Sphincter preservation for distal rectal cancer - a goal worth achieving at all costs?

To assess the merits of currently available treatment options in the management of patients with low rectal cancer, a review of the medical literature pertaining to the operative and non-operative management of low rectal cancer was performed, with particular emphasis on sphincter preservation, oncological outcome, functional outcome, morbidity, quality of life, and patient preference. Low anterior resection (AR) is technically feasible in an increasing proportion of patients with low rectal cancer. The cost of sphincter preservation is the risk of morbidity and poor functional outcome in a significant proportion of patients. Transanal and endoscopic surgery are attractive options in selected patients that can provide satisfactory oncological outcomes while avoiding the morbidity and functional sequelae of open total mesorectal excision. In complete responders to neo-adjuvant chemoradiotherapy, a non-operative approach may prove to be an option. Abdominoperineal excision (APE) imposes a permanent stoma and is associated with significant incidence of perineal morbidity but avoids the risk of poor functional outcome following AR. Quality of life following AR and APE is comparable. Given the choice, most patients will choose AR over APE, however patients following APE positively appraise this option. In striving toward sphincter preservation the challenge is not only to achieve the best possible oncological outcome, but also to ensure that patients with low rectal cancer have realistic and accurate expectations of their treatment choice so that the best possible overall outcome can be obtained by each individual.

Amiodarone hepatotoxicity complicating obstructive jaundice due to ampullary cancer

BAcKGROUND:The presence of dual pathology can cause diagnostic dilemmas. We present a case of adenocarcinoma of the ampulla of Vater with concurrent amiodarone hepatotoxicity. METHODS:Painless jaundice associated with a palpable gallbladder was investigated clinically,radiologically endoscopically and via liver biopsy. RESULTS:Liver biopsy showed amiodarone hepatotoxicity Endoscopic biopsy identified an ampullary adenoma However,the endoscopic ultrasound and intra-operative findings suggested a malignancy,which was confirmed postoperatively. cONcLUSIONS:While the classic findings of courvoisier’s Law are borne out in this case,the etiology of jaundice is twofold. Although dual pathology is uncommon it should always be considered.

Geometry optimization of a barrel silicon pixelated tracker

We have studied optimization of the design of a barrel-shaped pixelated tracker for given spatial boundaries. The optimization includes choice of number of layers and layer spacing. Focusing on tracking performance only,momentum resolution is chosen as the figure of merit. The layer spacing is studied based on Gluckstern＇s method and a numerical geometry scan of all possible tracker layouts. A formula to give the optimal geometry for curvature measurement is derived in the case of negligible multiple scattering to deal with trajectories of very high momentum particles. The result is validated by a numerical scan method, which could also be implemented with any track fitting algorithm involving material effects, to search for the optimal layer spacing and to determine the total number of layers for the momentum range of interest under the same magnetic field. The geometry optimization of an inner silicon pixel tracker proposed for BESIII is also studied by using a numerical scan and these results are compared with Geant4-based simulations.

Fibre Optic Protection System for concrete Structures

The design concepts, modelling and implementation of various fibre optic sensor protection systems for development in concrete structures were investigated. design concepts and on-site requirements for surface-mounted and embedded optical fibre sensor in concrete were addressed. Finite element （FE） modelling of selected sensor protection systems in strain-transfer efficiency from the structure to the sensing region was also studied. And experimental validation of specified sensor protection system was reported. Results obtained indicate that the protection system for the sensors performs adequately in concrete environment and there is very good correlation between results obtained by the protected fibre optic sensors and conventional electrical resistance strain gauges.

Several factitious foods were assessed for rearing the anthocorid predators Orius thripoborus （Hesse） and Orius naivashae （Poppius）（Hemiptera： Anthocoridae） in the laboratory. Developmental and reproductive traits of both Orius species were examined when offered frozen eggs of the Mediterranean flour moth, Ephestia kuehniella Zeller, frozen processed eggs of the medfly, ceratitis capitata Wiedemann, or mixed motile stages of the astigmatid mites Tyrophagusputrescentiae （Schrank） or carpoglyphus lactis （L）. Whereas c. lactis and T. putresecentiae proved to be an inferior food for rearing O. thripoborus and O. naivashae, eggs of c. capitata fully supported development and reproduction of both predators. Results on medfly eggs were similar or slightly inferior to those on E. kuehniella eggs, which is the standard food for culturing these anthocorid bugs. O. thripoborus could be maintained for 4 consecutive generations on c. capitata eggs indicating that processed medfly eggs can be a suitable and cheaper alternative to E. kuehniella eggs for prolonged rearing of these Orius spp.

In the suspended sludge system, when pH and temperature were suitable and carbon source was not limited, the denitrification and c-uptake rate followed a zero-order reaction. Mixed VFA c-source had higher denitrification rate than single VFA c-source consisting of them. When VFA were used as carbon source, consumed carbon quantity for denitrification was closed to 1. 07 mg VFA-c/mg NO-N. About 20% of the applied c/N was used for assimilative purposes. As rising up influent c/N by increasing the c load ,the sludge yield YN increased. The part of carbon consumption increased and the effluent oxidized nitrogen decreased. At pH 7. 5, 25℃ and sludge yield 0.72 mg VSS/mg NO-N, the calculated influent VFA-c /NO critical value was 1. 43 for complete denitrification.

cyclooxygenase-2 plays a central role in the genesis of pancreatitis and associated lung injury

BAcKGROUND: The exact mechanism by which cyclooxy- genase-2 (cOX-2) promotes inflammation in pancreatitis in obscure. This study was undertaken to investigate the role of cOX-2 inhibition in an animal model of pancreati- tis , a disease process characterized by a systemic inflamma- tory response and ensuing neutrophil-mediated lung injury. METHODS: Pancreatitis was induced in 24 Sprague-Daw- ley rats by intraperitoneal injection of 20% L-arginine (500 mg/100 g body weight). The animals were randomized into 3 groups (8 rats in each group); controls and rats with pancreatitis intravenously resuscitated with either normal saline (0.9% Nacl 3 ml/kg) at 24 and 48 hours or cOX-2 inhibitor (parecoxib 1 mg/kg). Pancreatic and lung inju- ries were assessed histologically. Lung injury was assessed utilizing wet;dry ratio and myeloperoxidase activity to in- dicate pulmonary neutrophil infiltration. A Western blot was used to determine cOX-2 protein expression in pancrea- tic tissue. RESULTS: The animals treated with cOX-2 inhibitors dis- played significantly less pancreatic and lung injuries than their normal saline counterparts. Histological pancreatic and lung injury scores were significantly reduced (P cOX-2 treated group. Lung wet: dry ratios were sig- nificantly improved and pulmonary neutrophil infiltration was attenuated in the cOX-2 group (Pconfirmed attenuated cOX-2 protein expression. cONcLUSION: This study shows, for the first time in a rat model, that adjuvant cOX-2 inhibition significandy attenu- ates the severity of both pancreatitis and its associated sys- temic inflammatory response and end-organ injury.

Listening to music manipulates attention to be more externally focused,which has the potential to improve muscular *** study aimed to determine the effect of listening to music on muscle activation during an isometric exercise task,and compare this effect to those of other attentional focus *** healthy subjects(n=35;16 men/19 women)completed an isometric elbow flexion task for 1 min in three randomized and counterbalanced conditions:internal focus(INT),external focus with a simple distraction task(EXT),or listening to music(MUS).Muscle activation of the biceps and triceps brachii and heart rate(HR)were recorded throughout the exercise *** of perceived exertion(RPE),affective valence,and motivation were measured at the end of each *** was no difference in muscle activation measures among the three *** during MUS was lower than EXT at 15 s([89.4±11.8]beats/min vs.[93.1±12.9]beats/min;p=0.018)and 30 s([90.6±12.4]beats/min vs.[94.2±12.5]beats/min;p=0.026),and lower than INT at 60 s([93.3±13.3]beats/min vs.[96.7±12.0]beats/min;p=0.016).Overall RPE was higher for INT(13.4±2.2)than for MUS([12.6±2.0];p=0.020)and EXT([11.94±2.22];pctive valence was higher for MUS than for INT([2.7±1.4]vs.[2.1±1.5];p=0.011).Manipulating attentional focus did not alter muscle activation for a light-intensity isometric muscular endurance task,though MUS was reported as more positive and requiring less exertion to complete than *** music can therefore be recommended during light-intensity isometric exercise based on the psychological benefits observed.

Taking the full advantage of the conformal growth characterizing atomic layer deposition(ALD),the possibility to grow co thin films,with thickness from several tens down to few nanometers on top of a granular topological insulator(TI)Sb2Tes film,exhibiting a quite high surface roughness(2-5 nm),was *** study the co growth on the Sb2Tes substrate,we performed simultaneous co depositions also on sputtered Pt substrates for *** conducted a thorough chemical-structural characterization of the co/Sb2Tes and co/Pt heterostructures,confirming for both cases,not only an excellent conformality,but also the structural continuity of the co layers.X-ray diffraction(XRD)and high-resolution transmission electron microscope(HRTEM)analyses evidenced that co on Sb2Te3 grows preferentially oriented along the[0oe]direction,following the underlying rhombohedric ***,co crystallizes in a cubic phase oriented along the[111]direction when deposited on *** work shows that,in case of deposition of crystalline materials,the ALD surface selectivity and conformality can be extended to the definition of local epitaxy,where in-plane ordering of the crystal structure and mosaicity of the developed crystallized grains are dictated by the underlying ***,a highly sharp and chemically-pure co/Sb2Tes interface was evidenced,which is promising for the application of this growth process for spintronics.

Preparation, characterization and Luminescent Properties of McM-41 Type Materials Impregnated with Rare Earth complex

Hybrid materials incorporating Eu-(TTA)(3). 2H(2)O (7hereafter designated as Eu-TTA, with TTA: thenoyltrifluoroacetone) in unmodified or modified McM-41 by 3-aminopropyl-triethoxysilane (APTES) were prepared by impregnation method. The obtained materials were characterized using X-ray diffraction (XRD), IR and diffuse reflectance spectroscopy and luminescence spectra. All the hybrid samples exhibited the characteristic emission bands of EU3+ under UV light excitation at room temperature, and the excitation spectra showed significant blue-shifts compared to the pure rare-earth complex. Although the red emission intensity in the modified hybrid was almost the half of the red emission intensity in the pure Eu-TTA complex at room temperature, the hybrid showed a much higher thermal stability due to the shielding character of the McM-41 host.
