Oxidation of aliphatic and aromatic halides by N-oxide functionalities supported on 4- vinyl pyridine, (4-VP), grafted cellulose is reported in the present manuscript. Synthesis of graft copolymer of cellulose and poly 4-vinyl pyridine, poly(4-VP), has been carried out using ceric ions as redox initiator. Post-grafting treatment of CellO-g-poly (4-VP) with 30% H2O2 in acetic acid gives Cellulose-g-poly (4-VP) N-oxide, the polymeric supported oxidizing reagent. The polymeric support, CellO-g-poly (4-VP) N-oxide, has been used for oxidation reactions of different alkyl / aryl halide such as 1-bromo-3-methyl butane, 2-bromo propane,1-bromo heptane and benzyl chloride. The polymeric reagent was characterized by IR and thermo-gravimetric analysis. The oxidized products were characterized by FTIR and H1NMR spectral methods. The reagent was reused for the oxidation of a fresh alkyl / aryl halides and it was observed that the polymeric reagent oxidizes the compounds successfully but with little lower product yield.

Background and Aims:Patients with cirrhosis and acuteon-chronic liver failure(ACLF)may have bleeding complications and need for invasive ***-of-care(POC)coagulation tests like thromboelastography(TEG)and Sonoclot may be better for guiding patient management than the standard coagulation tests(SCTs),like prothrombin time,platelet count and international normalized ***:We prospectively compared and validated the POC tests and SCTs in 70 persons with ACLF and 72 persons with decompensated cirrhosis who had clinical bleeding and checked for episodes of re-bleeding and transfusion *** assessed pre-procedure requirement of blood components when correction was done based on an SCT or POC ***:Episodes of bleeding were seen in 45%and 28%of ACLF and cirrhosis patient,respectively(p=0.036),with the major site of bleeding being gastrointestinal(31%and 16%,respectively).Platelet counts correlated with TEG-maximum amplitude in cirrhosis(p=0.045)and prothrombin time correlated positively with TEG-reaction(R)time(p=0.032),TEG-Clot kinetics(K)time(p=0.042),Son-activated clotting time(p=0.038)and negatively with clot rate(p=0.043)in ACLF,making these correctable target variables in POC transfusion *** 223 procedures,transfusion of fresh frozen plasma and platelet concentrate was reduced by 25%(p=0.035)and 20.8%(p=0.045)by using a POC strategy in 76 *** of deranged Son-activated clotting time and TEG-reaction time was noted in 68%and 72%after 24 h of fresh frozen plasma transfusion in ACLF and 85%and 80%in cirrhosis,***:Our study clinically validates that POC tests can better detect coagulation defects and transfusion thresholds in ACLF and cirrhosis,whereas use of conventional tests appear to be less suitable in patients with clinical *** Registration:NCT04332484.

介绍:结肠癌(CRC)在中国所有肿瘤的发生率和癌源性死亡癌症种类中均排名第5。CRC在出现急性结肠梗阻(ACO)之前往往容易被忽视,这种情况占7%-29%。急性左半结肠癌恶性梗阻(ALMCO)是危及生命的急症,对其处理上存在争议。急诊ALMCO手术与高致病率,高死亡率和结肠造瘘相关,内镜下大肠自膨式金属内支架(SEMS)于90年代问世,提供了一种采用微创和非手术治疗的方式来解除除道阻塞的方法。本文目的是比较采用结肠支架后择期开腹手术和直接急诊行哈特曼手术(HARTMANN’S手术)或结肠灌洗后行结肠切除并一期吻合手术两者之间临床效果和成本效益的差别。方法:采用回顾性的研究的方法,收集从2010年1月到2016年12月连医科大学附属第二医院所有结肠癌合并肠梗阻的确诊病例,经纳入和排除标准筛选后,将患者依据治疗方法分为3组:SEMS支架组,ICI结肠灌洗组,HP哈特曼术组。在置入支架后择期行开腹手术患者被分为SEMS支架组,而HP组和ICI组的患者为急诊行哈特曼手术和结肠灌洗后行结肠切除并一期吻合手术的患者。结果:82人(36.12%)从227名左半结肠癌梗阻患者中入选。SEMS组、ICI组和HP组分别有10、23和49例。男性占大多数,为64.6%,女性为35.4%,中位数年龄69.5岁(60.757.25)。乙状结肠病变例最常见占63.4%,98.8%的病变是腺癌。结肠支架置入成功率为100%和90%,只有一例病例患者置入结肠支架导致肠穿孔。结果显示使用SEMS支架的患者组与其余两组相比手术时间更短,一期吻合成功率更高,造瘘情况出现更少(P0.05)。结论:1、SEMS支架在左半结肠癌急性梗阻患者术前减压治疗中安全微创有效。2、SEMS置入后择期手术治疗优于急诊手术,术后发病率和死亡率均可接受。

肝脏损害是由于微循环障碍、肝细胞变性坏死而引起的肝功能异常,它影响到几乎每个年龄段人群肝脏细胞的重要生物活性,而常规用药来抑制肝脏损害是远远不够的。而且,由于对抗疗法所用药物往往具有肝毒性,反而加剧了肝脏功能的紊乱。为了寻找安全有效的替代药物,人们逐渐把目光转向了具有保肝作用的药用植物,并对其进行系统性的方法学研究和药用原理评估。药用植物治疗肝病具有悠久的历史,最早可追溯到公元前2100年的中国夏朝和古印度吠陀时期,但是能证明那个时期药用植物有效治疗肝病的证据却很难寻觅。本文旨在系统性地概述现今在临床治疗和实验研究中最常用以及最有效的保肝类药用植物。

Anterior cruciate ligament （ACL） reconstruction is usually recommended for young patients. Several recent articles have however reported comparable outcomes of ACL reconstruction between youth and patients in fourth or fifth age group. But in the literature there are not many reports about ACL reconstruction in patients over 70 years old. We report a case of a successful arthroscopic ACL reconstruction （using single bundle quadrupled hamstring graft） in an active 75-year-old medical practitioner. Successful outcome after ACL reconstruction can be achieved in selected older patients; chronological age is no barrier.

The paper presents a comparison between intra-ramet, intra-clonal and inter-clonal variations in girth at breast height (gbh), fiber length, fiber diameter, wall thickness, vessel element length, vessel element diameter and specific gravity in the ramets of L34, G3 and S7C15 clones of Populus deltoides at the age of 6 years old produced from planting material grown by macro- and micro-propagation techniques. Variance ratio (F) test indicated that intra-ramet variations were non-significant for all the characters except specific gravity for height in L34 for macro and specific gravity and vessel element diameter for radial locations for micro, and fiber length for G3 (micro) for height, and specific gravity for radial location and fiber length for height for S7C15 clone for both the techniques. The clone L34 showed the significantly higher girth followed by G3 and S7C15. The variations were significant for girth, vessel element length and specific gravity between the wood produced from planting stocks grown by two techniques. Intra-clonal variations were significant for fiber length, fiber diameter and vessel element length. In general wood element’s dimensions and specific gravity increased from bottom to top and pith to periphery radial locations. G3 clone was different from the L34 and S7C15 clone for the wood traits. The three different clones of Populus deltoides showed variability in wood anatomical properties and specific gravity in the woods grown from macro- and micro-propagated planting stock. So, the plantation raised by two techniques could not produce similar type of wood even from the same clone. G3 clone was the exception as it did not show variation in wood traits for two techniques. Intra-clonal variations in all the three clones of P. deltoides indicated that wood traits were not stable within the population of same clone grown by either method.

We read with great interest the article, ＂Non-alcoholic fatty liver disease may not be a severe disease at presentation among Asian Indians＂ by Madan et at in the recent issue of WJG. Twenty-eight （55%） out of 51 patients with nonalcoholic fatty liver disease （NAFLD） who presented with abnormal transaminases had histological evidence of nonalcoholic steatohepatitis （NASH）. The majority of patients had grade 1 [32 （63%）] or grade 2 [16 （31%）] inflammation and either had no [23 （45%）] fibrosis or stage I [19 （37%）] fibrosis. None of the patients had cirrhosisf11. We agree with Madan et al that Asian Indians with NAFLD who present with unexplained increase in transaminases may have mild disease at presentation on the basis of similar observations made by us. NAFLD has a spectrum which includes patients with only steatosis and NASH that can progress to cirrhotic and hepatocellular carcinoma.
