非心脏手术围手术期使用β阻断剂的证据有多强?对一系列随机对照试验的系统性回顾与荟萃分析

Obj.ctive: To determine the effect of p.riop.rative βblocker treatment in p.tients having non-cardiac surgery. Design: Systematic review and meta-analysis. Data sources: Seven search strategies, including searching two bibliograp.ic databases and hand searching seven medical j.urnals. Study selection and outcomes: We included randomised controlled trials that evaluated βblocker treatment in p.tients having non-cardiac surgery. p.riop.rative outcomes within 30 days of surgery included total mortality, cardiovascular mortality, non-fatal myocardial infarction, non-fatal cardiac arrest, non-fatal stroke, congestive heart failure, hyp.tension needing treatment, bradycardia needing treatment, and bronchosp.sm. Results: Twenty two trials that randomised a total of 2437 p.tients met the eligibility criteria. p.riop.rative βblockers did not show any statistically significant beneficial effects on any of the individual outcomes and the only nominally statistically significant beneficial relative risk was 0.44(95%confidence interval 0.20 to 0.97, 99%confidence interval 0.16 to 1.24) for the comp.site outcome of cardiovascular mortality, non-fatal myocardial infarction, and non-fatal cardiac arrest. Methods adap.ed from formal interim monitoring boundaries ap.lied to cumulative meta-analysis showed that the evidence failed, by a considerable degree, to meet standards for forgoing additional studies. The individual safety outcomes in p.tients treated with p.riop.rative βblockers showed a relative risk for bradycardia needing treatment of 2.27(95%CI 1.53 to 3.36, 99%CI 1.36 to 3.80) and a nominally statistically significant relative risk for hyp.tension needing treatment of 1.27(95%CI 1.04 to 1.56, 99%CI 0.97 to 1.66). Conclusion: The evidence that p.riop.rative βblockers reduce maj.r cardiovascular events is encouraging but too unreliable to allow definitive conclusions to be drawn.

Electron-p.onon interaction and related self-energy are fundamental to both the equilibrium p.op.rties and non-equilibrium relaxation dynamics of *** ele ctron-p.onon interaction has been suggested by various time-resolved measurements to be imp.rtant for the relaxation dynamics of grap.ene,the lack of energy-and momentum-resolved self-energy dynamics p.ohibits direct identification of the role of sp.cific p.onon modes in the relaxation ***,by p.rforming time-and angle-resolved p.otoemission sp.ctroscop. measurements on Kekulé-ordered grap.ene with folded Dirac cones at the Γ p.int, we have succeeded in resolving the self-energy effect induced by the coup.ing of electrons to two p.onons at Ω=177 meV and Ω=54 meV,and revealing its dynamical change in the time ***,these strongly coup.ed p.onons define energy thresholds,which sep.rate the hierarchical relaxation dynamics from ultrafast,fast to slow,thereby p.oviding direct exp.rimental evidence for the dominant role of mode-sp.cific p.onons in the relaxation dynamics.

背景:非心脏手术与高的心血管死亡率、发病率和费用相关。小样本试验表明β受体阻断剂可能预防接受非心脏手术治疗的患者发生心血管事件,但试验结果并不能得出确切结论。作者进行p.ISE试验的目的是确定β受体阻断剂在接受非心脏手术患者中的作用。方法:p.ISE试验是一项盲法、随机对照试验,以10000例接受非心脏手术且有发生围手术期心血管事件风险的患者为研究对象,比较应用美托洛尔控释剂与安慰剂的作用。患者将在手术之前2～4h接受试验药物,并持续至术后30d。
