Acupuncture for treating polycystic ovary syndrome:guidance for future randomized controlled trials

Objective： To provide guidance for future randomized controlled trials （RCTs） based on a review con- cerning acupuncture for treating polycystic ovary syndrome （PCOs）. Methods： A comprehensive literature search was conducted in October 2015 using MEDLINE, EMBAsE, sCIsEARCH, Cumulative Index to Nursing and Allied Health Literature, the Cochrane Menstrual Disorders and subfertility Group trials register, Allied and Complementary Medi- cine （AMED）, China National Knowledge Infrastructure （CNKI）, and the Wanfang databases. RCTs comparing either acupuncture with no/sham/pharmacological intervention or a combination of acupuncture and conventional therapy with conventional therapy in the treatment of PCOs were included in this review. A quality evaluation was performed for each of the included studies. Results： Thirty-one RCTs were included in the review and were divided into four categories according to the type of intervention used in the comparator or control group. Menstrual frequency, hor- mones, anthropometrics, insulin sensitivity, blood lipids, and fertility were used as the main measurements to assess the effects of acupuncture on the patients with PCOs. Thirty trials, except for one, showed an improvement in at least one of the indicators of PCOs after acupuncture treatment. However, normalizing the methodological and reporting format remains an issue. Conclusions： Based upon this review of current clinical trials concerning acupuncture for treating PCOs, we provide guidelines for better clinical trial design in the future.

Hepatic encephalopathy （HE） is a common neuropsychiatric abnormality, which complicates the course of patients with liver disease and results from hepatocellular failure and/or portosystemic shunting. The manifestations of HE are widely variable and involve a spectrum from mild subclinical disturbance to deep coma. Research interest has focused on the role of circulating gut-derived toxins, particularly ammonia, the development of brain swelling and changes in cerebral neurotransmitter systems that lead to global CNs depression and disordered function. Until recently the direct investigation of cerebral function has been difficult in man. However, new magnetic resonance imaging （MRI） techniques provide a non-invasive means of assessment of changes in brain volume （coregistered MRI） and impaired brain function （fMRI）, while proton magnetic resonance spectroscopy （^1H MRs） detects changes in brain biochemistry, including direct measurement of cerebral osmolytes, such as myoinositol, glutamate and glutamine which govern processes intrinsic to cellular homeostasis, including the accumulation of intracellular water. The concentrations of these intracellular osmolytes alter with hyperammonaemia. MRs-detected metabolite abnormalities correlate with the severity of neuropsychiatric impairment and since MR spectra return towards normal after treatment, the technique may be of use in objective patient monitoring and in assessing the effectiveness of various treatment regimens.

soil erosion is a complex process involving multiple natural and anthropic agents,causing the deterio-ration of multiple components comprising soil ***,we provide an estimate of the spatial pat-terns of cropland susceptibility to erosion by sheet and rill,gully,wind,tillage,and root crops harvesting and report the co-occurrence of these processes using a multi-model *** addition,to give a global overview of potential future changes,we identify the locations where these multiple concurrent soil erosion processes may be expected to intersect with projected dry/wet climate changes by *** a modelled 1.48 billion hectares(B ha)of global cropland,our results indicate that 0.56 B ha(-36%of the total area)are highly susceptible(classes 4 and 5)to a single erosion process,0.27 B ha(-18%of the total area)to two processes and 0.02 B ha(1.4%of the total area)to three or more *** estimated 0.82 B ha of croplands are susceptible to possible increases in water(0.68 B ha)and wind(0.14 B ha)*** contend that the presented set of estimates represents a basis for enhancing our founda-tional knowledge on the geography of soil erosion at the global *** generated insight on multiple erosion processes can be a useful starting point for decision-makers working with ex-post and ex-ante policy evaluation of the UN sustainable Development Goal 15(Life on Land)***,this work provides the hitherto most comprehensive assessment of soil erosion risks at the global scale,based on state-of-the-art models.

semi-Automated Method for Multi-Tasking Measurement of Microbial Growth, Capsule, and Biofilm Formation

The majority of bacterial infections involve the formation of biofilms. Biofilm formation is nutrient and growth dependent. Determination of the effects of nutrients on exopolysaccharide production and bacterial growth is labor and time intensive. We tested whether the Bioscreen C (Growth Curves, Inc.) would have utility as a high-throughput tool in the measurement of fundamental phenotype expression, as it relates to growth conditions. Within 48 - 72 hr, reproduceble, statistically significant data on the affects of growth conditions on generation time, capsule production and biofilm formation (maximally for 25 different conditions per 24 hr run cycle;n = 4) were obtained. Although all s. aureus strains produced similar amounts of capsule, sarA– and agr– strains grew significantly slower than parent strain (1.6 fold slower) and produced significantly (p 0.05) less biofilm (~2 fold). E. coli growth rate, biofilm and capsule production in simulated nephropathic urine medium was similar for urine with insulin (20 μU). Addition of insulin to urine medium with proline increased generation time, capsule and biofilm production. Findings from this study show that the Bioscreen C is a rapid, reproducible, and easily manipulated system to concurrently measure bacterial growth, biofilm formation, and capsule production. In addition, there is the potential for further applications of this system by expanding the types of detector dyes used.

Tolerance of soybean(Glycine max L.)to Protoporphyrinogen Oxidase Inhibitors and Very Long Chain Fatty Acid synthesis Inhibitors Applied Preemergence

Nine field experiments were conducted in 2011 and 2012 at various locations in southern Ontario, Canada to determine the tolerance of soybean (Glycine max (L.) Merr.) to herbicides inhibiting protoporphyrinogen oxidase (Protox) and very long chain fatty acid (VLCFA) synthesis applied alone and in combination. Preemergence applications were evaluated for soybean injury, plant height, shoot dry weight, and yield in the absence of weed competition. Early-season soybean injury from the Protox inhibitors persisted 4 weeks after soybean emergence (WAE) with 3%, 5%, and 18% injury for flumioxazin, saflufenacil, and sulfentrazone, respectively. When Protox inhibitors were tank mixed with VLCFA inhibitors (i.e., dimethenamid-P, s-metolachlor, and pyroxasulfone), additive interactions were observed for injury with saflufenacil and sulfentrazone;whereas synergistic interactions were observed with flumioxazin. However, injury subsided over time decreasing from as much as 34% injury 1 WAE for the flumioxazin + s-metolachlor tank mix down to 9% injury 4 WAE. In general, when saflufenacil or flumioxazin were tank mixed with VLCFA inhibitors, greater than expected reductions in height and dry weight were observed indicating synergistic responses;while no interactive effects were detected with sulfentrazone and VLCFA inhibitor tank mixes. For the flumioxazin tank mixes that contained dimethenamid-P or s-metolachlor, the reduction in yield was greater than expected indicating synergistic interactive effects. Yet, all the demonstrated impacts were transient as the yield for soybean treated with any of the Protox inhibitor and VLCFA inhibitor tank mixes tested were similar to the untreated control. Therefore, usage restriction on these mixtures, based on perceived negative yield impact, should be lifted so the herbicides could be combined to expand weed control options.

Objectives: The purpose of this study was to evaluate the influence of mode of delivery on sexual function. Design: One thousand six hundred and thirteen questionnaires containing 16 questions about sexual behavior and dyspareunia before, during and after pregnancy were sent out to primiparous, ethnically homogeneous (fluent in German) patients who delivered in a large tertiary referral center between 6 months and 2.5 year before. The returned questionnaires were merged to clinical data from our obstetric database in an anonymous fashion. The patients were subdivided into four groups (A) “spontaneous without injuries (except minor labial laceration)”, (B) “c-section”, (C)-“episiotomy or perineal laceration”, and (D) “operative vaginal delivery”. Results: The response rate of primiparae was 41%(655/1613). Forty-seven percent of women resumed sexual intercourse (sI) within 8 weeks after delivery. Altogether 31%of the women did not experience any pain during the first sI post-partum whereas 49%of all patients noted significant pain (medium, considerable or severe), depending on the mode of delivery (p = 0.007). Persistence of dyspareunia longer than 6 months was 3.5%(4/115; groupA), 3.4%(2/58; group B), 11%(34/316; group C), and 14%(20/114; group D). Conclusions: Recently, female sexuality may not have been prominent in any discussion concerning possible advantages and disadvantages of different modes of childbirth. Our results should be taken into consideration when counseling patients antenatally regarding mode of delivery.
