The use of complementary and alternative medicine (CAM)- in migraine is a gr owing phenomenon about which little is known. This study was undertaken to evalu ate the rates, pattern and presence of predictors of CAM use in a clinical popul ation of patients with different migraine subtypes. Four hundred and eighty- on e migraineurs attending a headache clinic were asked to undergo a physician- ad ministered structured interview designed to gather information on CAM use. Past use of CAM therapies was reported by 31.4% of the patients surveyed, with 17.1 % having used CAM in the previous year. CAM therapies were perceived as benefi cial by 39.5% of the patients who had used them. A significantly higher propor tion of transformed migraine patients reported CAM treatments as ineffective com pared with patients suffering from episodic migraine (73.1% vs. 50.7% , P graineurs’ recourse to CAM treatments was specif ically for their headache (89.3% ). Approximately 61% of CAM users had not in formed their medical doctors of their CAM use. The most common reason for decidi ng to try a CAM therapy was that it offered a ’ potential improvement of headac he’ (47.7% ). The greatest users of CAM treatments were: patients with a diag nosis of transformed migraine; those who had consulted a high number of speciali sts and reported a higher lifetime number of conventional medical visits; those with a comorbid psychiatric disorder; those with a high income; and those whose headache had been either misdiagnosed or not diagnosed at all. Our findings sugg est that headache clinic migraine patients, in their need of and quest for care, seek and explore both conventional and CAM approaches. Physicians should be mad e aware of this patient- driven change in the medical climate in order to preve nt misuse of healthcare resources and to be better equipped to meet patients’ needs.

Objectives.- To define predictors of migraine- related disability in patien ts with episodic and chronic migraine referred to a specialty migraine clinic, f ocusing on depressive symptoms and insecure attachment style that, because of th eir association with responses to pain and physical illness, might be predictive of greater migraine- related disability. Background.- The Migraine disability Assessment questionnaire (MIDAS) has proved to be a reliable and easy- to- us e instrument to assess migraine- related disability. As clinicians are increasi ngly using MIDAS in their diagnostic and treatment decisions for patient care, a n understanding of the factors influencing migraine- related disability is esse ntial for a rationale use of such an instrument. Methods.- Two- hundred patien ts suffering from episodic migraine without aura (EM), and chronic migraine (CM) with and without medication overuse, and referred to a specialty headache clini c were evaluated using the MIDAS, the Beck Depression Inventory (Bdi), and the A ttachment Style Questionnaire (ASQ). diagnosis of episodic and chronic migraine was operationally defined according to the International Headache Society (IHS) and Silberstein- Lipton criteria. Results.- Multiple regression analysis showe d that, in the total sample, disability was higher in those patients with CM, mo re severe depressive symptoms, an insecure style of attachment (as reflected by a lower score on the ASQ confidence scale), and experiencing more severe headach e pain intensity. In the subgroup of patients with episodic migraine, an insecur e style of attachment emerged as the most significant predictor of disability (o ther significant predictors were female sex and number of headache days per mont h). In contrast, in the subgroup of patients with CM, the only significant predi ctor of the total MIDAS score was a greater severity of depressive symptoms. Objectives. - This study was undertaken to evaluate the rates, pattern, and presence of predictors of complementary and alternative medicine use in a clinical population of patients with chronic tension- type headache. Background. - The use of complementary and alternative medicine in the treatment of headaches is a growing phenomenon about which little is known. Methods. - A total of 110 chronic tension- type headache patients attending a headache clinic participated in a physician- administered structured interview designed to gather information on complementary and alternative medicine use. Results. - Past use of complementary and alternative therapies was reported by 40% of the patients surveyed (22.7% in the previous year). Chronic tension- type headache patients prefer complementary and alternative practitioner- administered physical treatments to self- treatments, the most frequently used being chiropractic (21.9% ), acupuncture (17.8% ), and massage (17.8% ). Only 41.1% of the patients perceived complementary and alternative therapies to be beneficial. The most common source of recommendation of complementary and alternative medicine was a friend or relative (41.1% ). Most of the chronic tension- type headache patients used complementary and alternative treatment as a specific intervention for their headache (77.3% ). Almost 60% of complementary and alternative medicine users had not informed their medical doctors of their use of complementary and alternative medicine. The most common reasons given for choosing to use a complementary or alternative therapy was the “ potential improvement of headache” it offered (45.4% ). The patients who had used more complementary and alternative treatments were found to be those recording a higher lifetime number of visits to conventional medical doctors, those with a comorbid psychiatric disorder, those enjoying a higher (household) income, and those who had never tried a preventive pharmacological treatment. Schizophrenia is a severe mental illness which can have a devastating impact onan individual’s quality of life. Comorbidities are high amongst patients and lifeexpectancy is approximately 15 years less than the general population. Despite thewell-known increased mortality, little is known about the impact of gastrointestinaland liver disease on patients with schizophrenia. We aimed to reviewthe literature and to make recommendations regarding future care. Literaturesearches were performed on PubMed to identify studies related to gastrointestinaland liver disease in patients with schizophrenia. High rates of chronic liverdisease were reported, with Non-Alcoholic Fatty Liver disease being of particularconcern;antipsychotics and metabolic syndrome were contributing factors. Ratesof acute liver failure were low but have been associated with antipsychotic useand paracetamol overdose. Coeliac disease has historically been linked to schizophrenia;however, recent research suggests that a causal link is yet to be proven.Evidence is emerging regarding the relationships between schizophrenia andpeptic ulcer disease, inflammatory bowel disease and irritable bowel syndrome;clinical vigilance regarding these conditions should be high. Patients with schizophreniapoorly engage with bowel cancer screening programmes, leading to latediagnosis and increased mortality. Clozapine induced constipation is a significantissue for many patients and requires close monitoring. There is a significantburden of gastrointestinal and liver disease amongst patients with schizophrenia.Better levels of support from all members of the medical team are essential toensure that appropriate, timely care is provided.

Background The advancements of Artificial Intelligence,Big Data Analytics,and the Internet of Things paved the path to the emergence and use of digital Twins(DTs)as technologies to“twin”the life of a physical entity in different fields,ranging from industry to healthcare.At the same time,the advent of eXtended Reality(XR)in industrial and consumer electronics has provided novel paradigms that may be put to good use to visualize and interact with DTs.XR technologies can support human-to-human interactions for training and remote assistance and could transform DTs into collaborative intelligence tools.Methods We here present the Human Collaborative Intelligence empowered digital Twin framework(HCLINT-DT)integrating human annotations(e.g.,textual and vocal)to allow the creation of an all-in-one-place resource to preserve such knowledge.This framework could be adopted in many fields,supporting users to learn how to carry out an unknown process or explore others’past experiences.Results The assessment of such a framework has involved implementing a DT supporting human annotations,reflected in both the physical world(Augmented Reality)and the virtual one(Virtual Reality).Con-clusions The outcomes of the interface design assessment confirm the interest in developing HCLINT-DT-based applications.Finally,we evaluated how the proposed framework could be translated into a manufacturing context.

Modeling ferroelectric materials from first principles is one of the successes of density-functional theory and the driver of much development effort,requiring an accurate description of the electronic processes and the thermodynamic equilibrium that drive the spontaneous symmetry breaking and the emergence of macroscopic polarization.We demonstrate the development and application of an integrated machine learning model that describes on the same footing structural,energetic,and functional properties of barium titanate(BaTiO_(3)),a prototypical ferroelectric.The model uses ab initio calculations as a reference and achieves accurate yet inexpensive predictions of energy and polarization on time and length scales that are not accessible to direct ab initio modeling.These predictions allow us to assess the microscopic mechanism of the ferroelectric transition.The presence of an order-disorder transition for the Ti off-centered states is the main driver of the ferroelectric transition,even though the coupling between symmetry breaking and cell distortions determines the presence of intermediate,partly-ordered phases.Moreover,we thoroughly probe the static and dynamical behavior of BaTiO_(3)across its phase diagram without the need to introduce a coarse-grained description of the ferroelectric transition.Finally,we apply the polarization model to calculate the dielectric response properties of the material in a full ab initio manner,again reproducing the correct qualitative experimental behavior.
