Identifying pain-related concerns in routine follow-up clinics following oral and oropharyngeal cancer

AIM:To describe clinical characteristics of head and neck cancer(HNC)patients with pain and those wishing to discuss pain concerns during ***:Cross-sectional,questionnaire study using University of Washington Quality of life,version 4(UWQOl)and the Patients Concerns Inventory(PCI)in disease-free,post-treatment HNC *** pain on UW-QOl and indicating"Pain in head and neck"and"Pain elsewhere"on ***:One hundred and seventy-seven patientscompleted UW-QOl and *** prevalence of selfreported pain issues was 38%(67/177)comprising25%(44/177)with significant problems despite medications and 13%(23/177)with lesser or no problems but wishing to discuss *** aged under 65years and patients having treatment involving radiotherapy were more likely to have pain *** over half,55%(24/44)of patients with significant pain did not express a need to discuss *** with significant pain or others wanting to discuss pain in clinic had greater problems in physical and social-emotional functioning,reported suboptimal QOl,and also had more additional PCI items to discuss in clinic compared to those without significant pain and not wishing to discuss ***:Significant HNC-related pain is prevalent in the disease-free,posttreatment *** referral to a specialist pain team may be *** UW-QOl and PCI package is a valuable tool that may routinely screen for significant pain in outpatient clinics.

河流下切在岩溶区形成的多级层状洞穴,记录了地体隆升和地貌演化历史,但由于测年手段的限制,国内在这方面的研究一直进展不大。近年来,运用宇宙成因核素得到洞穴碎屑沉积物埋藏年龄的方法,已成为反演山地区域构造-地貌演化的新途径。本文尝试运用宇宙成因核素^(26)Al/^(10)Be埋藏测年技术,对属于贵州荔波地区4层洞穴系统的黑洞(第二层)碎屑沉积物进行年龄测定,假设样品进洞前无埋藏史,测年结果显示黑洞年龄至少有1.06±0.23Ma,并且该地区三岔河流域的侵蚀速率为55.1±2.3m/Ma。这是对该地区洞穴测年的一次新探索,体现出洞穴沉积物宇宙成因核素埋藏测年技术在我国晚新生代地质地貌演化研究方面具有良好的应用前景。

Purpose: To characterize a longitudinal relationship between highly active antiretroviral therapy immune recovery as defined by an increase in CD4 cell counts and any associated changes in intraocular pressure (IOP) in both patients with and patients without a history of cytomegalovirus (CMV) retinitis and to determine if human immunodeficiency (HIV)-induced reduction in IOP is reversible. Design: Retrospective analysis of patient data collected from 1997 through 2004. Participants: Cohort of patients from one eye center, including patients from the longitudinal Study of Ocular Complications of Acquired Immunodeficiency Syndrome. Methods: linear regression analyses were conducted within the CMV and non-CMV groups to determine the change in IOP per 100- unit change in CD4 count. Average changes in IOP per change in CD4 count were compared between the CMV and the non-CMV groups using a Wilcoxon rank-sum test. linear regression analyses were conducted within the CMV and non-CMV groups to determine the linear relationship between the 12- month change in IOP per 12- month 100- unit change in CD4 count. Main Outcome Measure: Intraocular pressure in relation to changes inCD4 cell counts. Results: Compared with the non-CMV group, the median IOP change per change in CD4 count was not statistically different from the CMV group (0.9 vs. 1.7 mmHg/100 CD4 cells, respectively; P=0.20). Analysis of the linear relationship between the 12- month change in IOP and the 12- month change in CD4 count within both the CMV and non-CMV groups showed a strong linear relationship: 67% of the variability in a 12- month IOP change for the CMV group (Plity in a 12- month IOP change for the non-CMV group (Plusions: Reduction in T-lymphocyte count in HIV infection is accompanied by a decrease in IOP in both CMV-infected and non-CMV-infected eyes, and immune recovery is associated with an increase in IOP.

Synthesis, characterization and biological applications of novel Schiff bases of 2-(trifluoromethoxy) aniline

A series of five new Schiff bases(1–5) were synthesized by reacting 2-(trifluoromethoxy) aniline with different aromatic aldehydes. The Schiff base compounds were characterized by spectroscopic and analytical methods. Crystal structure of one new compound was also reported. The pharmacological properties, including antibacterial(14 bacterial species), antifungal(7 strains), antimalarial, anti-trypanosomal and anti-HIV activities of the compounds, were investigated. Cytotoxicity of the tested compounds was evaluated against human cervix adernocarcinoma cells(He la). Bacterial minimum inhibitory concentration(MIC) results by broth microdilution method showed that Bacillus subtilis, Enterococcus faecalis, Staphylococcus aureus, Staphylococcus epidermidis, Proteus vulgaris, Klebsiella oxytoc and Klebsiella pneumonia were more sensitive in the presence of tested compounds with an MIC value of 15.6 μg/m l. All the tested compounds showed good to moderate activity against fungi. The sensitivity of Aspergillus fumigatus was higher than other strains with a minimum cell death concentration(MFC) of 15.6 μg/m l. Compound 1 showed greater antimalarial and anti-trypanosomal properties with very low to no cytotoxic effects against He la cells as compared with compound 5, while other compounds exhibited poor activity. Compounds 1–5 demonstrated good activity against HIV type-1. These Schiff base compounds could be used as good antimicrobial agents.

Best黄斑营养不良色觉异常的表型定量研究

Objectives: To quantify chromatic dysfunction in Best disease to reassess the classic categorization of macular chromatic damage and to investigate psychophysical and clinical correlations. Methods: Color-contrast discrimination was measured using 2 different psychophysical strategies in age-matched control (n=41) and patient (n=34) eyes. The first strategy measured performance along 3 main confusion lines (testing cone function), and the second evaluated discrimination ellipses (modified Cambridge Color Test). The main outcome measures were chromatic discrimination variables (confusion line length, ellipse length, angle, and axis ratio) and visual acuity (VA). Results: Significant loss of performance was seen in all color axes in our patients, and it increased monotonically with staging, becoming significant in Fishman stages 2 and 3. The classically assumed preferential type I red-green deficit was true only for stage 4. Substantial chromatic dysfunction occurred even with relatively preserved VA despite that negative correlations between all test variables and VA reached statistical significance. Partial correlation analysis showed that protan/deutan loss was related to VA independent of tritan loss. Statistically significant positive correlations were also found between lesion size and chromatic dysfunction. Conclusions: Chromatic discrimination is often impaired in Best disease, even when VA is still spared. Our quantitative psychophysical approach shows that the classic categorization as a type I red green deficit is valid only for disease stage 4.

Generation and Analysis of Hybrid-Electric Vehicle Transmission Shift Schedules with a Torque Split Algorithm

The increased concern over global climate change and lack of long-term sustainability of fossil fuels in the projected future has prompted further research into advanced alternative fuel vehicles to reduce vehicle emissions and fuel consumption. One of the primary advanced vehicle research areas involves electrification and hybridization of vehicles. As hybrid-electric vehicle technology has advanced, so has the need for more innovative control schemes for hybrid vehicles, including the development and optimization of hybrid powertrain transmission shift schedules. The hybrid shift schedule works in tandem with a cost function-based torque split algorithm that dynamically determines the optimal torque command for the electric motor and engine. The focus of this work is to develop and analyze the benefits and limitations of two different shift schedules for a position-3 (P3) parallel hybrid-electric vehicle. a traditional two-parameter shift schedule that operates as a function of vehicle accelerator position and vehicle speed (state of charge (SOC) independent shift schedule), and a three-parameter shift schedule that also adapts to fluctuations in the state of charge of the high voltage batteries (SOC dependent shift schedule). The shift schedules were generated using an exhaustive search coupled with a fitness function to evaluate all possible vehicle operating points. The generated shift schedules were then tested in the software-in-the-loop (SIl) environment and the vehicle-in-the-loop (VIl) environment and compared to each other, as well as to the stock 8l45 8-speed transmission shift schedule. The results show that both generated shift schedules improved upon the stock transmission shift schedule used in the hybrid powertrain comparing component efficiency, vehicle efficiency, engine fuel economy, and vehicle fuel economy.
