Right Radical Nephrectomy with Type-IV IVC Tumor Thrombectomy Using Venovenous Bypass Instead of Cardiopulmonary Bypass—A Case Report

A renal mass with level Level IV IVC thrombus is usually managed with radical nephrectomy and IVC thrombectomy. This procedure requires the assistance of a cardiac surgeon and is usually done under complete cardiopulmonary bypass. However, the use of cardiopulmonary bypass is associated with reduced cardiac venous return and can consequently decrease cardiac output, adversely affecting haemo-dynamic stability and systemic arterial perfusion. This can lead to relative ischemia of the abdominal viscera, lower limbs and kidneys. We report a case where radical nephrectomy with IVC thrombectomy was done under venovenous bypass, thus avoiding the complications associated with the cardiopulmonary bypass.

Dynamically reconfigurable Field Programmable Gate Array(dr-FPGA) based electronic systems on board mission-critical systems are highly susceptible to radiation induced hazards that may lead to faults in the logic or in the configuration memory. The aim of our research is to characterize self-test and repair processes in Fault Tolerant(FT) dr-FPGA systems in the presence of environmental faults and explore their interrelationships. We develop a Continuous Time Markov Chain(CTMC) model that captures the high level fail-repair processes on a dr-FPGA with periodic online Built-In Self-Test(BIST) and scrubbing to detect and repair faults with minimum latency. Simulation results reveal that given an average fault interval of 36 s, an optimum self-test interval of 48.3 s drives the system to spend 13% of its time in self-tests, remain in safe working states for 76% of its time and face risky fault-prone states for only 7% of its time. Further, we demonstrate that a well-tuned repair strategy boosts overall system availability, minimizes the occurrence of unsafe states, and accommodates a larger range of fault rates within which the system availability remains stable within 10% of its maximum level.

Background:Radiological imaging plays a pivotal role in forensic *** have the imaging techniques advances,so have the digital skeletal measurements inched towards *** trends of the population keep on changing in modem ***,finding the precise technique of bone measurement,with greater reproducibility,in modem population is always needed in making population specific biological *** and Objective:The aim of this study was to estimate the accuracy of the foramen magnum measurement,obtained by three dimensional multi-detector computed tomography using volume rendering technique with the cut off value of each variable,in sex determination of an *** and Methods:Two metric traits,an antero-posterior diameter(APD)and transverse diameter(TD),were measured digitally in an analysis of 130 radiological images having equal proportion of male and female *** magnum index and area of foramen magnum(Area by Radinsky's[AR],Area by Teixeira5s[AT])were derived from APD and ***:Descriptive statistical analysis,using unpaired t-test,showed significant higher value in males in all the *** Pearson correlation analysis,maximum correlation was observed between area(AT and AR r=0.999)and between area and TD(AR r=0.955 and AT r=0.945 respectively).When used individually,TD had the highest predictive value(67.7%)for sex detennination among all the parameters followed by AT(65.4%)and AR(64.6%).Cutoff value of variables TD,AR and AT were 29.9 mm,841.80 mm2 and 849.70 mm2 *** operating characteristic curve predicted male and female sex with 96.2%and 89.2%accuracy *** overall accuracy of the model was 92.7%.Conclusion:Measurements from 3D CT using volume rendering technique were precise,and the application of logistic regression analysis predicted the sex with more accuracy.

AIM To prospectively study the outcome of difficult gastroduodenal perforations(GDPs) treated by triple tube drainage(TTD) in order to standardize the *** Patients presenting to a single surgical unit of a tertiary hospital with difficult GDPs(large, unfavourable local and systemic factors) were treated with TTD(gastrostomy, duodenostomy and feeding jejunostomy). Postoperative parameters were observed like time to return of bowel sounds, time to start enteral feeds, time to start oral feeds, daily output of all drains, time to clamping/removal of all drains, time for skin to heal, complications, hospital stay, and, mortality. Descriptive statistics were used. RESULTS Between December 2013 and April 2015, 20 patients undergoing TTD for GDP were included, with mean age of 44.6 ± 19.8 years and male:female ratio of 17:3. Mean pre-operative APACHE Ⅱ scores were 10.85 ± 3.55; most GDPs were prepyloric(9/20; 45%) or proximal duodenal(8/20; 40%) and mean size was 1.83 ± 0.59 cm(largest 2.5 cm). Median times of resumption of enteral feeding, removal of gastrostomy, removal of duodenostomy,removal of feeding jejunostomy and oral feeding were 4 d(4-5 IQR), 13(12-16.5 IQR), 16(16.25-22.25 IQR), 18(16.5-24 IQR) and 12 d(10.75-18.5 IQR) respectively. Median hospital stay was 22 d(19-26 IQR) while mortality was 4/20(20%). CONCLUSION TTD for difficult GDP is feasible, easy in the emergency, and patients recover in two-three weeks. It obviates the need for technically demanding and riskier procedures.

A new impurity was detected during high performance liquid chromatographic （HPLC） analysis of eslicarbazepine acetate active pharmaceutical ingredient. The structure of unknown impurity was postulated based on liquid chromatography mass spectrometry using electrospray ionization and ion trap analyzer （LC/ESI-IT/MS） analysis. Proposed structure of impurity was unambiguously confirmed by synthesis followed by characterization using 1H, 13C nuclear magnetic resonance spectrometry （NMR）, 1H-1H correlation spectro-scopy （COSY） and infrared spectroscopy （IR）. Based on the spectroscopic and spectrometric data, unknown impurity was characterized as 5-carbamoyl-10,11-dihydro-5H-dibenzo[b,f]azepin-10-yl propionate.

Analysis and Predictions of Spread, Recovery, and Death Caused by COVID-19 in India

The novel coronavirus outbreak was first reported in late December 2019 and more than 7 million people were infected with this disease and over 0.40 million worldwide lost their lives. The first case was diagnosed on30 January 2020 in India and the figure crossed 0.24 million as of 6 June 2020. This paper presents a detailed study of recently developed forecasting models and predicts the number of confirmed, recovered, and death cases in India caused by COVID-19. The correlation coefficients and multiple linear regression applied for prediction and autocorrelation and autoregression have been used to improve the accuracy. The predicted number of cases shows a good agreement with 0.9992 R-squared score to the actual values. The finding suggests that lockdown and social distancing are two important factors that can help to suppress the increasing spread rate of COVID-19.

Antiplasmodial activity of silver nanoparticles: A novel green synthesis approach

Objective:To synthesize silver nanoparticles using silver nitrate by a green technique which involves different compositions of aqueous leaf extracts of Azadirachta indica(neem)and Ocimum sanctum(tulsi).Methods:Their shape and size were determined using transmission electron microscopy and UV-visible *** antiplasmodial activity was studied using the malarial parasite strain(Plasmodium falciparum,3D7).The parasite strain(3D7)was collected and revived in vitro using Trager and Jensen method in RPMI 1640 medium for *** maximal effective concentration values were calculated by nonlinear regression ***:Transmission electron microscopy results confirmed the formation of silver nanoparticles with size ranging from 4.74-39.32 nm and their size differs by varying the concentrations from 20%to 100%of neem extract in neem and tulsi *** was observed that samples B and C showed half maximum effective concentration of about 0.3μ***:It can be easily established that the aqueous leaf extracts of neem and tulsi in combination can be a good source for synthesis of silver nanoparticles with small size possessing appreciable antiplasmodial activity.

The pandemic situation in 2020 brought about a‘digitized new normal’and created various issues within the current education *** of the issues is the monitoring of students during online examination situations.A system to determine the student’s eye gazes during an examination can help to eradicate *** this work,we track the users’eye gazes by incorporating twelve facial landmarks around both eyes in conjunction with computer vision and the HAAR *** aim to implement eye gaze detection by considering facial landmarks with two different Convolutional Neural Network(CNN)models,namely the AlexNet model and the VGG16 *** proposed system outperforms the traditional eye gaze detection system which only uses computer vision and the HAAR classifier in several evaluation metric *** proposed system is accurate without the need for complex ***,it can be implemented in educational institutes for the fair conduct of examinations,as well as in other instances where eye gaze detection is required.

Electricity is one of the most widely used forms of energy. Being a renewable source of energy small hydropower is considered as an environment—friendly and cheap source of electricity. The installation cost of the small hydropower project depends mainly on two parts—civil works and electromechanical equipment. One of the most important element on the recovery of a small hydro-power plant is the electromechanical equipment (turbine-alternator). The present paper intends to develop a correlation to determine the cost based on the cost influencing parameters as power and head using three different methods, namely;sigma plot method, linest method and logest method. An attempt has also been made to identify the best correlation among the three models closer to the actual cost of electro-mechanical equipment as collected from recently developed projects.
