. The most active antibiotics were macrolides, aminoglycosides, and cefoxitin (methicillin) for Gram-positive cocci, carbapenems, and aminoglycosides for Gram-negative bacilli. Then, the monitoring of antibiotics resistance must be permanent.

Malnutrition and food insecurity are major concerns for the Malian authorities. The objective of this study was to strengthen the knowledge about the nutritional and physicochemical values of four underutilized edible wild fruits picked at two cities belonging to different bioclimatic zones of Mali. The physicochemical and nutritional parameters were performed using standard methods. The findings revealed that all these parameters varied from one fruit to another, this outcome could be associated with the provenances of the wild fruits (p-value R. sudanica fruits to 5.34 g/100g DM for those of B. aegyptiaca, all these samples were being harvested from Sikasso. The highest concentrations of vitamins are C (150,800 to 151,000 μg/100g DM), E (1,310 to 1,350 μg/100g DM) and A (38 to 40 μg/100g DM) respectively for the fruits of Z. mauritiana, B. aegyptiaca and S. senegalensis. In addition, these fruits would constitute a potential source of minerals such as iron, phosphorus and calcium. Incisional Hernias: Risk Factors and Treatment in a Context of Limited Resources

Background: The purpose of this study was to describe the etiologic factors and therapeutic modalities of incisional hernias in the department of surgery at the regional hospital of Tenkodogo, in Burkina Faso. Methods: It is a cross sectional study covering the period from 2010 to 2012. Sampling was comprehensive, including all the patients operated for incisional hernias during this period. Results: Fifty-four patients were included. Their average age was 22.3 years. There were 35 (64.8%) males and 19 (35.2%) female patients. We counted 28 large-size incisional hernias, 14 medium-size incisional hernias and 12 incisional hernias of small size. The average time from the first surgery to the diagnosis of incisional hernias was 8 months. Generalized acute peritonitis (44.4%) and acute intestinal obstructions (22.2%) were the main indications of initial surgical interventions. Vertical midline incisions have been involved in 37 cases (68.5%). Therapeutically, two surgical techniques have been used: open suture repair and open mesh repair. Forty-two patients (77.8%) were treated by open suture repair. Twelve patients (22.2%) benefited from open mesh repair. Post-operative complications were noted in 2 patients. Nine patients (16.7%) presented a recurrence after 12 months. Conclusion: In the rural area of Burkina Faso, incisional hernias occur in young patients. Emergencies with abdominal Infections are the main operative indications. Prostheses are relatively high cost and less accessible. The recidivism rate is high.

Factors Associated with Non-Adherence to Treatment in Sickle Cell Patients Monitored at the National Reference Center for Sickle Cell disease in Niger

Introduction: Sickle cell disease is a real public health problem in the world and particularly in Niger where the prevalence of the S gene is estimated at 25% and that of the homozygous forms at between 1% and 2%. Treatment combines quarterly follow-up of patients and management of complications. The objective of this study was to identify the potential explanatory factors of non-adherence to treatment in sickle cell patients followed at the national reference center for sickle cell disease in Niger. Methods: This is a cross-sectional study of sickle cell cases followed at the CNRD in Niger. The population consisted of all sickle cell patients followed in this center in 2021. The data collection techniques were individual interviews and documentary reviews. Non-adherence was assessed with the Girerd test. Descriptive statistical tests and simple and multiple logistic regression models were performed. Results: A total of 368 patients were enrolled. The median age is 7 years (4;10) and the sex ratio is 1.04. Ninety-eight (98) or 26.6% were compliant and 270 (73.4%) were non-compliant. In multivariate analysis, the factors independently and negatively associated with non-adherence to treatment were schooling (adjusted OR [95% CI], p-value), 0.17 [0.10 - 0.30];p Conclusion: The factors influencing treatment compliance identified in this study are all modifiable. To prevent the complications of sickle cell disease, we must fight against ignorance, make care services accessible and make care free.

Garcinia kola Heckel, called “petit cola” in Côte d’Ivoire and “bitter kola” in Nigeria, is a Non-Timber Forest Product of great socioeconomic importance. Unfortunately, this species is threatened due to overexploitation and deforestation. This study tested the effect of leaf area on stem cuttings to regenerate vegetatively in a non-mist poly-propagator without using exogenous hormones. Three stem cuttings types: greenwood, softwood and hardwood (12 - 15 cm in length) were collected from 24-month years old seedlings. The two top leaves of each cutting type were conserved at different leaf area: 0 cm2, 28.25 cm2, 56.5 cm2, 84.75 cm2 and 113 cm2 (entire leaf area). Observations were made on rooting, sprouting and leafing abilities of cuttings depending to treatments applied. Results showed that all cutting types have presented good ability to shooting and rooting. Cuttings with 113 cm2 leaf areas presented the best rooting (94.44% ± 2.42%) and shouting (95.55% ± 2.42%) percentage and longest primary root length (12.03 ± 0.50 cm). Hardwood, softwood and greenwood cuttings with 113 cm2 leaf area had the best rooting percentage (96.66% ± 3.33%, 96.66% ± 5.77% and 90.00% ± 5.77% respectively). The longest primary root length (13.50 ± 0.97 cm) was observed to greenwood cuttings with 113 cm2 leaf area. Regenerated plants in the forest presented well survival percentage (96.66%). This study shows that it is possible to regenerate entire plants by cuttings without using external hormones in non-mist poly-propagator.

Child Abdominal Trauma Due to Attack of Cattles While Working on Farms in Burkina Faso

Introduction: Cattle are strong animals very often used in agriculture in rural Africa. Unpredictable in their behavior, they regularly inflict injuries to farmers, some of whom are children in rural Africa. Objective: The purpose of this study was to describe the clinical, therapeutic and evolutionary aspects of abdominal trauma caused by bovidae to children on farms in Burkina Faso. Patients and Method: This is a descriptive study covering the period from January 2014 through December 2016 and was conducted in the General Surgery Unit of the Tenkodogo Regional Hospital in Burkina Faso. It included all patients under the age of 17, victims of abdominal trauma by cattle on farms. Result: A total of 68 patients were identified, which is 3% of all patients under 17 admitted to the service during the same period. The average age of patients was 10.5 years, with a standard variation of 3.6 years. The majority (73.5%) of the patients were boys. The main lesional mechanisms found were horn strokes (88.2%). We noted 35 eviscerations without visceral perforation, 15 intestinal perforations, 6 hepatic wounds and 5 splenic wounds. In 7 cases, there were non-penetrating wounds. All patients underwent surgical intervention under anesthesia. The average length of hospital was 6 days. Two deaths were recorded, making a mortality rate of 3%. Conclusion: Abdominal traumas of the child by attack of cattle are frequent during farm works in rural Burkina Faso. The injuries are sometimes serious. Wearing protective equipment for children should be considered during farm work.

Intra-Seasonal and Annual Variation of Aerosols and Their Radiative Impact in the Sahelian Zone of Burkina Faso

This paper deals with the characterization of aerosols in the Sahelian zone, particularly in Burkina Faso based on MOdiS observations and in situ measurements of the AERONET network on the Ouagadougou site (12.2°N, 1.4°W). Thus, a seasonal spatial distribution of aerosols made over the period from 2001 to 2016 gives a very great variability of aerosols in Burkina Faso, whose maxima are encountered in Spring, characterized by winds from the North East. This seasonality of aerosols is also shown by the annual cycles of optical, radiative and microphysical parameters measured by AERONET between 1999 and 2006. Moreover, an analysis of these parameters shows the prevalence of mineral dusts characterized by low values of the Angstrom coefficient (α440-870 0.9) and the cooling noticed in the bottom of atmosphere (BOA) and at the top of the atmosphere (TOA). Also, the climatology of the volume size distribution of aerosols shows a very great variability of particles in terms of size influenced by the thin and coarse pattern where most sizes are between 1 and 10 μm.

Genetic diversity of Local Rice Accessions Cultivated in Guinea Based on Agro-Morphological Traits and Identification of Sources of Tolerance to Iron Toxicity

In Guinea, traditional rice varieties are the most widely cultivated in rural areas despite their low yield and high susceptibility to iron toxicity. Moreover, the introduction of new improved varieties tolerant to iron toxicity poses a serious threat to the preservation of the genetic resources of these traditional varieties, whose genetic diversity remains poorly known. The present study therefore aims at a better valorisation of these local rice cultivars through the evaluation of their agromorphological diversity and the identification of genotypes potentially tolerant or resistant to iron toxicity. Thus, 90 accessions collected in the regions of kindia and Macenta and six controls susceptible or resistant to iron toxicity (AZUCENA, BOUAKE 189, CK 73, IR64, NERICAL 19 and AURYLUX 6) were evaluated in three replicate alpha lattice designs in two sites (Sérédou and kilissi) using 16 agromorphological traits. The results showed significant agromorphological variability of the traditional accessions at both sites for all qualitative and most quantitative traits studied. In addition, 30 local cultivars expressed similar or higher grain yields than the resistant or tolerant controls at both sites, of which 12 were found to be stable at both sites. Of the 12 cultivars identified, five were resistant and seven tolerant to iron toxicity. These 12 accessions could be used in the varietal improvement of lowland rice in Guinea Conakry.
