Pharmacologic vitreolysis:new strategy for treatment of anomalous vitreo-macular adhesion

Persistent anomalous vitreo-macular adhesion(VMA) is a well-known factor, associated with a variety of sight threatening diseases- including macular hole, vitreo-macular traction syndrome, cystoid and diabetic macular edema, exudative age- related macular degeneration, myopic traction maculopathy and others. With the advent of optical coherence tomography our understanding of these pathologies and the ability of their early diagnosis has gone much far in the past two decades. The release of macular traction has been of exclusive surgical capability. notwithstanding good results, vitrectomy is hampered by the inability of complete vitreo-retinal separation(i.e., smooth, bare internal limiting membrane), compulsory postoperative positioning in macular hole cases, surgical complications, and high costs. With aim to offer less invasive and safe treatment modality for anomalous VMA, investigators have made enormous progress in the past decade. Leading among the studied nonsurgical measures is the intravitreal application of pharmacologic agents for the induction of vitreo-retinal separation and vitreous liquefaction, a method termed pharmacologic vitreolysis. Several vitreolytic agents have been studied to date, the most potent among them proved to be plasmin. Recently, ocriplasmin(formerly known as microplasmin)- a more stable than plasmin recombinant product, proved to be safe and efficient in releasing VMA in large studies, and consequently received FDA approval. It's role in clinical practice is now in the process of being determined. This paper aims to review and summarize the current knowledge and status of investigation on this new approach for the treatment of VMA.

Quantitative Assessment of Foveal Avascular Zone in Patients with Early and Intermediate nonexudative Age-Related Macular Degeneration Using Optical Coherence Tomography-Angiography

Optical coherence tomography-angiography (OCT-A) is a relatively new non-invasive imaging technique which provides three-dimensional visualization of the retinal and choroidal microvasculature. Recently, OCT-A is widely used in the diagnosis of age-related macular degeneration (AMD). Purpose: To compare 3 quantitative indexes of foveal avascular zone (FAZ)—area (A), perimeter (Pm), and circularity (C) in patients with early and intermediate nonexudative AMD and healthy subjects using OCT-A. Methods: Eighty-nine patients with nonexudative AMD (n = 89 eyes) were enrolled in the study. They were compared with sex- and age-matched control group of 66 healthy subjects (n = 66 eyes). The patients with nonexudative AMD were divided into 2 subgroups—early AMD (n = 42 eyes) and intermediate AMD (n = 47 eyes). All participants underwent OCT-A (3 × 3 mm scans, Cirrus HD-OCT, Angioplex, Carl Zeiss Meditec, Dublin, CA). Results: There was no statistically significant difference in all 3 assessed indexes between the early AMD subgroup and the control group (A ? P = 0.139;Pm ? P = 0.230;and C ? P = 0.191). There was also no statistically significant difference in all 3 assessed indexes between the intermediate AMD subgroup and the control group (A ? P = 0.106, Pm ? P = 0.124, and C ? P = 0.102, respectively). Conclusions: According to our results from the assessment of FAZ in patients with early and intermediate AMD, we hypothesize that macular perfusion remains fairly unchanged in the nonexudative stages of the disease. OCT-A is a reliable technique for visualizing and quantifying FAZ in patients with nonexudative AMD.

Macular Morphology in Patients with Retinopathy of Prematurity

Purpose: To investigate macular morphology by spectral domain optical coherence tomography (SDOCT) in children with history of the retinopathy of prematurity (ROP). Materials and methods: In this cross-sectional study we included 35 eyes of 18 patients, aged 4 to 8 years, with history of ROP. Twenty-one eyes had received treatment for type 1 pre-threshold ROP. The others showed spontaneous regression of the disease. A 7 × 7-mm retinal area was sampled using a 3-dimensional scanning protocol with high-resolution spectral domain optical coherence tomography (SOCT Copernicus REVO). The central foveal thickness, mean macular thickness, macular volume and foveal depression were measured. The macular anatomy and the foveal contour were examined. Results: The mean birth weight was 1338.29 ± 324.05 grams. Mean gestational age at birth was 29.37 ± 2.07 weeks. Twenty-one eyes were treated with laser or cryotherapy. All of them showed regression of the disease. The mean best corrected visual acuity was 20/28. The average central foveal thickness was 262.26 ± 37.39, with 68.57% of preterm > 240 μm and 22.86% > 220 μm. In all eyes, the foveal contour is abnormal. A foveal depression was absent in 25 eyes (71%). Conclusion: In the majority of children with a history of ROP, we found changes in macular morphology, including retention of inner retinal layers and absent foveal depression. In our study foveal hypoplasia was associated with good functional outcome.

Evaluation of Serum Anti-Müllerian Hormone (AMH) Values for 28,016 Bulgarian Women: Prognostic Statistical Model of Age Specific AMH Declining

The present study aims to establish a relationship between serum AMH levels and age in a large group of women living in Bulgaria, as well as to establish reference age-specific AMH levels in women that would serve as an initial estimate of ovarian age. A total of 28,016 women on the territory of the Republic of Bulgaria were tested for serum AMH levels with a median age of 37.0 years (interquartile range 32.0 to 41.0). For women aged 20 - 29 years, the Bulgarian population has relatively high median levels of AMH, similar to women of Asian origin. For women aged 30 - 34 years, our results are comparable to those of women living in Western Europe. For women aged 35 - 39 years, our results are comparable to those of women living in the territory of India and Kenya. For women aged 40 - 44 years, our results were lower than those for women from the Western European and Chinese populations, close to the Indian and higher than Korean and Kenya populations, respectively. Our results for women of Bulgarian origin are also comparable to US Latina women at age 30, 35 and 40 ages. On the base on constructed a statistical model to predicting the decline in AMH levels at different ages, we found non-linear structure of AMH decline for the low AMH 3.5) the dependence of the decline of AMH on age was confirmed as linear. In conclusion, we evaluated the serum level of AMH in Bulgarian women and established age-specific AMH percentile reference values based on a large representative sample. We have developed a prognostic statistical model that can facilitate the application of AMH in clinical practice and the prediction of reproductive capacity and population health.

Clinical analysis on neuroprotection of transient ischemic attacks

Transient ischemic attack（TIA） is an acute cerebrovascular incident,and is generally considered the best opportunity for early neuroprotective treatment against cerebral *** study retrospectively analyzed 80 patients with TIA（38 males and 42 females）.Among 61 patients who received neuroprotective cerebrolysin treatment within 24 hours after TIA onset,13（21.31%） patients suffered subsequent *** 19 patients who received neuroprotective cerebrolysin treatment within 24-72 hours after TIA onset,seven（36.84%） developed cerebral *** was a significant difference in the proportion of subsequent strokes between patients receiving cerebrolysin treatment within 24 hours and 24-72 hours after TIA onset（P = 0.438）.These findings suggest that neuroprotective drugs administrated within 24 hours after TIA onset help reduce the incidence of subsequent *** results demonstrate usefulness of the ABCD2 score at TIA patients in the determination of short-term and long-term cerebrovascular risk,including the frequency of subsequent ischemic cerebral infarctions up to 12 months.

Self-healing effect of ceria electrodeposited thin films on stainless steel in aggressive 0.5 mol/L naCl aqueous solution

The self-healing effect of electrochemically deposited CeO2-Ce2O3 films on stainless steel OC404（SS） in 0.5 mol/L naCl solution was studied. It was established that the corrosion potential of the steel, after covering it with CeO2-Ce2O3 layer and thermal treatment（i.e. potential of the system CeO2-Ce2O3/SSt.t.）, was shifted sharply to a considerably more positive value, while the corrosion current was reduced noticeably. The X-ray photoelectron spectroscopy（XPS）, energy dispersive spectroscopy（EDS） and scanning electron microscopy（SEM） analyses on the observed scratched surface area of the system CeO2-Ce2O3/SSt.t., after exposure of investigated specimens to 0.5 mol/L naCl corrosion media, showed partial accumulation of ceria, as well as remarkable increase in the concentrations of oxides of Al, Cr and Fe, on the mechanically revealed steel surface. On the basis of the obtained results one could conclude that the secondary passive oxide/hydroxide films, formed after a definite time interval of exposure to corrosion media, acted as barriers, hindering the corrosion processes in active zones. A hypothesis was put forward about the mechanism of self-repairing oxide layers on the steel surface and their corrosion protection effect respectively.

Semen Quality in the General Population of Bulgaria

Background: With the accumulating evidence of semen difference between distinct populations we decided to conduct a population-based prospective study to assess the reproductive capabilities of the Bulgarian males. Objective: To evaluate the semen parameters in men from the general population of Bulgaria. To analyze the effects of BMI (Body mass index) and tobacco smoking on semen characteristics. Methods: 482 males without history of reproductive problems were included for the purpose of this study. The duration of our study was from April to May in 2016 and April in 2017. The volunteers were subdivided based on their smoking status and on their BMI. After semen collection, a conventional semen analysis was carried out manually by trained embryologists in an andrology laboratory according to WHO recommendations. The results were statistically analyzed and presented with their mean values. Result(s): After semen analysis we report a mean of 3.29 ml for semen volume, 40.68 × 106/ml for spermatozoa concentration, 128.38 × 106 for total sperm count, 52.54% of total motile spermatozoa in the ejaculate and a mean percentage of morphologically normal sperm standing at 9.6%. We did not find a negative effect of tobacco smoking on semen quality;however higher BMI is associated with lower sperm concentration per ml. Discussion and Conclusion(s): This is the first contemporary study, assessing semen quality in Bulgarians. The results obtained here show the lower quality of semen in Bulgaria, compared to other countries. no statistical differences were found between the semen quality of nonsmokers and smokers. Higher BMI was found to be associated with lower sperm concentration per milliliter.

ng>Background:ng> n>n>Answering the question of what is the optimal protocol for monitoring controlled ovarian stimulation (COS) still remains a challenge. The rapid introduction of new diagnostic methods and various components of telemedicine makes it possible to reduce the number of patient visits during ovarian stimulation, which will reduce the loss of time, costs, and risk for the patient from COVID-like situations. n>n>Methods:n>n> n>n>The different COS monitoring protocols are examined, thus proposing a new approach consisting of two successive phases. Results: In the first phase, E3G in urine samples is being examined, which is performed by the patient themselves with a small portable analyzer. Based on the results, the specialist prescribes the doses for stimulation. The second phase involves one single determination of the size and number of follicles at the end of stimulation, using TVUS, as well as the dynamics of serum levels of P4 and E2.
