Prevention of thromboembolic events after radical prostatectomy in patients with hereditary thrombophilia due to a factor V Leiden mutation by multidisciplinary coagulation management

Objective:To examine the perioperative impact of factor V Leiden mutation on thromboembolic events'risk in radical prostatectomy(RP)patients.With an incidence of about 5%,factor V Leiden mutation is the most common hereditary hypercoagulability among Caucasians and rarer in Asia.The increased risk of thromboembolic events is three-to seven-fold in heterozygous and to 80-fold in homozygous patients.Methods:Within our prospectively collected database,we analysed 33006 prostate cancer patients treated with RP between December 2001 and December 2020.Of those,patients with factor V Leiden mutation were identified.All patients received individualised recommendation of haemostaseologists for perioperative anticoagulation.Thromboembolic complications(deep vein thrombosis and pulmonary embolism)were assessed during hospital stay,as well as according to patient reported outcomes within the first 3 months after RP.Results:Overall,85(0.3%)patients with known factor V Leiden mutation were identified.Median age was 65(interquartile range:61-68)years.There was at least one thrombosis in 53(62.4%)patients and 31(36.5%)patients had at least one embolic event in their medical history before RP.Within all 85 patients with factor V Leiden mutation,we experienced no thromboembolic complications within the first 3 months after surgery.Conclusion:In our cohort of patients with factor V Leiden mutation,no thromboembolic events were observed after RP with an individualised perioperative coagulation management concept.This may reassure patients with this hereditary condition who are counselled for RP.

Detecting early changes in choroidal vascularity and thickness using optical coherence tomography in patients with corneal crosslinking for keratoconus

AIM:To investigate changes in choroidal thickness and vascularity in keratoconus patients treated with corneal crosslinking.METHODS:This study evaluated 28 eyes of 22 patients with keratoconus who underwent corneal crosslinking.The choroidal thicknesses were evaluated on enhanced depth imaging optical coherence tomography at the preoperative and postoperative 3d,1,and 3mo.Choroidal thickness in the four cardinal quadrants and the fovea were evaluated.The choroidal vascularity index was also calculated.RESULTS:There was no significant difference in central choroidal thickness between the preoperative and postoperative 3d,1mo(P>0.05).There was a significant increase in the 3mo(P=0.034)and a significant decrease in the horizontal choroidal vascularity index on the postoperative 3d(P=0.014),there was no statistically significant change in vertical axes and other visits in horizontal sections(P>0.05).CONCLUSION:This study sheds light on choroidal changes in postoperative corneal crosslinking for keratoconus.While it suggests the procedure’s relative safety for submacular choroid,more extensive research is necessary to confirm these findings and their clinical significance.

Effect of refractive error on temperament and character properties

AIM: To determine the effect of refractive error on temperament and character properties using Cloninger’s psychobiological model of personality.METHODS: Using the Temperament and Character Inventory(TCI), the temperament and character profiles of 41 participants with refractive errors(17 with myopia,12 with hyperopia, and 12 with myopic astigmatism) were compared to those of 30 healthy control participants.Here, temperament comprised the traits of novelty seeking, harm-avoidance, and reward dependence, while character comprised traits of self-directedness,cooperativeness, and self-transcendence.RESULTS: Participants with refractive error showed significantly lower scores on purposefulness,cooperativeness, empathy, helpfulness, and compassion(P <0.05, P <0.01, P <0.05, P <0.05, and P <0.01,respectively).CONCLUSION: Refractive error might have a negative influence on some character traits, and different types of refractive error might have different temperament and character properties. These personality traits may be implicated in the onset and/or perpetuation of refractive errors and may be a productive focus for psychotherapy.

Let A be an abelian category and M∈A.Then M is called a(D_(4))-object if,whenever A and B are subobjects of M with M=A⊕B and f:A→B is an epimorphism,Ker f is a direct summand of A.In this paper we give several equivalent conditions of(D_(4))-objects in an abelian category.Among other results,we prove that any object M in an abelian category A is(D_(4))if and only if for every subobject K of M such that K is the intersection K_(1)∩K_(2)of perspective direct summands K_(1)and K_(2)of M with M=K_(1)+K_(2),every morphismφ:M→M/K can be lifted to an endomorphismθ:M→M in End _(A)(M).

A novel Fe/HNT visible light-driven heterogeneous photocatalyst:Development as a semiconductor and photocatalytic application

Fe/HNT(Iron/Halloysite-nanotube) heterogeneous semiconductor catalysts operating effectively under visible light were developed by using FeCl3, FeSO4and Fe(OH)3sludge precipitated after electro-Fenton process and named as Fe/HNT-I, Fe/HNT-II and Fe/HNT-III, respectively. Chemical configuration and particle morphology of the catalysts were characterized with XRD, SEM-EDS and UV–vis DRS. Effect of the developed Fe/HNT photocatalysts was investigated for the degradation of Reactive Orange 16(RO16) textile dye under visible light. The photocatalytic decolorization of RO16 was 95.6%, 99.3% and 96.6%, respectively. It was found that the photocatalytic performance of Fe/HNT-III catalyst under visible light was effective compared to Fe/HNT-I and Fe/HNTII. The iron ratio in the catalyst’s structure(Fe:HNT ratio 0.25, 0.5 and 0.75(w/w)) and p H value(4, 7 and 9) in production phase were also changed to investigate the photocatalytic effect of Fe/HNT-III. An Fe:HNT ratio of 0.25 and a p H of 4 were determined as the optimum conditions for catalyst production. Optimum H2O2dosage value was also investigated for photocatalytic oxidation process and determined to be 10 m M. Finally, the optimum conditions were further used for the degradation of Terbinafine hydrochloride(Ter HCl) active drug and the treatment of wastewater from the textile and pharmaceutical industries.

Background:Shunt systems are used to provide cerebrospinal fluid drainage in the treatment of hydrocephalus.Recently,antibiotic-impregnated shunt systems are used to prevent colonization in the ventriculoperitoneal catheters.Methicillin-resistant Staphylococcus aureus(MRSA)is the most common causative microorganism of shunt infections.The aim of the study is to investigate effects of several substances on MRSA biofilms in the ventriculoperitoneal catheters.Methods:The present study consists of mainly eight groups(each has two subgroups as antibiotic-impregnated and nonantibiotic-impregnated catheters).In addition,each group contains six molds using MRSA strains.In this study,daptomycin(DAPT)(2 mg/ml),vancomycin(VAN)(10 mg/ml),linezolid(LIN)(2 mg/ml),N-acetylcysteine(NAC)(6 mg/ml),and various combinations of these substances were used to evaluate the treatment against MRSA using scanning electron microscope(SEM)images and microbiological enumeration.Results:The colony count in the antibiotic-impregnated samples significantly decreased compared to nonantibiotic-impregnated samples in the MRSA,MRSA+DAPT,and MRSA+LIN groups(p<0.01),respectively.Conversely,the colony count in antibiotic-impregnated samples significantly increased compared to nonantibiotic-impregnated samples in NAC+DAPT and NAC+VAN groups(p<0.01),respectively.Conclusions:The results showed that the use of antibiotic-impregnated catheters has a significant impact on the prevention of infection whereas the combination of NAC and DAPT showed better antibiofilm and antibacterial effects than other combinations on the prevention and treatment of nonantibiotic-impregnated catheter infections.

Exploring multidimensional aspects of walkability:An innovative analysis approach in Besiktas,Istanbul

This study develops a holistic approach that underlines the effect of spatial,behavioral,and perceptional dimensions on walkability at the neighborhood scale.The study recognizes quantitatively measured tangible factors and qualitatively assessed intangible values to evaluate the level of walkability in Besiktas,Istanbul(Turkey).The research explores the independent and combined effects of physical characteristics,individual perceptions,and walking behavior on walkability.Geographic Information Systems urban analyses,space syntax analysis,and observation to establish an analytical framework for spatial and behavioral dimensions of walkability in Besiktas central district are used as a methodology.Additionally,perceptional data is gathered through an experimental approach,which involves participants walking along pre-determined routes using cartography techniques and employing“shared walks”as a mobile method.The results of study confirmed previous findings from studies that the physical,social,and typological attributes of the built environment have a significant correlation between walking behavior and the physical and social aspects of the built environment.This study provides a comprehensive walkability assessment framework,encompassing quantitative and qualitative dimensions.By incorporating spatial,behavioral,and perceptual dimensions,this approach provides an efficient and integrated analytical methodology for evaluating walkability,distinguishing itself with its originality in the field of walkability research.

Construction Industry operates relying on various key economic indicators.One of these indicators is material prices.On the other hand,cost is a key concern in all operations of the construction industry.In the uncertain conditions,reliable cost forecasts become an important source of information.Material cost is one of the key components of the overall cost of construction.In addition,cost overrun is a common problem in the construction industry,where nine out of ten construction projects face cost overrun.In order to carry out a successful cost management strategy and prevent cost overruns,it is very important to find reliable methods for the estimation of construction material prices.Material prices have a time dependent nature.In order to increase the foreseeability of the costs of construction materials,this study focuses on estimation of construction material indices through time series analysis.Two different types of analysis are implemented for estimation of the future values of construction material indices.The first method implemented was Autoregressive Integrated Moving Average(ARIMA),which is known to be successful in estimation of time series having a linear nature.The second method implementedwas Non-LinearAutoregressive Neural Network(NARNET)which is known to be successful in modeling and estimating of series with non-linear components.The results have shown that depending on the nature of the series,both these methods can successfully and accurately estimate the future values of the indices.In addition,we found out that Optimal NARNET architectures which provide better accuracy in estimation of the series can be identified/discovered as result of grid search on NARNET hyperparameters.

Around 20%-30% of patients diagnosed with prostate cancer （PCa） still have high-risk PCa disease （HRPC） that requires aggressive treatment. Treatment of HRPC is controversial, and multimodality therapy combining surgery, radiation therapy, and androgen deprivation therapy have been suggested. There has been a trend toward performing radical prostatectomy （RP） in HRPC and currently, robot-assisted laparoscopic RP （RARP） has become the most common approach. Number of publications related to robotic surgery in HRPC is limited in the literature.
