Management strategies and the use of advanced technologies are equally important for determining the sample number and sampling frame for successful field sampling for animal disease prevalence *** quality of the biological samples collected in the field has a direct bearing on the integrity of the data generated,prevalence estimates and subsequent policy decisions on disease ***,compromising the quality of biological samples collected in the field could potentially undermine the priority setting principles in disease control *** samples collected from domestic animals in the field are precious materials and require meticulous planning for sample collection,sample storage in the field,transportation,and storage in the *** managed field sampling has a significant detrimental impact on the sample quality and quantity and directly affects the accuracy of disease prevalence data.A bad choice of sampling tools,containers,storage and transport all have a negative impact on the integrity of the sample and consequently have an impact on the *** the last two years,as part of our one health animal sampling work in ndia,we have observed challenges and opportunities in the field sampling of animals for disease prevalence *** paper aims to provide information on management practices and technologies for efficient biological sample collection from the field and ensure that good quality samples are available fortesting.

Outcome of patients with traumatic cranial nerve palsy admitted to a university hospital in Nepal

Background Cranial nerve palsy (CNP) is a common complication of traumatic brain injury (TBI). Despite a high incidence of TBI in Nepal (382 per 100,000), literature on the specific management and outcome of CNP is lacking. This study aimed to examine the outcomes of TBI patients involving single versus multiple *** A retrospective chart review of 170 consecutive TBI patients admitted to the tertiary neurosurgical center in Nepal between April 2020 and April 2022 was conducted. Demographic, clinical, and etiological characteristics; imaging findings; and management strategies were recorded, compared, and analyzed using descriptive statistics. The Glasgow Outcome Scale Extended (GOSE) was used to measure the outcomes in two groups of patients (single and multiple CNP) at 3 *** Out of 250 eligible patients, 80 were excluded and CNP was noted in 29 (17.1%) of the remaining 170. The median age was 34.9 years, and falls (60.6%) were the most common cause of trauma. TBI severity was categorized based on GCS: mild (82.4%), moderate (15.9%), and severe (1.8%). Cranial nerve involvement was seen in 29 (17.05%) patients: single cranial nerve involvement in 26 (89.65%) and multiple nerve involvement in 3 (10.34%). The most common isolated cranial nerve involved was the oculomotor nerve (37.9%). CT findings revealed a maximum of skull fractures with no significant association between CNP and CT *** CNP is a common consequence of TBI with the most common etiology being falls followed by RTA. Single CNP was more common than multiple CNP with no significant difference in the outcome in the 3-month GOSE score. Further research is needed to determine the burden of traumatic CNP and establish specific management guidelines for different types of CNP.

Retrograde jejunogastric intussusception following Braun's jejunojejunostomy

Jejunogastric intussusception is a rare long term complication of Billroth Ⅱ gastrectomy. The case reported here is a 50 year old man with history of a Billroth Ⅱ gastrectomy and Braun's side-to-side jejunojejunal anastomosis who presented with hematemesis. On abdominal examination, there was a mass in the left iliac fossa. Computed tomography scan showed a retrograde jejunogastric intussusception across the gastrojejunostomy. On laparotomy, a retrograde intussusception of the distal jejunum through the jejunojejunal anastomosis and across the gastrojejunostomy with a gangrenous intussusceptum was found. The jejunojejunal anastomosis was taken down, the gangrenous segment was resected and bowel continuity was restored with two jejunojejunal anastomoses, proximal and distal to the gastrojejunostomy. The gastrojejunostomy was preserved. This case brings out an unusual type of retrograde gangrenous intussusception which occurred at two points of a previous anastomosis, i.e., jejunojejunostomy and gastrojejunostomy simultaneously, which could be managed with jejunal resection.

Standard triple versus levofloxacin based regimen for eradication ofHelicobacter pylori

AIM:To compare the eradication rates for Helicobacter pylori(***) and ulcer recurrence of standard triple therapy(STT) and levofloxacin based therapy(LBT).METHODS:Seventy-four patients with perforated duodenal ulcer treated with simple closure and found to be *** infected on 3 mo follow up were randomized to receive either the STT group comprising of amoxicillin 1 g bid,clarithromycin 500 mg bid and omeprazole 20 mg bid or the LBT group comprising of amoxicillin 1 g bid,levofloxacin 500 mg bid and omeprazole 20 mg bid for 10 d *** *** eradication rates,side effects,compliance and the recurrence of ulcer were assessed in the two groups at 3 mo follow ***:Thirty-four patients in the STT group and 32 patients in the levofloxacin group presented at 3 mo follow *** eradication rates were similar with STT and the LBT groups on intention-to-treat(ITT) analysis(69% vs 80%,P = 0.425) and(79% vs 87%,P = 0.513) by per-protocol(PP) analysis *** recurrence in the STT and LBT groups on ITT analysis was(20% vs 14%,P = 0.551) and(9% vs 6%,P = 1.00) by PP *** and side effects were also comparable between the groups.A complete course of STT costs Indian Rupees(INR) 1060.00,while LBT costs only INR ***:*** eradication rates and the rate of ulcer recurrence were similar between the STT and *** LBT is a more economical option compared to STT.
