Inflammatory bowel diseases(IBd),including Crohn's disease and ulcerative colitis,are complex diseases that result from the chronic dysregulated immune response in the gastrointestinal tract. The exact etiology is not fully understood,but it is accepted that it occurs when an inappropriate aggressive inflammatory respon-se in a genetically susceptible host due to inciting environmental factors occurs. To investigate the path-ogenesis and etiology of human IBd,various animal models of IBd have been developed that provided indispensable insights into the histopathological and morphological changes as well as factors associated with the pathogenesis of IBd and evaluation of therapeutic options in the last few decades. The most widely used experimental model employs dextran sodium sulfate(dSS) to induce epithelial damage. The dSS colitis model in IBd research has advantages over other various chemically induced experimental models due to its rapidity,simplicity,reproducibility and controllability. In this manuscript,we review the newer publicized advances of research in murine colitis models that focus upon the disruption of the barrier function of the intestine,effects of mucin on the development of colitis,alterations found in microbial balance and resultant changes in the metabolome specifically in the dSS colitis murine model and its relation to the pathogenesis of IBd.

Intramural duodenal hematoma is a rare cause of a proximal gastrointestinal tract obstruction.Presentation of intramural duodenal hematoma most often occurs following blunt abdominal trauma in children,but spontaneous non-traumatic cases have been linked to anticoagulant therapy,pancreatitis,malignancy,vasculitis and endoscopy.We report an unusual case of spontaneous intramural duodenal hematoma presenting as an intestinal obstruction associated with acute pancreatitis in a patient with established von Willebrand disease,type 2B.The patient presented with abrupt onset of abdominal pain,nausea,and vomiting.Computed tomography imaging identified an intramural duodenal mass consistent with blood measuring 4.7 cm×8.7 cm in the second portion of the duodenum abutting on the head of the pancreas.Serum lipase was 3828 units/L.Patient was managed conservatively with bowel rest,continuous nasogastric decompression,total parenteral nutrition,recombinant factorⅧ(humateP)and transfusion.Symptoms resolved over the course of the hospitalization.This case highlights an important complication of an inherited coagulopathy.

Talking about space and Africa,Zambia stands out because of an incredible story.As early as 1964,a Zambian teacher named Edward Nkoloso famously vowed to help his country to beat America to become the first country to put a man on the moon,and then go to Mars.despite his passion to launch a rocket that would send 12 astronauts,a space girl and cats to the moon,Nkoloso’s dream never came to fruition.Through his Zambian Space Academy,Nkoloso’s team of Zambian astronauts,whom he called“Afronauts,”underwent strenuous training,but it all came to nought because the United Nations wouldn’t give him the$700 million he needed to fund the space programme.

This paper presents the results of thermal comfort and air quality studies in naturally ventilated residential buildings in Kampala City,Uganda.Questionnaire surveys were used for obtaining occupant subjective thermal sensation votes.Indoor and outdoor measurements for air temperature(T_(a)),Mean Radiant Temperature(MRT),relative humidity,air speed,and air quality were done for seven buildings over fifteen days during the month of June 2019.designBuilder software was used to develop the reference building model and to simulate strategies for improvement of thermal comfort.Survey results showed that 67.8%of the respondents were comfortable with the indoor thermal environment.The indoor air temperature range was 22.7℃-27.9℃ which lies within the 80%acceptable limits set by ASHRAE 55.The measured indoor MRT range was 24.24℃-25.57℃.Measured levels of indoor PM_(2.5)were double the World Health Organization recommended limits,while the levels of PM 10 were thrice the recommended limits of ASHRAE 62.1.The CO_(2)concentrations were within the limits set by ASHRAE 62.1.The developed model predicted comfortable indoor conditions with a temperature range of 23.6℃-25.3℃ based on the 80%acceptable limits set by ASHRAE 55.The results show that majority of occupants preferred cooler temperatures during the day which is justified by the use of adaptive measures to obtain thermal comfort.

Effective oral drugs and vaccines require high delivery efficiency across the gastrointestinal epithelia and protection of medically effective payloads(i.e.,immunogens)against gastric damage.In this study,hollowed nanocarriers(NCs:silica nanospheres and gold nanocages)with poly-l-lysine(PLL)coating and mammalian orthoreovirus cell attachment proteinσ1 functionalization(NC-PLL-σ1)were explored as functional oral drug delivery vehicles(OddVs).The transport of these OddVs to mucosal lymphoid tissues could be facilitated by microfold cells(M-cells)mediated transcytosis(viaσ1-α2–3-linked sialic acids adherence)across gastrointestinal epithelia.PLL coating provided protection and slow-release of rhodamine 6 G(R6G),a model payload.The transport effectiveness of these OddVs was tested on intestinal organoid monolayers in vitro.When compared with other experimental groups,the fully functionalized OddV system(with PLL-σ1)demonstrated two significant advantages:a significantly higher transport efficiency(198%over blank control at 48 h);and protection of payloads which led to both better transport efficiency and extended-release of payloads(61%over uncoated carriers at 48 h).In addition,it was shown that the M cell presence in intestinal organoid monolayers(modulated by Rank L stimulation)was a determining factor on the transport efficiency of the OddVs:more M-cells(induced by higher Rank L)in the organoid monolayers led to higher transport efficiency for OddV-delivered model payload(R6G).The fully functionalized OddVs showed great potential as effective oral delivery vehicles for drugs and vaccines.

Reconfigurable intelligent surfaces(RISs)have attracted significant attention due to their capability in customizing wireless communication environments to improve system performance.In this study,we investigate the performance of an RIS-assisted multi-user multiple-input single-output wireless communication system,considering the impact of channel aging caused by users’relative movements.In particular,first,we propose a model incorporating the joint effects of channel aging and channel estimation errors to investigate the performance of the RIS-assisted system.Then,we derive novel closed-form expressions for characterizing the sum spectral efficiency with zero-forcing precoding.From our analysis,we unveil that an increase in the temporal channel correlation coefficient,the number of base station antennas,and the received power at the users could help improve system performance.Furthermore,increasing the number of reflecting elements M of the RIS generally yields a good system performance,but with a diminishing return when M is sufficiently large.Finally,simulation results are presented to validate the accuracy of the analytical results.

Purpose Understanding the femur load environment during daily activity is necessary for the understanding of risk of femoral pain,pain related falls,and femoral fractures,which could help the design of osteogenic exercises or the preventa-tive methods for older adults.Methods Using the finite element femur analysis,this study was to estimate the femoral strains at 9 cross-sections along the long axis of femur for stair ascent and descent(n=17;age:50-65 years).Motion analysis and inverse dynamics were com-bined with musculoskeletal modelling and optimization,then were used as input to a 3-d femur model to estimate femoral strains.Strains at the hip contact force peaks were calculated.Results The compressive and tensile strains during stair descent were greater than ascent for all or most cross-sections,especially for the proximal cross-sections of the femur:stair ascent produced−324.0±103.8 to−483.7±191.0μεcompressive strains and descent produced−608.8±288.4 to 1016.0±444.1με;stair ascent produced 336.2±105.4 to 391.8±136.9μεtensile strains and descent produced 546.9±252.8 to 741.7±333.6με.Conclusion Strains represent the material deformation effect on the bone due to the sum of all the bone external loads.Using bone strains could help future studies analyze load conditions in a more comprehensive way for other physical activities,which predicts the risk of stress fractures and tests if alternative methods(gait type change)could reduce stress and strain effectively.

Background and purpose Low-income and middle-income countries have the greatest stroke burden,yet remain understudied.This study compared the utility of Framingham versus novel risk scores for prediction of total stroke and stroke types in Chinese adults.Methods China Kadoorie Biobank(CKB)is a prospective study of 512726 adults,aged 30-79 years,recruited from 10 areas in China in 2004-2008.By 1 January 2018,43234 incident first stroke cases(36310 ischaemic stroke(IS);8865 haemorrhagic stroke(HS))were recorded in 503842 participants with no history of stroke at baseline.We compared the predictive utility of the Framingham Stroke Risk Profile(FSRP)with novel CKB stroke risk scores and included recalibration,refitting,stratifying by study area and addition of other risk factors.discrimination was assessed using area under the receiver operating characteristic curve(AUC)and calibration was assessed using Greenwood-Nam-d’Agostinoχ^(2) statistics.Results Incidence of total stroke varied fivefold by area in China.The FSRP had good discrimination for total stroke(AUC(95%CI);men:0.78(0.77 to 0.79),women:0.77(95%CI 0.76 to 0.78)),but poor calibration(χ^(2);men:1,825,women:3,053),substantially underestimating absolute risks.Recalibration reducedχ^(2) by>80%,but did not improve discrimination.Refitting the FSRP did not materially improve discrimination,but further improved calibration.Stratification by area improved discrimination(AUC;men:0.82(0.82 to 0.83);women:0.82(0.82 to 0.83)),but not calibration.Adding other risk factors yielded modest,but statistically significant,improvements in the AUCs.The findings for IS and HS were similar to those for total stroke.Conclusions The FSRP reliably differentiated Chinese adults with incident stroke,but substantially underestimated the absolute risks of stroke.Novel local risk prediction equations that took account of differences in stroke incidence within China enhanced risk prediction of total stroke and major stroke pathological types.
